However, after half a century of animal experiments and clinicalstudies, clearly and intuitively revealing its therapeutic mechanism is still a great challengefor researchers. Therefore, this experiment is based on the combination of medicine andengineering to study the immediate effects of the acupuncture method in treating patientsby recording the electroencephalography (EEG) activities of subjects during the acupunctureprocess and to further reveal the therapeutic mechanism at the brain level. Methods: Thistrial is an exploratory, prospective, single-arm interventional study involving a total ofpatients with ischemic stroke. Physicians will record EEG data from patients duringacupuncture as the primary outcome indicator. After pre-processing the EEG data, researchers will use various methods to analyze the immediate effects of acupuncture toobtain brain effectiveness. Deep learning will then be used to identify acupoint stimuli andreceive correspondence between the acupuncture effect and the brain’s internal state.National Institutes of Health Stroke Scale score before and after the acupuncture process willbe used as the secondary outcome indicator. Conclusion: This is the first study protocol toapply dynamic changes in EEG to explore a range of mechanisms of action of acupuncture inthe treatment of ischemic stroke. We propose a method to analyze EEG signals ofacupuncture patients. The deep learning model will be applied for supervised training toobtain the compelling relationship between the acupuncture method and internal brainstates, providing a new prospect for the digitization of traditional Chinese medicine.展开更多

Methods: A randomized, single-blinded and controlled trial was conducted. A total of 124 cases conforming to the inclusion criteria were randomized by random number table into a treatment group and a control group, with 62 cases in each group. Patients in both groups received acupuncture therapy at the same acupoints, while patients in the treatment group received lower-reinforcing and upper-reducing method, and patients in the control group received even reinforcing-reducing manipulation. The treatment started around 10 d before menstruation and was conducted every other day. Patients received 5 treatments in each menstruation cycle for consecutive 3 cycles. The levels of E2, P and E2/P and clinical efficacy were measured before and after treatment. Results: After treatment, the breast lump size, pain intensity and concomitant symptoms score in both groups were substantially lower than those before treatment, showing statistical significances (all P〈0.01), and the improvement in the treatment group was higher than that in the control group, and the between-group comparisons showed statistical significances (all P〈0.01). After treatment, the overall effective rate was 91.9% in the treatment group, higher than 72.6% in the control group, and the between-group comparison showed a statistical significance (P〈0.01). After treatment, levels of E2, P and E2/P value showed no statistical significance when compared with those before treatment in the two groups (all P〉0.05). Conclusion: Lower-reinforcing and upper-reducing acupuncture method can effectively alleviate clinical symptoms and signs in HMG patients, and produce a better effect than even reinforcing-reducing manipulation. The majority of HMG patients' E2, P level and E2/P value were not beyond the normal ranges; therefore, acupuncture showed no substantial influence on E2 and P levels and E2/P value.展开更多

Methods One hundred and twenty cases were randomized into a conventional acupuncture group [Yangbai (阳白 GB 24), Sibai (四白 ST 2), Yingxiang (迎香 LI 20), etc.], a Renying (人迎ST 9) acupuncture group and an operation + acupuncture group [acupuncture at ST 9 as the main acupoint and the stellate ganglion block (SGB)], 40 cases in each one. The treatment was given once a day, 7 treatments made one session. After 3 sessions of treatment, the latency and the amplitude of the direct stimulation evoked potential of the facial nerve (ENoG) were compared before and after treatment in three groups, as well as R2 and R2 values of blink reflex (BR). The total clinical efficacy was assessed. Results The latency of ENoG was shortened and the amplitude was increased significantly in three groups. After treatment, ENoG latency was lower significantly in the operation + acupuncture group as compared with that in the conventional acupuncture group (P〈0.05). In the Renying (人迎 ST 9) acupuncture group, the amplitude of ENoG was increased as compared with P〈0.05). After treatment in three groups, those in the other two groups (all R2 and R2 values were decreased significantly. The differences in R1 and R2 values in the Renying (人迎 ST 9) acupuncture group and the operation + acupuncture group before and after treatment were bigger than those in the conventional acupuncture group (all P〈0.05), and the difference in R2 value in the operation + acupuncture group was bigger than that in the Renying (人迎 ST 9) acupuncture group (P〈O.05). The clinical markedly effective and curative rate was 87.5% (35/40) in the operation + acupuncture group, which was superior to 77.5% (32/40) in the Renying ()人迎 ST 9)acupuncture group and higher significantly than 65.0% (26/40) in the conventional acupuncture group (P〈0.05). Conclusion Compared with the conventional acupuncture, the efficacy on peripheral facial paralysis is much better in the Renying (人迎 ST 9) acupuncture group and the operation + acupuncture group. Moreover, the early reflex function of the damaged facial nerve is much better recovered in the operation + acupuncture group as compared with that in the Renying (人迎 ST 9) acupuncture group.展开更多

Whether acupuncture needles must be inserted in specifi...Recent clinical trial studies have demonstrated that the effects of acupuncture on pain improvement are small and no difference between acupoints and non-points. Whether acupuncture needles must be inserted in specific points depends on whether acupoint specificity exists that is still not resolved, and is now urgent. Previous anatomical studies have demonstrated that acupoints exist higher number of nerve fibers/trunks, blood vessels, hair follicles, and sweat glands as well as density of the gap junction. Recent evidence shows that nitric oxide(NO) level is elevated in the acupoints/meridians and is associated with an enhanced expression of NO synthase endowed with transient receptor potential vanilloid type-1. There is growing evidence from international groups showing that acupuncture induces NO-mediated vasodilatation, which increases local blood flow and allows for a flush of algesic or sensitizing substances, leading to pain relief. Previous studies, using a novel biocapture system, have demonstrated that NOx–(total nitrite and nitrate) and cyclic guanosine monophosphate(c GMP) concentrations are consistently increased over skin acupoints compared to non-meridian control regions(NMCR) in humans. Dermal microdialysis in humans showed that NO-c GMP releases in the subcutaneous tissue of acupoint are higher than those in NMCR and increased by electroacupuncture(EA). Recent studies have demonstrated that low-frequency electrical stimulation and manual acupuncture with low stimulating force and rate produce an elevation of NO release predominantly over acupoints. In contrast, NO levels over the areas of the skin regions are moderately reduced by high-frequency EA stimulation. The results from anatomical and biochemical studies consistently show that acupoints exist higher levels of NO signaling molecules, and stimulus-evoked NO release is also with a higher level at acupoints. Results suggest that NO signaling molecules contribute to the specificity of acupoints, and selecting well-trained acupuncturetists for using correct acupoints and appropriate parameters should improve acupuncture clinical trial studies.展开更多

文摘Objective: To observe the clinical effect of lower-reinforcing and upper-reducing acupuncture method for hyperplasia of mammary gland (HMG) and its influence on estradiol (E2) and progesterone (P). 文摘Recent clinical trial studies have demonstrated that the effects of acupuncture on pain improvement are small and no difference between acupoints and non-points. Results suggest that NO signaling molecules contribute to the specificity of acupoints, and selecting well-trained acupuncturetists for using correct acupoints and appropriate parameters should improve acupuncture clinical trial studies.
