The diagnoses were made by three radiologists em- ploying a 5-point rating scale, with postoperative pathologic results as the gold standard. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diag- nostic value of the three MSCT combination modes (arterial phase+portal-venous phase, arterial phase+delayed phase, arterial phase+portal-venous phase+delayed phase) and AFP levels for sHCC on the background of liver cirrhosis. RESULTS: The area under ROC curve (AUC), sensitivity, and specificity of the combination of arterial phase+portal- venous phase+delayed phase were 0.93, 93%, and 82%, respectively. The average AUC of the arterial phase+portal- venous phase+delayed phase combination was significantly greater than that of the arterial phase+portal-venous phase (AUC=0.84, P=0.01) and arterial phase+delayed phase (AUC=0.85, P=0.03). Arterial phase+portal-venous phase had a smaller AUC (0.84) than arterial phase+delayed phase (0.85), but the difference was insignificant (P=0.15). After combining MSCT enhancement scan with AFP, the AUC, sensitivity, and specificity were 0.95, 94%, and 83%, respectively, indicating a greatly increased diagnostic efficiency for sHCC. CONCLUSIONS: The combination of AFP and 3 phases MSCT enhancement scan could increase the diagnostic efficiency for sHCC on the background of liver cirrhosis. The application of ROC curve analysis has provided a new method and reference in HCC diagnosis.展开更多

In view of the critical shortage of organs, the indications for living-related liver transplantation (LRLT) have broadened since experience with the procedure has been achieved. This study was undertaken to assess the value of multi-slice spiral CT (MSCT) angiography in evaluating the hepatic arterial and veinous anatomy of potential donors for LRLT. METHODS: MSCT was performed after intravenous injection of contrast material at 3 ml/s. The total dose was calculated as 2 ml/kg. Twenty LRLT donors (2 men and 18 women) were subjected to MSCT angiography of hepatic blood vessels. These were generated by volume rendering and maximum intensity projection, while curved planar reformation was added in 5 patients. RESULTS: We identified 10 important hepatic vascular variants in 9 of the 20 donors (4 arterial, 4 venous, and 2 portal venous variants). In hepatic arterial variants, two had a replaced right hepatic artery arising from the superior mesenteric artery, an accessory right hepatic artery from the superior mesenteric artery and a replaced left hepatic artery arising from the left gastric artery. In hepatic venous variants, three had an accessory inferior right hepatic vein and one had two accessory inferior right hepatic veins. In hepatic portal venous variants, two had trifurcation of the main portal vein. CONCLUSIONS: As a non-invasive and reliable method, MSCT angiography is of value in the clinical evaluation of LRLT donors. MSCT angiography should be recommended as a routine preoperative examination for potential LRLT donors.展开更多

Methods: 106 patients with pathologically confirmed lung cancer received spiral CT examinations with a new revised TNM staging method of lung cancer which was taken as criterion of diagnosis. The images were reconstructed by using different postprocessing techniques such as MPR, MIP, SSD, VR, and their TNM staging capabilities were assessed by using postprocessing imaging and the results with axial CT images were compared. Results: In T staging, the accurate rates of axial CT images in T1-T4 staging were 77.3%, 60.6%, 60.7%, and 60.3% respectively; for postprocessing images, the accurate rates were 86.4%, 90.9%, 89.3%, and 87.0% respectively, In N staging, the accurate rates of axial CT images were 84.6%, 68.1%, 63.6%, and 64.2% in N0-N3 staging, respectively; but they were 92.3%, 90.9%, 90.9%, and 85.7% in postprocessing images, respectively. The postprocessing images were superior to axial CT images in T1-T3 and N1-N3 staging of lung cancer but there were no significant differences in T1 and NO staging. For metastasis, the postprocessing images may be helpful for the TNM staging of lung cancer. Conclusion: The postprocessing techniques of the spiral CT could improve the accurate rates of TNM staging of lung cancer, and represent a useful complement to the axial CT.展开更多

