The Guinean Water Company (SEG) faces challenges in supplying water to the entire city due to its limited area of approximately 420 km<sup>2</sup> and a population of 2 million. Moreover, the population growth rate is anticipated to increase dramatically, exacerbating the difficulty of fulfilling future water requirements. As a result, this study employs data from the Knoema website and the Institute of National Statistics of Guinea (INS) to examine the water production and distribution process by the SEG. The study examines the challenges related to water scarcity in Conakry and suggests strategies to assist the city in managing the present circumstances and planning for the future. The primary objective is to get a long-lasting water supply and uphold water quality to advance public health.展开更多

It is based on the principle of ultrasound, which enables liver tissue elasticity to be quantified using a probe, and fibrosis to be assessed. Fibroscan measures both elasticity correlated with hepatic fibrosis and CAP correlated with steatosis. The aim of this study was to evaluate hepatic fibrosis and steatosis using pulse elastometry (Fibroscan/CAP). Methods: This was a descriptive and analytical cross-sectional study in which 170 patients were included. It was conducted from October 1 2021 to December 31 2023, i.e. 27 months, in an outpatient clinic in the hepato-gastroenterology department of the Donka national hospital of the CHU Conakry. Results: Of the 170 patients identified, 87 were male (51%) and 83 female (49%), giving a M/F sex ratio of 1.04. The average age of our patients was 40. The 30 - 50 age group was the most affected, with a frequency of 58.23% (n = 99), followed by the 50 age group with a frequency of 29.41% (n = 50). Hepatomegaly, steatotic liver on ultrasonography, transaminase elevation and obesity were the main indications, respectively: (21.76%), (17.65%), (14.71%), and (13.53%). The examinations were requested by hepatogastroenterologists (47.06%), diabetologists (35.88%) and general practitioners (29%). Of the 170 patients, 100 patients (58.82%) had no significant fibrosis F0F1, 39 (22.94%) had moderate fibrosis F2, 20 patients (11.76%) had severe fibrosis F3 and 11 patients (6.47%) had fibrosis F4. Hepatic steatosis: 62 patients (36.47%) had no S0 steatosis;29.41% had S1 steatosis, 20% had S2 steatosis and 24 patients (14.11%) had S3 steatosis. Abdominal ultrasound revealed a normal liver in 67.05% of patients, hepatic steatosis in 29.41% and non-decompensated cirrhosis in 6 cases. Thus, 108 patients had the parameters required to calculate the Fatty Liver Index (FLI), steatosis was present in 20% of our patients, while 29.41% had an undetermined status and 24 14.11% had a normal FLI. Conclusion: Identifying subjects at risk of metabolic steatopathy, diagnosing and managing these patients is a public health issue and one of the future challenges of hepato-gastroenterology. Fibroscan is an increasingly popular screening tool for hepatic fibrosis and steatosis. The fight against obesity must be a priority.展开更多

However, this condition is increasingly observed in immunocompetent subjects. The aim of this study was to determine the endoscopic prevalence, clinical characteristics and risk factors for the occurrence of oesophageal mycosis in our department. Patients and Method: This was a prospective cross-sectional study of all patients who underwent oeso-gastroduodenal fibroscopy during the period from 1<sup>st </sup>January to 31<sup>st</sup> December 2022, i.e. one year, at the digestive endoscopy unit of the hepato-gastroenterology department of the Donka CHU national hospital in Conakry. All patients found to have oesophageal mycosis by FOGD were included. The endoscopy was performed using appropriate equipment: A Fujinon 4400 video endoscopy column;Three Fujinon EG 590 video gastroscopes;A hoover;Data were collected using a pre-established survey form and analysed using Epi info software version 6.0.4;Pearson’s Chi2 test as a test of independence and the exact 5% threshold ficher test. Results: Out of 1343 upper gastrointestinal endoscopies performed, 107 cases of oesophageal mycosis were found, representing a prevalence of 7.96%. The mean age was 40 years, with a male predominance of 55.42%. The sex ratio M/F was 1.24. The 45 and over age group was the most affected, with a prevalence of 40.43%, followed by the [35 - 45] age group, with a prevalence of 22.43%. Clinical symptoms were dominated by epigastralgia in 74.76% of cases, followed by odynophagia in 37.38% of cases, nausea and vomiting in 28.03% of cases, and pyrosis in 26.16% of cases. Oesophageal mycosis without oesophagitis was the most common endoscopic finding in 70% of cases. The main associated endoscopic lesions were erythemato-erosive and congestive gastropathy in 28.03% of cases, peptic oesophagitis (9.34%) and gastric ulcer (5.60%). The main risk factors found were positive HIV serology in 39.25% of cases, and diabetes in 24.30% of cases, with a statistically significant relationship of 0.02 and 0.01 respectively. Conclusion: Oesophageal mycosis is the most common opportunistic infection in patients with impaired cellular immunity. The prevalence of oesophageal mycosis in our series was 7.96%. This study enabled us to identify the main risk factors for the occurrence of oesophageal mycosis. Our country needs to step up its programme to combat and prevent immunodeficiency diseases, particularly HIV and diabetes.展开更多

The aim of this study was to contribute to improving the processing conditions of reusable care instruments in dental practices in the city of Conakry. Methods: We carried out an observational and cross-sectional study of a descriptive type lasting three months from January to April 2022 in public and private dental practices approved for activities. Results: A total of 48 dental practices were surveyed, including 7 public and 41 private. 91.67% had the trays for instrument sterilization. 77.08% had a sterilization device. The instruments were: cleaned 100%, disinfected 70.83%, sterilized 20.83% and 20.83% reused the instruments without being sterilized. 72.97% sterilized the instruments at the end of the day. 50% of respondents declared that there were six stages of sterilization. According to standard standards, one dental office or 2.70% respected the normal sterilization process and 13.51% respected the duration and temperature. The storage quality was inadequate at 95.83%. 95.83% of dental practices are at high risk of contamination. The difficulties encountered by staff in daily activity during our study were lack of hygienists (87.50%), insufficient material resources (58.33%), and lack of protocol procedure (45.83%). Conclusion: This study allowed us to note shortcomings of dental surgeons in the process of processing reusable care materials.展开更多

Method: This was a cross-sectional study carried out at tuberc...Objective: To determine the prevalence of diabetes and the level of cardiovascular risk in tuberculosis patients treated in Conakry and surrounding towns. Method: This was a cross-sectional study carried out at tuberculosis treatment centers in Conakry and surrounding towns (Coyah and Dubréka) on World TB Day, November 14, 2022, among 350 tuberculosis patients. We determined the proportion of TB patients with diabetes and high hyperglycemia. We determined the Chi-square to identify the factors associated with the occurrence of diabetes in tuberculosis patients, and a significance threshold with p Results: The average age of participants was 33.5 ± 14.5 years, and they were predominantly male (59.7%). The majority of participants lived in Conakry (96.3%). 5.7% were known diabetics and 4.6% were known hypertensives. We found a prevalence of diabetes of 9.4%. Among diabetics, 39% were diabetes discoveries. 30% were hypertensive or had high blood pressure. Of these hypertensive patients, 26.4% were unrecognized hypertensives. Age and hypertension were statistically significantly associated with the onset of diabetes in tuberculosis patients. Medium, high and very high levels of cardiovascular risk were present in 23.4%, 8.9% and 3.4% respectively. Conclusion: Diabetes is common in tuberculosis patients. Cardiovascular risk factors, notably hypertension and cardiovascular risk level, are frequently associated with tuberculosis. Tuberculosis patients with cardiovascular risk factors should be screened for diabetes.展开更多

