Objective: High-dose chemotherapy (HDC) followed by autologous stem cell transplantation (ASCT) is the standard of care in the upfront or relapsed/refractory setting in some patients with non-Hodgkin lymphoma (NHL). However, a proportion of patients do not respond to ASCT. lSF-fluorodeoxyglueose (FDG) positron emission tomography (PET)/computed tomography (CT) has been widely used for staging, response evaluation, and prognosis prediction. Here, we investigated the prognostic role of PET/CT in NHL patients before and after ASCT. Methods: A retrospective study was conducted at Peking University Cancer Hospital. All NHL patients who underwent ASCT between March 2010 and July 2016 were identified. Patients who had PET/CT scan before and after ASCT were included. Deauville criteria (5-point scale) were used to interpret PET scans. Univariate and multivariate survival analyses were performed using Cox regression. The predictive value of PET scanning was estimated by comparing the area under the receiver operating characteristic (ROC) curve. Results: In total, 79 patients were enrolled in this study. In univariate analysis, pre- and post-ASCT PET result was identified as prognostic factors for 3-year progression-free survival (PFS) and overall survival (OS). Patients with negative pre-ASCT PET result demonstrated significantly better PFS (84.2% vs. 54.2%) and OS (89.2% vs. 63.6%) than patients with positive pre-ASCT PET result. PFS (91.6% vs. 25.3%) and OS (96.5% vs. 36.8%) were also significantly different between patients with negative and positive post-ASCT PET result. Multivariate analysis also showed a significant association between survival and post-ASCT PET result. ROC analysis revealed that the predictive value of post-ASCT PET result was superior to that of pre-ASCT PET result alone. Combined pre- and post-ASCT PET result is better for predicting outcomes in patients with NHL receiving transplantation. Deauville criteria score 〉3 was identified as the best cutoffvalue for post-ASCT PET. Conclusions: Post-ASCT PET result was more important than pre-ASCT PET result in predicting outcomes for NHL patients who underwent ASCT. The prognostic significance can be improved when combining pre- ASCT PET result with post-ASCT PET result. Deauville criteria can be used for interpreting PET scans in this scenario. 展开更多

AIM:To investigate the capacity for 18F-fluorodeoxyglucose(18F-FDG) positron emission tomography(PET) to evaluate patients with gastrointestinal lesions of follicular lymphoma.METHODS:This retrospective case series consisted of 41 patients with follicular lymphoma and gastrointestinal involvement who underwent 18F-FDG-PET and endoscopic evaluations at ten different institutions between November 1996 and October 2011.Data for endoscopic,radiological,and biological examinations performed were retrospectively reviewed from clinical records.A semi-quantitative analysis of 18F-FDG uptake was performed for each involved area by calculating the maximum standardized uptake value(SUVmax).Based on the positivity of 18F-FDG uptake in the gastrointestinal lesions analyzed,patients were subdivided into two groups.To identify potential predictive factors for 18F-FDG positivity,these two groups were compared with respect to gender,age at diagnosis of lymphoma,histopathological grade,pattern of follicular dendritic cells,mitotic rate,clinical stage,soluble interleukin-2 receptor levels detected by 18F-FDG-PET,lactate dehydrogenase(LDH) levels,hemoglobin levels,bone marrow involvement,detectability of gastrointestinal lesions by computed tomography(CT) scanning,and follicular lymphoma international prognostic index(FLIPI) risk.RESULTS:Involvement of follicular lymphoma in the stomach,duodenum,jejunum,ileum,cecum,colon,and rectum was identified in 1,34,6,3,2,3,and 6 patients,respectively.No patient had esophageal involvement.In total,19/41(46.3%) patients exhibited true-positive 18F-FDG uptake in the lesions present in their gastrointestinal tract.In contrast,false-negative 18F-FDG uptake was detected in 24 patients(58.5%),while false-positive 18F-FDG uptake was detected in 5 patients(12.2%).In the former case,2/19 patients had both 18F-FDG-positive lesions and 18F-FDGnegative lesions in the gastrointestinal tract.In patients with 18F-FDG avidity,the SUVmax value of the involved gastrointestinal tract ranged from 2.6 to 17.4(median:4.7).For the 18F-FDG-negative(n = 22) and-positive(n = 19) groups,there were no differences in the male to female ratios(10/12 vs 4/15,P = 0.186),patient age(63.6 ± 2.4 years vs 60.1 ± 2.6 years,P = 0.323),presence of histopathological grade 1 vs 2(20/2 and 17/2,P = 1.000),follicular dendritic cell pattern(duodenal/nodal:13/5 vs 10/3,P = 1.000),mitotic rate(low/partly high,14/1 vs 10/3,P = 0.311),clinical stage according to the Ann Arbor system(stages ⅠE and ⅡE/other,15/7 vs 15/4,P = 0.499),clinical stage according to the Lugano system(stages Ⅰ and Ⅱ-1/other,14/8 vs 14/5,P = 0.489),soluble interleukin-2 receptor levels(495 ± 78 vs 402 ± 83,P = 0.884),LDH levels(188 ± 7 vs 183 ± 8,P = 0.749),hemoglobin levels(13.5 ± 0.3 vs 12.8 ± 0.4,P = 0.197),bone marrow involvement(positive/negative,1/8 vs 1/10,P = 1.000),detectability by CT scanning(positive/negative,1/16 vs 4/13,P = 0.335),and FLIPI risk(low risk/other,16/6 vs 13/6,P = 0.763),respectively in each case.CONCLUSION:These findings indicate that it is not feasible to predict 18F-FDG-avidity.Therefore,18FFDG-PET scans represent a complementary modality for the detection of gastrointestinal involvements in follicular lymphoma patients,and surveillance of the entire gastrointestinal tract by endoscopic examinations is required. 展开更多

