维生素D治疗全面性发育迟缓患儿的临床疗效研究

背景 除了某些有明确病因的代谢性疾病导致的全面性发育迟缓(GDD),康复治疗是GDD的主要治疗方式;维生素D通过影响神经营养因子在调节神经细胞的发育和分化方面发挥着重要的神经保护作用;但目前关于补充维生素D对GDD患儿临床疗效的研究... 背景 除了某些有明确病因的代谢性疾病导致的全面性发育迟缓(GDD),康复治疗是GDD的主要治疗方式;维生素D通过影响神经营养因子在调节神经细胞的发育和分化方面发挥着重要的神经保护作用;但目前关于补充维生素D对GDD患儿临床疗效的研究开展较少。目的 探讨补充不同剂量的维生素D对GDD患儿康复治疗的临床效果。方法 于2020年9月—2022年6月选取在郑州大学第三附属医院康复医学科首次住院就诊的120例GDD患儿为研究对象,采用随机区组化的方法将其分为常规组(38例)、400 U组(37例)和1 200 U组(35例)。常规组仅进行常规康复治疗;400 U组在常规康复治疗的基础上给予口服400 U/d维生素D;1 200 U组在常规康复训练的基础上给予口服1 200 U/d维生素D。收集3组患儿的性别、就诊年龄等基本资料;于入院时(治疗前)及第3个疗程末(治疗后)行血清25羟维生素D[25(OH)D]水平检测和Gesell发育量表评估[评估适应能力、大运动能力、精细运动能力、语言能力和社交能力5个能区的发育商(DQ)];记录发生在患儿住院期间不良事件的次数,并对上述资料进行分析比较。结果 3组患儿性别、居住地、出生季节、分娩方式、就诊年龄、出生体质量、出生胎龄、主要就诊原因比较,差异均无统计学意义(P>0.05)。治疗前,3组患儿25(OH)D水平、Gesell量表各能区DQ值比较,差异均无统计学意义(P>0.05);治疗后,1 200 U组患儿血25(OH)D水平、Gesell量表大运动能力、精细运动能力、语言能力DQ值高于常规组(P<0.05)。第1、2疗程期间,3组患儿不良事件发生率比较,差异无统计学意义(P>0.05);第3疗程期间,1 200 U组患儿不良事件发生率低于常规组及400 U组(P<0.05)。结论 补充1 200 U维生素D对GDD患儿的康复疗效有益,且能减少康复期间不良事件的发生率。 展开更多

目的探究维生素D水平对输卵管因素不孕患者体外受精(IVF)长方案助孕成功率的影响。方法回顾性分析保定市妇幼保健院2022年6月至2023年6月收治的167例行IVF长方案助孕的输卵管因素不孕患者的病历资料。根据25羟维生素D水平分为A组(维生素... 目的探究维生素D水平对输卵管因素不孕患者体外受精(IVF)长方案助孕成功率的影响。方法回顾性分析保定市妇幼保健院2022年6月至2023年6月收治的167例行IVF长方案助孕的输卵管因素不孕患者的病历资料。根据25羟维生素D水平分为A组(维生素D水平缺乏,n=74例)、B组(维生素D水平不足,n=51例)和C组(维生素D水平充足,n=42例)。比较三组的基线资料[年龄、不孕年限、体重指数(BMI)、卵泡刺激素(FSH)、黄体生成素(LH)、催乳素(PRL)、雌二醇(E_(2))、孕酮(P)、抗米勒管激素(AMH)]、促排卵情况[促性腺激素(Gn)总剂量、Gn天数和获卵数]、胚胎实验室指标[人绒毛膜促性腺激素(hCG)日内膜厚度、hCG日大卵泡数、可利用胚胎数、平均移植胚胎数]和助孕后妊娠结局。结果三组患者的年龄、不孕年限、BMI、FSH、LH、PRL、E_(2)、P、AMH比较,差异无统计学意义(P>0.05);三组Gn总剂量、Gn天数、获卵数、可利用胚胎数、优质胚胎数和优质胚胎率比较,差异无统计学意义(P>0.05);三组hCG日内膜厚度、hCG日大卵泡数、可利用胚胎数、平均移植胚胎数比较,差异无统计学意义(P>0.05);B组、C组临床妊娠率、生化妊娠率均高于A组(P<0.05),但B组、C组组间比较差异无统计学意义(P>0.05)。结论维生素D水平与输卵管因素不孕患者的卵巢功能和促排卵情况无显著相关性,但对IVF长方案助孕成功率存在一定影响。 展开更多

