Objective:To analyze the existing risks in breast milk management at the neonatal department and provide corresponding countermeasures.Methods:22 risk events were identified in 7 risk links in the process of bottle-fe...Objective:To analyze the existing risks in breast milk management at the neonatal department and provide corresponding countermeasures.Methods:22 risk events were identified in 7 risk links in the process of bottle-feeding of breast milk.Hazard Vulnerability Analysis based on the Kaiser model was applied to investigate and evaluate the risk events.Results:High-risk events include breast milk quality inspection,hand hygiene during collection,disinfection of collectors,cold chain management,hand hygiene during the reception,breast milk closed-loop management,and post-collection disposal.Root cause analysis of high-risk events was conducted and breast milk management strategies outside the hospital and within the neonatal department were proposed.Conclusion:Hazard Vulnerability Analysis based on the Kaiser model can identify and assess neonatal breast milk management risks effectively,which helps improve the management of neonatal breast milk.It is conducive to the safe development and promotion of bottle feeding of breast milk for neonates,ensuring the quality of medical services and the safety of children.展开更多
A recently proposed octahedral spherical hohlraum with six laser entrance holes(LEHs)is an attractive concept for an upgraded laser facility aiming at a predictable and reproducible fusion gain with a simple target de...A recently proposed octahedral spherical hohlraum with six laser entrance holes(LEHs)is an attractive concept for an upgraded laser facility aiming at a predictable and reproducible fusion gain with a simple target design.However,with the laser energies available at present,LEH size can be a critical issue.Owing to the uncertainties in simulation results,the LEH size should be determined on the basis of experimental evidence.However,determination of LEH size of an ignition target at a small-scale laser facility poses difficulties.In this paper,we propose to use the prepulse of an ignition pulse to determine the LEH size for ignition-scale hohlraums via LEH closure behavior,and we present convincing evidence from multiple diagnostics at the SGIII facility with ignition-scale hohlraum,laser prepulse,and laser beam size.The LEH closure observed in our experiment is in agreement with data from the National Ignition Facility.The total LEH area of the octahedral hohlraum is found to be very close to that of a cylindrical hohlraum,thus successfully demonstrating the feasibility of the octahedral hohlraum in terms of laser energy,which is crucially important for sizing an ignition-scale octahedrally configured laser system.This work provides a novel way to determine the LEH size of an ignition target at a small-scale laser facility,and it can be applied to other hohlraum configurations for the indirect drive approach.展开更多
The late Quaternary shallow-water carbonates have been altered by a variety of diagenetic processes,and further influenced by high-amplitude global and regional sea level changes.This study utilizes a new borehole dri...The late Quaternary shallow-water carbonates have been altered by a variety of diagenetic processes,and further influenced by high-amplitude global and regional sea level changes.This study utilizes a new borehole drilled on the Yongxing Island,Xisha Islands to investigate meteoric diagenetic alteration in the late Quaternary shallowwater carbonates.Petrographic,mineralogical,stable isotopic and elemental data provide new insights into the meteoric diagenetic processes of the reef limestone.The results show the variation in the distribution of aragonite,high-Mg calcite(HMC)and low-Mg calcite(LMC)divides the shallow-water carbonates in Core SSZK1 into three intervals,which are UnitⅠ(31.20-55.92 m,LMC),UnitⅡ(18.39-31.20 m,aragonite and LMC)and UnitⅢ(upper 18.39 m of core,aragonite,LMC and HMC).Various degrees of meteoric diagenesis exist in the identified three units.The lowermost UnitⅠhas suffered almost complete freshwater diagenesis,whereas the overlying UnitsⅡandⅢhave undergone incompletely meteoric diagenesis.The amount of time that limestone has been in the freshwater diagenetic environment has the largest impact on the degree of meteoric diagenesis.Approximately four intact facies/water depth cycles are recognized.The cumulative depletion of elements such as strontium(Sr),sodium(Na)and sulphur(S)caused by duplicated meteoric diagenesis in the older reef sequences are distinguished from the younger reef sequences.This study provides a new record of meteoric diagenesis,which is well reflected by whole-rock mineralogy and geochemistry.展开更多

