Therapeutic Effectiveness of Leukocyte-and Platelet-rich Fibrin for Diabetic Foot Ulcers:A Retrospective Study

Objective Diabetic foot ulcer(DFU)is one of the most serious complications of diabetes.Leukocyte-and platelet-rich fibrin(L-PRF)is a second-generation autologous platelet-rich plasma.This study aims to investigate the clinical effects of L-PRF in patients with diabetes in real clinical practice.Methods Patients with DFU who received L-PRF treatment and standard of care(SOC)from 2018 to 2019 in Tongji Hospital were enrolled.The clinical information including patient characteristics,wound evaluation(area,severity,infection,blood supply),SOC of DFU,and images of ulcers was retrospectively extracted and analyzed.L-PRF treatment was performed every 7±2 days until the ulcer exhibited complete epithelialization or an overall percent volume reduction(PVR)greater than 80%.Therapeutic effectiveness,including overall PVR and the overall and weekly healing rates,was evaluated.Results Totally,26 patients with DFU were enrolled,and they had an ulcer duration of 47.0(35.0,72.3)days.The severity and infection of ulcers varied,as indicated by the Site,Ischemia,Neuropathy,Bacterial Infection,and Depth(SINBAD)scores of 2–6,Wagner grades of 1–4,and the Perfusion,Extent,Depth,Infection and Sensation(PEDIS)scores of 2–4.The initial ulcer volume before L-PRF treatment was 4.94(1.50,13.83)cm3,and the final ulcer volume was 0.35(0.03,1.76)cm3.The median number of L-PRF doses was 3(2,5).A total of 11 patients achieved complete epithelialization after the fifth week of treatment,and 19 patients achieved at least an 80%volume reduction after the seventh week.The overall wound-healing rate was 1.47(0.63,3.29)cm3/week,and the healing rate was faster in the first 2 weeks than in the remaining weeks.Concurrent treatment did not change the percentage of complete epithelialization or healing rate.Conclusion Adding L-PRF to SOC significantly improved wound healing in patients with DFU independent of the ankle brachial index,SINBAD score,or Wagner grade,indicating that this method is appropriate for DFU treatment under different clinical conditions. 展开更多

BACKGROUND Postoperative pancreatic fistula(POPF)contributes significantly to morbidity and mortality after pancreaticoduodenectomy(PD).However,the underlying mechanisms remain unclear.This study explored this pathology in the pancreatic stumps and elucidated the mechanisms of POPF following PD.CASE SUMMARY Pathological analysis and 16S rRNA gene sequencing were performed on specimens obtained from two patients who underwent complete pancreatectomy for grade C POPF after PD.Gradient inflammation is present in the pancreatic stump.The apoptosis was lower than that in the normal pancreas.Moreover,neu-trophildominated inflammatory cells are concentrated in the ductal system.No-tably,neutrophils migrated through the ductal wall in acinar duct metaplasia-formed ducts.Additionally,evidence indicates that gut microbes migrate from the digestive tract.Gradient inflammation occurs in pancreatic stumps after PD.CONCLUSION The mechanisms underlying POPF include high biochemical activity in the pancreas,mechanical injury,and digestive reflux.To prevent POPF and address pancreatic inflammation and reflux,breaking the link with anastomotic dehi-scence is practical. 展开更多

目的:比较阿托品凝胶、盐酸环喷托酯和复方托吡卡胺在近视中小学生睫状肌麻痹验光中的效果,为科学验光和准确矫正提供理论依据。方法:选取2017-07/08在我院经小瞳验光诊断为近视的中小学生420例818眼,按年龄分为3组,分别采用阿托品凝胶... 目的:比较阿托品凝胶、盐酸环喷托酯和复方托吡卡胺在近视中小学生睫状肌麻痹验光中的效果,为科学验光和准确矫正提供理论依据。方法:选取2017-07/08在我院经小瞳验光诊断为近视的中小学生420例818眼,按年龄分为3组,分别采用阿托品凝胶、盐酸环喷托酯和复方托吡卡胺进行睫状肌麻痹验光(散瞳验光)及小瞳复光。结果:阿托品组、盐酸环喷托酯组、复方托吡卡胺组睫状肌麻痹验光与小瞳复光等效球镜符合率分别是:81.0%、81.3%和79.4%;睫状肌麻痹验光与小瞳复光等效球镜之差阿托品组为-0.113±0.226D,差异有统计学意义(t=-4.663,P<0.001);盐酸环喷托酯组为-0.025±0.192D,复方托吡卡胺组为-0.026±0.193D,差异均无统计学意义(t=-1.665,P=0.099;t=1.760,P=0.080)。结论:对>8岁的近视中小学生,首次睫状肌麻痹验光可采用快速散瞳,以减少对学习生活的影响;快速散瞳者复光配镜时可按小瞳结果直接给予处方,阿托品散瞳者小瞳复光近视屈光度高于散瞳时,配镜时需参考散瞳结果,选择最佳矫正视力的最低负镜度,避免近视过矫。 展开更多