目的:探讨维生素D与2型糖尿病(type 2 diabetes mellitus,T2DM)患者发生糖尿病视网膜病变(diabetic retinopathy,DR)的关系,及维生素D补充对DR患者临床指标的影响。方法:收集2023年6月—2024年6月在我院就诊的158例T2DM为研究对象,收集... 目的:探讨维生素D与2型糖尿病(type 2 diabetes mellitus,T2DM)患者发生糖尿病视网膜病变(diabetic retinopathy,DR)的关系,及维生素D补充对DR患者临床指标的影响。方法:收集2023年6月—2024年6月在我院就诊的158例T2DM为研究对象,收集患者性别、年龄、吸烟史、饮酒史、身高、体重、疾病史等一般信息、实验室血清25-羟维生素D3[25(OH)D3]水平检测数据等。根据眼底检查结果将患者分为DR组和非DR组,分析血清25(OH)D3水平与DR发生的关系。进一步将DR患者随机分为维生素D干预组和对照组,对照组采用常规治疗方法和干预,维生素D干预组给予口服活性维生素D3,观察两组患者干预3个月后血清25(OH)D3、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)水平,比较两组上述指标的差异。结果:158例T2DM患者中,共52例检出患有DR,患病率为32.91%。病程长、FPG水平高、HbA1c水平高是DR发生的独立危险因素,统计分析结果中均有OR>1、P<0.05,而血清25(OH)D3是DR发生的保护因素(OR<1、P<0.05)。维生素D干预结果显示,干预组患者血清25(OH)D3水平及HDL-C水平显著高于对照组,而血清FPG、HbA1c、TG、TC及LDL-C水平显著低于对照组。结论:T2DM患者DR患病率高,较高的血清25(OH)D3水平是其保护因素,且对DR患者进行维生素D补充,可改善患者血糖和血脂水平。 展开更多

目的探讨颗粒蛋白前体(progranulin,PGRN)、神经丝轻链蛋白(neurofilament light chain,NfL)和维生素D及其受体(vitamin D receptor,VDR)在临床孤立综合征(clinically isolated syndrome,CIS)患者发病及转归中的预测价值。方法收集2017... 目的探讨颗粒蛋白前体(progranulin,PGRN)、神经丝轻链蛋白(neurofilament light chain,NfL)和维生素D及其受体(vitamin D receptor,VDR)在临床孤立综合征(clinically isolated syndrome,CIS)患者发病及转归中的预测价值。方法收集2017年12月至2021年12月作者医院收治的96例CIS患者,其中男33例,女63例,年龄(33.13±8.86)岁;另收集性别、年龄和长期居住地与CIS患者相匹配的神经系统非炎症疾病(non-inflammatory neurological disease,NIND)患者30例为对照组,其中男11例,女19例,年龄(31.17±5.78)岁。采用酶联免疫吸附试验法(enzyme-linked immunosorbent assay,ELISA)检测纳入者和首次转归为多发性硬化(multiple sclerosis,MS)患者的血清和脑脊液PGRN、NfL水平,淋巴细胞上清液中VDR水平。采用超高效液相色谱-串联质谱法检测外周血上清液中维生素D水平[维生素D水平=25(OH)D2水平+25(OH)D_(3)水平]。对CIS患者进行每个月电话随访1次,3个月门诊随访1次,监测血常规、肝肾功能,如出现临床转归,行头和脊髓MRI检查。结果随访时间12~38个月,中位数为24个月。57例CIS患者转归为MS(59.38%)。与NIND组比较,CIS患者血清和脑脊液PGRN水平(血清:t=-2.746,P=0.007;脑脊液:t=-17.822,P=0.000)、NfL水平(血清:t=-17.627,P=0.000;脑脊液:t=-13.543,P=0.000)升高,血清25(OH)D_(3)(t=22.512,P=0.000)和VDR水平(t=12.315,P=0.000)降低。与未转归CIS患者相比较,转归MS的患者NfL基线水平(血清:t=25.052,P=0.000;脑脊液:t=8.362,P=0.000)增高,血清25(OH)D_(3)基线水平降低(t=-4.323,P=0.000);与转归MS前比较,转归MS后患者脑脊液PGRN水平升高(t=-5.909,P=0.000),血清维生素D(t=5.265,P=0.000)、25(OH)D_(3)水平(t=5.204,P=0.000)降低,血清和脑脊液NfL水平升高(血:t=-17.229,P=0.000;脑脊液:t=-8.949,P=0.000)。ROC分析显示,CIS患者外周血25(OH)D_(3)水平低于12.58 ng/mL、脑脊液NfL水平低于1383 pg/mL易转归为MS。结论CIS患者存在神经轴突损伤。PGRN尚不能预测CIS向MS转归,NfL是预测CIS转归为MS的潜在标志物。低维生素D水平可能是CIS致病及转归为MS的预测因素。 展开更多