Materials and Methods: This was a retrospective descriptive study, conducted over a period of five (05) years from January 1, 2018 to December 31, 2022, in the general surgery department of Ignace Deen Hospital, CHU Conakry. We included in our study all patient records in whom vulvectomy was performed. Results: We recorded 15 cases of vulvectomy out of 453 perineal surgeries, i.e. 3.31%, with a mean age of 43.56 years and extremes of 35 and 69 years. Vulvar cancer was the most common diagnosis (46.67%), followed by Buschke-Lowenstein (33.33%) and anal canal cancer extending to the vulva (20%). Six patients had undergone biopsy (40%). Vulvectomy with lymph node dissection was performed in only 9 patients (60%), and all surgical specimens were sent to anatomical pathology (100%). Conclusion: Vulvectomy is a surgical technique most often indicated for the treatment of vulvar cancer.展开更多

They affect different devi...Introduction: Thyroid pathologies are either the cause or a consequence of certain pathologies with which they interact, manifesting themselves through a polymorphic and non-specific clinic. They affect different devices and/or systems, making the clinical picture more complex or even a delay in diagnosis and treatment. These latter aspects, being then multidimensional, could be preceded by a very specific patient itinerary for the diagnosis and management of cases of thyroid gland pathologies. Methods: This was a descriptive cross-sectional study whose data were collected from May 1 to October 30, 2022. It focused on patients followed for thyroid diseases at the Conakry University Hospital. The objectives were to describe the patient’s journey before their diagnosis of thyroid disease. Results: The subjects were 86.1% women with a mean age of 43.7 ± 28 years. Of the total, 67.85% had a comorbidity including diabetes and/or hypertension/heart disease. They are followed for goiters/simple nodules (22.78%), hypothyroidism (16.46%) and hyperthyroidism (60.76%). They spend at least 9 months before resorting to a healthcare facility. The general medicine services of the Conakry University Hospital are the first recourse (51.9%) of these subjects. They consult 2 to 4 times before the diagnosis is suggested. Hyperthyroidism, hypothyroidism and goiters/simple nodules spend 5.4;6.4 and 13.7 months respectively before the diagnosis is made. 31.6% were hospitalized and treated mainly for malaria (19.0%), hypertension (13.1%), salmonellosis (13.1%) and diabetes (8.3%). A third of patients used traditional therapy before being in contact with the endocrinology department by referral (73.4%). Conclusion: This study made it possible to show a pre-diagnostic pathway which has a particular character concerning patients followed for thyroid pathologies in Guinea.展开更多

This leads to health risk factors for inexperienced staff. In the bodywork painting workshop, the staff seemed less interested in the risks probably due to negligence or by lack of knowledge. This work aimed to describe the working conditions and their impact on the workers’ health in a workshop of bodywork painting in Conakry. Material and Methods: This was a cross-sectional study over 06 months (from July 01, 2021, to December 31, 2021). Were included the bodybuilders-painters, the painters and the bodybuilders. The data was collected during an interview. We analysed the personal data of the workers, the physical environment factors (lighting, noise, etc.) and, the clinical manifestations felt by the workers. Results: The average age was 37 years extenting from 18 to 54 years and, they were all men. Over 80% of workers were exposed to more than 1000 lux and, 78.2% of workers were exposed to the vibratory intensity level of the cordless drill > 2.5 m/s2. The most frequent symptoms were back pain, headache, itchy eyes, and numbness of fingers and hands. The analysis of working conditions and clinical manifestations showed a significant relationship between the level of illumination and the tingling eyes (p = 0.0007), the vibratory intensity of the drill and the numbness of fingers and hands (p = 0.01). This study revealed that some of the complaints cited are related to the working conditions. Conclusion: Working conditions in a bodywork paint workshop are occupational risk factors that become dangerous if they are unknown. A longitudinal study on the assessment of working conditions could better enlighten us on this phenomenon.展开更多

The objective of this study was to musculoskeletal disorders among SOGEAC handlers. Methodology: This was a descriptive and analytic cross-sectional study over 6 months from November 2021 to April 2022. Results: We collected 110 handlers during the study. All of our population had postural constraints and were men. The average age of workers was 39.2 years. 93 handlers or 84.5% did manual handling, and 79% or 71.8% were baggage handlers. Regarding training, 88.2% or 97 handlers were trained, and 89.1% did alternating work. The standing posture was the most adopted with 89.1% followed by the kneeling position with 66.4%. 89.1% carried weights at work. 80 handlers or 73% had developed musculoskeletal disorders and the lumbar seat was found at 70%. The analysis of factors that may influence the occurrence of musculoskeletal disorders did not find any statistically significant relation. Conclusion: Our study shows that musculoskeletal disorders are an occupational health problem and recommends better compliance with occupational safety and health instructions.展开更多

It is one of the major causes of infant morbidity and mortality in developing countries. The general objective was to study the epidemiological, clinical and prognostic aspects of measles in the paediatric department of Conakry University Hospital. Methods: This was a prospective descriptive study lasting six months, from 01 October 2021 to 31 March 2022, in the paediatric wards of Conakry University Hospital. It included all patients aged between 0 and 15 years, received in consultation and/or hospitalised for measles and whose parents agreed to participate in the study. Results: Out of 3916 cases of paediatric pathology, we collected 380 cases of measles, i.e., 9.7%. The 0 - 4 age group was the most affected, accounting for 60.8%, with an average age of 26.6 ± 18.8 months and extremes of 3 months and 150 months. Females predominated in 53.2% of cases, with a sex ratio of 0.9. Non-vaccinated children accounted for 226 cases (59.5%). Fever was the most common clinical sign in the study (97.11%), followed by oculonasal catarrh (83.9%) and maculopapular lesions (82.1%). A notion of contagion was noted in 254 patients, i.e., a rate of 66.8%. The main complications were ocular in 229 cases (60.3%), pulmonary in 139 cases (36.6%) and digestive in 124 cases (32.6%). Malnutrition was noted in only 15 patients (3.93%). Treatment was symptomatic. The outcome was favourable in 92.3% of cases. We recorded 20 cases of death (5.3%) and 9 cases of escape (2.4%). Conclusion: Despite progress in the control and management of measles, much remains to be done to improve vaccination coverage with the aim of eradicating this disease.展开更多