目的:探讨姜黄素对体外培养的人翼状胬肉成纤维(HPF)细胞增殖和凋亡及迁移的影响。方法:收集我院2021-11-24/12-16手术切除的翼状胬肉组织7例,进行原代成纤维细胞体外培养,并采用免疫荧光染色法进行细胞鉴定。采用含等量二甲基亚砜的0... 目的:探讨姜黄素对体外培养的人翼状胬肉成纤维(HPF)细胞增殖和凋亡及迁移的影响。方法:收集我院2021-11-24/12-16手术切除的翼状胬肉组织7例,进行原代成纤维细胞体外培养,并采用免疫荧光染色法进行细胞鉴定。采用含等量二甲基亚砜的0、10、20、40、80、160μmol/L姜黄素处理HPF细胞24h后,用CCK8法检测细胞增殖情况。根据CCK8检测结果将细胞分为对照组(不含姜黄素)、20μmol/L姜黄素组、40μmol/L姜黄素组,每组分别处理HPF细胞24h。采用流式细胞仪检测细胞凋亡,Transwell迁移实验检测细胞迁移,实时荧光定量PCR法和Western blot法检测Bax、Bcl-2、Cyclin D1及MMP2的mRNA与蛋白表达。结果:与对照组相比,20μmol/L姜黄素组和40μmol/L姜黄素组均能抑制HPF细胞的增殖和迁移,并诱导其凋亡(均P<0.05)。与对照组相比,20μmol/L姜黄素组能下调Cyclin D1及MMP2,上调Bax的mRNA表达,下调Bcl-2的蛋白表达(均P<0.05)。与对照组相比,40μmol/L姜黄素组能下调Bcl-2、Cyclin D1及MMP2的mRNA与蛋白表达,上调Bax的mRNA与蛋白表达(均P<0.05)。与20μmol/L姜黄素组相比,40μmol/L姜黄素组能下调MMP2的mRNA表达,下调Bcl-2的蛋白表达,上调Bax的mRNA与蛋白表达(均P<0.05)。结论:姜黄素可通过抑制Cyclin D1的表达抑制HPF细胞的增殖,通过下调Bcl-2并上调Bax的表达来诱导HPF细胞的凋亡,通过下调MMP2的表达来抑制HPF细胞的迁移。 展开更多