目的:探讨维生素D联合粪菌移植治疗腹泻型肠易激综合征(IBS-D)的疗效及对疾病转归的影响。方法:选取2022年5月至2023年10月该院收治的IBS-D患者122例,采用随机数字表法分为对照组和研究组,每组61例。在常规治疗基础上,对照组患者给予粪... 目的:探讨维生素D联合粪菌移植治疗腹泻型肠易激综合征(IBS-D)的疗效及对疾病转归的影响。方法:选取2022年5月至2023年10月该院收治的IBS-D患者122例,采用随机数字表法分为对照组和研究组,每组61例。在常规治疗基础上,对照组患者给予粪菌移植治疗,研究组患者给予维生素D联合粪菌移植治疗。比较两组患者的临床疗效、肠道菌群、内脏敏感性指标及3个月复发率。结果:对照组58例、研究组59例患者完成研究。研究组患者的治疗总有效率高于对照组[94.9%(56/59)vs.81.0%(47/58)],差异有统计学意义(P<0.05)。治疗后,研究组患者IBS症状严重程度量表各维度评分及总分低于对照组,粪便双歧杆菌、乳杆菌含量高于对照组,肠球菌含量低于对照组,直肠初始感觉阈值、初始排便冲动阈值和最大耐受容量增加,血清5-羟色胺、P物质水平低于对照组,差异均有统计学意义(P<0.05)。两组治疗有效的患者中,研究组患者的复发率低于对照组[5.4%(3/56)vs.19.1%(9/47)],差异有统计学意义(P<0.05)。结论:维生素D联合粪菌移植能够提高IBS-D患者的临床疗效,减少疾病复发,而且能够调节肠道菌群和降低内脏高敏性。 展开更多

