目的探究超声多普勒引导下痔动脉结扎术(DG-HAL)联合吻合器痔上黏膜环切术(PPH)治疗重症痔疮的临床疗效及安全性。方法选择2016年1月-2017年1月于该院就诊的重症痔疮患者72例,按照随机数字表法分为对照组(n=35)与观察组(n=37),对照组患... 目的探究超声多普勒引导下痔动脉结扎术(DG-HAL)联合吻合器痔上黏膜环切术(PPH)治疗重症痔疮的临床疗效及安全性。方法选择2016年1月-2017年1月于该院就诊的重症痔疮患者72例,按照随机数字表法分为对照组(n=35)与观察组(n=37),对照组患者接受PPH治疗,观察组接受DGHAL联合PPH治疗。观察两组患者的手术相关指标、治疗效果及并发症发生率等。结果两组患者手术时间、术中出血量无明显差异(P>0.05)。观察组术后创面愈合时间及住院时间明显短于对照组,差异有统计学意义(P<0.05)。观察组术后1个月治疗有效率为94.6%,明显高于对照组有效率77.1%,差异有统计学意义(P<0.05)。两组患者术前和术后7 d视觉模拟评分(VAS)无明显差异(P>0.05)。观察组术后14 d VAS评分明显低于对照组(P<0.05)。观察组术后3个月Wexner评分明显低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为2.7%,明显低于对照组(P<0.05)。结论DG-HAL联合PPH治疗重症痔疮有较好的临床疗效,手术操作简单安全,术后并发症发生率低,值得临床推广使用。 展开更多

Background:There have been few reports on long-term survival of gastric cancer patients.This study analyzed the survival data of gastric cancer patients obtained from the population-based Qidong Cancer Registry between 1972 and 2011,providing a basis for evaluation of gastric cancer treatment and prognosis.Methods:The cumulative observed survival rate and relative survival rate of gastric cancer patients were calculated using Hakulinen's method via the SURV3.01 software,which was developed by the Finnish Cancer Registry.The date of the last follow-up for the survival status of the 15,401 registered cases was April 30,2012.Results:The 1-,5-,10-,20-,and 30-year observed survival rates were 33.82%,14.18%,10.35%,6.63%,and 4.19%,respectively,and the 1-,5-,10-,20-,and 30-year relative survival rates were 35.43%,18.13%,17.50%,21.96%,and32.84%,respectively.For males,the corresponding observed survival rates were 34.50%,14.40%,10.42%,6.46%,and4.05%,and the corresponding relative survival rates were 36.23%,18.67%,18.28%,23.73%,and 38.61%.For females,the corresponding observed survival rates were 32.62%,13.80%,10.22%,6.95%,and 4.46%,and the corresponding relative survival rates were 34.03%,17.21%,16.28%,19.70%,and 26.78%.Significant differences in relative survival rates were observed between sexes(P=0.003).For the 15-34,35-44,45-54,55-64,65-74,and 75+ age groups,the 5-year relative survival rates were 16.13%,21.77%,18.63%,12.61%,7.99%,and 2.94%,respectively,and the 10-year relative survival rates were 16.49%,22.83%,20.50%,15.97%,15.88%,and 15.73%,respectively.Remarkable improvement could be observed for the 5-,10-,and 15-year relative survival rates in Qidong beginning in the 1980 s.Conclusion:The survival outcome of registered gastric cancer cases in Qidong showed gradual progress over the past two decades. 展开更多

As the“throat”of the drilling well control system,ram blowout preventers(BOPs)can effectively prevent blowout accidents.However,the ram shear mechanism under complex working conditions is unclear,and it is difficult to evaluate the ram BOP shear force,leading to frequent shear failure accidents in oilfields.Aiming at the above problems,this paper takes the double-V ram BOP as the research object,and integrates the methods of theoretical analysis,simulation modeling,and test verification to analyze the shear force in the pipe shear process under both static and moving conditions.A ram BOP shear force evaluation method is proposed based on equivalent stress.Finally,by comparing with calculation data and experimental data,the error between them is less than 5%,demonstrating the applicability and effectiveness of the proposed method.The research results can provide a theoretical basis for oilfield operations of ram BOPs. 展开更多

The room temperature brittleness has been a long standing problem in bulk metallic glasses realm.This has seriously limited the application potential of metallic glasses and their composites.The elastic deformation behaviors of metallic glass matrix composites are closely related to their plastic deformation states.The elastic deformation behaviors of Cu48-xZr48Al4Nbx(x=0,3at.%)metallic glass matrix composites(MGMCs)with different crystallization degrees were investigated using an in-situ digital image correlation(DIC)technique during tensile process.With decreasing crystallization degree,MGMC exhibits obvious elastic deformation ability and an increased tensile fracture strength.The notable tensile elasticity is attributed to the larger shear strain heterogeneity emerging on the surface of the sample.This finding has implications for the development of MGMCs with excellent tensile properties. 展开更多
