BACKGROUND Malignant obstructive jaundice is mainly caused by cholangiocarcinoma.Only a few patients are indicated for surgical resection,and the 3-year survival rate is<50%.For patients who are not eligible for surgery,biliary stent placement can relieve biliary obstruction and improve liver function and quality of life.However,restenosis after biliary stents has a poor prognosis and is a clinical challenge.Biliary stent combined with iodine-125(125I)seed implantation can prolong stent patency and improve survival.AIM To evaluate the safety and efficacy of biliary stent combined with 125I seed strand implantation in malignant obstructive jaundice.METHODS We enrolled 67 patients between January 2016 and June 2018 with malignant obstructive jaundice and randomized them into a biliary stent combined with 125I seed strand treatment(combined)group(n=32)and biliary stent(control)group(n=35).All patients underwent enhanced computed tomography and magnetic resonance imaging and were tested for biochemical and cancer markers.Twelve patients underwent pathological examination before surgery.All patients were followed up by telephone or clinical visit.Postoperative liver function improvement,postoperative complications,stent patency time,and survival time were compared between the two groups.Prognostic risk factors were evaluated.RESULTS Technical success was achieved in all patients in both groups.Postoperative liver function improved significantly in all patients(total bilirubin,direct bilirubin,alanine aminotransferase,and aspartate aminotransferase decreased significantly in all patients,the P values were less than 0.05).There was no significant difference in preoperative or postoperative indexes between the two groups for changes in total bilirubin(P=0.147),direct bilirubin(P=0.448),alanine aminotransferase(P=0.120),and aspartate aminotransferase(P=0.387)between the two groups.The median stent patency time of the combined group was 9.0±1.4 mo[95%confidence interval(CI):6.3-11.8 mo],which was significantly longer than the that of the control group(6.0±0.3 mo,95%CI:5.5-6.5 mo,P=0.000).The median survival time of the combined group was 11.0±1.4 mo(95%CI:8.2-13.7 mo),which was significantly longer than that of the control group(7.0±0.3 mo,95%CI:6.4-7.6 mo,P=0.000).Location of obstruction and number of stents were independent risk factors affecting prognosis.CONCLUSION Biliary stent combined with 125I seed strand implantation is safe and effective in malignant obstructive jaundice and improves stent patency time and median survival time. 展开更多

Viral infections have led to many public health crises and pandemics in the last few centuries.Neurotropic virus infection-induced viral encephalitis(VE),especially the symptomatic inflammation of the meninges and brain parenchyma,has attracted growing attention due to its high mortality and disability rates.Understanding the infectious routes of neurotropic viruses and the mechanism underlying the host immune response is critical to reduce viral spread and improve antiviral therapy outcomes.In this review,we summarize the common categories of neurotropic viruses,viral transmission routes in the body,host immune responses,and experimental animal models used for VE study to gain a deeper understanding of recent progress in the pathogenic and immunological mechanisms under neurotropic viral infection.This review should provide valuable resources and perspectives on how to cope with pandemic infections. 展开更多

AIM: To investigate the relationship between semaphorin 7a expression and cell proliferation and migration in pterygium fibroblasts. METHODS: Twenty-six patients with surgically diagnosed pterygium were enrolled, including 15 cases of primary pterygium and 11 cases of recurrent pterygium. In addition, 12 cases of normal conjunctival tissue were collected. The expression of semaphorin 7a in normal conjunctival tissue, primary pterygium and recurrent pterygium was detected by real-time polymerase chain reaction. Recurrent pterygium fibroblasts were isolated and cultured, and the expression of semaphorin 7a was silenced by small interfering RNA(siRNA) interference technique. Furthermore, the effects of si-semaphorin 7a interference on the mRNA and protein levels of β1-integrin, vascular endothelial growth factor A(VEGFA) and vascular endothelial growth factor receptor(VEGFR), and on fibroblast proliferation were analyzed. Transwell assay was used to detect the effect of semaphorin 7a interference on fibroblast migration. RESULTS: Semaphorin 7a was highly expressed in the primary pterygium and recurrent pterygium samples than that of the normal conjunctival tissue. Compared with the primary pterygium, the expression of semaphoring 7a in the recurrent pterygium samples was significantly increased(P<0.05). The mRNA and protein expression levels of β1-integrin, VEGFA and VEGFR were decreased after si-semaphorin 7a transfection, and as well as the cell proliferation and migration. CONCLUSION: Semaphorin 7a might play important roles in the pathogenesis of pterygium by affecting the expression of β1-integrin, VEGFA and VEGFR. 展开更多

