Data was collected from the literature, field visi...The paper aims to determine the irrigation water supply efficiency of different irrigation methods used in the challenging environment of Bal’ad district in Somalia. Data was collected from the literature, field visits using field records and scheduled interviews, GPS coordinates and from ancillary information, such as remote sensing images and existing national maps. A comparison was done by use of secondary sources, such as academic journals using information from authorities on irrigation and water loss. Sampling was done by use of Snow balling. The results highlighted response rate for farmers being 80% while that of NGO employees is 75.76%. According to the results, the main ways through which irrigation water is lost is through: evaporation;seepage through the canal bunds;overtopping the bunds;overflow losses and overwatering with the average field application efficiency of 25% and conveyance efficiency of 30%. These generated a scheme irrigation efficiency of 7.5% which is poor for surface irrigation prevalent in the study area. The loss of irrigation water was found to be reduced by the following: daily supervision;proper maintenance;water allocation to farmers;good management;lining of canals;management of irrigation methods;ongoing evaluation;good land preparation;and training farmers.展开更多

However, few studies have monitored gender differences in individuals with delirium. OBJECTIVE: To explore gender differences in challenging behaviors, management and outcomes in age-matched elderly patients with delirium.DESIGN, TIME AND SETTING: A retrospective cohort study was performed in the Bankstown-Lidcombe Hospital, Sydney, Australia, from October 2008 to April 2009. METHODS: Patients, aged 65-90 years, diagnosed with delirium according to the International Classification of Diseases (ICD)-10 in the Psychogeriatric Unit of Bankstown Lidcombe Hospital from January 2002 to October 2008 were reviewed. All the patients were measured according to the Confusion Assessment Method upon admission. Those who developed delirium during hospitalization were excluded.MAIN OUTCOME MEASURES: Cause of delirium, wandering, aggression, duration of delirium, physical restraint, use of antipsychotic medicine, recovery from delirium, discharge back home, length of stay, one-to-one nursing care, falls and absconding rate.RESULTS: The 131 age-matched delirious patients comprised 54 males and 77 females. The behavioral disorders of wandering [odds ration (OR) = 2.612, 95% confidence interval (CI) = 1.26 -5.413, P = 0.009] and aggression (OR = 2.243, 95% CI = 1.028 - 4.891, P= 0.04) were more frequent in males than in females. More males received one-to-one nursing care (OR = 4.114, 95% CI = 1.355 - 12.491, P = 0.008), were more likely to receive antipsychotic medications (OR = 2.24, 95% CI = 1.095-4.583, P = 0.021) and more likely to be physically restrained (OR = 2.07, 95% CI = 1.02-1.02, P = 0.043) compared with female patients. All absconding patients (3/131, 2.3%) were male. In addition, male patients displayed a greater falling rate compared with females (OR = 4.57, 95% CI= 1.519-13.722, P = 0.004).CONCLUSION: There are gender differences in challenging behaviors, management and outcomes in elderly delirious patients. Males with delirium display more challenging behaviors that require physical restraint and pharmacological management including wandering and aggression; males also abscond and have a higher rate of falls compared with female patients.展开更多

There were 28 cases of island pectoralis major island myocutanuous flaps, 34 cases of latissimus dorsi island myocutaneous flaps, 19 cases of trapizius island myocutaneous flaps and 17 cases of rectus abdominis island myocutaneous flaps in this report. Results: All 98 patients with challenging wound on cervico-thoracic region were successfully treated with this method without complications, and obtained functional and cosmetic effectiveness. Conclusion:Challenging wounds in cervico-nuchal region can be repaired with pertoralis major island myocutaneous flap, latissimus dorsi island myocutaneous flap and trapizius island myocutaneous flap, while challenging wounds in thoracic region can be repaired with latissimus dorsi island myocutaneous flap and rectus abdominis island myocutaneous flap. Satisfactory functional and cosmetic results can be obtained.展开更多

