For years, foot ulcers linked with diabetes mellitus and neuropathy have significantly impacted diabetic patients’ health-related quality of life(HRQoL). Diabetes foot ulcers impact15% of all diabetic patients at some point in their lives. The facilities and resources used for DFU detection and treatment are only available at hospitals and clinics, which results in the unavailability of feasible and timely detection at an early stage. This necessitates the development of an at-home DFU detection system that enables timely predictions and seamless communication with users, thereby preventing amputations due to neglect and severity. This paper proposes a feasible system consisting of three major modules:an IoT device that works to sense foot nodes to send vibrations onto a foot sole,a machine learning model based on supervised learning which predicts the level of severity of the DFU using four different classification techniques including XGBoost,K-SVM,Random Forest, and Decision tree, and a mobile application that acts as an interface between the sensors and the patient. Based on the severity levels, necessary steps for prevention, treatment, and medications are recommended via the application. 展开更多

Background: The triple arthrodesis was described early by Myerson in 1923 and has been the gold standard surgical procedure for various conditions with the goal to reduce pain, improve deformity and facilitate function. Methods: The procedure consists of fusion of the subtalar joint, talonavicular joint and calcaneocuboid joint [1]. The procedure is traditionally done with internal fixation, however, Marinet al. has shown it to be performed with external fixation [2]. Dr. Marin and the associates have developed an alternative technique to perform a triple arthrodesis using internal and external fixation. This paper describes a step-by-step technique to perform a triple arthrodesis with both internal and external fixation. It will demonstrate the use of the prefabricated arches on the footplate for more precise and accurate placement of transosseous wires using arch-wire technique [3]. Results/Conclusion: We believe this technique will not only help increase the ability to achieve fusion with a less chance of non-union, but may also decrease healing time, which may allow patients to be mobile from the first week post-operatively and may diminish the risks associated with being non-weightbearing. 展开更多

The aim of this study was to investigate the effects of orthopaedic soles on the body posture. Methods. Forty-eight runners (21 men and 28 women) maintained a standing-up position on both feet with bare feet with neutral soles and orthopedic soles which contained bilaterally a podiatrist element of 3 mm height behind the metatarsal heads (Metatarsal Retro Capital Bar, MRCB). Stabilometric, plantar pressure and kinematic data in the sagittal plane on both sides were measured at 40 and 60 Hz, respectively. The position of the center of pressure on the anteroposterior axis (YCoP), the forefoot plantar pressure (FPP) and the anteroposterior position of the knee (Yk), the hip (YH), the shoulder (YS) and the ears (YE) with respect to the vertical axis passing through the joint of the ankle were determined for each experimental condition. Findings. The addition of a MRCB orthopedic element induced in backward displacement of CoP, hip, shoulder and ears (p CoP and FPP changes were significantly correlated with YH, YS and YE changes (p Conclusion. These results suggest that the addition of an orthopedic element located behind the metatarsal heads influences the overall position of the body and can help podiatrist in the care of their patients. 展开更多