BACKGROUND Peripheral FDG accumulation in a hepatic hemangioma presenting in a patient with prolonged fever is rare.Therefore,clinicians should pay close attention to patients with hepatic mass.CASE SUMMARY A 54-year-old woman with a 4-wk history of daily fevers was admitted to our hospital.A whole body^(18)-Fluordesoxyglucose(PET-FDG)positron emission tomography/computed tomography(PET/CT)was performed to elucidate the source of the fever.However,whole body^(18)-FDG PET/CT raised the suspicion of a malignant lesion because of peripheral FDG accumulation(SUVmax 3.5 g/mL)higher than that of the normal liver parenchyma(SUVmax 1.6 g/mL)surrounding a hypoactive area,and no other abnormalities were showed.Subsequently,the patient underwent liver mass resection.Histopathology showed a hepatic cavernous hemangioma with fatty infiltration around the lesion.The fever disappeared four days after surgery and the patient did not present any complications during follow-up.CONCLUSION Fatty infiltration in the peripheral parts of hepatic cavernous hemangioma may lead to subacute inflammation which further activate the Kupffer cells.This may cause prolonged fever and peripheral rim FDG accumulation on PET/CT. 展开更多

AIM: To evaluate the clinical role of 18F-fluorodeo-xyglucose positron emission and computed tomography (18F-FDG PET/CT) in detection of gastric cancer recurrence after initial surgical resection. METHODS: In the period from January 2007 to May 2008, 23 patients who had previous surgical resection of histopathologically diagnosed gastric cancer underwent a total of 25 18F-FDG PET/CT scans as follow-up visits in our center. The standard of reference for tumor recurrence consisted of histopathologic confirmation or clinical follow-up information for at least 5 mo after PET/CT examinations. RESULTS: PET/CT was positive in 14 patients (61%) and negative in 9 (39%). When correlated with final diagnosis, which was confirmed by histopathologic evidence of tumor recurrence in 8 of the 23 patients (35%) and by clinical follow-up in 15 (65%), PET/CT was true positive in 12 patients, false positive in 2, true negative in 8 and false negative in 2. Overall, the accuracy of PET/CT was 82.6%, the negative predictive value (NPV) was 77.7%, and the positive predictive value (PPV) was 85.7%. The 2 false positive PET/CT findings were actually chronic inflammatory tissue lesions. For the two patients with false negativePET/CT, the f inal diagnosis was recurrence of mucinous adenocarcinoma in the anastomosis in one patient and abdominal wall metastasis in the other. Importantly, PET/CT revealed true-positive findings in 11 (47.8%) patients who had negative or no definite findings by CT. PET/CT revealed extra-abdominal metastases in 7 patients and additional esophageal carcinoma in one patient. Clinical treatment decisions were changed in 7 (30.4%) patients after introducing PET/CT into their conventional post-operative follow-up program. CONCLUSION: Whole body 18F-FDG PET/CT was highly effective in discriminating true recurrence in post-operative patients with gastric cancer and had important impacts on clinical decisions in a considerable portion of patients. 展开更多