Myc...Objectives: To challenge the expiry dates of low concentration high purity mycotoxins standards. Literature Review: Aflatoxins (AFs) and Ochratoxin A (OTA) are persistent mycotoxins with adverse effects on humans. Mycotoxins standards are purchased to determine mycotoxin concentrations in food and may be stocked in some laboratories beyond expiry dates causing laboratories financial losses. Methods: Certified mycotoxins standards were purchased over the years from the same supplier at times and at other times from two different suppliers for quality control purposes. For AFs, six chromatographic runs for each of the mycotoxins standards were done to compare the difference among these standards having the following expiry dates (2008, 2012, 2013 and 2018). AFs standards purchased/obtained from two different suppliers in 2016 and expiring in 2018 were also compared. For OTA, the difference of concentration obtained between two years (2010 and 2018) was tested. All samples were run on a HPLC equipped with a fluorescence detector. Linearity of calibration curves and the points of lowest detection were determined for AFs components and for OTA from the unexpired mycotoxins standards. Results: At a 0.05 significance level and using non parametric tests, the statistical test revealed a p of 0.166, 0.153, 0.358 and 0.03 for B1, G1, B2 and G2 respectively among years for standards from same supplier and 0.037, 0.109, 0.182 and 0.182 for B1, G1, B2 and G2 respectively for unexpired standards from two different suppliers. For OTA, a p of 0.109 was obtained for standards of different expiry dates purchased from different suppliers. Conclusion: High purity low concentration mycotoxin standards purchased a decade ago (i.e. expired) did not differ from those purchased this current year (still valid). Hence, the expiry date can be renewed reducing the laboratories expenses. Manufacturers are urged to reconsider the expiry dates.展开更多
The dynamic motion of quadrupedal robots on challenging terrain generally requires elaborate spatial–temporal kinodynamic motion planning and accurate control at higher refresh rate in comparison with regular terrain...The dynamic motion of quadrupedal robots on challenging terrain generally requires elaborate spatial–temporal kinodynamic motion planning and accurate control at higher refresh rate in comparison with regular terrain.However,conventional quadrupedal robots usually generate relatively coarse planning and employ motion replanning or reactive strategies to handle terrain irregularities.The resultant complex and computation-intensive controller may lead to nonoptimal motions or the breaking of locomotion rhythm.In this paper,a kinodynamic optimization approach is presented.To generate long-horizon optimal predictions of the kinematic and dynamic behavior of the quadruped robot on challenging terrain,we formulate motion planning as an optimization problem;jointly treat the foot’s locations,contact forces,and torso motions as decision variables;combine smooth motion and minimal energy consumption as the objective function;and explicitly represent feasible foothold region and friction constraints based on terrain information.To track the generated motions accurately and stably,we employ a whole-body controller to compute reference position and velocity commands,which are fed forward to joint controllers of the robot’s legs.We verify the effectiveness of the developed approach through simulation and on a physical quadruped robot testbed.Results show that the quadruped robot can successfully traverse a 30°slope and 43% of nominal leg length high step while maintaining the rhythm of dynamic trot gait.展开更多

Techniques of in silico biology are not much explored for the investigation of a variety of bioactive phytochemicals of Ayurvedic herbs. Drug repurposing, reverse pharmacology, and polypharmacology in Ayurveda are areas in silico explorations that are needed to understand the rich repertoire of herbs, minerals, herbo-minerals, and assorted Ayurvedic formulations. This review emphasizes exploring the concept of Ayurveda with in silico approaches and the need for Ayurinformatics studies. It also provides an overview of in silico studies done on phytoconstituents of some important Ayurvedic plants, the utility of in silico studies in Ayurvedic phytoconstituents/formulations, limitations/challenges, and prospects of in silico studies in Ayurveda. This article discusses the convergence of in silico work, especially in the least explored field of Ayurveda. The focused coalesce of these two domains could present a predictive combinatorial platform to enhance translational research magnitude. This system, however, is prone to impersonations due to security vulnerabilities in current students’ verification system. These vulnerabilities include weak authentication, lack of encryption, and inadequate anti-counterfeiting measures. Additionally, advanced printing technologies and online marketplaces which claim to produce convincing fake identification documents make it easy to create convincing fake identity documents. The Improved Mechanism for Detecting Impersonations (IMDIs) system detects impersonations in in-class exams by integrating QR codes and dynamic question generation based on student profiles. It consists of a mobile verification app, built with Flutter and communicating via RESTful APIs, and a web system, developed with Laravel using