Objective Non-suicidal self-injury(NSSI)has a higher prevalence in adolescents with depressive disorders than in community adolescents.This study examined the differences in NSSI behaviors between adolescents with uni...Objective Non-suicidal self-injury(NSSI)has a higher prevalence in adolescents with depressive disorders than in community adolescents.This study examined the differences in NSSI behaviors between adolescents with unipolar depression(UD)and those with bipolar depression(BD).Methods Adolescents with UD or BD were recruited from 20 general or psychiatric hospitals across China.The methods,frequency,and function of NSSI were assessed by Functional Assessment of Self-Mutilation.The Beck Suicide Ideation Scale was used to evaluate adolescents’suicidal ideation,and the 10-item Kessler Psychological Distress Scale to estimate the anxiety and depression symptoms.Results The UD group had higher levels of depression(19.16 vs.17.37,F=15.23,P<0.001)and anxiety symptoms(17.73 vs.16.70,F=5.00,P=0.026)than the BD group.Adolescents with BD had a longer course of NSSI than those with UD(2.00 vs.1.00 year,Z=−3.39,P=0.001).There were no statistical differences in the frequency and the number of methods of NSSI between the UD and BD groups.Depression(r=0.408,P<0.01)and anxiety(r=0.391,P<0.01)were significantly and positively related to NSSI frequency.Conclusion Adolescents with BD had a longer course of NSSI than those with UD.More importantly,NSSI frequency were positively and strongly correlated with depression and anxiety symptoms,indicating the importance of adequate treatment of depression and anxiety in preventing and intervening adolescents’NSSI behaviors.展开更多
BACKGROUND Patients with foreign bodies in the digestive tract are often encountered,but complete penetration of a foreign body through the gastrointestinal tract is rare,and the choice of imaging method is very impor...BACKGROUND Patients with foreign bodies in the digestive tract are often encountered,but complete penetration of a foreign body through the gastrointestinal tract is rare,and the choice of imaging method is very important.Improper selection may lead to missed diagnosis or misdiagnosis.CASE SUMMARY An 81-year-old man was diagnosed as having a liver malignancy after he took magnetic resonance imaging and positron emission tomography/computed tomography(CT)examinations.The pain improved after the patient accepted gamma knife treatment.However,he was admitted to our hospital 2 mo later due to fever and abdominal pain.This time,he received a contrast-enhanced CT scan,which showed fish-boon-like foreign bodies in the liver with peripheral abscess formation,then he went to the superior hospital for surgery.It lasted for more than 2 mo from the onset of the disease to the surgical treatment.A 43-year-old woman with a 1 mo history of a perianal mass with no obvious pain or discomfort was diagnosed as having an anal fistula with the formation of a local small abscess cavity.Clinical perianal abscess surgery was performed,and fish bone foreign body was found in perianal soft tissue during the operation.CONCLUSION For patients with pain symptoms,the possibility of foreign body perforation should be considered.Magnetic resonance imaging is not comprehensive and that a plain computed tomography scan of the pain area is necessary.展开更多
BACKGROUND Pulmonary hypertension is a rare cardiopulmonary disease,with an insidious onset that usually worsens rapidly.Amyloid light chain(AL)amyloidosis is a rare systemic disease caused by extracellular deposition...BACKGROUND Pulmonary hypertension is a rare cardiopulmonary disease,with an insidious onset that usually worsens rapidly.Amyloid light chain(AL)amyloidosis is a rare systemic disease caused by extracellular deposition of pathologic,insoluble,and proteinaceous fibrils in organs and tissues;however,it is difficult to diagnose given its varied and nonspecific symptoms.To date,rare cases of amyloidosis with pulmonary hypertension have been reported.Of note,the optimal treatments for cardiac amyloidosis complicated with pulmonary hypertension remain unclear.CASE SUMMARY We report a case of a 51-year-old woman who presented with progressively worsening dyspnea.Transthoracic echocardiography indicated severe pulmonary hypertension.Twenty-seven months after first admission,the patient returned with symptoms of progressive heart failure.A myocardial tissue sample stained with Congo red was positive,and the patient was ultimately diagnosed with AL amyloidosis with cardiac involvement.CONCLUSION Although pulmonary hypertension may be idiopathic,it is frequently associated with other conditions.In rare cases,pulmonary hypertension can be a complication of AL amyloidosis,which should be seriously considered in any adult presenting with nonspecific signs or symptoms of cardiac distress.展开更多

To improve the wear resistance of the high chromium cast irons(HCCIs), mechanical vibration was employed during the solidification of the HCCIs. The effects of vibration frequency on the microstructure and performance of the HCCIs under as-cast, as-quenched and as-tempered conditions were investigated. The results indicated that the microstructures of the LFC-produced HCCIs were refined due to the introduction of mechanical vibration, and the hardness was improved compared to that of the alloy without vibration. However, only a slight improvement in hardness was found in spite of the increase of vibration frequency. In contrast, the impact toughness of the as-tempered HCCIs increased with an increase in the vibration frequency. In addition, the wear resistance of the HCCIs was improved as a result of the introduction of vibration and increased with an increase in the vibration frequency.展开更多