AIM: To observe the protective effect of glucagon-like peptide-2(GLP-2) on the intestinal barrier of rats with obstructive jaundice and determine the possible mechanisms of action involved in the protective effect.METHODS: Thirty-six Sprague-Dawley rats were randomly divided into a sham operation group, an obstructive jaundice group, and a GLP-2 group; each group consisted of 12 rats. The GLP-2 group was treated with GLP-2 after the day of surgery, whereas the other two groups were treated with the same concentration of normal saline. Alanine aminotransferase(ALT), total bilirubin, and endotoxin levels were recorded at 1, 3, 7, 10 and 14 d. Furthermore, on the 14 th day, body weight, the wet weight of the small intestine, pathological changes of the small intestine and the immunoglobulin A(Ig A) expressed by plasma cells located in the small intestinal lamina propria were recorded for each group.RESULTS: In the rat model, jaundice was obvious, and the rats' activity decreased 4-6 d post bile duct ligation. Compared with the sham operation group, the obstructive jaundice group displayed increased yellow staining of abdominal visceral serosa, decreased small intestine wet weight, thinning of the intestinal muscle layer and villi, villous atrophy, uneven height, fusion, partial villous epithelial cell shedding, substantial inflammatory cell infiltration and significantly reduced Ig A expression. However, no significant gross changes were noted between the GLP-2 and sham groups. With time, the levels of ALT, endotoxin and bilirubin in the GLP-2 group were significantly increased compared with the sham group(P < 0.01). The increasing levels of the aforementioned markers were more significant in the obstructive jaundice group than in the GLP-2 group(P < 0.01).CONCLUSION: GLP-2 reduces intestinal mucosal injuries in obstructive jaundice rats, which might be attributed to increased intestinal Ig A and reduced bilirubin and endotoxin. 展开更多

Systematic research on the performance of Self-designed plasma reactor with higher power for synthesizing diamond film was carried out. Microwave input power can reach up to 10 kW, the plasma reactor has good adaptability for the deviation of microwave frequency f at 2.45 GHz ± 20 MHz. The secondary plasma could be eliminated by adjusting the working gas pressure and the cylinder adjustment structure,it is helpful to improve the deposition rate with less energy loss. The concentrated and steady plasma could be obtained device with higher gas pressure and relative lower microwave input power. Gas supply modes and inlet gas flow rate were optimized, which would be beneficial to synthesize the film with good quality. 展开更多

BACKGROUND Crouzon syndrome(CS;OMIM 123500)is an autosomal dominant inherited craniofacial disorder caused by mutations in the fibroblast growth factor receptor 2(FGFR2)gene.CS is characterized by craniofacial dysostosis,exophthalmos,and facial anomalies with hypoplastic maxilla and relative mandibular prognathism.CASE SUMMARY Our report involves a 6-year-old fraternal twin boy with many caries in the oral cavity who presented with characteristic features of CS based on clinical and radiographic examinations along with Sanger sequencing.The fraternal girl did not show any abnormalities indicating CS.Carious teeth and poor oral hygiene were managed promptly through administering appropriate behavior guidance,orthodontic treatment was planned,and preventive procedures were described.CONCLUSION CS could occur in a fraternal twin caused by a de novo mutation of the FGFR2 gene.Oral hygiene instruction,preventive programs on oral hygiene,orthodontic treatment,and maxillary osteotomy were required for treatment. 展开更多