Acacetin(5,7-dihydroxy-4′-methoxyflavone), a potential neuroprotective agent, has an inhibitory effect on lipopolysaccharide-induced neuroinflammatory reactions. However, whether acacetin has an effect on inflammatory corpuscle 3(NLRP3) after cerebral ischemia-reperfusion injury has not been fully determined. This study used an improved suture method to establish a cerebral ischemia-reperfusion injury model in C57BL/6 mice. After ischemia with middle cerebral artery occlusion for 1 hour, reperfusion with intraperitoneal injection of 25 mg/kg of acacetin(acacetin group) or an equal volume of saline(0.1 mL/10 g, middle cerebral artery occlusion group) was used to investigate the effect of acacetin on cerebral ischemia-reperfusion injury. Infarct volume and neurological function scores were determined by 2,3,5-triphenyltetrazolium chloride staining and the Zea-Longa scoring method. Compared with the middle cerebral artery occlusion group, neurological function scores and cerebral infarction volumes were significantly reduced in the acacetin group. To understand the effect of acacetin on microglia-mediated inflammatory response after cerebral ischemia-reperfusion injury, immunohistochemistry for the microglia marker calcium adapter protein ionized calcium-binding adaptor molecule 1(Iba1) was examined in the hippocampus of ischemic brain tissue. In addition, tumor necrosis factor-α, interleukin-1β, and interleukin-6 expression in ischemic brain tissue of mice was quantified by enzyme-linked immunosorbent assay. Expression of Iba1, tumor necrosis factor-α, interleukin-1β and interleukin-6 was significantly lower in the acacetin group compared with the middle cerebral artery occlusion group. Western blot assay results showed that expression of Toll-like receptor 4, nuclear factor kappa B, NLRP3, procaspase-1, caspase-1, pro-interleukin-1β, and interleukin-1β were significantly lower in the acacetin group compared with the middle cerebral artery occlusion group. Our findings indicate that acacetin has a protective effect on cerebral ischemia-reperfusion injury, and its mechanism of action is associated with inhibition of microglia-mediated inflammation and the NLRP3 signaling pathway. 展开更多

Objective:To To investigate the effect of acupuncture on the tumor necrosis factor- α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP),nitric oxide synthase(NOS) content and muscular tension of spasticity cerebral palsy rat model.Methods:The rats with spastic cerebral palsy were randomly divided into the control group,model group and acupuncture group.After successful modeling,the muscular tension and the content of TNF- α,IL-6,CRP.NOS were measured.Results:The serum TNF- α,IL—6,CRP,NOS content were significantly decreased in the acupuncture group(P<0.05).The low and high shear viscosity of whole blood of the acupuncture group were significantly lower than the control group and the model group(P<0.05).The erythrocyte electrophoresis indexes in the acupuncture group were significantly lower than that in the model group and the control group(P<0.05).Acupuncture significantly reduced the muscular tension of spastic cerebral palsy rat and increased the active extent in the paralytie extremity(P<0.05),but it could not be restored to normal level.Compared with the control group,the difference had significant(P<0.05).Conclusions:Acupuncture treatment can inhibit the release of inflammatory cells after brain injury,then reduce immune injury,relieve muscle spasms and reduce muscular tension. 展开更多

AIM:To reconstruct the lamellar cornea using human amniotic epithelial(HAE) cells and rabbit cornea stroma in vitro using tissue engineering technology.·METHODS:Human amnia taken from uncomplicated caesarean sections were digested by collagenase to obtain HAE cells,and the cells were cultured to proliferate.Rabbit corneal epithelial cells were removed by n-heptanol to make lamellar matrix sheets.The second passage of HAE cells were cultured on the corneal stroma sheets for 1 or 2 days,then transferred to an air-liquid interface environment to culture for 2weeks.Tissue engineered lamellar cornea(TELC)morphology was observed by Hematoxylin-eosin(HE)staining;its ultrastructure was observed by transmission electron microscopy(TEM) and scanning electron microscopy(SEM);corneal epithelial cell-specific keratin3 and keratin 12 were detected with immunofluorescence microscopy.·RESULTS:HAE cells grew on the rabbit corneal stroma,forming a monolayer after 1-2 days.About 4-5 layers of epithelial cells developed after 2 weeks of air-liquid interface cultivation,a result similar to normal corneal epithelium.Rabbit corneal stromal cells were significantly reduced after one week,then almost completely disappeared after 2 weeks.TEM showed desmosomes between the epithelial cells;hemidesmosomes formed between the epithelial cells and the basement membrane.SEM revealed that the HAE cells which grew on the lamellar cornea had abundant microvilli.The tissue-engineered cornea expressed keratin 3 and keratin 12,as detected by immunofluorescence assay.·CONCLUSION:Functional tissue-engineered lamellar corneal grafts can be constructed in vitro using HAE cells and rabbit corneal stroma. 展开更多