AIM To compare ^(18)F-fluorodeoxyglucose positron emission tomography/computed tomography(^(18)F-FDG PET/CT) features in gastric lymphoma and gastric carcinoma.METHODS Patients with newly diagnosed gastric lymphoma or gastric carcinoma who underwent ^(18)F-FDG PET/CT prior to treatment were included in this study. We reviewed and analyzed the PET/CT features of gastric wall lesions,including FDG avidity,pattern(focal/diffuse),and intensity [maximal standard uptake value:(SUVmax)]. The correlation of SUVmax with gastricclinicopathological variables was investigated by χ~2 test,and receiver-operating characteristic(ROC) curve analysis was performed to determine the differential diagnostic value of SUVmax-associated parameters in gastric lymphoma and gastric carcinoma. RESULTS Fifty-two patients with gastric lymphoma and 73 with gastric carcinoma were included in this study. Abnormal gastric FDG accumulation was found in 49 patients(94.23%) with gastric lymphoma and 65 patients(89.04%) with gastric carcinoma. Gastric lymphoma patients predominantly presented with type Ⅰ and type Ⅱ lesions,whereas gastric carcinoma patients mainly had type Ⅲ lesions. The SUVmax(13.39 ± 9.24 vs 8.35 ± 5.80,P < 0.001) and SUVmax/THKmax(maximal thickness)(7.96 ± 4.02 vs 4.88 ± 3.32,P < 0.001) were both higher in patients with gastric lymphoma compared with gastric carcinoma. ROC curve analysis suggested a better performance of SUVmax/THKmax in the evaluation of gastric lesions between gastric lymphoma and gastric carcinoma in comparison with that of SUVmax alone.CONCLUSION PET/CT features differ between gastric lymphoma and carcinoma,which can improve PET/CT evaluation of gastric wall lesions and help differentiate gastric lymphoma from gastric carcinoma. 展开更多

AIM: To investigate the correlation of 18F-fluorodeoxy- glucose (18F-FDG) positron emission tomography (PET) with clinical features and the prediction of treatment response. METHODS: A total of 83 hepatocellular carcinoma (HCC) patients undergoing 18F-FDG PET before transar- terial chemolipiodolization with systemic chemo-infusion between October, 2006 and May, 2009 were retrospec-tively enrolled. The patients included 68 men and 15 women (mean age, 60 ~ 10.7 years). The effect of 18F- FDG-monitored PET uptake on clinical features and on the evaluated treatment response was ascertained with modified Response Evaluation Criteria in Solid Tumors. The PET parameters of maximal standardized uptake value of the tumor (Tsuvmax), the ratio of the tumor maximal standardized uptake value (SUV) to the liver maximal SUV (Tsuvmax/Lsuwax) and the ratio of tumor maximal SUV to the liver mean SUV (msuvmax/LSUVrnean) were tested as predictive factors. RESULTS: Among the 3 SUV parameters, the TSUV- =maxdLsuvmean ratio (cutoff value of 1.90) was significantly associated with tumor burden including tumor size, tu- mor number, α-fetoprotein levels and tumor stage (P 〈 0.001, P = 0.008, P = 0.011, P 〈 0.001, respectively). The objective response rates in patients with a high SUV ratio (≥ 1.90) were significantly better than those with a low SUV ratio (〈 1.90) (P = 0.020). The overall survival rates of patients exhibiting a low Tsuvmax/Lsu- Vmean ratio (〈 1.90) and those with a high SUV ratio (≥1.90) was 38.2 and 10.3 mo, respectively (P 〈 0.01). However, the time to progression showed no significant difference between the groups (P = 0.15). CONCLUSION: 18F-FDG PET can be an important predictor of HCC treatment. In particular, the Tsuvmax/ Lsuwean ratio (cutoff value of 1.90) can provide useful information in treatment prognosis for HCC patients treated with Iocoregional therapy. 展开更多