Four blue-violet light emitting InGaN/GaN multiple quantum well(MQW) structures with different well widths are grown by metal–organic chemical vapor deposition. The carrier localization effect in these samples is investigated mainly by temperature-dependent photoluminescence measurements. It is found that the localization effect is enhanced as the well width increases from 1.8 nm to 3.6 nm in our experiments. The temperature induced PL peak blueshift and linewidth variation increase with increasing well width, implying that a greater amplitude of potential fluctuation as well as more localization states exist in wider wells. In addition, it is noted that the broadening of the PL spectra always occurs mainly on the low-energy side of the PL spectra due to the temperature-induced band-gap shrinkage, while in the case of the widest well, a large extension of the spectral curve also occurs in the high energy sides due to the existence of more shallow localized centers. 展开更多

Objective:To observe the influence of ganglioside on the blood gas analysis and serum levels of inflammatory cytokines in newborns with anoxic ischemic encephalopathy.Method: A total of 100 newborns with anoxic ischemic encephalopathy in our hospital were selected and randomly divided into 2 groups: the control group and the observation group. Conventional oxygen inhalation, reducing intracranial pressure, controlling eclampsia and neurotrophic drug treatment were given to the observation group. Treatment of ganglioside was given to the control group on the basis of observation group. Blood gas analysis and serum levels of inflammatory cytokines were detected before treatment (T0), 3 d after treatment (T1), and 7 d after treatment (T2).Result: (1) The comparison of pH, PaO2, PaCO2, SaO2 in the two groups in T0 was not statistically significant. The comparison of pH, PaO2, PaCO2, SaO2 in T0, T1, T2 was considered to be statistically significant. Among these, the result of comparision of pH, PaO2, SaO2: T0<T1<T2. comparision of PaCO2: T0>T1>T2. The pH, PaO2, SaO2 in observation group were higher, PaCO2 in observation group was lower compared with that in control group in T1 and T2. The difference was considered to be statistically significant. (2) The comparision of IL-2, IL-6, hs-CRP, TNF-α in the two groups in T0 was not statistically significant. IL-2 in the observation in T1 and T2 was higher than that in the control group, IL-6, hs-CRP, TNF-α in the observation in T1 and T2 was lower than that in the control group. The difference was considered to be statistically significant.Conclusion: Ganglioside can improve blood gas analysis indexes, decrease the serum inflammatory cytokines in newborns with anoxic ischemic encephalopathy. 展开更多

Cs and I can migrate through fuel-cladding interfaces and accelerate the cladding corrosion process induced by the fuel-cladding chemical interaction.Cr coating has emerged as an important candidate for mitigating this chemical interaction.In this study,first-principles calculations were employed to investigate the diffusion behavior of Cs and I in the Cr bulk and grain boundaries to reveal the microscopic interaction mitigation mechanisms at the fuel-cladding interface.The interaction between these two fission products and the Cr coating were studied systematically,and the Cs and I temperature-dependent diffusion coefficients in Cr were obtained using Bocquet’s oversized solute-atom model and Le Claire’s nine-frequency model,respectively.The results showed that the Cs and I migration barriers were significantly lower than that of Cr,and the Cs and I diffusion coefficients were more than three orders of magnitude larger than the Cr self-diffusion coefficient within the temperature range of Generation-IV fast reactors(below 1000 K),demonstrating the strong penetration ability of Cs and I.Furthermore,Cs and I are more likely to diffuse along the grain boundary because of the generally low migration barriers,indicating that the grain boundary serves as a fast diffusion channel for Cs and I. 展开更多

Evaluation of elastography combined with serological indexes for hepatic fibrosis in patients with chronic hepatitis B 被引量:23