背景:研究证明绝经后骨质疏松症与女性生殖特点和运动膳食有关,但是鲜有研究证明女性生殖特点及运动膳食是否对绝经后女性维生素D有影响。目的:探讨北京市绝经后女性维生素D水平与女性生殖特点和运动膳食情况的关系及其影响因素。方法:... 背景:研究证明绝经后骨质疏松症与女性生殖特点和运动膳食有关,但是鲜有研究证明女性生殖特点及运动膳食是否对绝经后女性维生素D有影响。目的:探讨北京市绝经后女性维生素D水平与女性生殖特点和运动膳食情况的关系及其影响因素。方法:选取2017年9月至2018年5月北京市多个城区的17个社区现场问卷调查的727例46-75岁绝经后女性作为研究对象,问卷内容包括受试者的基本信息和月经史、绝经史、妊娠史、运动膳食情况等。采用电化学发光免疫分析仪及其配套试剂测定25-(OH)D水平;使用双能X射线骨密度检测仪测定腰椎(L1-L4、整体)和双髋关节(股骨颈、整体)的骨密度和T值;观察维生素D与生殖特点和运动膳食的相关性,并进行维生素D的影响因素分析。结果与结论:①维生素D正常组136例(18.7%),维生素D不足组389例(53.5%),维生素D缺乏组202例(27.8%);平均维生素D水平为(15.60±5.85)ng/mL。②随着维生素D水平的升高,不同部位的骨密度值及T值也随之升高。③维生素D水平与初潮年龄、月经持续天数、绝经阶段、运动情况、饮食类型、饮食偏好均呈正相关(P<0.05);与月经周期、绝经年龄、生育次数均呈负相关(P<0.05);与初孕年龄、怀孕次数无相关性(P>0.05)。④多因素有序Logistic回归分析结果显示,喝奶制品频率对绝经后女性维生素D值存在正向相关(P<0.05),且奶制品是维生素D水平的保护因素。⑤调查结果说明,北京市绝经后女性维生素D水平普遍不足、骨密度值普遍低下,维生素D水平与部分生殖特点、运动膳食情况存在关联,且其中喝奶制品是其保护因素。因此可以通过开展相关骨质疏松的健康教育,对绝经后女性膳食情况进行调整加以干预,增加体内维生素D含量,以期增加骨密度值,减少绝经后骨质疏松患病率。 展开更多

BACKGROUND Diabetes and thyroiditis are closely related.They occur in combination and cause significant damage to the body.There is no clear treatment for type-2 diabetes mellitus(T2DM)with Hashimoto's thyroiditis(HT).While single symptomatic drug treatment of the two diseases is less effective,combined drug treatment may improve efficacy.AIM To investigate the effect of a combination of vitamin D,selenium,and hypoglycemic agents in T2DM with HT.METHODS This retrospective study included 150 patients with T2DM and HT treated at The Central Hospital of Shaoyang from March 2020 to February 2023.Fifty patients were assigned to the control group,test group A,and test group B according to different treatment methods.The control group received low-iodine diet guidance and hypoglycemic drug treatment.Test group A received the control treatment plus vitamin D treatment.Test group B received the group A treatment plus selenium.Blood levels of markers of thyroid function[free T3(FT3),thyroid stimulating hormone(TSH),free T4(FT4)],autoantibodies[thyroid peroxidase antibody(TPOAB)and thyroid globulin antibody(TGAB)],blood lipid index[low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triacylglycerol(TG)],blood glucose index[fasting blood glucose(FBG),and hemoglobin A1c(HbA1c)]were measured pre-treatment and 3 and 6 months after treatment.The relationships between serum 25-hydroxyvitamin D3[25(OH)D3]level and each of these indices were analyzed.RESULTS The levels of 25(OH)D3,FT3,FT4,and LDL-C increased in the order of the control group,test group A,and test group B(all P<0.05).The TPOAB,TGAB,TC,TG,FBG,HbA1c,and TSH levels increased in the order of test groups B,A,and the control group(all P<0.05).All the above indices were compared after 3 and 6 months of treatment.Pre-treatment,there was no divergence in serum 25(OH)D3 level,thyroid function-related indexes,autoantibodies level,blood glucose,and blood lipid index between the control group,test groups A and B(all P>0.05).The 25(OH)D3 levels in test groups A and B were negatively correlated with FT4 and TGAB(all P<0.05).CONCLUSION The combination drug treatment for T2DM with HT significantly improved thyroid function,autoantibody,and blood glucose and lipid levels. 展开更多