The research aim was to improve the...Based on the outpatient interview and literature review, the initial framework of the outpatient experience of human caring scale was formed with 9 dimensions of outpatient process. The research aim was to improve the scale by Delphi method. Sixteen experts in medical management, human caring or medical education were invited to evaluate the importance of the dimensions and items of the scale and provided some expertise via filling out the DeLphi consultation questionnaires twice in the consulting round. In the first round, the recovery rate showing the experts' positivity was 80%; the coefficient of reliability (Cr) ascertaining the authority of the evaluation was 0.92; the mean and full mark ratios responding the concentration of the evaluation were 2.88-4.94 and 6.25%-93.75% respectively; the coefficients of variation (CV) and the Kendall's W determining the concordance of the evaluation were 5.06%-52.15% and 0.21-0.24 respectively. In the second round, the recovery rate was 93.75%; the Cr was 0.93; the mean was 3.93-4.93; the full mark ratios were 26.67%-93.33%; the Kendall's W was 0.14- 0.31, the CV was 5.25%-23.61%. Via the two-round Delphi study, the scale that included 10 dimensions and 61 items has been improved. Ten dimensions are pre-hospital medical service, guidance, registration, waiting, diagnosis & treatment, paying, inspection & assay, medicine receiving, therapy/injection/transfusion and global evaluation. It was concluded that Chinese scholars have paid high attention to human caring and outpatient experience. The experts have given high agreements about the dimensions which were established with Chinese outpatient process. The dimensions are different from the similar researches about outpatient experience study. In the future, it is necessary to survey the outpatients to test the construct validity, internal consistency reliability and others of the scale to improve the scale.展开更多
BACKGROUND In early gastric cancer(GC),tumor markers are increased in the blood.The levels of these markers have been used as important indexes for GC screening,early diagnosis and prognostic evaluation.However,specif...BACKGROUND In early gastric cancer(GC),tumor markers are increased in the blood.The levels of these markers have been used as important indexes for GC screening,early diagnosis and prognostic evaluation.However,specific tumor markers have not yet been discovered.Diagnosis based on a single tumor marker has limited significance.The detection rate of GC is still very low.AIM To improve the diagnostic value of blood markers for GC.METHODS We used a multiparameter joint analysis of 77 indexes of malignant GC and gastric polyp(GP),64 indexes of GC and healthy controls(Ctrls).RESULTS By analyzing the data,there are 27 indexes in the final Ctrls vs GC with P values<0.01,the area under the curve(AUC)of albumin is the largest in Ctrls vs GC,and the AUC was 0.907.30 indexes in GP vs GC have P values<0.01.Among them,the D-dimer showed an AUC of 0.729.The 27 indexes in Ctrls vs GC and 30 indexes in GP vs GC were used for binary logistic regression,discriminant analysis,classification tree analysis and artificial neural network analysis model.For the ability to distinguish between Ctrls vs GC,GP vs GC,artificial neural networks had better diagnostic value when compared with classification tree,binary logistic regression,and discriminant analysis.When compared Ctrl and GC,the overall prediction accuracy was 92.9%,and the AUC was 0.992(0.980,1.000).When compared GP and GC,the overall prediction accuracy was 77.9%,and the AUC was 0.969(0.948,0.990).CONCLUSION The diagnostic effect of multi-parameter joint artificial neural networks analysis is significantly better than the single-index test diagnosis,and it may provide an assistant method for the detection of GC.展开更多
BACKGROUND Early detection,early diagnosis,and early treatment are currently accepted methods that can effectively improve the efficacy of colorectal cancer(CRC)treatment.Exosomes were demonstrated to be potential tum...BACKGROUND Early detection,early diagnosis,and early treatment are currently accepted methods that can effectively improve the efficacy of colorectal cancer(CRC)treatment.Exosomes were demonstrated to be potential tumor molecular markers.AIM To evaluate the diagnostic value of CRC by detecting four exosomal microRNAs(miRNAs)(miR-15b,miR-16,miR-21,and miR-31)that were demonstrated to have potential diagnostic value in serum.METHODS Relative expression levels of miR-15b,miR-16,miR-21,and miR-31 in 123 CRC,117 colorectal adenoma,and 150 healthy controls were detected,and single and panel models were evaluated.The 2-ΔΔCt method was used to calculate the relative expression of miRNA compared to the internal control(U6).Eighty-one CRC patients,67 colorectal adenoma patients,and 90 healthy controls were used for validation.RESULTS Compared to the healthy control group,the best indicator of the four miRNAs was miR-15b,and the sensitivity and specificity were 81.33%and 91.80%,respectively.For miR-15b,miR-21,and miR-31 individually,the sensitivity and specificity were 91.95%and 97.62%,95.06%and 94.44%,respectively.Compared to the colorectal adenoma group,miR-15b,miR-16,and miR-21 in the CRC group showed significant differences(P<0.05).The best single indicator was miR-16,with a sensitivity and specificity of 79.05%and 71.55%.The sensitivity and specificity of a panel that included miR-15b,miR-16,and miR-21 were 81.21%and 81.03%,and 85.19%and 82.09%,respectively,in the validation.CONCLUSION We built and validated a diagnostic model containing miR-15b,miR-21,and miR-31 expression levels to discriminate the healthy control group and CRC group,and its sensitivity and specificity were 95.06%and 94.44%,respectively.The miR-15b,miR-16,and miR-21 panel was used to discriminate the colorectal adenoma group and CRC group with a sensitivity and specificity of 85.19%and 82.09%,respectively.展开更多
Cyano substitution has been established as a viable approach to optimize the performance of all-small-molecule organic solar cells.However,the effect of cyano substitution on the dynamics of photo-charge generation re...Cyano substitution has been established as a viable approach to optimize the performance of all-small-molecule organic solar cells.However,the effect of cyano substitution on the dynamics of photo-charge generation remains largely unexplored.Here,we report an ultrafast spectroscopic study showing that electron transfer is markedly promoted by enhanced intermolecular charge-transfer interaction in all-small-molecule blends with cyanided donors.The delocalized excitations,arising from intermolecular interaction in the moiety of cyano-substituted donor,undergo ultrafast electron transfer with a lifetime of∼3 ps in the blend.In contrast,some locally excited states,surviving in the film of donor without cyano substitution,are not actively involved in the charge separation.These findings well explain the performance improvement of devices with cyanided donors,suggesting that manipulating intermolecular interaction is an efficient strategy for device optimization.展开更多