AIM To investigate the value of ultrasound elastography combined with serological indexes in diagnosing liver fibrosis and assessing its severity.METHODS A total of 338 chronic hepatitis B(CHB) patients were divided into a disease group(patients with hepatic fibrosis) and control group(subjects without hepatic fibrosis). The disease group was further divided into S1-S4 according to the degree of fibrosis. Independent risk factors for hepatic fibrosis were analyzed using multivariate logistic regression. The diagnostic values of hepatic fibrosis from different indicators were compared using receiver operating characteristic(ROC) curves. The combination of elastography and serological indexes was explored to assess the severity of hepatic fibrosis.RESULTS The multivariate logistic regression analysis results revealed that shear wave velocity(SWV), hyaluronic acid(HA), type Ⅳ collagen(CⅣ) and aspartate aminotransferase-to-platelet ratio index(APRI) significantly affected the occurrence of hepatic fibrosis. The ROC curve revealed that the accuracy of the diagnosis of hepatic fibrosis for SWV and HA were 87.3% and 84.8%, respectively. The accuracy of SWV combined with HA was 88.9%. The multiple linear regression analysis revealed that SWV, aspartate aminotransferase(AST)/alanine aminotransferase(ALT), HA, CⅣ, APRI and fibrosis index based on the 4 factor(FIB-4) were screened as statistically significant independent factors. The established regression equation was: Fibrosis level =-4.046 + 1.024 × SWV + 1.170 × AST/ALT + 0.011 × HA + 0.020 × CⅣ + 0.719 × APRI + 0.379 × FIB-4. CONCLUSION SWV combined with serological indexes can improve the accuracy of diagnosis for CHB hepatic fibrosis. Serum indexes can help diagnose the degree of hepatic fibrosis. 展开更多

AIM:To investigate the prevalence and physicians' detection rate of depressive and anxiety disorders in gastrointestinal(GI)outpatients across China. METHODS:A hospital-based cross-sectional survey was conducted in the GI outpatient departments of 13 general hospitals.A total of 1995 GI outpatients were recruited and screened with the Hospital Anxiety and Depression Scale(HADS).The physicians of the GI departments performed routine clinical diagnosis and management without knowing the HADS score results.Subjects with HADS scores≥8 were subsequently interviewed by psychiatrists using the Mini International Neuropsy-chiatric Interview(MINI)to make further diagnoses. RESULTS:There were 1059 patients with HADS score ≥8 and 674(63.64%)of them undertook the MINI interview by psychiatrists.Based on the criteria of Diagnostic and Statistical Manual of Mental Disorders (4th edition),the adjusted current prevalence for depressive disorders,anxiety disorders,and comorbidity of both disorders in the GI outpatients was 14.39%, 9.42%and 4.66%,respectively.Prevalence of depressive disorders with suicidal problems[suicide attempt or suicide-related ideation prior or current;module C (suicide)of MINI score≥1]was 5.84%in women and 1.64%in men.The GI physicians'detection rate of depressive and anxiety disorders accounted for 4.14%. CONCLUSION:While the prevalence of depressive and anxiety disorders is high in Chinese GI outpatients, the detection rate of depressive and anxiety disorders by physicians is low. 展开更多

