矿井涌水是矿山安全开采中难以解决又必须面对的问题。GMS(groundwater modeling system)具有强大的前、后处理功能,实现了地质模型、水文地质参数等的可视化,提供的自检功能为调试模型提供了极大方便。以宁夏某矿为例,应用GMS对其水文... 矿井涌水是矿山安全开采中难以解决又必须面对的问题。GMS(groundwater modeling system)具有强大的前、后处理功能,实现了地质模型、水文地质参数等的可视化,提供的自检功能为调试模型提供了极大方便。以宁夏某矿为例,应用GMS对其水文地质条件进行概化,建立水文地质结构模型和水文地质概化模型,通过MODFLOW模块对其涌水量进行预测。结果表明:三维地质模型具有很好地可视性,研究区地层能够更加直观的得以展示,模拟所得水位与实测水位拟合效果在置信区间内,模拟区首采面的涌水量的预测值为1450 m~3/h。为矿山开采、工程建设中涌水量预测的研究提供了更为直观、更为便捷的方法。 展开更多

It is well known that angiogenesis is critical in the transition from premalignant to malignant lesions.Consequently,early detection and diagnosis based on morphological changes to the microvessels are crucial.In the last few years,new imaging techniques which utilize the properties of light-tissue interaction have been developed to increase early diagnosis of gastrointestinal(GI) tract neoplasia.We analyzed several "red-flag" endoscopic techniques used to enhance visualization of the vascular pattern of preneoplastic and neoplastic lesions(e.g.trimodal imaging including autofluorescence imaging,magnifying endoscopy and narrow band imaging).These new endoscopic techniques provide better visualization of mucosal microsurface structure and microvascular architecture and may enhance the diagnosis and characterization of mucosal lesions in the GI tract.In the near future,it is expected that trimodal imaging endoscopy will be practiced as a standard endoscopy technique as it is quick,safe and accurate for making a precise diagnosis of gastrointestinal pathology,with an emphasis on the diagnosis of early GI tract cancers.Further large-scale randomized controlled trials comparing these modalities in different patient subpopulations are warranted before their endorsement in the routine practice of GI endoscopy. 展开更多

AIM:To investigate perception of natural orifice transluminal endoscopic surgery(NOTES)as a potential technique for appendectomy.METHODS:One hundred patients undergoing endoscopy and 100 physicians were given a questionnaire describing in detail the techniques of NOTES and laparoscopic appendectomy.They were asked about the reasons for their preference,choice of orifice,and extent of complication risk they were willing to accept.RESULTS:Fifty patients(50%)and only 21 physicians(21%)preferred NOTES(P<0.001).Patients had previously heard of NOTES less frequently(7%vs73%,P<0.001)and had undergone endoscopy more frequently(88%vs 36%,P<0.001)than physicians.Absence of hernia was the most common reason for NOTES preference in physicians(80%vs 44%,P= 0.003),whereas reduced pain was the most common reason in patients(66%vs 52%).Physicians were more likely to refuse NOTES as a novel and unsure technique(P<0.001)and having an increased risk of infection(P<0.001).The preferred access site in both groups was colon followed by stomach,with vagina being rarely preferred.In multivariable modeling,those with high-school education[odds ratio(OR):2.68,95% confidence interval(CI):1.23-5.83]and prior colonoscopy(OR:2.10,95%CI:1.05-4.19)were more likely to prefer NOTES over laparoscopic appendectomy.There was a steep decline in NOTES preference with increased rate of procedural complications.Male patients were more likely to consent to their wives vaginal NOTES appendectomy than male physicians(P=0.02).CONCLUSION:The preference of NOTES for appendectomy was greater in patients than physicians and was related to reduced pain and absence of hernia rather than lack of scarring. 展开更多

Objective To investigate the efficacy of hematopoietic stem cell (HSC) transplantation via the hepatic artery vs. the portal vein for end-stage liver disease (ESLD). Methods Patients with hepatic decompensation were prospectively recruited from September 2010 to September 2012 to receive HSC transplantation via the hepatic artery or the portal vein. Liver function was examined at 3, 6, and 12 months after transplantation. Liver biopsy results were analyzed using the Knodell score. Results Eighty patients (58 males and 22 females) were enrolled in the study. The Child-Pugh score was grade B in 69 cases, and grade C in the remaining 11 cases. HSC transplantation was performed via the portal vein in 36 patients and via the hepatic artery in 44 patients. ALT levels decreased while serum albumin levels increased significantly in both groups at 6 and 12 months after HSC transplantation (P〈0.05 compared with pre-transplantation levels). Total biliruhin levels decreased significandy in both groups at 3, 6, and 12 months after HSC transplantation (P〈0.05 compared with pre-transplantation levels). Additionally, prothromhin time decreased in both groups at 12 months after HSC transplantation (P〈0.05 compared with pre-transplantation level). There were no significant differences in ALT, total bilirubin and prothromhin time between the two groups either before or after transplantation. Moreover, Knodell score decreased significantly at 6 and 12 months. Histological examination showed that liver cell edema, degeneration, necrosis, and inflammation were significantly relieved at 3, 6, and 12 months after transplantation. The incidence of portal vein thrombosis, upper gastrointestinal bleeding, and hepatic encephalopathy were 1.25%, 3.75%, and 2.5% respectively. The one-year survival rate was 100%. Conclusions Autologous HSC transplantation improves liver function and histology in ESLD patients The administration route of HSC has no significant impact on the efficacy of transplantation. 展开更多