Methods: A QCC, consisting of inform...Objective: To carry out empirical research on the role of project-achieving quality control circle (QCC) in constructing a new model of contactless medical service for outpatients. Methods: A QCC, consisting of information office members from a grade A tertiary hospital in Wenzhou, was established to conduct a research project with the theme “Constructing a new model of contactless medical service based on outpatients’ experience.” According to the ten steps and PDCA cycle, an analysis was carried out before and after the QCC activities, focusing on improving pre-consultation services, providing steward-like services, and facilitating post-consultation management. Results: After the QCC activities, the mobile appointment rate, missed appointment rate, the proportion of smart check-ins, and the average check-in time were 55.68%, 4.02%, 39.75%, and 8.24 ± 3.66 min, respectively;in contrast, before the activities, they were 32.00%, 7.88%, 0.00%, and 14.96 ± 4.98 min, respectively;the difference between the two groups was statistically significant (χ2 = 3480.112, 4994.496;Fisher’s exact probability = 963788.570;t = 5.323, P < 0.001). Many experts have also visited the hospital to learn about this system, thus rendering social and economic benefits. Conclusion: Project-achieving QCC activities are suitable for complex situations, such as constructing a new model of contactless medical service, and can significantly improve outpatient service quality, enhance patients’ experience, and improve the abilities of circle members.展开更多
Objective:To analyze the risk factors of neonatal medical adhesive-related skin injury and put forward targeted preventive measures,so as to provide reference for the care and prevention of neonatal medical adhesive-r...Objective:To analyze the risk factors of neonatal medical adhesive-related skin injury and put forward targeted preventive measures,so as to provide reference for the care and prevention of neonatal medical adhesive-related skin injuries.Methods:Using the convenience sampling method,262 neonates admitted to the neonatal intensive care unit(NICU)of a tertiary general hospital in Wenzhou from April 2021 to May 2022 were selected as the study subjects.The incidence of medical adhesive-related skin injuries in these neonates was retrospectively analyzed.Results:Among the 262 children,43 cases had skin injuries,with an incidence rate of 16.4%.Single factor analysis showed that the occurrence of medical adhesive-related skin injury was related to gestational age,weight,electrocardiogram(ECG)monitoring,venous access,ambient temperature,and mechanical ventilation(P<0.05).Multivariate logistic regression showed that gestational age,ECG monitoring,and ambient temperature were independent risk factors of medical adhesive-related skin injury(OR values were 0.700,0.431,and 6.365,respectively).Conclusion:The high incidence of neonatal medical adhesive-related skin injury may be caused by one or more factors.Clinical measures should be taken,such as selecting the appropriate type of adhesive according to gestational age and using skin-protecting membrane,minimizing ECG monitoring,etc.,to prevent the occurrence of neonatal medical adhesive-related skin injury.展开更多