HTML, CSS, and JavaScript. The two components communicate through APIs, with MySQL managing the database. The mobile app and web server interact to ensure efficient verification and security during examinations. The implemented IMDIs system was validated by a mobile application which is integrated with a QR codes scanner for capturing codes embedded in student Identity Cards and linking them to a dynamic question generation model. The QG model uses natural language processing (NLP) algorithm and Question Generation (QG) techniques to create dynamic profile questions. Results show that the IMDIs system could generate four challenging profile-based questions within two seconds, allowing the verification of 200 students in 33 minutes by one operator. The IMDIs system also tracks exam-eligible students, aiding in exam attendance and integrates with a Short Message Service (SMS) to report impersonation incidents to a dedicated security officer in real-time. The IMDIs system was tested and found to be 98% secure, 100% convenient, with a 0% false rejection rate and a 2% false acceptance rate, demonstrating its security, reliability, and high performance.展开更多

The Gaoligong Mountains(GLGM),located in southwestern China,extend north to south along the western border of the Hengduan Mountains,spanning approximately 600 km.In this study,we consolidated findings from 17 bird su...The Gaoligong Mountains(GLGM),located in southwestern China,extend north to south along the western border of the Hengduan Mountains,spanning approximately 600 km.In this study,we consolidated findings from 17 bird surveys conducted in the GLGM between 2010 and 2022.We found that the GLGM harbors tremendous bird diversity,with a total of 796 documented bird species in the region.Nearly a quarter(23.0%)of these species are listed as state key protected species or as Chinese and global threatened species.Analysis of species richness at the county level showed a decreasing trend with increasing latitude,with the greatest diversity in Yingjiang(661 species).Observations indicated that the GLGM belongs to the Oriental realm,primarily composed of bird species from southern and southwestern China.The GLGM plays an important role in avian conservation by sheltering exceptional bird diversity,providing corridors and flyways for bird migration and dispersal,and mitigating the effects of climate change.In response to the conservation needs of birds and other wildlife,the Chinese government has established numerous protected areas within the GLGM.Despite these efforts,avian conservation still faces considerable challenges in the GLGM due to limitations in the protected area network,transboundary nature of the regions,and existing gaps in monitoring and research.展开更多

BACKGROUND Inflammatory bowel disease,including ulcerative colitis,microscopic colitis,and Crohn’s disease(CD),has a global impact.This review focuses on duodenal CD(DCD),a rare subtype affecting the duodenum.DCD’s ...BACKGROUND Inflammatory bowel disease,including ulcerative colitis,microscopic colitis,and Crohn’s disease(CD),has a global impact.This review focuses on duodenal CD(DCD),a rare subtype affecting the duodenum.DCD’s rarity and asymptomatic nature create diagnostic challenges,impacting prognosis and patient well-being.Delayed diagnosis can worsen DCD outcomes.AIM To report a rare case of DCD and to discuss the diagnostic challenges and its implications on prognosis.METHODS A systematic literature search,following the PRISMA statement,was conducted.Relevant studies were identified and analysed using specific Medical Subject Terms(MeSH)from PubMed/MEDLINE,American Journal of Gastroenterology,and the University of South Wales database.Data collection included information from radiology scans,endoscopy procedures,biopsies,and histopathology results.RESULTS The review considered 8 case reports and 1 observational study,involving 44 participants diagnosed with DCD,some of whom developed complications due to delayed diagnosis.Various diagnostic methods were employed,as there is no gold standard workup for DCD.Radiology scans[magnetic resonance imaging(MRI),computed tomography(CT),and upper gastrointestinal X-ray],endoscopy procedures(colonoscopy and esophagogastroduodenoscopy),biopsies,and clinical suspicions were utilized.CONCLUSION This review discusses DCD diagnosis challenges and the roles of CT,MRI,and fluoroscopy.It notes their limitations and compares findings with endoscopy and histopathology studies.Further research is needed to improve diagnosis,emphasizing scan interpretation,endoscopy procedures,and biopsies,especially in high-risk patients during routine endoscopy.展开更多

The loss of irrigation water was found to be reduced by the following: daily supervision;proper maintenance;water allocation to farmers;good management;lining of canals;management of irrigation methods;ongoing evaluation;good land preparation;and training farmers.