The objectives of this study were to describe the sociodemographic characteristics, the different types of perineal tears and to identify the associated risk factors. Patients and Method: The maternity of the CMC of Ratoma served as a setting for carrying out this study. This was a prospective study of a descriptive and analytical type, which took place over a period of 6 months from July 1, 2021 to January 1, 2022. Results: The frequency of perineal tear was 9%. The average age of the patients concerned was 23.99 years ± 5.18 years, with extremes of 15 and 38 years. Housewives were the most numerous (30.88%). Most of our parturient had undergone excision (99.6%) and they were more frequently primiparous (40.44%) and pauciparous (47.79%). The multiplicity of risk factors was a remarkable fact (scarred perineum, edematous, short, and infected). The presentation was cephalic in 96.33%, posterior variety in 3.81% and the variety of positions could not be specified by the health worker who provided care to the parturient in 46.56%. The expulsion took place in OP (92.37%) and in OS (2.30%). In most cases, midwives carried out deliveries (93%). Perineal tears were 1st degree (54.41%), 2nd degree (39.70%), 3rd degree (5.15%) and 4th degree (0.74%). Conclusion: Prevention of perineal obstetric injuries is based on better knowledge of risk factors and delivery by qualified personnel. The main limitation of this study is the impossibility of highlighting occult perineal lesions due to the weakness of the technical platform.展开更多

The a...Introduction: The management of urinary lithiasis in our settings has long been conventional surgery. The recent introduction of endoscopy of the upper urinary tract represents a major advance in our department. The aim was to contribute to the study of the treatment of lithiasis of the upper urinary tract in Conakry and to report on our initial experience of ureteroscopy in Conakry. Methodology: This is a prospective descriptive study lasting one year, from January 1 to December 31, 2022, carried out at the Urology Department of the Ignace Deen National Hospital and at the PERCHIN Urological Clinic. The study included all patients treated surgically for lithiasis of the upper urinary tract who met the selection criteria. Results: The study included 57 patients. The mean age was 30 years, with extremes of 20 and 73 years;the most represented age group was 31 to 40 years, followed by 20 to 30 years with 29.9% and 26.3% respectively. Males predominated in 36 cases (63.15%), with a M/F sex ratio of 1.71. Left-sided renal colic was the most common, at 75.43% (n=43), and right-sided renal colic at 40.35%, associated with digestive signs at 43.85%. 77.19% had normal creatinine levels before surgery, versus 22.81% with elevated creatinine levels, i.e. 15.78% improvement in renal function after surgery. Urinary tract infection was found in 47.36%, and Escherichia coli in 31.57%, followed by staphylococcus aureus in 8.77%. Overall, 82.45% of patients had organic damage to the kidneys or ureters. Right ureterohydronephrosis was noted in 56.14%, followed by left hydronephrosis in 26.32%. Open surgery was predominant in 52.63% of cases, with endoscopic surgery (URS) accounting for a significant 47.36% during the study period. The endoscopic treatment used was laser ureteroscopy. In our study, 73.68% underwent drainage of the upper excretory tract, including 64.91% with a JJ catheter. The average length of stay was 07 days, with extremes of 02 and 28 days, and 81.4% of patients who underwent URS had a length of stay of less than 4 days. Conventional surgery had the highest complication rate (14.28%). The stone free rate for the two surgical treatment methods (open and endoscopic surgery) was 73% and 92.59%.展开更多

Material and Methods: This is a prospective, descriptive, cross-sectional study lasting six (6) months, from July 1, 2022 to December 31, 2022. Patients with BPH on comorbidity condition taken care of during the study period AND have agreed to participate in the study. Results: During our study, 49 cases of benign prostatic hypertrophy with comorbidities were collected, representing a frequency of 29%. The average age ranges for the patients were 43 - 70 years. The age group most affected was 70 to 79 years old (38.80%). Nocturnal urinary frequency was the main reason for consultation present in all our patients. The most frequent comorbidity was hypertension, i.e. 83.70%. The PSA rate between 4 and 10 was the most represented, i.e. 42.86%. The prostate volume was between 61 and 100 ml in 40.82% of patients. Histology showed that it was a benign adenomatous hypertrophy of the prostate in 85.70% and a prostatic adenomyoma in 14.29%. Trans-bladder adenomectomy alone was the most performed technique, i.e. 49%, followed by trans urethral resection of the prostate, i.e. 38.80%. Retention of urine after removal of the catheter was the most observed complication, i.e. 12.20%. Conclusion: Benign prostatic hypertrophy with comorbidities constitutes a frequent association. Because their presence can affect effectiveness and lead to complications.展开更多

It quickly spreads like wildfire to all continents in ...The coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), dates back to December 29, 2019, in Wuhan, China. It quickly spreads like wildfire to all continents in the following months. In Guinea, the first case of COVID-19 and death were all reported respectively on March 12 and April 16, 2020. Since then, several studies have found a relationship between certain environmental conditions such as the meteorological factors to have the potential of contributing to the spread of the virus. Thus, this study aims at examining the extent to which observed meteorological factors might have contributed to the spread of the coronavirus disease 2019 (COVID-19) cases in Conakry, from March 1 to May 31, 2020. Meteorological factors such as temperature (T</span><sub><span style="font-family:Verdana;">min</span></sub><span style="font-family:Verdana;">, T</span><sub><span style="font-family:Verdana;">mean</span></sub><span style="font-family:Verdana;"> and T</span><sub><span style="font-family:Verdana;">max</span></sub><span style="font-family:Verdana;">) and relative humidity (RH</span><sub><span style="font-family:Verdana;">min</span></sub><span style="font-family:Verdana;">, RH</span><sub><span style="font-family:Verdana;">mean</span></sub><span style="font-family:Verdana;"> and RH</span><sub><span style="font-family:Verdana;">max</span></sub><span style="font-family:Verdana;">) were analyzed together with the data on the COVID-19. The dynamic of the COVID-19 in Guinea was analyzed along with that of some west African countries. The analysis on the dynamic of the COVID-19 pandemic in West Africa indicated Guinea as one of the most affected countries by the pandemic after Nigeria and Ghana. The study found that in general an increase in the temperature is linked to a decline in the COVID-19 number of cases and deaths, while an increase in the humidity is positively correlated to the number of cases and deaths. Nevertheless, from this study it was also observed that low temperature, mild diurnal temperature and high humidity are likely to favor its transmission. The study therefore, recommends that habitations and hospital rooms should be kept in relatively low humidity and relatively higher temperature to minimize the spread of the (SARS-CoV-2).展开更多