Portal vein tumor thrombus must be distinguished from portal vein blood thrombus, and this identification plays a very important role in management of HCC. Conventional imaging modalities have limitations in discrimination of portal vein tumor thrombus. The application of positron emission tomography (PET) with 18F-fluorodeoxyglucose (18F-FDG) for discrimination between tumor extension and blood thrombus has been reported in few cases of HCC, while portal tumor thrombosis and portal vein clot identified by 18F-FDG PET/CT in HCC patients has not been reported so far. We present two HCC cases, one with portal vein tumor thrombus and one thrombosis who were identified with 18F-FDG PET/CT. This report illustrates the complimentary value of combining the morphological and functional imaging in achieving a correct diagnosis in such clinical situations. 展开更多

Objective: The DNA repair capacity (DRC) of tumor cells is an important contributor to resistance to radiation and platinum-based drugs. Because DRC may be affected by tumor cell metabolism, we measured DRC in lymphocytes from patients with non-small-cell lung cancer (NSCLC) and compared the findings with the maximum standardized uptake value (SUVmax) on 18 F-fluorodeoxyglucose positron emission tomography (FDG PET) after (chemo)radiation therapy. Methods: This study included 151 patients with stage IA-IV NSCLC who had FDG PET at a single institution and donated blood samples before chemotherapy. We assessed the correlation of DRC, measured in peripheral T lymphocytes by a host-cell reac-tivation assay with SUVmax and their associations with overall survival (OS) time by hazards ratios calculated with a Cox pro-portional hazards regression model. Results: SUVmax of the primary tumor at diagnosis was inversely associated with lymphocyte DRC (r=-0.175, P=0.032), particularly among patients with advanced disease (r = -0.218, P = 0.015). However, △SUVmax of primary tumor was not significantly associated with DRC (r=0.005, P=0.968). SUVmax of regional lymph nodes at diagnosis (r=-0.307, P=0.0008) and after (chemo)radiation treatment (r=-0.329, P=0.034) and SUVmax of the primary tumor after (chemo)radiation treatment (r=-0.253, P=0.045) were also inversely associated with OS time. Conclusion: DRC was inversely associated with primary tumor SUVmax before treatment but not with △SUVmax after (chemo)radiation. 展开更多

We report on two cases of patients with fat-equivalent masses in computed tomography(CT),referred to our department for dynamic positron emission tomography/CT(dPET/CT)with18F-fluorodeoxyglucose(18FFDG)in order to investigate their dignity.Both qualitative and quantitative information,as derived from dPET/CTs,couldn’t exclude a high-grade liposarcoma:Visual evaluation,revealed a large hypermetabolic focus of intense18F-FDG uptake in each patient(average SUVs 8.3 and 11.3).Regression-based parametric imaging demonstrated an enhanced distribution volume,which correlates to perfusion,and a high phosphorylation rate that correlates to cell viability.Kinetic analysis,based on a two-tissue compartment model demonstrated an enhanced FDG transport k1and an enhanced phosphorylation rate k3.A non-compartmental approach based on fractal dimension revealed also enhanced values.However,final diagnosis was based on biopsy,which revealed hibernoma,a benign brown fat tumor.Brown adipose contains increased numbers of mitochondria and a high-rate of glucose metabolism.Therefore,they have increased FDG uptake.The evaluation of lipomatous lesions on CT,with high FDG uptake,should include the possibility of hibernoma as a differential diagnosis. 展开更多

RESULTS:The tumors ranged from 1.3 to 11.0 cm in diameter.Thirty of the 31 patients(97%) had a positive FDG-PET study.There were 5 patients classified as TS1,15 as TS2 and 11 as TS3-4.The sensitivity of FDG-PET,computed tomography(CT) and magnetic resonanceimaging(MRI) were 100%,40%,0%in TS1,93%,93%,89%in TS2 and 100%,100%,100%in TS3-4. The sensitivity of FDG-PET was significantly higher in comparison to CT and MRI in patients with TS1(P< 0.032) .The mean standardized uptake values(SUVs) did not show a significant difference in relation to the TS(TS1:5.8±4.5,TS2:5.7±2.2,TS3-4:8.2±3.9) ,respectively.All the TS1 tumors(from 13 to 20 mm) showed higher SUVs in FDG-PET with dual time point evaluation in the delayed phase compared with the early phase,which suggested the lesions were malignant. CONCLUSION:These results indicate that FDG-PET with dual time point evaluation is a useful modality for the detection of small pancreatic cancers with a diameter of less than 20 mm. 展开更多