OBJECTIVE : To observe the forms of foramen ovales in healthy adults displayed by volume rendering and multi-planar reconstruction after three-dimensional spiral CT thin-slice scan of skull base, and measure the longitudinal diameter and transverse diameter. DESIGN : A repetitive observation and measurement SETTINGS : Department of Neurosurgery and Department of Medical Imaging, Foshan Hospital of Traditional Chinese Medicine. PARTICIPANTS: Fifty healthy adults (100 sides), who were examined with three-dimensional spiral CT scan, were randomly selected from the Department of Medical Imaging, Foshan Hospital of Traditional Chinese Medicine from January 2005 to January 2006, including 26 males and 24 females, aged 25-68 years with an average of 48 years old. They were all informed and agreed with the examination. METHODS : The subjects were examined with the Philips 16-slice spiral CT-Mx 8000 IDT CT apparatus (Philips Company, Holland), the scanning ranged from 2 cm below the canthomeatal line to the level of suprasellar cistem. The width of collimator was 0.75 mm, pitch was 0.663; tube current was 350 mA, voltage was 120 kV, resolution was 512×512 matrix; slice thickness of reconstruction was 1 mm, and interval was 0.5 mm. After the three-dimensional spiral CT thin-slice scan of skull base, the image post-processing techniques including volume rendering and multi-planar reconstruction were applied to observe the forms of foramen ovales, and measure the size, longitudinal diameter and transverse diameter of the foramen ovales. The figures of the foramen ovales were drawn with mouse along the boundary of bone porous margin and soft tissue. According to the indications, the diameters were measured with computer to observe the forms of foramen ovales. MAIN OUTCOME MEASURES : The longitudinal diameter, transverse diameter and form of foramen ovales were observed. RESULTS: All the 50 healthy adults (100 sides) were involved in the analysis of results. (1) It was observed in the volume rendering images that foramen ovales had four forms of oval shape (77 sides), kidney shape (12 sides), round shape (7 sides), ribbon shape (4 sides). (2) The longitudinal diameters of left and right foramen ovales were (7.67±1.32) and (7.98±1.45) mm, and the transverse diameters were (4.04±0.83), (4.09±1.07) mm; There was no obvious difference between left and right longitudinal diameters (t = 1.63, P = 0.11 ), and left and right transverse diameters were close (t = 0.45, P= 0.65). CONCLUSION : The non-invasive techniques of volume rendering and multi-planar reconstruction after three-dimensional spiral CT thin-slice scan can clearly display the formand size of foramen ovale in healthy adults.展开更多

Recently, three dimensions (3D) spiral CT imaging has been used in surgical diseases gradually. This study was designed to evaluate the diagnostic value of 3D spiral CT imaging of cholangiopancreatic ducts on obstructive jaundice. Methods: Thirty patients with obstructive jaundice had received B-mode ultrasonography, CT, percutaneous transhepatic cholangiography (PTC) or endoscopic retrograde cholangiopancreatography (ERCP), and 3D spiral CT imaging of cholangiopancreatic ducts preoperatively. Then the diagnose accordance rate of these examinational methods were compared after operations. Results: The diagnose accordance rate of 3D spiral CT imaging of cholangiopancreatic ducts was higher than those of B-mode ultrasonography, CT, or single PTC or ERCP, which showed clear images of bile duct tree and pathological changes. As to malignant obstructive jaundice, this examinational technique could clearly display the adjacent relationship between tumor and liver tissue, biliary ducts, blood vessels, and intrahepatic metastases. Conclusion: 3D spiral CT imaging of cholangiopancreatic ducts has significant value for obstructive diseases of biliary ducts, which provides effective evidence for the feasibility of tumor-resection and surgical options.展开更多

文摘In order to improve the diagnosis and analysis ability of 3D spiral CT and to reconstruct the contour of 3D spiral CT damage image,a contour reconstruction method based on sharpening template enhancement for 3D spiral CT damage image is proposed. This method uses the active contour LasSO model to extract the contour feature of the 3D spiral CT damage image and enhances the information by sharpening the template en.hancement technique and makes the noise separation of the 3D spiral CT damage image. The spiral CT image was procesed with ENT,and the statistical shape model of 3D spiral CT damage image was established.The.gradient algorithm is used to decompose the feature to realize the analysis and reconstruction of the contour feature of the 3D spiral CT damage image,so as to improve the adaptive feature matching ability and the ability to locate the abnormal feature points. The simulation results show that in the 3D spiral CT damage image contour reconstruction, the proposed method performs well in the feature matching of the output pixels, shortens the contour reconstruction time by 20/ms,and provides a strong ability to express the image information. The normalized reconstruction error of CES is 30%,which improves the recognition ability of 3D spiral CT damage image, and increases the signal-to noise ratio of peak output by 40 dB over other methods.
文摘Objective: To evaluate the role of spiral CT imaging postprocessing techniques in preoperative TNM staging of lung cancer. Conclusion: The postprocessing techniques of the spiral CT could improve the accurate rates of TNM staging of lung cancer, and represent a useful complement to the axial CT.

文摘BACKGROUND: The accurate measurements of various data of the bone diameters of foramen ovale of living person can change the methods of puncturing trigeminal gasserian ganglion via foramen ovale for treating trifacial neuralgia from the experience of puncture operator only to puncture by taking the objective data of measurement as the evidence, which is good for improving the accuracy of puncturing trigeminal ganglion and reducing side effects. CONCLUSION : The non-invasive techniques of volume rendering and multi-planar reconstruction after three-dimensional spiral CT thin-slice scan can clearly display the formand size of foramen ovale in healthy adults.

文摘To accelerate the scan speed and improve the image quality, a new type of CT configuration, "double-source double-multislice spiral CT" (DSDMS-CT), which is based on two sets of single-source multislice spiral CT was proposed with a special reconstruction algorithm. Simulation results using the fan-beam filtered backprojection algorithm with a special interpolation method were presented for both single-source multislice spiral CT and DSDMS-CT. The results of new CT model show that it scans faster than the traditional spiral CT and has a better slice sensitivity profile (SSP) with larger pitch value.