Methodology: This was a retrospective, and 15-year descriptive study from January 1, 2000 to December 31, 2014 conducted at the Department of Anatomy and Pathological Cytology of the National Hospital Donka in collaboration with the obstetric gynecology departments of the Conakry University Hospital. Results: We collected 296 cases of malignant and benign endometrial hyperplasia in 15 years, accounting for 37% of all endometrial biopsy curettages examined. The age group 47 to 56 years was the most affected (81 cases) or 27, 36%. The mean age was 53.6 years with extremes of 27 and 83 years. Metrorrhagia was the main reason for consultation (206 cases), i.e. 69.59%. The suspicion of endometrial hyperplasia by physicians was the most frequently diagnosed circumstance (149 cases) or 50.33%. Biopsy curettage was the most frequently used method (176 cases), is 59.45%. Histological endometrial lesions of atypical complex adenomatous hyperplasia (79 cases) represented 26.69%. Benign behavior was most frequently observed in (235 cases) or 79.39%. Conclusion: Endometrial hyperplasia is an endometrial lesion whose atypical histological types represent the borderline lesions between benignity and malignancy.展开更多

Only few studies have been done on this rainfall peak ...Observed rainfall data of the National Meteorological Service of Guinea (NMS) exhibit that synoptic station usually records the largest rainfall amount in Guinea. Only few studies have been done on this rainfall peak observed in Conakry. This work better analyses the atmospheric dynamics leading to rainfall particularity. Using NMS data from 1981 to 2010, the monthly contribution and mean seasonal cycle of each station has been done. These findings of the study show that between July and August (rainfall season peak), the coastline particularly Conakry records the largest amount of rainfall. Using Era Interim data for the common period (1981-2010), we also investigate the rainfall dynamics in the lower level (1000 hPa - 850 hPa) from precipitable water, divergence, and moisture flow transport. There is a west and southwest moisture flow transport explained by a strong moisture convergence in the coastal region (Lower-Guinea). Furthermore, values of precipitable water in the same region are found, in agreement with the high moisture flow transport gradient. These incoming flow (west and south-west) undergo a return by blocking’s Kakoulima range (foehn effect) and Fouta Djallon massif to initiate convection clouds on the Guinean coast. These processes enhance a convergence of moisture associated with orographic origin convection. This has an important effect by increasing the rainfall amount in Conakry.展开更多

The objective of this study was to evaluate the level of knowledge, attitudes and practices of health care personnel in the management of hypertension in Conakry. Methods: Descriptive transversal study of 6 months, from April 20, 2018 to September 20, 2018, which took place in the internal medicine departments of the two university hospital centers (CHU) of Conakry (Donka and Ignace Deen). The study population consisted of general practitioners and specialists, practicing in both departments for at least two years and who agreed to submit to our questionnaire. Results: At the end of the study, we included 140 doctors, 61% at Donka University Hospital and 39% at Ignace Deen University Hospital. The predominance was male with 113 men for 27 women with a sex ratio H/F = 4.18. General practitioners were the most represented with 95% against 5% specialists. Sixty-four percent of physicians gave an exact definition of hypertension, 56% of physicians confirmed the diagnosis of hypertension after 2 to 3 consultations. 19% offered monotherapy as first-line treatment, only 72% said that the duration of Antihypertensive treatment was for life. Conclusion: Theseresults demonstrate that it is essential to reflect on our daily practices for a good improvement in the quality of care for patients.展开更多

The objectives of this study were to determine the frequency of TMJA, describe the clinical aspects and assess management. <strong>Materials and Method:</strong> It was a retrospective study that was carried out in the Department of Odontostomatology and Maxillofacial Surgery of Donka National Hospital for a period of 5 years (January 2016 to December 2020). Included were all records of inpatients and managed cases of TMJA during the study period. Socio-demographic, clinical and therapeutic variables were analyzed. <strong>Results:</strong> During the study 13 TMJA cases were collected with the frequency of 0.47%. The age group 1 - 9 was the most affected (61.54%) with extremes of 4 and 28 years. Men were the most concerned (53.87%). Restriction of mouth opening was the main reason for consultation (69.23%). The etiologies of ankylosis were dominated by infection including cellulitis of dental origin (53.85%) followed by facial trauma (30.77%). Arthroplasty was the most commonly used technique (73%), two cases of recurrence were noted (15.38%). <strong>Conclusion: </strong>TMJA affects mainly children and the etiology is dominated by cellulite of dental origin hence the interest of sensitization of the population for early management of oral diseases.展开更多

Methodology: It consisted of a prospective study over a period of 6 months from 01/02 to 31/07/2018. All patients transferred to the maternity ward of the Donka National Hospital of Conakry University Hospital. Results: The frequency of obstetric transfers was 13.79%. The epidemiological profile was that of a parturient woman of an average age of 25.7 years, married, and housewife, unschooled, who was on her first pregnancy and from the city of Conakry. The average distance covered was 16 km with extremes of 3 and 50 km. The transfer to the referral maternity clinic was not medicalized in 94% of cases. The venous route was not taken in 96% of cases. The parturient was not escorted by a health worker in 98% of cases. Bleeding was the most frequent reason for evacuation, followed by acute fetal sufferings. The average number of prenatal consultation was 2 with extremes of 0 and 9. The average length of stay was 3.6 days with extremes of 1 and 28 days. The majority of transferred women had a full-term pregnancy. The Cesarean section was 79.4%. The Retro placental hematoma was the most common complication found and was 29.4%. The counter-reference was not made in 97.79%. We recorded 8 maternal deaths, for a lethality rate of 1.77%. Possible interventions to reduce the dramatic situation of obstetric evacuations require first of all the decentralization of health care structures capable of performing a cesarean section. This approach should aim to create medical centers with a surgical antenna in all municipalities. These decentralized units would reduce the delay in case management and thus, limit the number of complications.展开更多