Background ^18F-fluorodeoxyglucose (FDG) is the most widely used radiotracer in tumor imaging,but its use for brain gliomas and recurrence is limited by the high ^18F-FDG uptake in normal brain tissue.^11C-methionine (MET) has low uptake in the normal brain tissue,providing potential advantages over ^18F-FDG.The aim of the study was to investigate the diagnostic value of ^11C-MET compared to ^18F-FDG positron emission tomography and computed tomography (PET/CT) in patients with suspected primary and residual/recurrent gliomas.Methods Eighty paired PET/CT scans using ^11C-MET and 18F-FDG were performed on 44 newly diagnosed patients with suspected gliomas and 36 post-operative patients with suspected residual/recurrent tumors.PET/CT results were evaluated by visual and semiquantitative analysis.The sensitivity,specificity and accuracy for detection of gliomas and residual/recurrent tumors were calculated using visual analysis.Tumor to contralateral normal gray matter (T/G) ratio was calculated for semiquantitative analysis.Results Final pathology of the 44 newly diagnosed patients included 26 gliomas (14 high-grade and 12 low-grade tumors) and 18 non-glioma benign lesions.Residual/recurrent gliomas were verified in 28 patients and excluded in 8/36 post-operative patients by subsequent histopathologic examination and/or clinical follow-up for more than six months.The sensitivity,specificity and accuracy of 11C-MET PET/CT were 88.5%,83.3% and 86.4% for gliomas and 96.4%,87.5% and 94.4% for residual/recurrent gliomas,respectively.The sensitivity,specificity and accuracy of 18F-FDG PET/CT were 50.0%,88.9% and 65.9% for gliomas and 46.4%,100.0% and 58.3% for residual/recurrent gliomas,respectively.11C-MET had a higher sensitivity than 18F-FDG (83.3% vs.33.3%,P=0.031) in low-grade gliomas,but had no significant difference in sensitivity from 18F-FDG for high-grade gliomas (92.9% vs.64.3%,P=0.219).11C-MET T/G uptake ratios in high-grade gliomas,low-grade gliomas and benign lesions were 1.94±0.53,1.78±0.61 and 1.06±0.34,respectively.18F-FDG T/G uptake ratios in high-grade gliomas,low-grade gliomas and benign lesions were 1.05±0.37,0.66±0.14 and 0.63±0.17,respectively.Conclusions 11C-MET PET/CT is superior to 18F-FDG PET/CT in detecting and delineating gliomas and residual/recurrent tumors,especially low-grade gliomas and residual/recurrent lesions present in gray matter,but its role in non-invasive grading of the tumors is limited. 展开更多

Background Rheumatoid arthritis (RA) is a chronic,systemic autoimmune inflammatory disorder.Many methods have been used to observe the progress of RA.The purpose of this study was to observe the progress of RA in rats with 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT),magnetic resonance (MR) imaging and arthritis score,and analyze the relationships among different methods in evaluation of RA.Methods Sixteen healthy Sprague Dawley (SD) rats about 8-week old were randomly assigned to a RA group and a control group.Bovine type Ⅱ emulsified incomplete Freud's adjuvant was used to induce arthritis in the RA group.Arthritis score of the rats in two groups were recorded,and 18F-FDG PET/CT,MR imaging were performed both on the corresponding rats every 3 days.All the rats were sacrificed at week 5,and histopathological examination was performed on rat knees stained with haematoxylin and eosin.Results The arthritis score and the standard uptake value (SUV) of knee joints in RA rats increased with the progression of arthritis gradually.Both peaks of arthritis score and SUV appeared at 21 days after the first immune injection,then the arthritis score and SUV of knee joints decreased slowly.The arthritis scores of knee joints in RA rats were positively correlated with their SUV changes.The MR images were confirmed by the histopathological studies.Conclusion PET/CT can detect the earliest molecular metabolism changes of RA,and MR imaging can follow up the dynamical anatomical changes of RA,all of which indicated that PET/CT and MR imaging may be applied as useful tools to monitor the progress of RA. 展开更多

