Information regarding current status of handwashing knowledge, beliefs, attitudes and compliance with handwashing practice in nursing students in Jordan is limited. This study aims to explore handwashing knowledge, beliefs, attitudes, and self-reported compliance and to examine significant variables predicting compliance with handwashing practice among nursing students. The study used a cross-sectional survey design. Results found a statistically significant relationship between nursing students’ handwashing compliance and their handwashing knowledge, attitudes, and beliefs (r = 0.68, 0.65, 0.58, P β = 0.423, P = 0.000), followed by attitudes (β = 0.322, P = 0.000), and beliefs (β = 0.171, P = 0.027). Specific handwashing training courses integrated in nursing education curriculum are recommended. These courses need to consider nursing students’ handwashing attitudes and beliefs in conjunction with handwashing knowledge. 展开更多

Access to Sanitation Facilities and Handwashing Practices among Physically Challenged Persons in Homes for the Disabled in Ibadan, Nigeria

Background: Disabled people in the streets have been adjudged to have the least access to sanitation facilities. Information is inadequate on accessibility to sanitation and hygiene services in homes for the disabled in Nigeria. Purpose: To investigate accessibility to sanitation facilities and handwashing practices among Physically Challenged Persons (PCPs) in selected homes for the disabled in Ibadan. Materials and Methods: Sixty-four consenting PCPs selected from three homes for the disabled in a cross-sectional study were interviewed using questionnaire. Field observation and Focus Group Discussion (FGDs) were conducted using a checklist and FGD guide respectively. Results: Mean age of PCPs was 19.5 ± 6.8 years, and 56.2% were males. All PCPs reported they had water closet toilet-types in their homes, however, 42% reported that toilet usage was difficult because there were no assistive devices. Observation revealed there were no hand rails or ropes to aid in toilet usage. The FGDs revealed water supply for flushing was inadequate. Although 98.4% reported handwashing with soap after toilet use, only 54.7% of the PCPs reported that soap was readily available in their homes/centres. Discussion and Conclusion: Reports by the PCP and field observation showed that there were inadequate assistive devices such as wheelchair ramps, hand rails, or ropes to aid in toilet usage. Sanitation facilities in homes for the disabled should be appropriately built with necessary support to facilitate usage and easy accessibility. 展开更多

Background Hand hygiene can be a simple,inexpensive,and effective method for preventing the spread of infectious diseases.However,a reliable and consistent method for monitoring adherence to the guidelines within and outside healthcare settings is challenging.The aim of this study was to provide an approach for monitoring handwashing compliance and quality in hospitals and communities.Methods We proposed a deep learning algorithm comprising three-dimensional convolutional neural networks(3D CNNs)and used 230 standard handwashing videos recorded by healthcare professionals in the hospital or at home for training and internal validation.An assessment scheme with a probability smoothing method was also proposed to optimize the neural network’s output to identify the handwashing steps,measure the exact duration,and grade the standard level of recognized steps.Twenty-two videos by healthcare professionals in another hospital and 28 videos recorded by civilians in the community were used for external validation.Results Using a deep learning algorithm and an assessment scheme,combined with a probability smoothing method,each handwashing step was recognized(ACC ranged from 90.64%to 98.87%in the hospital and from 87.39%to 96.71%in the community).An assessment scheme measured each step’s exact duration,and the intraclass correlation coefficients were 0.98(95%CI:0.97-0.98)and 0.91(95%CI:0.88-0.93)for the total video duration in the hospital and community,respectively.Furthermore,the system assessed the quality of handwashing,similar to the expert panel(kappa=0.79 in the hospital;kappa=0.65 in the community).Conclusions This work developed an algorithm to directly assess handwashing compliance and quality from videos,which is promising for application in healthcare settings and communities to reduce pathogen transmis-sion. 展开更多

The COVID-19 pandemic put every government in the globe on red alert;safety protocol and long-standing infection prevention and control (IPC) measures such as hand and oral hygiene, social/ physical distancing, use of face mask were rigorously enforced. This study evaluated microbial (bacterial, fungal) contents of rinsate, bowl water, water from tap, and the neighborhood drainage in selected hand washing stations in Agege, Alimosho, Mainland, Island and Shomolu local government areas (LGAs) in Lagos, Nigeria. The identities of bacterial isolates were confirmed with 16S rRNA sequencing while the fungi were identified by colonial appearances. The antibiotics susceptibility testing (AST) of the bacteria against structurally unrelated antibiotics was performed and interpreted according to standard guidelines. Sixty-seven (67) bacterial and fifty-one (51) fungal isolates were recovered from mainly water bucket with tap and rinsates across all the LGAs. There were marked microbial loads (some too numerous to count at 10<sup>-3</sup> dilution) across Alimosho, Lagos Island and Mainland. Forty (40) representative bacterial isolates were selected for 16S rRNA sequencing. The occurrence of microbial isolates in the samples was at varying degrees;Klebsiella spp. (37.5%), Bacillus spp. (32.5%), Enterobacter spp. (17.5%), were the predominant bacteria while Aspergillus spp. (46.2%), yeasts (34.6%), Sporothrix schenckii (11.5%) and Penicillium spp. (7.7%) constituted the fungal isolates. The biological weapon, B. anthracis was recovered from a water bucket with tap in Alimosho. The antibiotics susceptibility testing of the bacteria showed high degrees of resistance profile;45 (69.2%) to ampicillin, 41.5% amoxicillin/clavulanate while 47.6% were resistant to two (2) or more antibiotics. This study demonstrated high microbial load during the pandemic at the study LGAs, presence of environmental commensals reputed for debilitating opportunistic infections in man and risk of passing heavy load of these potential pathogens to the public. 展开更多

