目的:评估基于注射用全氟丁烷微球[商品名示卓安(Sonazoid)]超声造影Kupffer期的深度学习模型预测肝细胞癌(hepatocellular carcinoma,HCC)微血管侵犯(microvascular invasion,MVI)的效能,并将其与影像组学模型及临床模型进行比较。方法...目的:评估基于注射用全氟丁烷微球[商品名示卓安(Sonazoid)]超声造影Kupffer期的深度学习模型预测肝细胞癌(hepatocellular carcinoma,HCC)微血管侵犯(microvascular invasion,MVI)的效能,并将其与影像组学模型及临床模型进行比较。方法:回顾并纳入2020年7月—2022年9月于广西医科大学第一附属医院接受Sonazoid超声造影检查的146例原发性HCC患者,以7∶3随机划分为训练集102例和验证集44例。基于肿瘤感兴趣区,使用ResNet101模型通过迁移学习提取深度学习特征,使用PyRadiomics提取影像组学特征。采用Mann-Whitney U检验、最小绝对收缩和选择算子(least absolute shrinkage and selection operator,LASSO)算法进行特征降维。LASSO回归用于构建深度学习模型和影像组学模型,同时还基于临床特征构建一个临床模型。采用受试者工作特征曲线的曲线下面积(area under the curve,AUC)、灵敏度、特异度和准确度评估模型的诊断效能。DeLong检验用于比较模型间的诊断效能。结果:在训练集中,深度学习模型、影像组学模型、临床模型的AUC(95%CI)分别为0.931(0.880~0.981)、0.823(0.744~0.903)、0.719(0.614~0.824)。在验证集中,深度学习模型、影像组学模型、临床模型的AUC(95%CI)分别为0.895(0.757~1.000)、0.711(0.514~0.909)、0.606(0.390~0.822)。DeLong检验表明在训练集和验证集中,深度学习模型的诊断效能均优于影像组学模型及临床模型(P<0.05)。单因素及多因素logistic回归分析示甲胎蛋白和巴塞罗那临床肝癌分期可作为HCC患者MVI的独立预测因子(P<0.01)。结论:基于Sonazoid超声造影Kupffer期的深度学习模型在预测HCC患者MVI方面表现出优异的性能,有望成为预测MVI的无创影像学生物标志物。展开更多
Submicron scale temperature sensors are crucial for a range of applications,particularly in micro and na-noscale environments.One promising solution involves the use of active whispering gallery mode(WGM)microresonato...Submicron scale temperature sensors are crucial for a range of applications,particularly in micro and na-noscale environments.One promising solution involves the use of active whispering gallery mode(WGM)microresonators.These resonators can be remotely excited and read out using free-space structures,simplifying the process of sensing.In this study,we present a submicron-scale temperature sensor with a remarkable sensitivity up to 185 pm/℃based on a trian-gular MAPbI3 nanoplatelet(NPL)laser.Notably,as temperature changes,the peak wavelength of the laser line shifts lin-early.This unique characteristic allows for precise temperature sensing by tracking the peak wavelength of the NPL laser.The optical modes are confined within the perovskite NPL,which measures just 85 nm in height,due to total internal reflec-tion.Our NPL laser boasts several key features,including a high Q of~2610 and a low laser threshold of about 19.8μJ·cm^(−2).The combination of exceptional sensitivity and ultra-small size makes our WGM device an ideal candidate for integration into systems that demand compact temperature sensors.This advancement paves the way for significant prog-ress in the development of ultrasmall temperature sensors,opening new possibilities across various fields.展开更多
目的探讨婴儿色素性神经外胚层瘤(melanotic neuroectodermal tumour of infancy,MNTI)的临床病理特征、诊断及鉴别诊断。方法收集7例MNTI临床资料,行形态学观察、免疫组化、FISH以及电镜检测;分析其临床病理特征并复习相关文献。结果7...目的探讨婴儿色素性神经外胚层瘤(melanotic neuroectodermal tumour of infancy,MNTI)的临床病理特征、诊断及鉴别诊断。方法收集7例MNTI临床资料,行形态学观察、免疫组化、FISH以及电镜检测;分析其临床病理特征并复习相关文献。结果7例MNTI患儿平均发病年龄3个月,发生部位:头面部骨4例,附睾3例。临床表现为迅速增大的无痛性肿块;影像学检查无特异性。6例肿瘤细胞呈腺泡状排列,位于中心的是原始小神经元细胞,部分病例细胞间可见神经纤维丝连接;腺泡周边为富含黑色素颗粒的上皮样细胞,呈卵圆形或梭形,边界不清;1例穿刺组织显示巢、片状排列的原始小细胞,未见黑色素颗粒。免疫表型:原始小神经元细胞表达Syn(7/7)、NSE(4/7)、CD99(4/7)、FLI-1(4/7)、CgA(3/7)和CD56(2/7),富含黑色素的上皮样细胞表达CK(AE1/AE3)(7/7)、HMB-45(7/7)、Melan-A(2/7)和EMA(1/7)。FISH检测:7例均未检测到EWSR1基因断裂。电镜:2例可见细胞连接及黑色素颗粒。5例获得随访资料,随访时间34~56个月,有3例患儿局部复发,5例患儿均无瘤生存。结论MNTI是具有恶性潜能的交界性肿瘤,肿瘤由原始的小神经元细胞及富含黑色素颗粒的上皮样细胞构成,需与腺泡状横纹肌肉瘤、淋巴瘤等鉴别。展开更多