For decades, ^(18)F-fluorodeoxyglucose(FDG) positron emission tomography(PET) has been widely used for staging, predicting prognosis, and detecting cancer recurrence in various types of malignant diseases. Due to low sensitivity of FDG PET for detecting intrahepatic HCC lesions, the clinical value of FDG PET in HCC patients has been limited. However, recent studies with diverse analytic methods have shown that FDG PET has promising role in aiding management of HCC patients. In this review, we will discuss the clinical role of FDG PET for staging, predicting prognosis, and evaluating treatment response in HCC. Further, we will focus on recent clinical studies regarding implication of volumetric FDG PET parameters, the significance of FDG uptake in HCC for selecting treatment and predicting treatment response, and the use of radiomics of FDG PET in HCC. 展开更多

目的:探讨18 MHz彩色多普勒超声对黄斑前膜的诊断价值。方法:收集2020-01/2022-01期间在我院经眼底检查拟诊白内障和玻璃体混浊的患者44例80眼,分别通过光学相干断层扫描法(OCT)与18 MHz彩色多普勒超声对患眼进行检查,比较18 MHz彩色多... 目的:探讨18 MHz彩色多普勒超声对黄斑前膜的诊断价值。方法:收集2020-01/2022-01期间在我院经眼底检查拟诊白内障和玻璃体混浊的患者44例80眼,分别通过光学相干断层扫描法(OCT)与18 MHz彩色多普勒超声对患眼进行检查,比较18 MHz彩色多普勒超声与OCT对黄斑前膜诊断敏感度、特异性、准确度的差异。结果:经18 MHz彩色多普勒超声检测80眼中,62眼为黄斑前膜,18眼为非黄斑前膜;经OCT确诊确实为黄斑前膜的54眼,非黄斑前膜的13眼,漏诊5眼,误诊8眼;18 MHz彩色多普勒超声与OCT诊断一致性较高(Kappa=0.892,P<0.05);18 MHz彩色多普勒超声检测黄斑前膜敏感性为92%,特异性为62%,漏诊率为8%,误诊率为38%,正确率为84%;与OCT检测相比,18 MHz彩色多普勒超声检测特异性、正确率、阳性预测准确率、阴性预测准确率降低,误诊率升高(均P<0.05),诊断敏感性与漏诊率比较无差异(均P>0.05)。结论:18 MHz彩色多普勒超声对黄斑前膜病变具有一定鉴定价值,与OCT检验具有一致性。 展开更多

Positron emission tomography (PET) with <sup>18</sup>F-fluorodeoxyglucose (FDG) is a diagnostic tool to evaluate metabolic activity by measuring accumulation of FDG, an analogue of glucose, and has been widely used for detecting small tumors, monitoring treatment response and predicting patients’ prognosis in a variety of cancers. However, the molecular mechanism of FDG accumulation into tumors remains to be investigated. It is well-known that most cancers are metabolically active with elevated glucose metabolism, a phenomenon known as the Warburg effect. The underlying mechanisms for elevated glucose metabolism in cancer tissues are complex. Recent reports have indicated the potential of FDG-PET/CT scans in predicting mutational status (e.g., KRAS gene mutation) of colorectal cancer (CRC), which suggests that FDG-PET/CT scans may play a key role in determining therapeutic strategies by non-invasively predicting treatment response to anti-epidermal growth factor receptor (EGFR) therapy. However, the immune suppression state leads to the occurrence of lymphoproliferative disorder (MTX-LPD). The purpose of this study was to describe the findings of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) in MTX-LPD patients, and compare it with non-MTX-related malignant lymphoma (ML). Materials and Methods: We retrospectively reviewed 11 MTX-LPD patients (9 female, mean age 68.3 years) and 21 ML patients (7 female, mean age 60.6 years) with a histopathological diagnosis. FDG-PET imaging was performed using a standard oncology procedure. We assessed the disease distribution based on FDG-PET images and measured the maximum standardized up take values (SUVmax) for each region. Results: Mean values of SUVmax in MTX-LPD and ML were 14.6 and 17.2, respectively (p = 0.49). In MTX-LPD, 55 lesions met the Cotswold classification, consisting of 37 nodal and 18 extranodal lesions. In ML, 82 lesions were found, consisting of 68 nodal and 14 extranodal lesions. MTX-LPD showed a higher incident of the involvement in extranodal lesions throughout the whole body (p < 0.001). Conclusion: Because this disease occurs widely throughout the whole body, we need to pay attention to the less frequent sites as well when performing PET imaging in patients with MTX-LPD. 展开更多