AIM:To investigate the performance of a new software-based colonoscopy quality assessment system.METHODS:The software-based system employs a novel image processing algorithm which detects the levels of image clarity,withdrawal velocity,and level of the bowel preparation in a real-time fashion from live video signal.Threshold levels of image blurriness and the withdrawal velocity below which the visualization could be considered adequate have initially been determined arbitrarily by review of sample colonoscopy videos by two experienced endoscopists.Subsequently,an overall colonoscopy quality rating was computed based on the percentage of the withdrawal time with adequate visualization(scored 1-5;1,when the percentage was 1%-20%;2,when the percentage was 21%-40%,etc.).In order to test the proposed velocity and blurriness thresholds,screening colonoscopy withdrawal videos from a specialized ambulatory colon cancer screening center were collected,automatically processed and rated.Quality ratings on the withdrawal were compared to the insertion in the same patients.Then,3 experienced endoscopists reviewed the collected videos in a blinded fashion and rated the overall quality of each withdrawal(scored 1-5;1,poor;3,average;5,excellent) based on 3 major aspects:image quality,colon preparation,and withdrawal velocity.The automated quality ratings were compared to the averaged endoscopist quality ratings using Spearman correlation coefficient.RESULTS:Fourteen screening colonoscopies were assessed.Adenomatous polyps were detected in 4/14(29%) of the collected colonoscopy video samples.As a proof of concept,the Colometer software rated colonoscope withdrawal as having better visualization than the insertion in the 10 videos which did not have any polyps(average percent time with adequate visualization:79% ± 5% for withdrawal and 50% ± 14% for insertion,P < 0.01).Withdrawal times during which no polyps were removed ranged from 4-12 min.The median quality rating from the automated system and the reviewers was 3.45 [interquartile range(IQR),3.1-3.68] and 3.00(IQR,2.33-3.67) respectively for all colonoscopy video samples.The automated rating revealed a strong correlation with the reviewer's rating(ρ coefficient= 0.65,P = 0.01).There was good correlation of the automated overall quality rating and the mean endoscopist withdrawal speed rating(Spearman r coefficient= 0.59,P = 0.03).There was no correlation of automated overall quality rating with mean endoscopists image quality rating(Spearman r coefficient= 0.41,P = 0.15).CONCLUSION:The results from a novel automated real-time colonoscopy quality feedback system strongly agreed with the endoscopists' quality assessments.Further study is required to validate this approach. 展开更多

In manufacture of precise optical products, it is important to inspect and classify the potential defects existing on the products’ surfaces after precise machining in order to obtain high quality in both functionality and aesthetics. The existing methods for detecting and classifying defects all are low accuracy or efficiency or high cost in inspection process. In this paper, a new inspection system based on machine vision has been introduced, which uses automatic focusing and image mosaic technologies to rapidly acquire distinct surface image, and employs Case-Based Reasoning(CBR)method in defects classification. A modificatory fuzzy similarity algorithm in CBR has been adopted for more quick and robust need of pattern recognition in practice inspection. Experiments show that the system can inspect surface diameter of 500mm in half an hour with resolving power of 0.8μm diameter according to digs or 0.5μm transverse width according to scratches. The proposed inspection principles and methods not only have meet manufacturing requirements of precise optical products, but also have great potential applications in other fields of precise surface inspection. 展开更多

METHODS: We reviewed endoscopic photographs and medical records of patients with gastric polyps who underwent endoscopic mucosal resection from April 1996 through February 2003. RESULTS: A total of 85 gastric polyps from 74 patients were reviewed, Male-to-female ratio was 1:1,96, Mean age was 59.9 =1:10,8 years. Multiple polyps were observed in 10,8%, Gastric polyps occurred most frequently in the antrum (58.8%), Pathological results on resected specimens were as follows: tubular adenoma 45.9%, hyperplastic polyp 31.8%, inflammatory polyp 9.4%, hamartoma 3.5%, fundic gland polyp 2.4%, tubulovillous adenoma 2.4%, adenocarcinoma 2.4%, dysplasia 1.1%, and mucosal pseudolipomatosis 1.1%. Discrepancy rate between endoscopic biopsy and pathology of resected specimens was 27.1%. There was no relationship between the size of the polyp and concordance rate. CONCLUSION: There is considerable discrepancy in histologic findings between endoscopic forceps biopsy and resected specimens. Approaches to review of the histology of an entire polyp should be performed, especially when an adenoma is suspected. 展开更多