文摘BACKGROUND: Gender differences have been reported in some common mental disorders. However, few studies have monitored gender differences in individuals with delirium. OBJECTIVE: To explore gender differences in challenging behaviors, management and outcomes in age-matched elderly patients with delirium.DESIGN, TIME AND SETTING: A retrospective cohort study was performed in the Bankstown-Lidcombe Hospital, Sydney, Australia, from October 2008 to April 2009. METHODS: Patients, aged 65-90 years, diagnosed with delirium according to the International Classification of Diseases (ICD)-10 in the Psychogeriatric Unit of Bankstown Lidcombe Hospital from January 2002 to October 2008 were reviewed. All the patients were measured according to the Confusion Assessment Method upon admission. Those who developed delirium during hospitalization were excluded.MAIN OUTCOME MEASURES: Cause of delirium, wandering, aggression, duration of delirium, physical restraint, use of antipsychotic medicine, recovery from delirium, discharge back home, length of stay, one-to-one nursing care, falls and absconding rate.RESULTS: The 131 age-matched delirious patients comprised 54 males and 77 females. The behavioral disorders of wandering [odds ration (OR) = 2.612, 95% confidence interval (CI) = 1.26 -5.413, P = 0.009] and aggression (OR = 2.243, 95% CI = 1.028 - 4.891, P= 0.04) were more frequent in males than in females. More males received one-to-one nursing care (OR = 4.114, 95% CI = 1.355 - 12.491, P = 0.008), were more likely to receive antipsychotic medications (OR = 2.24, 95% CI = 1.095-4.583, P = 0.021) and more likely to be physically restrained (OR = 2.07, 95% CI = 1.02-1.02, P = 0.043) compared with female patients. All absconding patients (3/131, 2.3%) were male. In addition, male patients displayed a greater falling rate compared with females (OR = 4.57, 95% CI= 1.519-13.722, P = 0.004).CONCLUSION: There are gender differences in challenging behaviors, management and outcomes in elderly delirious patients. Males with delirium display more challenging behaviors that require physical restraint and pharmacological management including wandering and aggression; males also abscond and have a higher rate of falls compared with female patients.
文摘Objective:To introduce the experiences in the application of island myocutaneous flap for challenging wound on cervico-thoracic region. Methods: Different myocutaneous flaps were selected according to the location, peculiarity and etiological factor of wound. There were 28 cases of island pectoralis major island myocutanuous flaps, 34 cases of latissimus dorsi island myocutaneous flaps, 19 cases of trapizius island myocutaneous flaps and 17 cases of rectus abdominis island myocutaneous flaps in this report. Results: All 98 patients with challenging wound on cervico-thoracic region were successfully treated with this method without complications, and obtained functional and cosmetic effectiveness. Conclusion:Challenging wounds in cervico-nuchal region can be repaired with pertoralis major island myocutaneous flap, latissimus dorsi island myocutaneous flap and trapizius island myocutaneous flap, while challenging wounds in thoracic region can be repaired with latissimus dorsi island myocutaneous flap and rectus abdominis island myocutaneous flap. Satisfactory functional and cosmetic results can be obtained.