背景:椎间盘中差异表达的RNA结合蛋白在椎间盘退变中发挥着关键作用,其中RNA结合蛋白KRT18水平的降低与椎间盘退行性病变相关,但其在椎间盘髓核细胞中的具体作用尚未完全确定。目的:探讨KRT18与mRNA及长链非编码RNA结合互作对椎间盘髓... 背景:椎间盘中差异表达的RNA结合蛋白在椎间盘退变中发挥着关键作用,其中RNA结合蛋白KRT18水平的降低与椎间盘退行性病变相关,但其在椎间盘髓核细胞中的具体作用尚未完全确定。目的:探讨KRT18与mRNA及长链非编码RNA结合互作对椎间盘髓核细胞的影响及机制。方法:对因腰部骨折或椎间盘退行性病变而接受椎间融合术的患者进行人体髓核组织取样获得正常髓核细胞和退变髓核细胞,并进行iRIP-seq、功能富集分析以及DNA微阵列分析,随后根据分析结果在髓核细胞中敲低KRT18,通过蛋白免疫印迹及qRt-PCR在蛋白和RNA水平检测相关基因水平的表达。结果与结论:通过iRIP-seq分析发现GUAAUC和AGCCUC序列中存在大量的KRT18结合位点,表明KRT18可参与调控RNA的转录、翻译、稳定性或在细胞信号传导途径中发挥作用。其能够与成熟的mRNA稳定结合,其中表达较高的基因包括CRLF1及IGFBP4等,同时与其结合的长链非编码RNA的峰值基因包括SNHG25、SNHG12、NEAT1、USP32、EIF4A2和CDH4,这些基因多涉及细胞凋亡、炎症等多种生物过程,并且能介导细胞外基质代谢的相关通路,KRT18能够调控它们的稳定性、转运、翻译、剪接等多个方面的功能,进而影响基因的表达和细胞功能。实验结果验证了在髓核细胞中敲低KRT18,细胞外基质代谢水平被抑制出现失衡,导致体外椎间盘退变。该研究首次从KRT18与mRNA及长链非编码RNA结合的角度探讨其调控机制,并推测了KRT18在椎间盘退变发病机制中的潜在功能,为今后KRT18关键功能的研究奠定了基础。 展开更多

AIM To assess the relationship using multimodality imaging between intermediary citrate/choline metabolism as seen on proton magnetic resonance spectroscopic imaging(1H-MRSI) and glycolysis as observed on ^(18)F-fluorodeoxyglucose positron emission tomography/computed tomography(^(18)F-FDG-PET/CT) in prostate cancer(PCa) patients. METHODS The study included 22 patients with local PCa who were referred for endorectal magnetic resonance imaging/1HMRSI(April 2002 to July 2007) and ^(18)F-FDG-PET/CT and then underwent prostatectomy as primary or salvage treatment. Whole-mount step-section pathology was used as the standard of reference. We assessed the relationships between PET parameters [standardized uptake value(SUVmax and SUVmean)] and MRSI parameters [choline + creatine/citrate(CC/Cmax and CC/Cmean) and total number of suspicious voxels] using spearman's rank correlation, and the relationships of PET and 1H-MRSI index lesion parameters to surgical Gleason score.RESULTS Abnormal intermediary metabolism on 1H-MRSI was present in 21/22 patients, while abnormal glycolysis on ^(18)F-FDG-PET/CT was detected in only 3/22 patients. Specifically, index tumor localization rates were 0.95(95%CI: 0.77-1.00) for 1H-MRSI and 0.14(95%CI: 0.03-0.35) for ^(18)F-FDG-PET/CT. Spearman rank correlations indicated little relationship(ρ =-0.36-0.28) between 1H-MRSI parameters and ^(18)F-FDG-PET/CT parameters. Both the total number of suspicious voxels(ρ = 0.55, P = 0.0099) and the SUVmax(ρ = 0.46, P = 0.0366) correlated weakly with the Gleason score. No significant relationship was found between the CC/Cmax, CC/Cmean or SUVmean and the Gleason score(P = 0.15-0.79). CONCLUSION The concentration of intermediary metabolites detected by 1H MRSI and glycolytic flux measured ^(18)F-FDG PET show little correlation. Furthermore, only few tumors were FDG avid on PET, possibly because increased glycolysis represents a late and rather ominous event in the progression of PCa. 展开更多