A healthcare-associated infection (defined as an infection acquired within a healthcare facility), such as due to transmission via medical equipment or by healthcare providers is the most frequent adverse event in the healthcare delivery system. But why does the problem persist, when infection control measures are known, simple, and low-cost? We reviewed some biological- and treatment-factors in Part 1, and we now review some human-factors. Healthcare-associated infections are a major public health problem even in advanced healthcare systems. They affect hundreds of millions of patients each year, and are responsible for increased morbidity, mortality, and financial burden. This is perplexing, since good-hygiene practices are known and promoted. Disinfection, sterilization, handwashing, and alcohol rubs should be more effective, but human-factors interfere. The persistent high prevalence of nosocomial infections, despite known hygienic practices, is attributable to two categories of factors: biological and inherent shortcomings of some practices (considered in Part 1), and human factors (considered here). Since infection control measures are known, simple, and low-cost, we examine why the problem of healthcare-associated infections persists. Hundreds of millions of patients each year are affected by a healthcare-associated infection, with negative medical outcome and financial cost. It is a major public health problem even in countries with advanced healthcare systems. This is a bit perplexing, given that hygienic practices have been known and actively promoted. The objective is to address the question: doesn’t the use of disinfection, sterilization, handwashing, and alcohol rubs prevent the spread of pathogenic organisms? We conclude that the persistent high prevalence of nosocomial infections despite known hygienic practices is attributable to two categories of factors: biological and inherent shortcomings of some practices (considered in Part 1), and human factors (considered in Part 2). Evaluation of the Effectiveness of Hygenall<sup>©</sup>Leadoff<sup>TM</sup>Foaming Soap in Reducing Lead on Workers’ Hands and the Uptake of Lead on Bridge Painting Projects

Although considerable research has been conducted regarding airborne lead exposures during lead paint removal, little data has been collected measuring the lead concentration on workers’ hands in the construction industry. As a result, there are gaps in our understanding of the effectiveness of a standard work practice control (handwashing) in the prevention and control of elevated blood lead levels (BLLs) in the workplace. The primary objective of this study was to assess if a National Institute for Occupational Safety and Health (NIOSH) formulation intended to remove lead from skin (Hygenall<sup><span style="white-space:nowrap;">&#169; </span></sup>Leadoff<sup>TM</sup> Foaming Soap) is more effective than a commonly used soap in the industrial painting industry (Zep Cherry Bomb Soap) in reducing the concentration of lead on workers’ hands after exposure. A secondary aim was to assess if using Leadoff<sup>TM</sup> Foaming Soap during handwashing reduces worker uptake of lead on bridge painting projects. We evaluated hand wipe, air sample and blood lead level data collected by two industrial bridge painting contractors. Airborne exposures ranged from 350 μg/m<sup>3</sup> to 19,700 μg/m<sup>3</sup>. The geometric mean lead concentration remaining on workers’ hands after using Zep Cherry Bomb Soap was 77 μg per hand wipe compared with 61 μg per hand wipe after use of Leadoff<sup>TM</sup> Foaming Soap for all work classifications. The geometric lead concentration on workers’ hands decreased by 16 μg/hand wipe for all work classifications combined after using Leadoff<sup>TM</sup> Foaming Soap—a decrease of 21%. The use of Hygenall Leadoff<sup>TM</sup> Foaming Soap reduced the maximum lead concentration on workers’ hands by 85%. The geometric mean BLL for all work classifications was 12.1 μg/dl before the use of Leadoff<sup>TM</sup>. After two months of airborne exposure and use of Leadoff<sup>TM</sup> Foaming Soap at breaks and at the end of the workday, the geometric mean BLL for all work classifications increased to 18.1 μg/dl. Worker BLLs increased despite the reduction in the concentration of lead measured on workers’ hands—most likely a result of ineffective inhalation exposure controls. We found that the Leadoff<sup>TM</sup> Foaming Soap was more effective in removing lead from workers’ hands than Zep Cherry Bomb. 展开更多