We must examine each situation with the many questions that arise and which sometimes require a long reflection. We report a case of separation of pygopages conjoined twins as well as a review of the literature. Patients and Observations: a pair of conjoined twins aged 11 days, weighing 3080 g between them, was referred to the neonatology department from the Faranah prefecture, 300 km from Conakry, for care. The mother, a 30-year-old housewife, multipara, eight gestures and nine parities, had not followed any prenatal consultation. The delivery took place at home in a village with the death of the mother immediately postpartum. The clinical and paraclinical investigation had concluded with the diagnosis of Siamese type pygopage. After multidisciplinary consultation, the surgical treatment by separation of the twins was carried out at the age of 50 days with success and preservation of the physiological functions. Conclusion: The birth of conjoined twins is extremely rare. Each pair of Siamese is different and the surgical strategy must be adapted according to the shared organs. The perfect multidisciplinary work of the medical staff has been the main contributor to our success.展开更多
文摘The capital of Guinea, Conakry, faces a notable difficulty due to a water shortage, which is worsened by the rapid increase in population and urban development. The Guinean Water Company (SEG) faces challenges in supplying water to the entire city due to its limited area of approximately 420 km<sup>2</sup> and a population of 2 million. Moreover, the population growth rate is anticipated to increase dramatically, exacerbating the difficulty of fulfilling future water requirements. As a result, this study employs data from the Knoema website and the Institute of National Statistics of Guinea (INS) to examine the water production and distribution process by the SEG. The study examines the challenges related to water scarcity in Conakry and suggests strategies to assist the city in managing the present circumstances and planning for the future. The primary objective is to get a long-lasting water supply and uphold water quality to advance public health.
文摘Introduction: Fibroscan is a recent, non-invasive and non-irradiating diagnostic method. It is based on the principle of ultrasound, which enables liver tissue elasticity to be quantified using a probe, and fibrosis to be assessed. Fibroscan measures both elasticity correlated with hepatic fibrosis and CAP correlated with steatosis. The aim of this study was to evaluate hepatic fibrosis and steatosis using pulse elastometry (Fibroscan/CAP). Methods: This was a descriptive and analytical cross-sectional study in which 170 patients were included. It was conducted from October 1 2021 to December 31 2023, i.e. 27 months, in an outpatient clinic in the hepato-gastroenterology department of the Donka national hospital of the CHU Conakry. Results: Of the 170 patients identified, 87 were male (51%) and 83 female (49%), giving a M/F sex ratio of 1.04. The average age of our patients was 40. The 30 - 50 age group was the most affected, with a frequency of 58.23% (n = 99), followed by the 50 age group with a frequency of 29.41% (n = 50). Hepatomegaly, steatotic liver on ultrasonography, transaminase elevation and obesity were the main indications, respectively: (21.76%), (17.65%), (14.71%), and (13.53%). The examinations were requested by hepatogastroenterologists (47.06%), diabetologists (35.88%) and general practitioners (29%). Of the 170 patients, 100 patients (58.82%) had no significant fibrosis F0F1, 39 (22.94%) had moderate fibrosis F2, 20 patients (11.76%) had severe fibrosis F3 and 11 patients (6.47%) had fibrosis F4. Hepatic steatosis: 62 patients (36.47%) had no S0 steatosis;29.41% had S1 steatosis, 20% had S2 steatosis and 24 patients (14.11%) had S3 steatosis. Abdominal ultrasound revealed a normal liver in 67.05% of patients, hepatic steatosis in 29.41% and non-decompensated cirrhosis in 6 cases. Thus, 108 patients had the parameters required to calculate the Fatty Liver Index (FLI), steatosis was present in 20% of our patients, while 29.41% had an undetermined status and 24 14.11% had a normal FLI. Conclusion: Identifying subjects at risk of metabolic steatopathy, diagnosing and managing these patients is a public health issue and one of the future challenges of hepato-gastroenterology. Fibroscan is an increasingly popular screening tool for hepatic fibrosis and steatosis. The fight against obesity must be a priority.
文摘Introduction: Oesophageal mycosis (OM) is one of the most common opportunistic infections in patients infected with HIV (Human Immunodeficiency Virus). However, this condition is increasingly observed in immunocompetent subjects. The aim of this study was to determine the endoscopic prevalence, clinical characteristics and risk factors for the occurrence of oesophageal mycosis in our department. Patients and Method: This was a prospective cross-sectional study of all patients who underwent oeso-gastroduodenal fibroscopy during the period from 1<sup>st </sup>January to 31<sup>st</sup> December 2022, i.e. one year, at the digestive endoscopy unit of the hepato-gastroenterology department of the Donka CHU national hospital in Conakry. All patients found to have oesophageal mycosis by FOGD were included. The endoscopy was performed using appropriate equipment: A Fujinon 4400 video endoscopy column;Three Fujinon EG 590 video gastroscopes;A hoover;Data were collected using a pre-established survey form and analysed using Epi info software version 6.0.4;Pearson’s Chi2 test as a test of independence and the exact 5% threshold ficher test. Results: Out of 1343 upper gastrointestinal endoscopies performed, 107 cases of oesophageal mycosis were found, representing a prevalence of 7.96%. The mean age was 40 years, with a male predominance of 55.42%. The sex ratio M/F was 1.24. The 45 and over age group was the most affected, with a prevalence of 40.43%, followed by the [35 - 45] age group, with a prevalence of 22.43%. Clinical symptoms were dominated by epigastralgia in 74.76% of cases, followed by odynophagia in 37.38% of cases, nausea and vomiting in 28.03% of cases, and pyrosis in 26.16% of cases. Oesophageal mycosis without oesophagitis was the most common endoscopic finding in 70% of cases. The main associated endoscopic lesions were erythemato-erosive and congestive gastropathy in 28.03% of cases, peptic oesophagitis (9.34%) and gastric ulcer (5.60%). The main risk factors found were positive HIV serology in 39.25% of cases, and diabetes in 24.30% of cases, with a statistically significant relationship of 0.02 and 0.01 respectively. Conclusion: Oesophageal mycosis is the most common opportunistic infection in patients with impaired cellular immunity. The prevalence of oesophageal mycosis in our series was 7.96%. This study enabled us to identify the main risk factors for the occurrence of oesophageal mycosis. Our country needs to step up its programme to combat and prevent immunodeficiency diseases, particularly HIV and diabetes.
文摘Introduction: The effective sterilization of reusable instruments in dental care is a crucial issue for public health. The aim of this study was to contribute to improving the processing conditions of reusable care instruments in dental practices in the city of Conakry. Methods: We carried out an observational and cross-sectional study of a descriptive type lasting three months from January to April 2022 in public and private dental practices approved for activities. Results: A total of 48 dental practices were surveyed, including 7 public and 41 private. 91.67% had the trays for instrument sterilization. 77.08% had a sterilization device. The instruments were: cleaned 100%, disinfected 70.83%, sterilized 20.83% and 20.83% reused the instruments without being sterilized. 72.97% sterilized the instruments at the end of the day. 50% of respondents declared that there were six stages of sterilization. According to standard standards, one dental office or 2.70% respected the normal sterilization process and 13.51% respected the duration and temperature. The storage quality was inadequate at 95.83%. 95.83% of dental practices are at high risk of contamination. The difficulties encountered by staff in daily activity during our study were lack of hygienists (87.50%), insufficient material resources (58.33%), and lack of protocol procedure (45.83%). Conclusion: This study allowed us to note shortcomings of dental surgeons in the process of processing reusable care materials.