文摘Objectives: To challenge the expiry dates of low concentration high purity mycotoxins standards. Literature Review: Aflatoxins (AFs) and Ochratoxin A (OTA) are persistent mycotoxins with adverse effects on humans. Mycotoxins standards are purchased to determine mycotoxin concentrations in food and may be stocked in some laboratories beyond expiry dates causing laboratories financial losses. Methods: Certified mycotoxins standards were purchased over the years from the same supplier at times and at other times from two different suppliers for quality control purposes. For AFs, six chromatographic runs for each of the mycotoxins standards were done to compare the difference among these standards having the following expiry dates (2008, 2012, 2013 and 2018). AFs standards purchased/obtained from two different suppliers in 2016 and expiring in 2018 were also compared. For OTA, the difference of concentration obtained between two years (2010 and 2018) was tested. All samples were run on a HPLC equipped with a fluorescence detector. Linearity of calibration curves and the points of lowest detection were determined for AFs components and for OTA from the unexpired mycotoxins standards. Results: At a 0.05 significance level and using non parametric tests, the statistical test revealed a p of 0.166, 0.153, 0.358 and 0.03 for B1, G1, B2 and G2 respectively among years for standards from same supplier and 0.037, 0.109, 0.182 and 0.182 for B1, G1, B2 and G2 respectively for unexpired standards from two different suppliers. For OTA, a p of 0.109 was obtained for standards of different expiry dates purchased from different suppliers. Conclusion: High purity low concentration mycotoxin standards purchased a decade ago (i.e. expired) did not differ from those purchased this current year (still valid). Hence, the expiry date can be renewed reducing the laboratories expenses. Manufacturers are urged to reconsider the expiry dates.

文摘The dynamic motion of quadrupedal robots on challenging terrain generally requires elaborate spatial–temporal kinodynamic motion planning and accurate control at higher refresh rate in comparison with regular terrain.However,conventional quadrupedal robots usually generate relatively coarse planning and employ motion replanning or reactive strategies to handle terrain irregularities.The resultant complex and computation-intensive controller may lead to nonoptimal motions or the breaking of locomotion rhythm.In this paper,a kinodynamic optimization approach is presented.To generate long-horizon optimal predictions of the kinematic and dynamic behavior of the quadruped robot on challenging terrain,we formulate motion planning as an optimization problem;jointly treat the foot’s locations,contact forces,and torso motions as decision variables;combine smooth motion and minimal energy consumption as the objective function;and explicitly represent feasible foothold region and friction constraints based on terrain information.To track the generated motions accurately and stably,we employ a whole-body controller to compute reference position and velocity commands,which are fed forward to joint controllers of the robot’s legs.We verify the effectiveness of the developed approach through simulation and on a physical quadruped robot testbed.Results show that the quadruped robot can successfully traverse a 30°slope and 43% of nominal leg length high step while maintaining the rhythm of dynamic trot gait.

文摘Coalescence of traditional medicine Ayurveda and in silico technology is a rigor for supplementary development of future-ready effective traditional medicine. Ayurveda is a popular traditional medicine in South Asia, emanating worldwide for the treatment of metabolic disorders and chronic illness. Techniques of in silico biology are not much explored for the investigation of a variety of bioactive phytochemicals of Ayurvedic herbs. Drug repurposing, reverse pharmacology, and polypharmacology in Ayurveda are areas in silico explorations that are needed to understand the rich repertoire of herbs, minerals, herbo-minerals, and assorted Ayurvedic formulations. This review emphasizes exploring the concept of Ayurveda with in silico approaches and the need for Ayurinformatics studies. It also provides an overview of in silico studies done on phytoconstituents of some important Ayurvedic plants, the utility of in silico studies in Ayurvedic phytoconstituents/formulations, limitations/challenges, and prospects of in silico studies in Ayurveda. This article discusses the convergence of in silico work, especially in the least explored field of Ayurveda. The focused coalesce of these two domains could present a predictive combinatorial platform to enhance translational research magnitude. In nutshell, it could provide new insight into an Ayurvedic drug discovery involving an in silico approach that could not only alleviate the process of traditional medicine research but also enhance its effectiveness in addressing health care.