The different enzyme activity indexes were measured, and the correlation analysis and the principal component analysis (PCA) method were applied to evaluate and screen the tree species with advanced restoration. The results demonstrate that there are some correlations among the different soil enzymes, including some very significant positive correlations among urease, phosphatase, invertase and protease. The best species in terms of repair is privet, and the worst is Haltong. This study provides a scientific basis for the selection of restoration-capable tree species in the reclamation area of the coal mine.展开更多

The arterial wall was assumed to be vis- coelastic and the blood was assumed to be incompressible and non-Newtonian fluid, which is more close to human arte- rial system. Numerical results indicated that the mass trans- fer from the arterial surface to flowing blood in regions of disturbed flow is positively related with the wall shear rates and it is significantly enhanced in regions of disturbed flow with a local minimum around the reattachment point which is higher than the average value of the downstream. There- fore, it may be implied that the accumulation of cholesterol or lipids within atheromatous plaques is not caused by the reduced efflux of cholesterol or lipids, but by the infiltration of the LDL (low-density lipoprotein) from the flowing blood to the arterial wall.展开更多

The flow curves exhibited typical flow softening. The initial discontinuous yielding behavior was observed at higher strain rates, which was not found in other traditional Zr alloys. The apparent deformation activation energy was calculated to be 103 kJ/mol and constitutive equationdescribing the flow stress as a function of the strain rate and deformation temperature was proposed. The analysis indicated that the hot deformation mechanism was mainly dominated by dynamic recovery. However, dynamic recrystallization was delayed by dynamic recovery. Thereafter, the processing map was calculated to evaluate the efficiency of the forging process at the temperatures and strain rates investigated and to optimize processing parameters of hot deformation. The optimum processing parameters were found to be 830?850 °C and 0.56?1 s?1 for hot the deformation of Zr50Ti50 alloy in the β phase region.展开更多

The CH3 fragments have been probed via (2+1) resonance-enhanced multiphoton ionization. Images are measured for CH3 formed in the ground and excited states (v2=0 and 1) of the umbrella vibrational mode. For acetaldehyde dissociation on T1 state after intersystem crossing from S1 state, the products are formed with high translational energy release and low internal excitation. The rotational and vibrational energy of both fragments increases with increasing photodissociation energy. The triplet barrier height is estimated at 3.8814-0.006 eV above the ground state of acetaldehyde.展开更多

The CO productwas detected through (2+1) resonance enhanced multiphoton ionization (REMPI). Imageswere obtained for CO products formed in the ground and vibrational excited state (v=0 and v=1). The energy distributions and product angular distributions were obtained from the CO velocity imaging. The correlated NH(a^1Δ) rovibrational state distributions were determined.The vibrational branching ratio of 1NH (v=1/v=0) increases as the rotational state of CO(v=0) increases initially and decreases afterwards, which indicates a special state-tostate correlation between the 1NH and CO products. About half of the available energy was partitioned into the translational degree of freedom. The negative anisotropy parameter indicates that it is a vertical direct dissociation process.展开更多