Vitamin D is a steroid hormone that is naturally produced in the body or obtained through dietary sources,primarily under the influence of UVB radiation.This essential nutrient has a vital role in numerous physiological processes,encom-passing immune function,cell growth,differentiation,insulin regulation,and cardiovascular well-being,along with its pivotal role in sustaining the delicate equilibrium of calcium and phosphate concentrations in the body.Moreover,vitamin D reinforces mucosal defense and bolsters the immune system through immunomodulation,making it a critical component of overall health.Numerous studies have unveiled the profound connection between vitamin D and the predisposition to respiratory tract infections,including well-known viruses such as influenza and the novel severe acute respiratory syndrome coronavirus 2.Vita-min D deficiency has been consistently linked to increased severity of coronavirus disease 2019(COVID-19)and a heightened risk of mortality among afflicted individuals.Retrospective observational studies have further substantiated these findings,indicating that levels of vitamin D are linked with both the occurrence and severity of COVID-19 cases.Vitamin D has its influence on viral infections th-rough a multitude of mechanisms,such as promoting the release of antimicrobial peptides and fine-tuning the responses of the immune system.Additionally,vitamin D is intertwined with the intricate network of the renin–angiotensin system,suggesting a potential impact on the development of complications related to COVID-19.While further clinical trials and extensive research are warranted,the existing body of evidence strongly hints at the possible use of vitamin D as a valuable tool in the prophylaxis and management of COVID-19 and other viral infectious diseases. 展开更多

The association between Vitamin D and IBS has attracted growing interest because of vitamin D’s involvement in immune system function and reduction of inflammation. IBS, a prevalent gastrointestinal condition marked by symptoms such as stomach pain, bloating, and changed bowel habits, implies that sufficient levels of Vitamin D may impact the symptomatology and quality of life associated with IBS. This review aimed to unveil current insights on vitamin D intake, status and symptoms of irritable bowel syndrome. A search was conducted on Embase, Scopus, Medline, and Cochrane Library, as well as a reference list of relevant studies. This paper also examined the current therapeutic practices for using vitamin D in the management of IBS. Although several studies demonstrate a positive association between vitamin D administration and the alleviation of IBS symptoms, the data is not yet definitive. Moreover, the review emphasised the necessity for additional rigorous clinical studies to determine a definitive therapeutic function of vitamin D in IBS based on the literature. The role of vitamin D in the modulation of gut health, particularly in IBS, has garnered significant attention due to its potential impact on both inflammation and the integrity of the gut barrier. Background: The role of vitamin D and parathyroid hormone in the metabolic profile of type 2 diabetes mellitus in sub-Saharan Africa has not been adequately assessed. The aim of this study was to determine the prevalence of low vitamin D level and secondary hyperparathyroidism and their association with insulin sensitivity and β-cell secretory function among Congolese type 2 diabetics. Methodology: Fasting glycaemia, fasting insulin, 25OH D3 and human parathyroid hormone (hPTH) were measured in one hundred and eighty-four type 2 diabetic patients followed as outpatients in South Kivu. Levels of 25OH D3 65 pg/ml defined low vitamin D and elevated parathyroid hormone levels, respectively. The HOMA model was used to measure insulin sensitivity and β-cell secretory function. Results: Medians (IQR) were 25.3 (20.4 - 32.4) ng/ml for 25OH D3 and 53.7 (38.4 - 115.7) pg/ml for hPTH. 58.7% of diabetics had insulin resistance, 126 (68.5%) had low vitamin D and 80 (43.5%) had hyperparathyroidism. In multivariate analysis, hPTH (partial r = −0.28;p = 0.0002) and 25OH D3 (partial r = 0.16;p = 0.03) showed an independent association with insulin sensitivity after adjustment for body mass index and waist circumference. Finally, hPTH (partial r = 0.27;p = 0.0002) was the sole determinant of β-cell secretory function. Conclusions: This study confirms the high prevalence of low vitamin D level and secondary hyperparathyroidism and their association with insulin resistance and impaired islet β-cell secretory function among Congolese with type 2 diabetes mellitus. Vitamin D and calcium supplementation should be envisaged for cases of deficiency in this region. 展开更多