文摘Accurate diagnosis is the foundation of clinical care but accurate diagnosis is not easily reached in some cases.In rare instances,even a sophisticated multidisciplinary team at an academic medical center cannot reliably reach an accurate diagnosis after extensive testing and imaging,and has to wait until histological diagnosis or even autopsy results are available.The underlying reason of challenging diagnoses is mostly conflicting data from history,tests,and imaging that point to different diagnoses.In this issue of World Journal of Clinical Cases,Huffaker et al reported such a challenging case of a tricuspid mass in a patient with Li-Fraumeni syndrome.The case by Huffaker et al powerfully illustrates the occasional diagnostic challenges inherent in our current diagnostic approach and the current technology.Clinicians should realize that in rare situations,agnosticism in diagnosis is unavoidable but a treatment has to be initiated so long as the principle of primum non nocere is upheld.
文摘BACKGROUND Combined spinal-epidural(CSE)anesthesia is the preferred anesthesia method for cesarean delivery.The use of an epidural catheter is essential for administering additional drugs intraoperatively and managing postoperative pain.However,the insertion of epidural catheters is associated with various complications,such as total spinal anesthesia,symptoms indicative of spinal nerve root irritation,and challenges in epidural catheter removal.CASE SUMMARY We present a case report of a challenging epidural catheter removal due to knotting.The lumbar computed tomography scan results revealed that the catheter formed a tight knot in the epidural space.We used a novel extubation method and successfully removed the catheter.CONCLUSION The operator can use opposite forces to"spiral"apart the spinal joints by positioning the patient's body in a specific position.The findings indicate that,when combined with imaging examination results,this method is effective for the removal of epidural catheters.
文摘Currently, most public higher learning institutions in Tanzania rely on traditional in-class examinations, requiring students to register and present identification documents for examinations eligibility verification. This system, however, is prone to impersonations due to security vulnerabilities in current students’ verification system. These vulnerabilities include weak authentication, lack of encryption, and inadequate anti-counterfeiting measures. Additionally, advanced printing technologies and online marketplaces which claim to produce convincing fake identification documents make it easy to create convincing fake identity documents. The Improved Mechanism for Detecting Impersonations (IMDIs) system detects impersonations in in-class exams by integrating QR codes and dynamic question generation based on student profiles. It consists of a mobile verification app, built with Flutter and communicating via RESTful APIs, and a web system, developed with Laravel using HTML, CSS, and JavaScript. The two components communicate through APIs, with MySQL managing the database. The mobile app and web server interact to ensure efficient verification and security during examinations. The implemented IMDIs system was validated by a mobile application which is integrated with a QR codes scanner for capturing codes embedded in student Identity Cards and linking them to a dynamic question generation model. The QG model uses natural language processing (NLP) algorithm and Question Generation (QG) techniques to create dynamic profile questions. Results show that the IMDIs system could generate four challenging profile-based questions within two seconds, allowing the verification of 200 students in 33 minutes by one operator. The IMDIs system also tracks exam-eligible students, aiding in exam attendance and integrates with a Short Message Service (SMS) to report impersonation incidents to a dedicated security officer in real-time. The IMDIs system was tested and found to be 98% secure, 100% convenient, with a 0% false rejection rate and a 2% false acceptance rate, demonstrating its security, reliability, and high performance.
文摘Diabetes mellitus(DM)and obesity have become public issues of global concern.Bariatric surgery for the treatment of obesity combined with type 2 DM has been shown to be a safe and effective approach;however,there are limited studies that have systematically addressed the challenges of surgical treatment of obesity combined with DM.In this review,we summarize and answer the most pressing questions in the field of surgical treatment of obesity-associated DM.I believe that our insights will be of great help to clinicians in their daily practice.

文摘The Gaoligong Mountains(GLGM),located in southwestern China,extend north to south along the western border of the Hengduan Mountains,spanning approximately 600 km.In this study,we consolidated findings from 17 bird surveys conducted in the GLGM between 2010 and 2022.We found that the GLGM harbors tremendous bird diversity,with a total of 796 documented bird species in the region.Nearly a quarter(23.0%)of these species are listed as state key protected species or as Chinese and global threatened species.Analysis of species richness at the county level showed a decreasing trend with increasing latitude,with the greatest diversity in Yingjiang(661 species).Observations indicated that the GLGM belongs to the Oriental realm,primarily composed of bird species from southern and southwestern China.The GLGM plays an important role in avian conservation by sheltering exceptional bird diversity,providing corridors and flyways for bird migration and dispersal,and mitigating the effects of climate change.In response to the conservation needs of birds and other wildlife,the Chinese government has established numerous protected areas within the GLGM.Despite these efforts,avian conservation still faces considerable challenges in the GLGM due to limitations in the protected area network,transboundary nature of the regions,and existing gaps in monitoring and research.