文摘Objective: To determine the prevalence of diabetes and the level of cardiovascular risk in tuberculosis patients treated in Conakry and surrounding towns. Method: This was a cross-sectional study carried out at tuberculosis treatment centers in Conakry and surrounding towns (Coyah and Dubréka) on World TB Day, November 14, 2022, among 350 tuberculosis patients. We determined the proportion of TB patients with diabetes and high hyperglycemia. We determined the Chi-square to identify the factors associated with the occurrence of diabetes in tuberculosis patients, and a significance threshold with p Results: The average age of participants was 33.5 ± 14.5 years, and they were predominantly male (59.7%). The majority of participants lived in Conakry (96.3%). 5.7% were known diabetics and 4.6% were known hypertensives. We found a prevalence of diabetes of 9.4%. Among diabetics, 39% were diabetes discoveries. 30% were hypertensive or had high blood pressure. Of these hypertensive patients, 26.4% were unrecognized hypertensives. Age and hypertension were statistically significantly associated with the onset of diabetes in tuberculosis patients. Medium, high and very high levels of cardiovascular risk were present in 23.4%, 8.9% and 3.4% respectively. Conclusion: Diabetes is common in tuberculosis patients. Cardiovascular risk factors, notably hypertension and cardiovascular risk level, are frequently associated with tuberculosis. Tuberculosis patients with cardiovascular risk factors should be screened for diabetes.
文摘Introduction: The aim of this study was to report the indications and evaluate the results of vulvectomy in the general surgery department of Ignace Deen Hospital, CHU Conakry. Materials and Methods: This was a retrospective descriptive study, conducted over a period of five (05) years from January 1, 2018 to December 31, 2022, in the general surgery department of Ignace Deen Hospital, CHU Conakry. We included in our study all patient records in whom vulvectomy was performed. Results: We recorded 15 cases of vulvectomy out of 453 perineal surgeries, i.e. 3.31%, with a mean age of 43.56 years and extremes of 35 and 69 years. Vulvar cancer was the most common diagnosis (46.67%), followed by Buschke-Lowenstein (33.33%) and anal canal cancer extending to the vulva (20%). Six patients had undergone biopsy (40%). Vulvectomy with lymph node dissection was performed in only 9 patients (60%), and all surgical specimens were sent to anatomical pathology (100%). Conclusion: Vulvectomy is a surgical technique most often indicated for the treatment of vulvar cancer.
文摘Introduction: Thyroid pathologies are either the cause or a consequence of certain pathologies with which they interact, manifesting themselves through a polymorphic and non-specific clinic. They affect different devices and/or systems, making the clinical picture more complex or even a delay in diagnosis and treatment. These latter aspects, being then multidimensional, could be preceded by a very specific patient itinerary for the diagnosis and management of cases of thyroid gland pathologies. Methods: This was a descriptive cross-sectional study whose data were collected from May 1 to October 30, 2022. It focused on patients followed for thyroid diseases at the Conakry University Hospital. The objectives were to describe the patient’s journey before their diagnosis of thyroid disease. Results: The subjects were 86.1% women with a mean age of 43.7 ± 28 years. Of the total, 67.85% had a comorbidity including diabetes and/or hypertension/heart disease. They are followed for goiters/simple nodules (22.78%), hypothyroidism (16.46%) and hyperthyroidism (60.76%). They spend at least 9 months before resorting to a healthcare facility. The general medicine services of the Conakry University Hospital are the first recourse (51.9%) of these subjects. They consult 2 to 4 times before the diagnosis is suggested. Hyperthyroidism, hypothyroidism and goiters/simple nodules spend 5.4;6.4 and 13.7 months respectively before the diagnosis is made. 31.6% were hospitalized and treated mainly for malaria (19.0%), hypertension (13.1%), salmonellosis (13.1%) and diabetes (8.3%). A third of patients used traditional therapy before being in contact with the endocrinology department by referral (73.4%). Conclusion: This study made it possible to show a pre-diagnostic pathway which has a particular character concerning patients followed for thyroid pathologies in Guinea.
文摘Context: Working conditions in the car repair sector are difficult in general. This leads to health risk factors for inexperienced staff. In the bodywork painting workshop, the staff seemed less interested in the risks probably due to negligence or by lack of knowledge. This work aimed to describe the working conditions and their impact on the workers’ health in a workshop of bodywork painting in Conakry. Material and Methods: This was a cross-sectional study over 06 months (from July 01, 2021, to December 31, 2021). Were included the bodybuilders-painters, the painters and the bodybuilders. The data was collected during an interview. We analysed the personal data of the workers, the physical environment factors (lighting, noise, etc.) and, the clinical manifestations felt by the workers. Results: The average age was 37 years extenting from 18 to 54 years and, they were all men. Over 80% of workers were exposed to more than 1000 lux and, 78.2% of workers were exposed to the vibratory intensity level of the cordless drill > 2.5 m/s2. The most frequent symptoms were back pain, headache, itchy eyes, and numbness of fingers and hands. The analysis of working conditions and clinical manifestations showed a significant relationship between the level of illumination and the tingling eyes (p = 0.0007), the vibratory intensity of the drill and the numbness of fingers and hands (p = 0.01). This study revealed that some of the complaints cited are related to the working conditions. Conclusion: Working conditions in a bodywork paint workshop are occupational risk factors that become dangerous if they are unknown. A longitudinal study on the assessment of working conditions could better enlighten us on this phenomenon.
文摘Introduction: Musculoskeletal disorders are disorders of the musculoskeletal system related to work. The objective of this study was to musculoskeletal disorders among SOGEAC handlers. Methodology: This was a descriptive and analytic cross-sectional study over 6 months from November 2021 to April 2022. Results: We collected 110 handlers during the study. All of our population had postural constraints and were men. The average age of workers was 39.2 years. 93 handlers or 84.5% did manual handling, and 79% or 71.8% were baggage handlers. Regarding training, 88.2% or 97 handlers were trained, and 89.1% did alternating work. The standing posture was the most adopted with 89.1% followed by the kneeling position with 66.4%. 89.1% carried weights at work. 80 handlers or 73% had developed musculoskeletal disorders and the lumbar seat was found at 70%. The analysis of factors that may influence the occurrence of musculoskeletal disorders did not find any statistically significant relation. Conclusion: Our study shows that musculoskeletal disorders are an occupational health problem and recommends better compliance with occupational safety and health instructions.
文摘Introduction: Measles is a highly contagious viral infectious disease. It is one of the major causes of infant morbidity and mortality in developing countries. The general objective was to study the epidemiological, clinical and prognostic aspects of measles in the paediatric department of Conakry University Hospital. Methods: This was a prospective descriptive study lasting six months, from 01 October 2021 to 31 March 2022, in the paediatric wards of Conakry University Hospital. It included all patients aged between 0 and 15 years, received in consultation and/or hospitalised for measles and whose parents agreed to participate in the study. Results: Out of 3916 cases of paediatric pathology, we collected 380 cases of measles, i.e., 9.7%. The 0 - 4 age group was the most affected, accounting for 60.8%, with an average age of 26.6 ± 18.8 months and extremes of 3 months and 150 months. Females predominated in 53.2% of cases, with a sex ratio of 0.9. Non-vaccinated children accounted for 226 cases (59.5%). Fever was the most common clinical sign in the study (97.11%), followed by oculonasal catarrh (83.9%) and maculopapular lesions (82.1%). A notion of contagion was noted in 254 patients, i.e., a rate of 66.8%. The main complications were ocular in 229 cases (60.3%), pulmonary in 139 cases (36.6%) and digestive in 124 cases (32.6%). Malnutrition was noted in only 15 patients (3.93%). Treatment was symptomatic. The outcome was favourable in 92.3% of cases. We recorded 20 cases of death (5.3%) and 9 cases of escape (2.4%). Conclusion: Despite progress in the control and management of measles, much remains to be done to improve vaccination coverage with the aim of eradicating this disease.