In contrast to conventional gas-bearing rocks, gas shale has extremely low permeability due to its nano- scale pore networks. Organic matter which is dispersed in the shale matrix makes gas flow characteristics more complex. The traditional Darcy's law is unable to estimate matrix permeability due to the particular flow mechanisms of shale gas. Transport mechanisms and influence factors are studied to describe gas transport in extremely tight shale. Then Lattice Boltzmann simulation is used to establish a way to estimate the matrix permeability numerically. The results show that net desorption, diffu- sion, and slip flow are very sensitive to the pore scale. Pore pressure also plays an important role in mass fluxes of gas. Temperature variations only cause small changes in mass fluxes. The Lattice Boltzmann method can be used to study the flow field in the micropore spaces and then provides numerical solutions even in complex pore structure models. Understanding the transport characteristics and establishing a way to estimate potential gas flow is very important to guide shale gas t'eserve estimation and recovery schemes. 展开更多

Objective Thyroid-associated ophthalmopathy(TAO)is an autoimmune disorder involving the orbital tissue.This study aimed to understand the role of regulatory T cells(Tregs)in TAO during 12-week systemic glucocorticoid(GC)treatment.Methods Thirty-two moderate-severe TAO patients with a clinical activity score(CAS)≥3/7 or with prolonged T2 relaxation time(T2RT)on at least one side of extraocular muscle(EOM)were enrolled.The percentage of the peripheral CD4+CD25(high)CD127(−/low)Tregs was analyzed using flow cytometry before and after the GC treatment.The activity and severity of TAO,T2RT,and the clinical outcomes after the GC treatment were assessed.Their correlation with the peripheral Tregs was investigated.Results There was no significant association between the baseline Treg fraction and the activity and severity of TAO or the treatment response.A significant reduction of Tregs was observed after the GC therapy merely in patients without any clinical improvement.Conclusion Treg reduction after systemic GC therapy is indicative of a poor therapeutic response.Accordingly,dynamic alterations of Tregs could help to evaluate the effectiveness of the GC treatment. 展开更多

·AIM: To explore the feasibility that human amniotic epithelial cells (hAECs) have the potential to differentiate into corneal epithelial -like cells under the microenvironment replicated by spontaneously immorta... ·AIM: To explore the feasibility that human amniotic epithelial cells (hAECs) have the potential to differentiate into corneal epithelial -like cells under the microenvironment replicated by spontaneously immortalized human corneal epithelial cells (S-ihCECs). ·METHODS: hAECs were isolated by enzyme digestion, and flow cytometry was used to analysis the expression of CD29/90/166/73/34 and HLA -DR. Recovered and cultured S -ihCECs, immunocytochemistry was used to detect the expression of CK3/12. The proliferation of S - ihCECs handled by different concentrations of mitomycin was detected by CCK -8. The proliferation of hAECs cultured by S-ihCECs culture media collected at different time was analyzed by CCK -8. After filtered out the optimal conditions, we collected S-ihCECs culture media for 5 days, then prepared conditioned medium to incubate hAECs, inverted phase contrast microscope and scanning electron microscope were used to observe the change of morphology in hAECs. Quantitative real -time reverse transcription -polymerase chain reaction (QRT - PCR) was carried out to evaluate the expression of Oct - 4, NANOG, PAX6, and CK12 in the differentiation period. Immunocytochemistry and western bloting were used to detect the expression of CK3/12. ·RESULTS: The culture media collected every 12h, from 20μg/mL mitomycin pretreatment S -ihCECs could significantly promote the proliferation of hAECs. In the period of differentiation, the morphology of differentiated hAECs was obviously different compared with the control group, and the distinctive CK3/12 for corneal epithelial cells was detected.·CONCLUSION: This study showed that hAECs can differentiate into corneal epithelial -like cells by replication of the corneal epithelial microenvironment, using the culture media collected from S -ihCECs, and it is possible that S -ihCECs culture media could be used in corneal tissue engineering. · 展开更多