文摘The integration of 7 Tesla magnetic resonance imaging(7 T MRI)in adult patients has marked a revolutionary stride in radiology.In this article we explore the feasibility of 7 T MRI in paediatric practice,emphasizing its feasibility,applications,challenges,and safety considerations.The heightened resolution and tissue contrast of 7 T MRI offer unprecedented diagnostic accuracy,particularly in neuroimaging.Applications range from neuro-oncology to neonatal brain imaging,showcasing its efficacy in detecting subtle structural abnormalities and providing enhanced insights into neurological conditions.Despite the promise,challenges such as high cost,discomfort,and safety concerns necessitate careful consideration.Research suggests that,with precautions,7 T MRI is feasible in paediatrics,yet ongoing studies and safety assessments are imperative.
文摘BACKGROUND Inflammatory bowel disease,including ulcerative colitis,microscopic colitis,and Crohn’s disease(CD),has a global impact.This review focuses on duodenal CD(DCD),a rare subtype affecting the duodenum.DCD’s rarity and asymptomatic nature create diagnostic challenges,impacting prognosis and patient well-being.Delayed diagnosis can worsen DCD outcomes.AIM To report a rare case of DCD and to discuss the diagnostic challenges and its implications on prognosis.METHODS A systematic literature search,following the PRISMA statement,was conducted.Relevant studies were identified and analysed using specific Medical Subject Terms(MeSH)from PubMed/MEDLINE,American Journal of Gastroenterology,and the University of South Wales database.Data collection included information from radiology scans,endoscopy procedures,biopsies,and histopathology results.RESULTS The review considered 8 case reports and 1 observational study,involving 44 participants diagnosed with DCD,some of whom developed complications due to delayed diagnosis.Various diagnostic methods were employed,as there is no gold standard workup for DCD.Radiology scans[magnetic resonance imaging(MRI),computed tomography(CT),and upper gastrointestinal X-ray],endoscopy procedures(colonoscopy and esophagogastroduodenoscopy),biopsies,and clinical suspicions were utilized.CONCLUSION This review discusses DCD diagnosis challenges and the roles of CT,MRI,and fluoroscopy.It notes their limitations and compares findings with endoscopy and histopathology studies.Further research is needed to improve diagnosis,emphasizing scan interpretation,endoscopy procedures,and biopsies,especially in high-risk patients during routine endoscopy.

文摘Metal-organic frameworks(MOFs)possess a distinct advantage over conventional heterogeneous photocatalysts because of their carefully defined architecture and particular pores,which facilitate the targeted incorporation of other efficient cocatalysts or semiconductor materials.The integration of MOFs with other materials has resulted in significant breakthroughs,as the coupled materials improve the performance due to the combined effect.The unique MOF structures allow them to host foreign materials,which results in harvesting the visible region of the solar spectrum and effectively mitigating charge recombination by promoting charge separation.The review presents an evaluation of the latest developments in the utilization of surface and/or pore chemistry of MOF-supported heterojunctions for photocatalytic green-hydrogen generation with a basic understanding of the mechanism involved.The review begins with the basic principles of photocatalysis,the significance of MOFs,their optical properties,the methods used for synthesizing MOFs,and their coordination with other inorganic and polymeric materials.Furthermore,methods to increase photocatalytic H2 evolution using MOF-supported heterojunction have been proposed as standard practice.Lastly,to address environmental challenges,we highlight the future potential of MOF-supported heterojunctions for use in green-energy production.We hope that this review provides guidance to researchers in the development of effective heterojunctions based on MOFs to address challenges in energy applications and catalytic processes.