Background Due to the broad application of next-generation sequencing,the molecular diagnosis of genetic disorders in pediatric neurology is no longer an unachievable goal.However,treatments for neurological genetic d...Background Due to the broad application of next-generation sequencing,the molecular diagnosis of genetic disorders in pediatric neurology is no longer an unachievable goal.However,treatments for neurological genetic disorders in children remain primarily symptomatic.On the other hand,with the continuous evolution of therapeutic viral vectors,gene therapy is becoming a clinical reality.From this perspective,we wrote this review to illustrate the current state regarding viral-mediated gene therapy in childhood neurological disorders.Data sources We searched databases,including PubMed and Google Scholar,using the keywords"adenovirus vector,""lentivirus vector,"and"AAv"for gene therapy,and"immunoreaction induced by gene therapy vectors,""administration routes of gene therapy vectors,"and"gene therapy"with"NCL,""SMA,""DMD,""congenital myopathy,""MPS""leukodystrophy,"or"pediatric metabolic disorders".We also screened the database of ClinicalTrials.gov using the keywords"gene therapy for children"and then filtered the results with the ones aimed at neurological disorders.The time range of the search procedure was from the inception of the databases to the present.Results We presented the characteristics of commonly used viral vectors for gene therapy for pediatric neurological disorders and summarized their merits and drawbacks,the administration routes of each vector,the research progress,and the clinical application status of viral-mediated gene therapy on pediatric neurological disorders.Conclusions Viral-mediated gene therapy is on the brink of broad clinical application.Viral-mediated gene therapy will dramatically change the treatment pattern of childhood neurological disorders,and many children with incurable diseases will meet the dawn of a cure.Nevertheless,the vectors must be optimized for better safety and efficacy.展开更多

Herein,we fabricated all-polymer solar cells(all-PSCs)based on a fluorinated wide-bandgap p-type conjugated polymer PM6 as the donor,and a narrow bandgap n-type conjugated polymer PZ1 as the acceptor.In addition to th...Herein,we fabricated all-polymer solar cells(all-PSCs)based on a fluorinated wide-bandgap p-type conjugated polymer PM6 as the donor,and a narrow bandgap n-type conjugated polymer PZ1 as the acceptor.In addition to the complementary absorption and matching energy levels,the optimized blend films possess high cystallinity,predominantly face-on stacking,and a suitable phase separated morphology.With this active layer,the devices exhibited a high Vocof 0.96 V,a superior Jscof 17.1 mA cm^-2,a fine fill factor(FF)of 68.2%,and thus an excellent power conversion efficiency(PCE)of 11.2%,which is the highest value reported to date for single-junction all-PSCs.Furthermore,the devices showed good storage stability.After 80 d of storage in the N2-filled glovebox,the PCE still remained over 90%of the original value.Large-area devices(1.1 cm^2)also demonstrated an outstanding performance with a PCE of 9.2%,among the highest values for the reported large-area all-PSCs.These results indicate that the PM6:PZ1 blend is a promising candidate for scale-up production of large area high-performance all-PSCs.展开更多
Globozoospermia has been reported to be a rare but severe causation of male infertility,which results from the failure of acrosome biogenesis and sperm head shaping.Variants of dpy-19-like 2(DPY19L2)are highly related...Globozoospermia has been reported to be a rare but severe causation of male infertility,which results from the failure of acrosome biogenesis and sperm head shaping.Variants of dpy-19-like 2(DPY19L2)are highly related to globozoospermia,but related investigations have been mainly performed in patients from Western countries.Here,we performed a screening of DPY19L2 variants in a cohort of Chinese globozoospermic patients and found that five of nine patients carried DPY19L2 deletions and the other four patients contained novel DPY19L2 point mutations,as revealed by whole-exome sequencing.Patient 3(P3)contained a heterozygous variant(c.2126+5G>A),P6 contained a homozygous nonsense mutation(c.1720C>T,p.Arg574*),P8 contained compound heterozygous variants(c.H82-1184delATCf p.Leu394_Ser395deIinsPhe;c.368A>T,p.Hisl23Arg),and P9 contained a heterozygous variant(c.H82-1184delATCTTI frameshift).We also reported intracytoplasmic sperm injection(ICSI)outcomes in the related patients,finding that ICSI followed by assisted oocyte activation(AOA)with calcium ionophore achieved high rates of live births.In summary,the infertility of these patients results from DPY19L2 dysfunction and can be treated by ICSI together with AOA.展开更多
In recent years,conjugated polymers have attracted great attention in the application as photovoltaic donor materials in polymer solar cells(PSCs).Broad absorption,lower-energy bandgap,higher hole mobility,relatively ...In recent years,conjugated polymers have attracted great attention in the application as photovoltaic donor materials in polymer solar cells(PSCs).Broad absorption,lower-energy bandgap,higher hole mobility,relatively lower HOMO energy levels,and higher solubility are important for the conjugated polymer donor materials to achieve high photovoltaic performance.Side-chain engineering plays a very important role in optimizing the physicochemical properties of the conjugated polymers.In this article,we review recent progress on the side-chain engineering of conjugated polymer donor materials,including the optimization of flexible side-chains for balancing solubility and intermolecular packing(aggregation),electron-withdrawing substituents for lowering HOMO energy levels,and two-dimension(2D)-conjugated polymers with conjugated side-chains for broadening absorption and enhancing hole mobility.After the molecular structural optimization by side-chain engineering,the2D-conjugated polymers based on benzodithiophene units demonstrated the best photovoltaic performance,with powerconversion efficiency higher than 9%.展开更多