文摘Introduction: Obstetric perineal tears are a common complication during childbirth. The objectives of this study were to describe the sociodemographic characteristics, the different types of perineal tears and to identify the associated risk factors. Patients and Method: The maternity of the CMC of Ratoma served as a setting for carrying out this study. This was a prospective study of a descriptive and analytical type, which took place over a period of 6 months from July 1, 2021 to January 1, 2022. Results: The frequency of perineal tear was 9%. The average age of the patients concerned was 23.99 years ± 5.18 years, with extremes of 15 and 38 years. Housewives were the most numerous (30.88%). Most of our parturient had undergone excision (99.6%) and they were more frequently primiparous (40.44%) and pauciparous (47.79%). The multiplicity of risk factors was a remarkable fact (scarred perineum, edematous, short, and infected). The presentation was cephalic in 96.33%, posterior variety in 3.81% and the variety of positions could not be specified by the health worker who provided care to the parturient in 46.56%. The expulsion took place in OP (92.37%) and in OS (2.30%). In most cases, midwives carried out deliveries (93%). Perineal tears were 1st degree (54.41%), 2nd degree (39.70%), 3rd degree (5.15%) and 4th degree (0.74%). Conclusion: Prevention of perineal obstetric injuries is based on better knowledge of risk factors and delivery by qualified personnel. The main limitation of this study is the impossibility of highlighting occult perineal lesions due to the weakness of the technical platform.
文摘Introduction: The management of urinary lithiasis in our settings has long been conventional surgery. The recent introduction of endoscopy of the upper urinary tract represents a major advance in our department. The aim was to contribute to the study of the treatment of lithiasis of the upper urinary tract in Conakry and to report on our initial experience of ureteroscopy in Conakry. Methodology: This is a prospective descriptive study lasting one year, from January 1 to December 31, 2022, carried out at the Urology Department of the Ignace Deen National Hospital and at the PERCHIN Urological Clinic. The study included all patients treated surgically for lithiasis of the upper urinary tract who met the selection criteria. Results: The study included 57 patients. The mean age was 30 years, with extremes of 20 and 73 years;the most represented age group was 31 to 40 years, followed by 20 to 30 years with 29.9% and 26.3% respectively. Males predominated in 36 cases (63.15%), with a M/F sex ratio of 1.71. Left-sided renal colic was the most common, at 75.43% (n=43), and right-sided renal colic at 40.35%, associated with digestive signs at 43.85%. 77.19% had normal creatinine levels before surgery, versus 22.81% with elevated creatinine levels, i.e. 15.78% improvement in renal function after surgery. Urinary tract infection was found in 47.36%, and Escherichia coli in 31.57%, followed by staphylococcus aureus in 8.77%. Overall, 82.45% of patients had organic damage to the kidneys or ureters. Right ureterohydronephrosis was noted in 56.14%, followed by left hydronephrosis in 26.32%. Open surgery was predominant in 52.63% of cases, with endoscopic surgery (URS) accounting for a significant 47.36% during the study period. The endoscopic treatment used was laser ureteroscopy. In our study, 73.68% underwent drainage of the upper excretory tract, including 64.91% with a JJ catheter. The average length of stay was 07 days, with extremes of 02 and 28 days, and 81.4% of patients who underwent URS had a length of stay of less than 4 days. Conventional surgery had the highest complication rate (14.28%). The stone free rate for the two surgical treatment methods (open and endoscopic surgery) was 73% and 92.59%.
文摘Goal: To determine the type of comorbidity and highlight the complications of adenomectomy and comorbidities. Material and Methods: This is a prospective, descriptive, cross-sectional study lasting six (6) months, from July 1, 2022 to December 31, 2022. Patients with BPH on comorbidity condition taken care of during the study period AND have agreed to participate in the study. Results: During our study, 49 cases of benign prostatic hypertrophy with comorbidities were collected, representing a frequency of 29%. The average age ranges for the patients were 43 - 70 years. The age group most affected was 70 to 79 years old (38.80%). Nocturnal urinary frequency was the main reason for consultation present in all our patients. The most frequent comorbidity was hypertension, i.e. 83.70%. The PSA rate between 4 and 10 was the most represented, i.e. 42.86%. The prostate volume was between 61 and 100 ml in 40.82% of patients. Histology showed that it was a benign adenomatous hypertrophy of the prostate in 85.70% and a prostatic adenomyoma in 14.29%. Trans-bladder adenomectomy alone was the most performed technique, i.e. 49%, followed by trans urethral resection of the prostate, i.e. 38.80%. Retention of urine after removal of the catheter was the most observed complication, i.e. 12.20%. Conclusion: Benign prostatic hypertrophy with comorbidities constitutes a frequent association. Because their presence can affect effectiveness and lead to complications.
文摘The coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), dates back to December 29, 2019, in Wuhan, China. It quickly spreads like wildfire to all continents in the following months. In Guinea, the first case of COVID-19 and death were all reported respectively on March 12 and April 16, 2020. Since then, several studies have found a relationship between certain environmental conditions such as the meteorological factors to have the potential of contributing to the spread of the virus. Thus, this study aims at examining the extent to which observed meteorological factors might have contributed to the spread of the coronavirus disease 2019 (COVID-19) cases in Conakry, from March 1 to May 31, 2020. Meteorological factors such as temperature (T</span><sub><span style="font-family:Verdana;">min</span></sub><span style="font-family:Verdana;">, T</span><sub><span style="font-family:Verdana;">mean</span></sub><span style="font-family:Verdana;"> and T</span><sub><span style="font-family:Verdana;">max</span></sub><span style="font-family:Verdana;">) and relative humidity (RH</span><sub><span style="font-family:Verdana;">min</span></sub><span style="font-family:Verdana;">, RH</span><sub><span style="font-family:Verdana;">mean</span></sub><span style="font-family:Verdana;"> and RH</span><sub><span style="font-family:Verdana;">max</span></sub><span style="font-family:Verdana;">) were analyzed together with the data on the COVID-19. The dynamic of the COVID-19 in Guinea was analyzed along with that of some west African countries. The analysis on the dynamic of the COVID-19 pandemic in West Africa indicated Guinea as one of the most affected countries by the pandemic after Nigeria and Ghana. The study found that in general an increase in the temperature is linked to a decline in the COVID-19 number of cases and deaths, while an increase in the humidity is positively correlated to the number of cases and deaths. Nevertheless, from this study it was also observed that low temperature, mild diurnal temperature and high humidity are likely to favor its transmission. The study therefore, recommends that habitations and hospital rooms should be kept in relatively low humidity and relatively higher temperature to minimize the spread of the (SARS-CoV-2).