目的:探讨糖尿病黄斑水肿(diabetic macular edema,DME)患者视力与椭圆体带完整性的相关性及视力的相关影响因素。方法:纳入糖尿病视网膜病变(diabetic retinopathy,DR)合并DME患者39例62眼,根据频域光学相干断层扫描(spectral domain o... 目的:探讨糖尿病黄斑水肿(diabetic macular edema,DME)患者视力与椭圆体带完整性的相关性及视力的相关影响因素。方法:纳入糖尿病视网膜病变(diabetic retinopathy,DR)合并DME患者39例62眼,根据频域光学相干断层扫描(spectral domain optical coherence tomography,SD-OCT)技术测量的椭圆体带破坏程度进行分组:A组椭圆体带完整连续,B组椭圆体带缺损长度≤200μm,C组椭圆体带缺损长度>200μm。测量三组患者的最佳矫正视力(best corrected visual acuity,BCVA)和黄斑中心凹视网膜厚度(central macular thickness,CMT),观察是否存在浆液性视网膜脱离(serous retinal detachment,SRD)和硬性渗出(hard exudates,HE)等。采用Pearson相关法分析年龄、性别、糖尿病病程、HbA1c、DME病程和类型、DR分期、椭圆体带缺损长度、是否存在SRD和HE等因素与BCVA的相关性。结果:治疗前三组患者BCVA分别为0.44±0.18、0.64±0.16、0.74±0.21,差异有统计学意义(P<0.001)。三组患者CMT分别为403.40±90.32、408.44±95.98、421.29±98.32μm,差异无统计学意义(P=0.805)。Pearson相关分析提示,椭圆体带缺损长度、CMT均与BCVA呈负相关(r=-0.673、-0.344,均P<0.001),且椭圆体带缺损长度与BCVA呈强相关,CMT与BCVA呈弱相关,其它因素如SRD、HE、DME病程等均与BCVA无明显相关性。结论:椭圆体带的完整性与BCVA存在较强的相关性,CMT与BCVA存在弱相关性。 展开更多

Molecularly imprinted polymers for dimethoate recognition were synthesized by the precipitation polymerization technique using methyl methacrylate (MMA) as the functional monomer and ethylene glycol dimethacrylate (EGDMA) as the cross-linker. The morphology, adsorption and recognition properties were investigated by scanning electron microscopy (SEM), static adsorption test, and competitive adsorption test. To obtain the best selectivity and binding performance, the synthesis and adsorption conditions of MIPs were optimized through single factor experiments. Under the optimized conditions, the resultant polymers exhibited uniform size, satisfactory binding capacity and significant selectivity. Furthermore, the imprinted polymers were successfully applied as a specific solid-phase extractants combined with high performance liquid chromatography (HPLC) for determination of dimethoate residues in the cucumber samples. The average recoveries of three spiked samples ranged from 78.5% to 87.9% with the relative standard deviations (RSDs) less than 4.4% and the limit of detection (LOD) obtained for dimethoate as low as 2.3 μg/mL. 展开更多

Dynamic self-heating effect(SHE)of silicon-on-insulator(SOI)MOSFET is comprehensively evaluated by ultrafast pulsed I-V measurement in this work.It is found for the first time that the SHE complete heating response and cooling response of SOI MOSFETs are conjugated,with two-stage curves shown.We establish the effective thermal transient response model with stage superposition corresponding to the heating process.The systematic study of SHE dependence on workload shows that frequency and duty cycle have more significant effect on SHE in first-stage heating process than in the second stage.In the first-stage heating process,the peak lattice temperature and current oscillation amplitude decrease by more than 25 K and 4%with frequency increasing to 10 MHz,and when duty cycle is reduced to 25%,the peak lattice temperature drops to 306 K and current oscillation amplitude decreases to 0.77%.Finally,the investigation of two-stage(heating and cooling)process provides a guideline for the unified optimization of dynamic SHE in terms of workload.As the operating frequency is raised to GHz,the peak temperature depends on duty cycle,and self-heating oscillation is completely suppressed. 展开更多