文摘Objective Non-suicidal self-injury(NSSI)has a higher prevalence in adolescents with depressive disorders than in community adolescents.This study examined the differences in NSSI behaviors between adolescents with unipolar depression(UD)and those with bipolar depression(BD).Methods Adolescents with UD or BD were recruited from 20 general or psychiatric hospitals across China.The methods,frequency,and function of NSSI were assessed by Functional Assessment of Self-Mutilation.The Beck Suicide Ideation Scale was used to evaluate adolescents’suicidal ideation,and the 10-item Kessler Psychological Distress Scale to estimate the anxiety and depression symptoms.Results The UD group had higher levels of depression(19.16 vs.17.37,F=15.23,P<0.001)and anxiety symptoms(17.73 vs.16.70,F=5.00,P=0.026)than the BD group.Adolescents with BD had a longer course of NSSI than those with UD(2.00 vs.1.00 year,Z=−3.39,P=0.001).There were no statistical differences in the frequency and the number of methods of NSSI between the UD and BD groups.Depression(r=0.408,P<0.01)and anxiety(r=0.391,P<0.01)were significantly and positively related to NSSI frequency.Conclusion Adolescents with BD had a longer course of NSSI than those with UD.More importantly,NSSI frequency were positively and strongly correlated with depression and anxiety symptoms,indicating the importance of adequate treatment of depression and anxiety in preventing and intervening adolescents’NSSI behaviors.

文摘BACKGROUND Patients with foreign bodies in the digestive tract are often encountered,but complete penetration of a foreign body through the gastrointestinal tract is rare,and the choice of imaging method is very important.Improper selection may lead to missed diagnosis or misdiagnosis.CASE SUMMARY An 81-year-old man was diagnosed as having a liver malignancy after he took magnetic resonance imaging and positron emission tomography/computed tomography(CT)examinations.The pain improved after the patient accepted gamma knife treatment.However,he was admitted to our hospital 2 mo later due to fever and abdominal pain.This time,he received a contrast-enhanced CT scan,which showed fish-boon-like foreign bodies in the liver with peripheral abscess formation,then he went to the superior hospital for surgery.It lasted for more than 2 mo from the onset of the disease to the surgical treatment.A 43-year-old woman with a 1 mo history of a perianal mass with no obvious pain or discomfort was diagnosed as having an anal fistula with the formation of a local small abscess cavity.Clinical perianal abscess surgery was performed,and fish bone foreign body was found in perianal soft tissue during the operation.CONCLUSION For patients with pain symptoms,the possibility of foreign body perforation should be considered.Magnetic resonance imaging is not comprehensive and that a plain computed tomography scan of the pain area is necessary.
文摘BACKGROUND Pulmonary hypertension is a rare cardiopulmonary disease,with an insidious onset that usually worsens rapidly.Amyloid light chain(AL)amyloidosis is a rare systemic disease caused by extracellular deposition of pathologic,insoluble,and proteinaceous fibrils in organs and tissues;however,it is difficult to diagnose given its varied and nonspecific symptoms.To date,rare cases of amyloidosis with pulmonary hypertension have been reported.Of note,the optimal treatments for cardiac amyloidosis complicated with pulmonary hypertension remain unclear.CASE SUMMARY We report a case of a 51-year-old woman who presented with progressively worsening dyspnea.Transthoracic echocardiography indicated severe pulmonary hypertension.Twenty-seven months after first admission,the patient returned with symptoms of progressive heart failure.A myocardial tissue sample stained with Congo red was positive,and the patient was ultimately diagnosed with AL amyloidosis with cardiac involvement.CONCLUSION Although pulmonary hypertension may be idiopathic,it is frequently associated with other conditions.In rare cases,pulmonary hypertension can be a complication of AL amyloidosis,which should be seriously considered in any adult presenting with nonspecific signs or symptoms of cardiac distress.