Mixed bacteria were enriched from heavy metals mine soil for cadmium(Cd(Ⅱ))-containing wastewater treatment. Batch adsorption experiment results showed that the optimal pH, temperature, initial Cd(Ⅱ) concentration, and biomass dosage were 6.0, 30 ℃, 20 mg/L, and 1 g/L, respectively. Living biomass exhibited better Cd(Ⅱ) removal efficiency(91.97%) than autoclaved biomass(79.54%) under optimal conditions. The isotherms and kinetics of living biomass conformed to the Langmuir isotherm model and pseudo-first-order kinetic model, respectively. FTIR results implied that amine groups, hydroxyl groups and phosphoric acid play an important role in the Cd(Ⅱ) adsorption process, while XRD results showed that crystalline Cd(OH)and CdO were obtained. After Cd(Ⅱ)-containing wastewater treatment exposure, the dominant bacteria genera included Comamonas(39.94%), unclassified_f__Enterobacteriaceae(34.96%), Ochrobactrum(14.07%), Alcaligenes(4.84%), Bordetella(2.07%), Serratia(1.04%), and Bacillus(1.01%). Function prediction showed that the abundance of metabolic genes changed significantly. This study proposes the potential application of mixed bacteria for Cd(Ⅱ)-containing wastewater treatment. 展开更多

BACKGROUND Malignant adenomyoepithelioma(AME)of the breast is a rare tumor in which malignancy can arise from either epithelial or myoepithelial components,or from both cell types.The incidence and prognosis of malignant AME of the breast are difficult to assess due to its rarity.Therefore,the optimal treatment for this disease is still controversial.CASE SUMMARY We present two middle-aged women(48 and 56 years old)with malignant AME of the breast.Core needle biopsy was performed before surgery.However,breast adenoma and malignant tumors were observed.The preoperative diagnosis of malignant AME of the breast is still challenging for pathologists and clinicians.Both patients underwent mastectomy and sentinel lymph node biopsy,both of which were negative,followed by adjuvant chemotherapy.CONCLUSION The follow-up duration of the two patients was two years and four months,respectively.No signs of relapse or metastasis have been observed thus far. 展开更多

20C,a bibenzyl compound isolated from Gastrodia elata,possesses antioxidative properties in PC12 cells,but its in-depth molecular mechanisms against rotenone-induced neurotoxicity remains unknown.Recent studies indica... 20C,a bibenzyl compound isolated from Gastrodia elata,possesses antioxidative properties in PC12 cells,but its in-depth molecular mechanisms against rotenone-induced neurotoxicity remains unknown.Recent studies indicate that without intact DJ-1,nuclear factor erythroid 2-related factor(Nrf2)protein becomes unstable,and the activity of Nrf2-mediated downstream antioxidant enzymes are thereby suppressed.Therefore,increasing the nuclear translocation of Nrf2 by DJ-1 may present a helpful means for the prevention and treatment of chronic diseases related to oxidative stress.Our results showed that 20C clearly protected PC12 and SH-SY5Y cells against rotenone-induced oxidative injury in a concentration-dependent manner.Furthermore,20C markedly up-regulated the levels of DJ-1,which in turn activated phosphoinositide-3-kinase(PI3K)/Akt signaling and inhibited glycogen synthase kinase 3β(GSK3β)activation,eventually promoting Nrf2 nuclear translocation and inducing the expression of Nrf2-mediated downstream antioxidative enzymes such as HO-1.The antioxidative effects of 20C could be partially blocked by ShR NA-mediated knockdown of DJ-1 and inhibition of the PI3K/Akt pathways with Akt1/2 kinase inhibitor in PC12 and SH-SY5Y cells,respectively.Conclusively,our findings confirm that DJ-1 is necessary for 20C-mediated protection against rotenone-induced oxidative damage,at least in part,by activating PI3K/Akt signaling,and subsequently enhancing the nuclear accumulation of Nrf2.The findings from our investigation suggest that 20C should be developed as a novel candidate for preventing or alleviating the consequences of PD in the future. 展开更多

