BACKGROUND We report a case of infective endocarditis(IE)in a patient with congenital heart valve lesions accompanied by IE,which was diagnosed based on blood culture analysis that revealed the presence of a gram-nega...BACKGROUND We report a case of infective endocarditis(IE)in a patient with congenital heart valve lesions accompanied by IE,which was diagnosed based on blood culture analysis that revealed the presence of a gram-negative bacterium,Streptococcus gordonii.CASE SUMMARY The patient had a history of precordial valve disease diagnosed by cardiac ultrasound,as well as a 4-mo history of fever.He was subjected to comprehensive anti-infection and anti-heart failure treatment in the internal medicine department.Further examination revealed sudden dislodgement from and perforation through the aortic valve by the superfluous organisms,as well as occurrence of bacterial emboli dislodgement,which caused bacteremia and infectious shock.He recovered and was discharged from the hospital after surgical and postoperative anti-infection treatments.CONCLUSION We review the treatment process and highlight inspirations and reflections from this case;suggest possible future changes in treatment modalities.展开更多

Inkjet 3D printing has potential in the additive manufacturing of electronic circuits and devices.However,inks that can be used for printing layers with T5%>300℃ or hardness>200 MPa have been rarely reported.Cy...Inkjet 3D printing has potential in the additive manufacturing of electronic circuits and devices.However,inks that can be used for printing layers with T5%>300℃ or hardness>200 MPa have been rarely reported.Cyanate ester(CE)polymers have excellent thermal stability,high strength,and low shrinkage compared to other common dielectric inks for inkjet 3D printing,but cannot be quickly shaped by ultraviolet(UV)irradiation or thermal treatment.Combining CEs with UV-curable monomers may be a possible way to accelerate crosslinking,but there are challenges from the adverse effects of the dilution of both monomers.In this study,dielectric inks with acrylate and cyanate moieties were developed.The low viscosity and surface tension of the CE precursor(Bisphenol E cyanate ester)were combined with photocurable acrylate diluent monomers and cross-linker to realize an ink suitable for inkjet 3D printing.An internal dual three-dimensional cross-linked network structure resin was prepared by a combination of photocuring and thermal curing with T5%up to 326.69℃,hardness up to 431.84 MPa,dielectric constant of 2.70 at 8 GHz,and shrinkage of 1.64%.The developed dielectric inks can be applied to the 3D printing of printed circuit boards and other electronic devices that require dielectric properties.展开更多
An energy model for the structure transformation of pile-ups of grain boundary dislocations(GBD)at the triple-junction of the grain boundary of ultrafine-grain materials was proposed.The energy of the pile-up of the G...An energy model for the structure transformation of pile-ups of grain boundary dislocations(GBD)at the triple-junction of the grain boundary of ultrafine-grain materials was proposed.The energy of the pile-up of the GBD in the system was calculated by the energy model,the critical geometric and mechanical conditions for the structure transformation of head dislocation of the pile-up were analyzed,and the influence of the number density of the dislocations and the angle between Burgers vectors of two decomposed dislocations on the transformation mode of head dislocation was discussed.The results show when the GBD is accumulated at triple junction,the head dislocation of the GBD is decomposed into two Burgers vectors of these dislocations unless the angle between the two vectors is less than 90°,and the increase of applied external stress can reduce the energy barrier of the dislocation decomposition.The mechanism that the ultrafine-grained metal material has both high strength and plasticity owing to the structure transformation of the pile-up of the GBD at the triple junction of the grain boundary is revealed.展开更多
The phase field crystal method and Continuum Modeling are applied to study the cooperative dislocation motion of the grain boundary(GB)migration,the manner of the nucleation of the grain and of the grain growth in two...The phase field crystal method and Continuum Modeling are applied to study the cooperative dislocation motion of the grain boundary(GB)migration,the manner of the nucleation of the grain and of the grain growth in two dimensions(2 D)under the deviatoric deformation at high temperature.Three types of the nucleation modes of new finding are observed by the phase field crystal simulation:The first mode of the nucleation is generated by the GB splitting into two sub-GBs;the second mode is of the reaction of the sub-GB dislocations,such as,the generation and annihilation of a pair of partial Frank sessile dislocation in 2 D.The process can be considered as the nucleation of dynamic recrystallization;the third mode is caused by two oncoming rows of the dislocations of these sub-GBs,crossing and passing each other to form new gap which is the nucleation place of the new deformed grain.The research is shown that due to the nucleation of different modes the mechanism of the grain growth by means of the sub-GB migration is different,and therefore,the grain growth rates are also different.Under the deviatoric deformation of the applied biaxial strain,the grain growth is faster than that of the grain growth without external applied stress.It is observed that the cooperative dislocation motion of the GB migration under the deviatoric deformation accompanies with local plastic flow and the state of the stress of the system changes sharply.When the system is in the process of recrystallized grain growth,the system energy is in an unstable state due to the release of the strain energy to cause that the reverse movement of the plastic flow occurs.The area growth of the deformed grain is approximately proportional to the strain square and also to the time square.The rule of the time square of the deformed grain growth can also be deduced by establishing the continuum dynamic equation of the biaxial strain-driven migration of the GB.The copper metal is taken as an example of the calculation,and the obtained result is a good agreement with that of the experiment.展开更多
Background:Histidine-tryptophan-ketoglutarate(HTK)is a solution commonly used for organ transplantation.However,there is no certified fixed regimen for on-pump heart surgery in neonates.We aimed to retrospectively eva...Background:Histidine-tryptophan-ketoglutarate(HTK)is a solution commonly used for organ transplantation.However,there is no certified fixed regimen for on-pump heart surgery in neonates.We aimed to retrospectively evaluate the outcomes related to different HTK dosages and to analyze the safety of high-dosage perfusion.Methods:A total of 146 neonates who underwent on-pump heart surgery with single-shot HTK perfusion were divided into two groups according to HTK dosages:a standard-dose(SD)group(n=63,40 mL/kg<HTK≤60 mL/kg)and a high-dose(HD)group(n=83,HTK>60 mL/kg).Propensity score matching(PSM)was performed to control confounding bias.Results:The SD group had a higher weight(3.7±0.4 vs.3.4±0.4 kg,P<0.0001),a lower proportion of complete transposition of the great artery(69.8%vs.85.5%,P=0.022),a lower cardiopulmonary bypass(CPB)time(123.5[108.0,136.0]vs.132.5[114.8,152.5]min,P=0.034),and a lower aortic x-clamp time(82.9±27.1 vs.95.5±26.0 min,P=0.005).After PSM,44 patients were assigned to each group;baseline characteristics and CPB parameters between the two groups were comparable.There were no significant differences in peri-CPB blood product consumption after PSM(P>0.05).The incidences of post-operative complications were not significantly different between the two groups.There were no significant differences in ventilation time,intensive care unit stay,and post-operative hospital stay(P>0.05).Follow-up echocardiography outcomes at 1 month,3 to 6 months,and 1 year showed that left ventricular ejection fraction and end-diastolic dimension were comparable between the two groups.Conclusions:In neonatal on-pump cardiac surgery patients,single-shot HD(>60 mL/kg)HTK perfusion had a comparable heart protection effect and short-term post-operative prognosis as standard dosage perfusion of 40 to 60 mL/kg.Thus,this study provides supporting evidence of the safety of HD HTK perfusion.展开更多