文摘In the present research, high chromium cast irons(HCCIs) were prepared using the lost foam casting(LFC) process. To improve the wear resistance of the high chromium cast irons(HCCIs), mechanical vibration was employed during the solidification of the HCCIs. The effects of vibration frequency on the microstructure and performance of the HCCIs under as-cast, as-quenched and as-tempered conditions were investigated. The results indicated that the microstructures of the LFC-produced HCCIs were refined due to the introduction of mechanical vibration, and the hardness was improved compared to that of the alloy without vibration. However, only a slight improvement in hardness was found in spite of the increase of vibration frequency. In contrast, the impact toughness of the as-tempered HCCIs increased with an increase in the vibration frequency. In addition, the wear resistance of the HCCIs was improved as a result of the introduction of vibration and increased with an increase in the vibration frequency.
文摘Based on the outpatient interview and literature review, the initial framework of the outpatient experience of human caring scale was formed with 9 dimensions of outpatient process. The research aim was to improve the scale by Delphi method. Sixteen experts in medical management, human caring or medical education were invited to evaluate the importance of the dimensions and items of the scale and provided some expertise via filling out the DeLphi consultation questionnaires twice in the consulting round. In the first round, the recovery rate showing the experts' positivity was 80%; the coefficient of reliability (Cr) ascertaining the authority of the evaluation was 0.92; the mean and full mark ratios responding the concentration of the evaluation were 2.88-4.94 and 6.25%-93.75% respectively; the coefficients of variation (CV) and the Kendall's W determining the concordance of the evaluation were 5.06%-52.15% and 0.21-0.24 respectively. In the second round, the recovery rate was 93.75%; the Cr was 0.93; the mean was 3.93-4.93; the full mark ratios were 26.67%-93.33%; the Kendall's W was 0.14- 0.31, the CV was 5.25%-23.61%. Via the two-round Delphi study, the scale that included 10 dimensions and 61 items has been improved. Ten dimensions are pre-hospital medical service, guidance, registration, waiting, diagnosis & treatment, paying, inspection & assay, medicine receiving, therapy/injection/transfusion and global evaluation. It was concluded that Chinese scholars have paid high attention to human caring and outpatient experience. The experts have given high agreements about the dimensions which were established with Chinese outpatient process. The dimensions are different from the similar researches about outpatient experience study. In the future, it is necessary to survey the outpatients to test the construct validity, internal consistency reliability and others of the scale to improve the scale.

文摘BACKGROUND In early gastric cancer(GC),tumor markers are increased in the blood.The levels of these markers have been used as important indexes for GC screening,early diagnosis and prognostic evaluation.However,specific tumor markers have not yet been discovered.Diagnosis based on a single tumor marker has limited significance.The detection rate of GC is still very low.AIM To improve the diagnostic value of blood markers for GC.METHODS We used a multiparameter joint analysis of 77 indexes of malignant GC and gastric polyp(GP),64 indexes of GC and healthy controls(Ctrls).RESULTS By analyzing the data,there are 27 indexes in the final Ctrls vs GC with P values<0.01,the area under the curve(AUC)of albumin is the largest in Ctrls vs GC,and the AUC was 0.907.30 indexes in GP vs GC have P values<0.01.Among them,the D-dimer showed an AUC of 0.729.The 27 indexes in Ctrls vs GC and 30 indexes in GP vs GC were used for binary logistic regression,discriminant analysis,classification tree analysis and artificial neural network analysis model.For the ability to distinguish between Ctrls vs GC,GP vs GC,artificial neural networks had better diagnostic value when compared with classification tree,binary logistic regression,and discriminant analysis.When compared Ctrl and GC,the overall prediction accuracy was 92.9%,and the AUC was 0.992(0.980,1.000).When compared GP and GC,the overall prediction accuracy was 77.9%,and the AUC was 0.969(0.948,0.990).CONCLUSION The diagnostic effect of multi-parameter joint artificial neural networks analysis is significantly better than the single-index test diagnosis,and it may provide an assistant method for the detection of GC.

文摘BACKGROUND Early detection,early diagnosis,and early treatment are currently accepted methods that can effectively improve the efficacy of colorectal cancer(CRC)treatment.Exosomes were demonstrated to be potential tumor molecular markers.AIM To evaluate the diagnostic value of CRC by detecting four exosomal microRNAs(miRNAs)(miR-15b,miR-16,miR-21,and miR-31)that were demonstrated to have potential diagnostic value in serum.METHODS Relative expression levels of miR-15b,miR-16,miR-21,and miR-31 in 123 CRC,117 colorectal adenoma,and 150 healthy controls were detected,and single and panel models were evaluated.The 2-ΔΔCt method was used to calculate the relative expression of miRNA compared to the internal control(U6).Eighty-one CRC patients,67 colorectal adenoma patients,and 90 healthy controls were used for validation.RESULTS Compared to the healthy control group,the best indicator of the four miRNAs was miR-15b,and the sensitivity and specificity were 81.33%and 91.80%,respectively.For miR-15b,miR-21,and miR-31 individually,the sensitivity and specificity were 91.95%and 97.62%,95.06%and 94.44%,respectively.Compared to the colorectal adenoma group,miR-15b,miR-16,and miR-21 in the CRC group showed significant differences(P<0.05).The best single indicator was miR-16,with a sensitivity and specificity of 79.05%and 71.55%.The sensitivity and specificity of a panel that included miR-15b,miR-16,and miR-21 were 81.21%and 81.03%,and 85.19%and 82.09%,respectively,in the validation.CONCLUSION We built and validated a diagnostic model containing miR-15b,miR-21,and miR-31 expression levels to discriminate the healthy control group and CRC group,and its sensitivity and specificity were 95.06%and 94.44%,respectively.The miR-15b,miR-16,and miR-21 panel was used to discriminate the colorectal adenoma group and CRC group with a sensitivity and specificity of 85.19%and 82.09%,respectively.