Objective: The objective of this study was to compare the underestimation rate of invasive carcinoma cases with ductal carcinoma in situ (DCIS) at percutaneous ultrasound-guided core biopsies of breast lesions between 14-gauge automated core needle biopsy (ACNB) and 11-gauge vacuum-assisted biopsy (VAB), and analyze the diagnostic advantages and insufficiencies in DCIS between this two methods, and to determine the relationship between the lesion type (masses or microcalcifications on radiological findings ) and DCIS underestimation rate. Methods: We collected 152 breast lesions which were diagnosed as DCIS by retrospectively reviewing data about ultrasound-guided biopsies of breast lesions (from February 2003 to July 2010). There were 98 lesions in 95 patients by 14-gauge ACNB, and 54 lesions in 52 patients by 11-gauge VAB (The system used in this study called Mammatome, MMT). The clinical and radiological findings were reviewed; meanwhile all the selected patients had histological results of the biopsies and follow-up surgeries which also achieved the reliable pathological results to compare with the biopsy results. The differences between two correlated histological results defined as underestimation, and the histological DCIS underestimation rates were compared between the two groups. According to the radiological characteristics, each group was classified into two subgroups (masses or micrecalcifications group), and the differences between subgroups were also analyzed. Results: The DCIS underestimation rate was 45.9% (45/98) for 14-gauge ACNB and 16.6% (9/54) for MMT. According to the lesion type on ultrasonography, DCIS underestimation was 31.0% (26/84) in masses (43.1% using ACNB and 12.1% using MMT; P = 0.003) and 42.6% (29/68) in microcalcifications (48.9% using ACNB and 23,8% using MMT; P = 0,036), Conclusion: The underestimation rate of invasive carcinoma in cases with DCIS at ultrasound-guided core biopsies is significantly higher for ACNB than for MMT. Furthermore, this difference does not alter among the two lesion types presented on ultrasonography. So ultrasound-guided VAB (MMT system) could be an effective and useful method for the diagnosis of DCIS lesions no matter what the lesion type is. 展开更多

It draws on Fish's concept of "interpretive communities", which argues that interpretation is an institutional practice, and that consequently readers hold shared prior assumptions that constrain their interpretive strategies (Fish 1980). Not surprisingly then, some responses to Saudi authors are based on the ideological belief that their novels consist of acts of rebellion against a conservative culture. A close reading of the conflict between Saudi novelists and the social responses to their works can reflect how cultural and social contexts shape the reception of contemporary Saudi novels, and can also help to construct public attitudes toward these texts. Saudi novelists have faced a number of social constraints and factors which have affected the development of the novel in Saudi Arabia. For example, works by al-Gosaibi, Munif, Khal, al-Hamad, al-Mohaimeed, Alsanea, and al-luhani have all been banned because they were seen to pose a major threat to the dominant, patriarchal Saudi ideology. While the social controversy around these writers was raging, some other writers applied self-censorship to avoid touching upon what were perceived to be the most sensitive issues. 展开更多

Celiac disease (CD) is an autoimmune inflammatory disease of the small intestine as a result of reaction to wheat protein, gluten. Exclusion of dietary gluten is the mainstay of the treatment that necessitates a precise diagnosis of the disease. Serological screen ing may aid in identifying patients with suspected CD, which should be confirmed by intestinal biopsy. It has been shown that duodenal biopsies are good for de- tection of the disease in most patients. However, there is a group of patients with positive serology and incon- clusive pathology. As a result of the widespread use of serology, many patients with equivocal findings grow quickly. Unfortunately current endoscopic methods can only diagnose villous atrophy, which can be present in the later grades of disease (i.e., Marsh m). To diag- nose CD correctly, going deeper in the intestine may be necessary. Enteroscopy can reveal changes in CD in the intestinal mucosa in 10%-17% of cases that have negative histology at initial workup. Invasiveness of the method limits its use. Capsule endoscopy may be a good substitute for enteroscopy. However, both tech- niques should be reserved for patients with suspected diagnosis of complications. All vessels and offshore units require regular inspections to prevent structural or functional damages. Periodical inspections are based on classification society's requirements and include both visual surveys and non-destructive test interventions, to look for structural damage, pitting, corrosion, thickness measurement, usually documented by measurements or images. Different approaches of inspection can be proposed for underwater and for dry areas, using unmanned vehicles or service robots able to improve traditional inspection methods. Several excellent proposals are today available: nevertheless, reasons of costs, operation complexity and impossibility to cover all types of inspection with a unique autonomous unit limit and conditions the applicability of automatic inspection systems. The paper is the first of a series describing the main results of a wide research activity carried out at DREAMS Lab (Laboratory of Drives and Experimental Automation for Marine Systems) of the University of Genoa. The studies are oriented to the realization of a kit of low cost and unsophisticated self-moving units, devoted to support some recursive inspections in dry areas of ships and on offshore plants as bulkheads, holds, double bottoms and ducts. The paper describes the design and prototyping of a pneumatronic unit oriented to inspection and maintenance of ship holds. Conceived to realize a portable and user-friendly tool which could take part of an "inspection kit" for the use of inspectors, able to simplify and speed up the inspection visits with and automatic generation of survey reports. 展开更多