However, the mechanisms of NMDA receptor participation in the formation and maintenance of DNP remain poorly understood. OBJECTIVE: To evaluate the role NMDA receptor plays in DNP and effects on p38 mitogen activated protein kinase (p38 MAPK) in a rat model of DNP. DESIGN, TIME AND SETTING: A randomized, controlled, animal experiment was performed at the Human Embryonic Stem Cell Research Institute of Yunyang Medical College Affiliated Taihe Hospital between July 2005 and September 2007. MATERIALS: Streptozotocin was provided by Sigma, USA; p38 MAPK inhibitor (SB203580) was provided by Shanghai KangChen Biotech, China; NMDA receptor antagonist (MK-801) was purchased from Shanghai Yope Biotech, China. METHODS: A total of 128 healthy, Wistar rats of clean grade, aged 3 months and weighing 180- 220 g, were randomly assigned to 4 groups: control, DNP model, p38 MAPK, and NMDA receptor. Each group contained 32 rats. DNP was established in all groups except for the control group by intraperitoneal injection of streptozocin (65 mg/kg). Subsequently, 1 mg/kg SB203580 and 1 mg/kg MK-801 were injected once each week via intraperitoneal injection in the p38 MAPK and NMDA receptor groups, respectively. MAIN OUTCOME MEASURES: At the end of 2, 4, 6, and 8 weeks following streptozotocin injection, mechanical withdrawal threshold was measured in 8 animals from each group following von Frey filament stimulation. The rats were anesthetized and nerve conduction velocity of the left sciatic nerve was measured. Subsequently, the right sciatic nerve, the lumbar segment of the spinal cord, and dorsal root ganglia were removed from the L3-6 segment for microscopic examination, p38 MAPK expression was determined using immunohistochemistry and Western blot analysis. Expression of NMDA receptor 1 mRNA in dorsal root ganglion and spinal cord neurons was detected using RT-PCR. RESULTS: Mechanical withdrawal threshold and nerve conduction velocity were significantly reduced, and p38 MAPK and NMDA receptor 1 mRNA expression in the spinal cord and dorsal root ganglia were significantly increased, in the model, p38 MAPK, and NMDA receptor groups compared with the control group at all time points (P 〈 0.05). At 4-8 weeks following successful DNP model establishment, SB203580 and MK-801 increased mechanical withdrawal threshold, accelerated nerve conduction velocity, and attenuated p38 MAPK expression, compared with the model group. The NMDA receptor group exhibited downregulated mRNA expression of NMDA receptor 1 compared with the model and p38 MAPK groups (P 〈 0.05). CONCLUSION: NMDA receptor was highly expressed in the brains of DNP rats and was involved in DNP development via activation of the p38 MAPK signal pathway.展开更多

Introduction:The global incidence of dengue fever has increased significantly over the past two decades,and China faces a significant upward trend in dengue control challenges.Methods:Data were obtained from China’s ...Introduction:The global incidence of dengue fever has increased significantly over the past two decades,and China faces a significant upward trend in dengue control challenges.Methods:Data were obtained from China’s NNDRS from 2005 to 2023.Joinpoint regression software was used to analyze temporal trends,while SaTScan software was used to analyze spatial,seasonal,and spatiotemporal distributions.ArcGIS software was used to visualize clusters.Results:A total of 117,892 dengue cases were reported from 2005 to 2023,with significant fluctuation in annual reported cases.Dengue was not endemic in China.Autochthonous outbreaks most likely occurred in the southwestern,southeastern coastal,and inland areas of China.These outbreaks have occurred between June and November,generally peaking in September or October,around epidemiological week(EW)40.Conclusions:Dengue challenges in China are increasing.Timely case monitoring,proactive control interventions,and staff mobilization should be implemented before June to ensure a timely response to autochthonous outbreaks.展开更多