文摘Cyano substitution has been established as a viable approach to optimize the performance of all-small-molecule organic solar cells.However,the effect of cyano substitution on the dynamics of photo-charge generation remains largely unexplored.Here,we report an ultrafast spectroscopic study showing that electron transfer is markedly promoted by enhanced intermolecular charge-transfer interaction in all-small-molecule blends with cyanided donors.The delocalized excitations,arising from intermolecular interaction in the moiety of cyano-substituted donor,undergo ultrafast electron transfer with a lifetime of∼3 ps in the blend.In contrast,some locally excited states,surviving in the film of donor without cyano substitution,are not actively involved in the charge separation.These findings well explain the performance improvement of devices with cyanided donors,suggesting that manipulating intermolecular interaction is an efficient strategy for device optimization.
文摘In order to study the soil enzyme content at the mine reclamation area and choose a tree species with superior restoration capabilities, this paper takes Huainan Datong Mine as the study area, and five different enzymes under nine tree species as the study subject. The different enzyme activity indexes were measured, and the correlation analysis and the principal component analysis (PCA) method were applied to evaluate and screen the tree species with advanced restoration. The results demonstrate that there are some correlations among the different soil enzymes, including some very significant positive correlations among urease, phosphatase, invertase and protease. The best species in terms of repair is privet, and the worst is Haltong. This study provides a scientific basis for the selection of restoration-capable tree species in the reclamation area of the coal mine.

文摘The effect of disturbed flow on the mass trans- fer from arterial surface to flowing blood was studied nu- merically, and the results were compared with that of our previous work. The arterial wall was assumed to be vis- coelastic and the blood was assumed to be incompressible and non-Newtonian fluid, which is more close to human arte- rial system. Numerical results indicated that the mass trans- fer from the arterial surface to flowing blood in regions of disturbed flow is positively related with the wall shear rates and it is significantly enhanced in regions of disturbed flow with a local minimum around the reattachment point which is higher than the average value of the downstream. There- fore, it may be implied that the accumulation of cholesterol or lipids within atheromatous plaques is not caused by the reduced efflux of cholesterol or lipids, but by the infiltration of the LDL (low-density lipoprotein) from the flowing blood to the arterial wall.

文摘The deformation behavior and microstructure of the Zr50Ti50 alloy in β phase field were investigated by isothermal compression tests at temperatures ranging from 700 to 850 °C and strain rates ranging from 0.001 to 1 s?1. The flow curves exhibited typical flow softening. The initial discontinuous yielding behavior was observed at higher strain rates, which was not found in other traditional Zr alloys. The apparent deformation activation energy was calculated to be 103 kJ/mol and constitutive equationdescribing the flow stress as a function of the strain rate and deformation temperature was proposed. The analysis indicated that the hot deformation mechanism was mainly dominated by dynamic recovery. However, dynamic recrystallization was delayed by dynamic recovery. Thereafter, the processing map was calculated to evaluate the efficiency of the forging process at the temperatures and strain rates investigated and to optimize processing parameters of hot deformation. The optimum processing parameters were found to be 830?850 °C and 0.56?1 s?1 for hot the deformation of Zr50Ti50 alloy in the β phase region.

文摘The photodissociation dynamics of acetaldehyde in the radical channel CH3+HCO has been reinvestigated using time-sliced velocity map imaging technique in the photolysis wavelength range of 275-321 nm. The CH3 fragments have been probed via (2+1) resonance-enhanced multiphoton ionization. Images are measured for CH3 formed in the ground and excited states (v2=0 and 1) of the umbrella vibrational mode. For acetaldehyde dissociation on T1 state after intersystem crossing from S1 state, the products are formed with high translational energy release and low internal excitation. The rotational and vibrational energy of both fragments increases with increasing photodissociation energy. The triplet barrier height is estimated at 3.8814-0.006 eV above the ground state of acetaldehyde.

文摘The NH(a^1Δ)+CO(X^1Σ+) product channel for the photodissociation of isocyanic acid(HNCO) on the first excited singlet state S1 has been investigated by means of time-sliced ionvelocity map imaging technique at photolysis wavelengths around 201 nm. The CO productwas detected through (2+1) resonance enhanced multiphoton ionization (REMPI). Imageswere obtained for CO products formed in the ground and vibrational excited state (v=0 and v=1). The energy distributions and product angular distributions were obtained from the CO velocity imaging. The correlated NH(a^1Δ) rovibrational state distributions were determined.The vibrational branching ratio of 1NH (v=1/v=0) increases as the rotational state of CO(v=0) increases initially and decreases afterwards, which indicates a special state-tostate correlation between the 1NH and CO products. About half of the available energy was partitioned into the translational degree of freedom. The negative anisotropy parameter indicates that it is a vertical direct dissociation process.

文摘Side-chain engineering has emerged as a highly effective strategy for tailoring the aggregation behavior and charge transport properties of non-fullerene small molecule acceptors(SMAs).In this study,we designed and synthesized two SMAs,namely BTPSi-Bu and BTPSi-Pr,respectively incorporating tributylsilyloxy and trisopropylsilyloxy groups in their outer positions.Notably,BTPSi-Bu exhibited better planarity,crystallization,and favorable phase separation when paired with PM6 donor polymer compared to its counterpart,BTPSi-Pr.The resulting organic solar cells,utilizing the PM6:BTPSi-Bu blend,demonstrated a remarkable power conversion efficiency of 17.41%and a high open-circuit voltage of 0.859 V.These findings underscore the significance of integrating trialkylsilyloxy side chains into SMAs as a rational design approach for enhancing the performance of photovoltaic systems.