AIM: To systematically compare the efficacy and safety of off-label bevacizumab versus licensed ranibizumab intravitreal injections as well as monthly regimen versus pro re nata [PRN(as needed)] regimen in the treatment of neovascular age-related macular degeneration(n AMD).METHODS: Relevant publications were identified through automatically retrieve of database and manually retrieving. The methodological quality of studies included was assessed using the Jadad score and the risk-of-bias assessment. The efficacy estimates were measured by the weight mean difference(WMD) for the improvement of best-corrected visual acuity(BCVA) and central retinal thickness(CRT) reduction. The safety estimates were measured by odds ratios(OR) for adverse events rates. Statistical analysis was conducted by Revman 5.2.7.RESULTS: Seven studies were included in the Metaanalysis. There were no statistically significant differences between bevacizumab and ranibizumab in BCVA at 1 and 2y(P =0.37, P =0.18, respectively),However, both drugs has better BCVA given monthly than given as needed at 1 and 2y(P 【0.05). The results demonstrated the mean decrease in CRT was less in bevacizumab group than ranibizumab group at 1y(P 【0.05),while the difference was not significant at 2y(P =0.24).Treatment monthly gained much more decrease in CRT at 1 and 2y(P 【0.005).There were no differences between drugs in the rates of death, arterial thrombotic events and venous thrombotic events(P =0.41, P =0.55, P =0.10,respectively), while the rates of medical dictionary for regulatory activities(Med DAR) system organ class events and ≥1 systemic serious adverse events were higher in bevacizumab group than ranibizumab group(P 【0.05).But the incidences of death, arterial thrombotic events,venous thrombotic events, Med DAR system organ class events as well as ≥1 systemic serious adverse events were not statistically different between both treatment regimens of monthly and as needed(P =0.14, P =0.76,P =0.73, P =0.12, P =0.11, respectively).· CONCLUSION: Bevacizumab was equivalent to ranibizumab for BCVA, however bevacizumab tended to gain less decrease in CRT and had higher rates of serious adverse events. Compared with treatment as needed, treatment monthly showed superior efficacy in BCVA improvement and CRT reduction, while the rates of adverse events were similar in the two dosing regimens. 展开更多

Fungicolous fungi are a very large,diverse,ecological and trophic group of organisms that are associated with other fungi.This association occurs with species of different lineages across the fungal kingdom.They are recognized as symbionts,mycoparasites,saprotrophs,and even neutrals.Wherever fungi have been found,fungicolous taxa have also been found.Homogeneous environments favour the development of highly adapted and coevolved fungicolous species,which could have led to host-specificity aspects.As a primary consumer,fungicolous fungi decrease the turnaround time of certain nutrients in food webs,due to their special often-rapid life cycles.They may also significantly affect population dynamics and population sizes of their hosts in aquatic or terrestrial ecosystems.As mycoparasites of pathogenic fungi,some fungicolous fungi have been explored as biocontrol agents.They may also cause serious diseases of cultivated edible and medicinal mushrooms,decreasing both yield and quality.Fungicolous fungi could be used as model organisms that may help determine better understanding of species interactions,fungal evolution and divergence,and fungicolous mechanisms.This review summarizes our current understanding of fungicolous fungi,with a particular focus on the terminology,diversity,global distribution,and interaction with their hosts.We also provide a checklist including 1552 fungicolous fungal taxa so far recorded following the updated classification schemes.There is a need for further investigations on this ecologically important group of fungi to better understand their biology,ecological aspects,origin and divergence,hostspecificity and application in biocontrol.Accurate identification of these fungi as pathogens and their significance in quarantine purposes on the mushroom industry need further evaluations so that efficient control measures can be developed for better disease management purposes. 展开更多

The emissions trading scheme(ETS)is considered an effective policy tool for achieving emission reductions.At present,China's ETS pilots have completed several compliance periods and provided rich experience for a national ETS.Taking advantage of the variations in the regional ETS pilots across regions and sectors and over time,this study used a difference-in-difference-in-differences(DDD)model to examine the effects of ETS pilots and differential program design on industry risk and the impacts of risk on future investment horizon.First,we found that the ETS pilots significa ntly increase profit variability(total risk)in China's industry;that is,ETS in duces greater uncertainty in the covered sectors.In terms of industry heterogeneity,non-eleclricily sectors(specifically ferrous metal)engage in higher levels of risk taking.Second,the influence of such program features as the allowance allocation(total amounts and approaches),China Certification Emission Reduction(CCER)trading volumes,and penalties are heterogeneous.Finally,during the period around the treatment,industry risk can significantly encourage longer rather than shorter investment.In particular,the punishment mechanism and benchmarking allocation play a positive role in regulating the risk-investment relationship. 展开更多