This study aimed to provide a comprehensive overview of the pigment and non-pigment phenolic profile of polyphenol extracts of jujube peel and their bioactivities.Firstly,the pigment and non-pigment phenolic composition as well as antioxidant activities of free and bound polyphenol extracts in jujube peel were compared among twelve varieties systematically.Then the antioxidant and lipid-lowering activities of free and bound polyphenol extracts of‘Junzao’jujube peel were investigated representatively.Overall,23 phenolic compounds were identified,in which flavonols were the main pigments and all varieties showed similar qualitative patterns but different quantitative characteristics.The antioxidant activities significantly correlated with the content of total phenolics and total flavonoids.Furtherly,both free and bound polyphenol extracts of‘Junzao’jujube peel exhibited DPPH,ABTS free radical scavenging capacity,and oxygen radical absorbance capacity in a dose-dependent manner.Besides,both of them improved the resistance of Caenorhabditis elegans under H2O2 oxidative stress and heat stress at 37◦C(20-100μg/mL).What’s more,compared to free polyphenol extract,bound polyphenol extract decreased the level of body lipid accumulation and triglyceride more effectively.These results suggest that jujube peel is an exploitable source of natural flavonoid pigments and phenolic ingredients with strong antioxidant and lipid-lowering properties. 展开更多

Background:Scrub typhus is a life-threatening disease caused by O rientia tsu tsu gom u sh i,and specific antimicrobial medicine is available.Early and accurate diagnosis is essential for reducing the risk of severe complications and death.In this study,we aimed to evaluate the case diagnosis situation among medical care institutions and geographical regions in China,and the results will benefit both clinical practice and the disease surveillance system.Methods:We extracted individual scrub typhus case data 2006-2016 from a national disease surveillance system in China.The diagnosis category and interval time from illness onset to diagnosis were compared among three levels of medical care institutions and provinces.The descriptive analysis method was performed in our study.Results:During the 11-year study period,93481 scrub typhus cases,including 57 deaths,were recorded in the nationwide surveillance system.The overall proportion of laboratory-confirmed cases was only 4.7%,and this proportion varied greatly among primary medical centres(2.8%),county level hospitals(4.2%),and city level hospitals(6.3%).Notably,the proportion of laboratory-confirmed cases has consistently decreased from 16.3%in 2006 to 2.6%in 2016,and the same decreasing trend was found among all three levels of medical care institutions.The interval from illness onset to case diagnosis(Tdiag)for all cases was 5 days(interquartile range[IQR]:2-9 days)and decreased from 7 days(IQR:3-11 days)in 2006 to 5 days(IQR:2-8 days)in 2016.The risk of death for patients with a Tdiag of>7 days was 2.2 times higher(OR=22),(95%CI:1.05-5.21)than that of patients with a Tdiag of<2 days.Conclusions:The interval time from illness onset to diagnosis for scrub typhus cases decreased greatly in China;however,the diagnosis rate of cases with laboratory-confirmed results must be increased among all levels of medical care institutions to reduce both the risk of death and the misuse of antibiotics associated with scrub typhus. 展开更多

Poly(propylene carbonate) (PPC) is gaining significant momentum as a biodegradable polymer using CO_(2) as an abundant, low-cost and renewable feedstock. Yet, PPC suffers from unsatisfactory thermal and mechanical properties, and its blends with other synthetic polymers as the second component typically exhibit poor compatibility. Herein, novel furan-based non-isocyanate polyurethanes (NIPUs) with high molecular weights and narrow distributions have been synthesized starting from 5-hydroxymethylfurfural (HMF) via a four-step procedure including reduction, glycidylation, carbonation of CO_(2) and step-growth polymerization with diamines. Owing to the structural similarity and intermolecular hydrogen bonding interactions, furan-based NIPUs display good compatibility with PPC. A sea-island morphology can be observed in SEM images of PPC/NIPU blends, where the NIPU phases are randomly dispersed in the PPC matrix. Moreover, the blends exhibit only one Tg (higher than both that of PPC and the corresponding NIPU) on the DSC curves, which first increase and then decrease when increasing the content of NIPUs from 0 wt% to 15 wt%. Notably, the physical crosslinking formed by hydrogen bonding interactions, coupled with the rigidity of furan rings, confer simultaneous improvements of elongation at the break and tensile strength, affording high-performance CO_(2)-based biodegradable polymeric materials with low carbon footprint, good sustainability, and ideal biocompatibility. 展开更多