目的分析胆汁淤积性肝病患儿的维生素D水平,为该病患儿补充维生素D治疗提供理论依据。方法选取2022年1月—2024年1月于首都医科大学附属北京儿童医院中医科初次就诊的胆汁淤积性肝病患儿116例,按照性别、年龄、补充维生素D剂量、病程、... 目的分析胆汁淤积性肝病患儿的维生素D水平,为该病患儿补充维生素D治疗提供理论依据。方法选取2022年1月—2024年1月于首都医科大学附属北京儿童医院中医科初次就诊的胆汁淤积性肝病患儿116例,按照性别、年龄、补充维生素D剂量、病程、病因分别进行分组比较,并收集患儿的血清25-羟基维生素D(25-OH-D)水平及相关生化指标,分析维生素D水平与生化指标的相关性。计数资料组间比较采用χ^(2)检验或Fisher精确概率法。相关分析采用Spearman秩相关。结果116例患儿中76例(65.5%)存在维生素D缺乏或不足。维生素D缺乏或不足在男性中占65.7%(46/70),在女性中占65.2%(30/46),差异无统计学意义(χ^(2)=0.003,P=0.956)。维生素D缺乏或不足在从未补充过维生素D患儿中占83.3%(25/30),在每日补充500 IU者中占58.7%(27/46),在每日补充700 IU者中占64.3%(18/28),在每日补充>700 IU者中占50.0%(6/12),差异无统计学意义(χ^(2)=6.460,P=0.091)。不同病因比较结果显示,维生素D缺乏或不足在感染性疾病组占57.7%(15/26),在遗传代谢性疾病组占66.7%(10/15),在药物性肝损伤组占66.7%(6/9),在胆道系统结构异常组占100.0%(8/8),在病因未明组占63.8%(37/58),差异无统计学意义(χ^(2)=5.304,P=0.252)。不同病程比较结果显示,维生素D缺乏或不足在<1个月组占78.4%(29/37),在1~3个月组占54.3%(25/46),在>3~6个月组占53.3%(8/15),在>6个月组占77.8%(14/18),差异无统计学意义(χ^(2)=7.432,P=0.059)。不同年龄段比较结果显示,儿童组维生素D缺乏或不足占比明显高于婴儿组(82.5%vs 52.5%,χ^(2)=9.504,P=0.018)。相关性分析结果显示,血清AST、ALT与25-OH-D无显著相关性(P值均>0.05);血清ALP(r=−0.286,P=0.002)、GGT(r=−0.248,P=0.007)、TBil(r=−0.353,P<0.001)、DBil(r=−0.299,P=0.001)、总胆汁酸(r=−0.236,P=0.011)与25-OH-D均呈负相关;血清钙(r=0.263,P=0.004)、磷(r=0.385,P<0.001)与25-OH-D均呈正相关。结论大部分胆汁淤积性肝病患儿存在维生素D缺乏或不足;血清ALP、GGT、TBil、DBil或总胆汁酸水平升高,钙或磷水平降低,对维生素D缺乏或不足具有提示作用。 展开更多

Mental disorders seriously affect people’s health and social stability.This Mendelian randomization(MR)study was designed to investigate the causal relationship between circulating vitamin C(VC)or 25-hydroxyvitamin D(25(OH)D)levels and mental disorders.The data used for the MR analysis were derived from the summary genome-wide association studies(GWAS)database for VC and 25(OH)D and from the Finn Gen consortium for fourteen mental disorders.Based on the inverse variance weighted(IVW)method,we found a potential causal association between circulating VC and anxiety disorders(IVW:OR=1.139,95%CI:1.023-1.269,P=0.018).However,no causal association was found between VC or 25(OH)D and other mental disorders(P>0.05).In the reverse MR analysis,individuals with Alzheimer’s disease was causally associated with higher concentrations of circulating VC(P=0.012),while individuals with anxiety disorders had a negative association between the concentrations of 25(OH)D(P=0.012).However,the current evidence does not support a causal relationship between VC or 25(OH)D and other mental disorders.In addition,there was no causal association between circulating VC and 25(OH)D(P>0.05).Future studies are needed to confirm these findings and to elucidate the mechanisms of potential causality. 展开更多