SFTS virus(SFTSV)is a novel bunyavirus,which was discovered as the etiological agent of severe fever with thrombocytopenia syndrome(SFTS)in China in 2009,and was now prevalent in at least 25 provinces in China.SFTS wa...SFTS virus(SFTSV)is a novel bunyavirus,which was discovered as the etiological agent of severe fever with thrombocytopenia syndrome(SFTS)in China in 2009,and was now prevalent in at least 25 provinces in China.SFTS was subsequently identified in South Korea and Japan in 2012.To explore themolecular evolution and genetic characteristics of this newly identified pathogen,we reported 72 whole genome sequences of SFTSV,and built a dataset of SFTSV genome sequences containing 292 L-segment,302 M-segment and 502 S-segment.We clearly divided SFTSV into six genotypes,Genotype A-F.It was found that genotype F was the dominant epidemic genotype of Japan,South Korea,and Zhejiang province of China.The coalescent analysis supported that SFTSV originated in the early 18th century from Zhejiang province,and Genotype F was the most primitive one.Henan,Hubei,and Anhui provinces which are located in Dabie Mountain area weremainly epidemic of Genotype A,which emerged relatively late but distributed widely.A total of 37 recombination events were identified,making SFTSV with a high recombination frequency(L segment 5.1%,Msegment 3.6%,S segment 0.8%)among negative-strand segmented RNA viruses.It was identified that 19 reassortant strains belonged to 12 reassortment forms of SFTSV genome containing 6 newly identified forms.The reassortment virus and recombination in tick were both found for the first time.We also found many of genotype-specific mutation sites,7 of which could be considered as potential molecular marker for genotype classification.This study promoted a more comprehensive understanding of the phylogeny and origin,and the genetic diversity of SFTSV,and it could help the studies of other newly discovered tick-borne bunyavirus as reference data and research ideas.展开更多

MAIN OUTCOME MEASURES: At the end of 2, 4, 6, and 8 weeks following streptozotocin injection, mechanical withdrawal threshold was measured in 8 animals from each group following von Frey filament stimulation. The rats were anesthetized and nerve conduction velocity of the left sciatic nerve was measured. Subsequently, the right sciatic nerve, the lumbar segment of the spinal cord, and dorsal root ganglia were removed from the L3-6 segment for microscopic examination, p38 MAPK expression was determined using immunohistochemistry and Western blot analysis. Expression of NMDA receptor 1 mRNA in dorsal root ganglion and spinal cord neurons was detected using RT-PCR. RESULTS: Mechanical withdrawal threshold and nerve conduction velocity were significantly reduced, and p38 MAPK and NMDA receptor 1 mRNA expression in the spinal cord and dorsal root ganglia were significantly increased, in the model, p38 MAPK, and NMDA receptor groups compared with the control group at all time points (P 〈 0.05). At 4-8 weeks following successful DNP model establishment, SB203580 and MK-801 increased mechanical withdrawal threshold, accelerated nerve conduction velocity, and attenuated p38 MAPK expression, compared with the model group. The NMDA receptor group exhibited downregulated mRNA expression of NMDA receptor 1 compared with the model and p38 MAPK groups (P 〈 0.05). CONCLUSION: NMDA receptor was highly expressed in the brains of DNP rats and was involved in DNP development via activation of the p38 MAPK signal pathway.

文摘We propose a method for retrieving a beam wavefront from its near-field intensity distribution after a 4f system by simply inserting a benchmark at the Fourier plane.Through a convolution of the mark-blocked spatial frequency component and the original optical field with the 4f system,the separation between the focus of any sub-aperture and the benchmark can be determined to reconstruct the beam wavefront.Theoretical and experimental studies demonstrate the validity of this method,which has potential applications in real-time wavefront sensing.

文摘[Objectives] To compare different extraction methods for total alkaloids from Abrus Cantoniensis Hance taking chloroform as solvent. [Methods] The ethanol extraction method,ultrasonic extraction method,acid water extraction method,and Soxhlet extraction method were used to extract total alkaloids,and the total alkaloids of Abrus Cantoniensis Hance were measured at 238 nm wavelength. [Results] The extraction rate of ethanol extraction method was 14. 76 mg/100 g; the extraction rate of ultrasonic extraction method was 56. 85 mg/100 g; the extraction rate of acid water extraction method was 56. 63 mg/100 g; the extraction rate of Soxhlet extraction method was 58. 19 mg/100 g.[Conclusions]Results indicate that Soxhlet extraction method can obtain the highest extraction rate.