文摘Summary: Estimate the incidence of endometrial hyperplasia according to socio-demographic parameters and the type of lesions histological. Methodology: This was a retrospective, and 15-year descriptive study from January 1, 2000 to December 31, 2014 conducted at the Department of Anatomy and Pathological Cytology of the National Hospital Donka in collaboration with the obstetric gynecology departments of the Conakry University Hospital. Results: We collected 296 cases of malignant and benign endometrial hyperplasia in 15 years, accounting for 37% of all endometrial biopsy curettages examined. The age group 47 to 56 years was the most affected (81 cases) or 27, 36%. The mean age was 53.6 years with extremes of 27 and 83 years. Metrorrhagia was the main reason for consultation (206 cases), i.e. 69.59%. The suspicion of endometrial hyperplasia by physicians was the most frequently diagnosed circumstance (149 cases) or 50.33%. Biopsy curettage was the most frequently used method (176 cases), is 59.45%. Histological endometrial lesions of atypical complex adenomatous hyperplasia (79 cases) represented 26.69%. Benign behavior was most frequently observed in (235 cases) or 79.39%. Conclusion: Endometrial hyperplasia is an endometrial lesion whose atypical histological types represent the borderline lesions between benignity and malignancy.
文摘Observed rainfall data of the National Meteorological Service of Guinea (NMS) exhibit that synoptic station usually records the largest rainfall amount in Guinea. Only few studies have been done on this rainfall peak observed in Conakry. This work better analyses the atmospheric dynamics leading to rainfall particularity. Using NMS data from 1981 to 2010, the monthly contribution and mean seasonal cycle of each station has been done. These findings of the study show that between July and August (rainfall season peak), the coastline particularly Conakry records the largest amount of rainfall. Using Era Interim data for the common period (1981-2010), we also investigate the rainfall dynamics in the lower level (1000 hPa - 850 hPa) from precipitable water, divergence, and moisture flow transport. There is a west and southwest moisture flow transport explained by a strong moisture convergence in the coastal region (Lower-Guinea). Furthermore, values of precipitable water in the same region are found, in agreement with the high moisture flow transport gradient. These incoming flow (west and south-west) undergo a return by blocking’s Kakoulima range (foehn effect) and Fouta Djallon massif to initiate convection clouds on the Guinean coast. These processes enhance a convergence of moisture associated with orographic origin convection. This has an important effect by increasing the rainfall amount in Conakry.
文摘Introduction: High blood pressure is a real public health problem affecting more than one in three adults. The objective of this study was to evaluate the level of knowledge, attitudes and practices of health care personnel in the management of hypertension in Conakry. Methods: Descriptive transversal study of 6 months, from April 20, 2018 to September 20, 2018, which took place in the internal medicine departments of the two university hospital centers (CHU) of Conakry (Donka and Ignace Deen). The study population consisted of general practitioners and specialists, practicing in both departments for at least two years and who agreed to submit to our questionnaire. Results: At the end of the study, we included 140 doctors, 61% at Donka University Hospital and 39% at Ignace Deen University Hospital. The predominance was male with 113 men for 27 women with a sex ratio H/F = 4.18. General practitioners were the most represented with 95% against 5% specialists. Sixty-four percent of physicians gave an exact definition of hypertension, 56% of physicians confirmed the diagnosis of hypertension after 2 to 3 consultations. 19% offered monotherapy as first-line treatment, only 72% said that the duration of Antihypertensive treatment was for life. Conclusion: Theseresults demonstrate that it is essential to reflect on our daily practices for a good improvement in the quality of care for patients.
文摘<strong>Introduction: </strong>Temporomandibular joint ankylosis (TMJA) is a fusion of joint surfaces by fibrous or bone tissue and the resulting limitation of mouth opening. The objectives of this study were to determine the frequency of TMJA, describe the clinical aspects and assess management. <strong>Materials and Method:</strong> It was a retrospective study that was carried out in the Department of Odontostomatology and Maxillofacial Surgery of Donka National Hospital for a period of 5 years (January 2016 to December 2020). Included were all records of inpatients and managed cases of TMJA during the study period. Socio-demographic, clinical and therapeutic variables were analyzed. <strong>Results:</strong> During the study 13 TMJA cases were collected with the frequency of 0.47%. The age group 1 - 9 was the most affected (61.54%) with extremes of 4 and 28 years. Men were the most concerned (53.87%). Restriction of mouth opening was the main reason for consultation (69.23%). The etiologies of ankylosis were dominated by infection including cellulitis of dental origin (53.85%) followed by facial trauma (30.77%). Arthroplasty was the most commonly used technique (73%), two cases of recurrence were noted (15.38%). <strong>Conclusion: </strong>TMJA affects mainly children and the etiology is dominated by cellulite of dental origin hence the interest of sensitization of the population for early management of oral diseases.
文摘Objectives: To describe the socio-demographic aspects of transferred parturient women;To identify the means of transport used by the evacuated parturient women to the Donka maternity ward at the University Hospital of Conakry;To describe the difficulties met;And to assess maternal and fetal prognosis. Methodology: It consisted of a prospective study over a period of 6 months from 01/02 to 31/07/2018. All patients transferred to the maternity ward of the Donka National Hospital of Conakry University Hospital. Results: The frequency of obstetric transfers was 13.79%. The epidemiological profile was that of a parturient woman of an average age of 25.7 years, married, and housewife, unschooled, who was on her first pregnancy and from the city of Conakry. The average distance covered was 16 km with extremes of 3 and 50 km. The transfer to the referral maternity clinic was not medicalized in 94% of cases. The venous route was not taken in 96% of cases. The parturient was not escorted by a health worker in 98% of cases. Bleeding was the most frequent reason for evacuation, followed by acute fetal sufferings. The average number of prenatal consultation was 2 with extremes of 0 and 9. The average length of stay was 3.6 days with extremes of 1 and 28 days. The majority of transferred women had a full-term pregnancy. The Cesarean section was 79.4%. The Retro placental hematoma was the most common complication found and was 29.4%. The counter-reference was not made in 97.79%. We recorded 8 maternal deaths, for a lethality rate of 1.77%. Possible interventions to reduce the dramatic situation of obstetric evacuations require first of all the decentralization of health care structures capable of performing a cesarean section. This approach should aim to create medical centers with a surgical antenna in all municipalities. These decentralized units would reduce the delay in case management and thus, limit the number of complications.
文摘Introduction: The birth of conjoined twins is an event extremely rare that offers unique therapeutic challenges and circumstances. We must examine each situation with the many questions that arise and which sometimes require a long reflection. We report a case of separation of pygopages conjoined twins as well as a review of the literature. Patients and Observations: a pair of conjoined twins aged 11 days, weighing 3080 g between them, was referred to the neonatology department from the Faranah prefecture, 300 km from Conakry, for care. The mother, a 30-year-old housewife, multipara, eight gestures and nine parities, had not followed any prenatal consultation. The delivery took place at home in a village with the death of the mother immediately postpartum. The clinical and paraclinical investigation had concluded with the diagnosis of Siamese type pygopage. After multidisciplinary consultation, the surgical treatment by separation of the twins was carried out at the age of 50 days with success and preservation of the physiological functions. Conclusion: The birth of conjoined twins is extremely rare. Each pair of Siamese is different and the surgical strategy must be adapted according to the shared organs. The perfect multidisciplinary work of the medical staff has been the main contributor to our success.