文摘Background Due to the broad application of next-generation sequencing,the molecular diagnosis of genetic disorders in pediatric neurology is no longer an unachievable goal.However,treatments for neurological genetic disorders in children remain primarily symptomatic.On the other hand,with the continuous evolution of therapeutic viral vectors,gene therapy is becoming a clinical reality.From this perspective,we wrote this review to illustrate the current state regarding viral-mediated gene therapy in childhood neurological disorders.Data sources We searched databases,including PubMed and Google Scholar,using the keywords"adenovirus vector,""lentivirus vector,"and"AAv"for gene therapy,and"immunoreaction induced by gene therapy vectors,""administration routes of gene therapy vectors,"and"gene therapy"with"NCL,""SMA,""DMD,""congenital myopathy,""MPS""leukodystrophy,"or"pediatric metabolic disorders".We also screened the database of ClinicalTrials.gov using the keywords"gene therapy for children"and then filtered the results with the ones aimed at neurological disorders.The time range of the search procedure was from the inception of the databases to the present.Results We presented the characteristics of commonly used viral vectors for gene therapy for pediatric neurological disorders and summarized their merits and drawbacks,the administration routes of each vector,the research progress,and the clinical application status of viral-mediated gene therapy on pediatric neurological disorders.Conclusions Viral-mediated gene therapy is on the brink of broad clinical application.Viral-mediated gene therapy will dramatically change the treatment pattern of childhood neurological disorders,and many children with incurable diseases will meet the dawn of a cure.Nevertheless,the vectors must be optimized for better safety and efficacy.

文摘Herein,we fabricated all-polymer solar cells(all-PSCs)based on a fluorinated wide-bandgap p-type conjugated polymer PM6 as the donor,and a narrow bandgap n-type conjugated polymer PZ1 as the acceptor.In addition to the complementary absorption and matching energy levels,the optimized blend films possess high cystallinity,predominantly face-on stacking,and a suitable phase separated morphology.With this active layer,the devices exhibited a high Vocof 0.96 V,a superior Jscof 17.1 mA cm^-2,a fine fill factor(FF)of 68.2%,and thus an excellent power conversion efficiency(PCE)of 11.2%,which is the highest value reported to date for single-junction all-PSCs.Furthermore,the devices showed good storage stability.After 80 d of storage in the N2-filled glovebox,the PCE still remained over 90%of the original value.Large-area devices(1.1 cm^2)also demonstrated an outstanding performance with a PCE of 9.2%,among the highest values for the reported large-area all-PSCs.These results indicate that the PM6:PZ1 blend is a promising candidate for scale-up production of large area high-performance all-PSCs.
文摘Globozoospermia has been reported to be a rare but severe causation of male infertility,which results from the failure of acrosome biogenesis and sperm head shaping.Variants of dpy-19-like 2(DPY19L2)are highly related to globozoospermia,but related investigations have been mainly performed in patients from Western countries.Here,we performed a screening of DPY19L2 variants in a cohort of Chinese globozoospermic patients and found that five of nine patients carried DPY19L2 deletions and the other four patients contained novel DPY19L2 point mutations,as revealed by whole-exome sequencing.Patient 3(P3)contained a heterozygous variant(c.2126+5G>A),P6 contained a homozygous nonsense mutation(c.1720C>T,p.Arg574*),P8 contained compound heterozygous variants(c.H82-1184delATCf p.Leu394_Ser395deIinsPhe;c.368A>T,p.Hisl23Arg),and P9 contained a heterozygous variant(c.H82-1184delATCTTI frameshift).We also reported intracytoplasmic sperm injection(ICSI)outcomes in the related patients,finding that ICSI followed by assisted oocyte activation(AOA)with calcium ionophore achieved high rates of live births.In summary,the infertility of these patients results from DPY19L2 dysfunction and can be treated by ICSI together with AOA.

文摘In recent years,conjugated polymers have attracted great attention in the application as photovoltaic donor materials in polymer solar cells(PSCs).Broad absorption,lower-energy bandgap,higher hole mobility,relatively lower HOMO energy levels,and higher solubility are important for the conjugated polymer donor materials to achieve high photovoltaic performance.Side-chain engineering plays a very important role in optimizing the physicochemical properties of the conjugated polymers.In this article,we review recent progress on the side-chain engineering of conjugated polymer donor materials,including the optimization of flexible side-chains for balancing solubility and intermolecular packing(aggregation),electron-withdrawing substituents for lowering HOMO energy levels,and two-dimension(2D)-conjugated polymers with conjugated side-chains for broadening absorption and enhancing hole mobility.After the molecular structural optimization by side-chain engineering,the2D-conjugated polymers based on benzodithiophene units demonstrated the best photovoltaic performance,with powerconversion efficiency higher than 9%.