文摘Introduction:The global incidence of dengue fever has increased significantly over the past two decades,and China faces a significant upward trend in dengue control challenges.Methods:Data were obtained from China’s NNDRS from 2005 to 2023.Joinpoint regression software was used to analyze temporal trends,while SaTScan software was used to analyze spatial,seasonal,and spatiotemporal distributions.ArcGIS software was used to visualize clusters.Results:A total of 117,892 dengue cases were reported from 2005 to 2023,with significant fluctuation in annual reported cases.Dengue was not endemic in China.Autochthonous outbreaks most likely occurred in the southwestern,southeastern coastal,and inland areas of China.These outbreaks have occurred between June and November,generally peaking in September or October,around epidemiological week(EW)40.Conclusions:Dengue challenges in China are increasing.Timely case monitoring,proactive control interventions,and staff mobilization should be implemented before June to ensure a timely response to autochthonous outbreaks.

文摘What is already known about this topic?Severe fever with thrombocytopenia syndrome(SFTS)is a serious tick-borne disease in East Asia with high mortality,particularly affecting the elderly.Since its discovery in 2010,inconsistencies in small-scale studies and the lack of decade-long research on antibody levels in large population samples after natural infection,along with the absence of an effective vaccine,highlight the need for large-scale,long-term data in highincidence regions of China.What is added by this report?This study of 1,410 serum samples from SFTS patients in high-incidence regions of China reveals that immunoglobulin M(IgM)levels peak at 8–14 days post-infection,declining to nearly undetectable levels by 180 days.Immunoglobulin G(IgG)and neutralizing antibodies(NAb)levels peak at 22–180 days,persisting up to 10 years.IgM levels correlate with viral load and various immune and coagulation parameters,with lower levels observed in fatal cases.During convalescence,elderly patients have lower IgG levels,whereas females exhibit higher IgG levels compared with males.What are the implications for public health practice?The study’s findings on long-term antibody dynamics in SFTS patients can significantly improve vaccine development,optimize therapy scheduling,inform public health policies,and enhance diagnostic tools,leading to better disease management and prevention in high-incidence areas.

文摘SFTS virus(SFTSV)is a novel bunyavirus,which was discovered as the etiological agent of severe fever with thrombocytopenia syndrome(SFTS)in China in 2009,and was now prevalent in at least 25 provinces in China.SFTS was subsequently identified in South Korea and Japan in 2012.To explore themolecular evolution and genetic characteristics of this newly identified pathogen,we reported 72 whole genome sequences of SFTSV,and built a dataset of SFTSV genome sequences containing 292 L-segment,302 M-segment and 502 S-segment.We clearly divided SFTSV into six genotypes,Genotype A-F.It was found that genotype F was the dominant epidemic genotype of Japan,South Korea,and Zhejiang province of China.The coalescent analysis supported that SFTSV originated in the early 18th century from Zhejiang province,and Genotype F was the most primitive one.Henan,Hubei,and Anhui provinces which are located in Dabie Mountain area weremainly epidemic of Genotype A,which emerged relatively late but distributed widely.A total of 37 recombination events were identified,making SFTSV with a high recombination frequency(L segment 5.1%,Msegment 3.6%,S segment 0.8%)among negative-strand segmented RNA viruses.It was identified that 19 reassortant strains belonged to 12 reassortment forms of SFTSV genome containing 6 newly identified forms.The reassortment virus and recombination in tick were both found for the first time.We also found many of genotype-specific mutation sites,7 of which could be considered as potential molecular marker for genotype classification.This study promoted a more comprehensive understanding of the phylogeny and origin,and the genetic diversity of SFTSV,and it could help the studies of other newly discovered tick-borne bunyavirus as reference data and research ideas.

文摘Optics surface phase defects induced intensity modulation in high-power laser facility for inertial confinement fusion research is studied. Calculations and experiments reveal an exact mapping of the modulation patterns and the optics damage spot distributions from the surface phase defects. Origins are discussed during the processes of optics manufacturing and diagnostics, revealing potential improvements for future optics manufacturing techniques and diagnostic index, which is meaningful for fusion level laser facility construction and its operation safety.