This project looks at a novel way to enhance the sensory experience of vitamin D ingestion by incorporating it into marshmallows. This investigation used a human panel taste test with twelve individuals, an index of swelling, and a stability evaluation. Samples of vitamin D infused marshmallows were prepared and given to participants in the human panel taste test, which evaluated mouthfeel and flavor. By analyzing dissolving behavior, the swelling index test revealed unexpected erosion. In addition, a temperature threshold for storage conditions was found through a temperature sensitivity test. All of these techniques assessed the feasibility and palatability of vitamin D supplementation with marshmallow flavor, offering insights into both the possible advantages and difficulties. The marshmallow infusion technique effectively covered up the disagreeable taste of vitamin D pills, leading to reviews that were overwhelmingly favorable (“Moderate Sweet”) and that indicated a pleasant mouthfeel. During the swelling index test, it showed erosion behavior, suggesting a certain kind of dissolution that is advantageous for nutritional absorption. Furthermore, the study discovered a temperature sensitivity threshold, highlighting how crucial proper storage conditions are. 展开更多

BACKGROUND Non-alcoholic fatty liver disease(NAFLD),which is a significant liver condition associated with metabolic syndrome,is the leading cause of liver diseases globally and its prevalence is on the rise in most nations.The protective impact of vitamin D on NAFLD and its specific mechanism remains unclear.AIM To examine the role of vitamin D in NAFLD and how vitamin D affects the polarization of hepatic macrophages in NAFLD through the vitamin D receptor(VDR)-peroxisome proliferator activated receptor(PPAR)γpathway.METHODS Wild-type C57BL/6 mice were provided with a high-fat diet to trigger NAFLD model and administered 1,25-dihydroxy-vitamin D[1,25(OH)_(2)D_(3)]supplementation.1,25(OH)_(2)D_(3) was given to RAW264.7 macrophages that had been treated with lipid,and a co-culture with AML12 hepatocytes was set up.Lipid accumulation,lipid metabolism enzymes,M1/M2 phenotype markers,proinflammatory cytokines and VDR-PPARγpathway were determined.RESULTS Supplementation with 1,25(OH)_(2)D_(3) relieved hepatic steatosis and decreased the proinflammatory M1 polarization of hepatic macrophages in NAFLD.Administration of 1,25(OH)_(2)D_(3) suppressed the proinflammatory M1 polarization of macrophages induced by fatty acids,thereby directly relieving lipid accumulation and metabolism in hepatocytes.The VDR-PPARγpathway had a notable impact on reversing lipid-induced proinflammatory M1 polarization of macrophages regulated by the administration of 1,25(OH)_(2)D_(3).CONCLUSION Supplementation with 1,25(OH)_(2)D_(3) improved hepatic steatosis and lipid metabolism in NAFLD,linked to its capacity to reverse the proinflammatory M1 polarization of hepatic macrophages,partially by regulating the VDRPPARγpathway.The involvement of 1,25(OH)_(2)D_(3) in inhibiting fatty-acid-induced proinflammatory M1 polarization of macrophages played a direct role in relieving lipid accumulation and metabolism in hepatocytes. 展开更多

This letter discusses the publication by Feng et al.Iodine,selenium,and vitamin D are closely associated with thyroid hormone production in humans;however,the efficacy of selenium and vitamin D supplementation for type 2 diabetes mellitus(T2DM)patients with Hashimoto’s thyroiditis(HT)remains controversial.In the retrospective study we discuss herein,the authors highlighted significant improvements in thyroid function,thyroid antibodies,blood glucose,and blood lipid in T2DM patients with HT following addition of vitamin D and selenium to their antidiabetic regimens,underscoring the value of these supplements.Our team is currently engaged in research exploring the relationship between micronutrients and HT,and we have obtained invaluable insights from the aforementioned study.Based on this research and current literature,we recommend a regimen of 4000 IU/day of vitamin D and 100-200μg/day of selenium for over three months to six months for patients with HT,particularly for those with concurrent T2DM. 展开更多
