Perioperative nursing care for hip arthroplasty patients with concomitant hypertension: A minireview

Hip replacement(HA)is mainly indicated for the elderly,who generally suffer from various underlying diseases such as hypertension.This article provides a review of the key points of perioperative nursing care for patients with hyper-tension undergoing HA.It analyzes the key points of care during the periop-erative period(preoperative,intraoperative,and postoperative)and proposes directions for the development of perioperative nursing care for HA.The pro-gnosis for patients can be improved through the modification of traditional medical approaches and the application of new technologies and concepts.

Evaluation of Clinical Application of ERAS Concept in Perioperative Nursing of HoLEP Patients

[Objectives]To observe the application effect of the concept of accelerated recovery after surgery(ERAS)on patients undergoing trans-urethral holmium laser enucleation of prostate(HoLEP)during perioperative period.[Methods]HoLEP patients with benign prostatic hyperplasia admitted to the Department of Urology,Affiliated Hospital of Jinggangshan University from June to December in 2022 were divided into the observation and control group.40 patients in the control group were given perioperative routine nursing,while 40 patients in the observation group were given perioperative systematic nursing under the concept of ERAS.The nursing effect of different measures were evaluated.[Results]The perioperative related indexes of patients in the observation group were lower than those in the control group(P<0.05).The postoperative feeding,exhaust,spontaneous urination,getting out of bed and hematuria stop time of patients in the observation group were all shorter than those in the control group(P<0.05).The IPSS score and RUV of patients in the observation group were significantly lower than those in the control group,while their Qmax was higher than that in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).[Conclusions]The application of perioperative systematic nursing under ERAS concept in HoLEP patients can promote patients rapid recovery and reduce their complications.

Perioperative Nursing for Adult Congenital Heart Disease with Severe Pulmonary Arterial Hypertension

Objectives: To explore the main points of perioperative nursing for adult congenital heart disease with severe pulmonary arterial hypertension. Methods: A retrospective study of 13 patients with congenital heart disease and severe pulmonary arterial hypertension who admitted to the perioperative period of care from January 2018 to December 2019. To prevent perioperative complications of the patients, the focus is on respiratory and circulatory system care, followed by blood coagulation monitoring, digestive system protection and psychological care. Results: All 13 patients passed the perioperative period and were discharged from ICU. Conclusion: Adult congenital heart disease with severe pulmonary arterial hypertension has high perioperative risk, respiratory and circulatory system care is the key.

Perioperative Nursing of a Case of Right Temporal Lobe Brain Abscess Ruptured into the Ventricle

Objective: To explore the perioperative nursing method of brain abscess breaking into the ventricle. Methods: By reviewing the clinical data of 1 case of right temporal lobe brain abscess into the ventricle, the effective nursing methods were summarized. Results: The patient’s condition was advanced quickly, and the diagnosis was treated in time to avoid brain hernia, but the treatment was difficult because of the critical condition. Conclusion: Brain abscess is a very serious intracranial infectious disease. It is of great significance to observe the changes in patients’ condition and take effective nursing measures.

Analysis of the Effects of Perioperative Nursing Intervention for Patients with Cerebrovascular Intervention

Objective:To explore the clinical application effects of perioperative nursing intervention in the treatment of patients with cerebrovascular intervention.Methods:176 patients with cerebrovascular intervention in our hospital from October 2018 to March 2020 were selected and randomly divided into 2 groups,with 88 cases in the control group and 88 cases in the observation group.Comparative analysis of blood pressure and incidence of complications was performed.Results:During the operation,the systolic blood pressure of the observation group increased by(11.85±1.66)mmHg,and the diastolic blood pressure was(4.63±0.45)mmHg.The control group patients'systolic blood pressure increased by(22.12±1.98)mmHg and the diastolic blood pressure was(8.36±3.69)mmHg,the two results were significantly different.The blood pressure of the patients in the control group fluctuated more;and the incidence of complications in the observation group is lower than that of the control group.The observation group's incidence of hematoma,low back discomfort,dysuria,and insomnia and dreaminess was 2.27%,2.27%,1.14%,0%,respectively,with total incidence of 5.68%;in the control group,the incidence of hematoma,low back discomfort,dysuria,insomnia and dreaminess were 6.81%,5.68%,3.41%,4.55%respectively,total incidence being 20.45%.The difference between observation group and control group is obvious.Conclusion:Through perioperative nursing intervention,the recovery speed of patients can be improved,and the incidence of postoperative complications can be reduced.The mental state of the patient before the operation was adjusted,the emotions were calmed,the patient's compliance was improved,and the patient's resistive emotions were reduced.At the same time,if patients can get good nursing care after surgery,the probability of postoperative complications is reduced,so that patients have better results in surgery,and its clinical application is worthy of promotion.

Perioperative Nursing Management of an Animal Model of Axial Flow Blood Pump Implantation

This study aimed to explore the characteristics of perioperative nursing of experimental goats using self-made axial-flow blood pump implantation and provided theoretical nursing knowledge and practice-based evidence for the clinical application of domestically manufactured artificial cardiac pumps. Methods: Seven experimental goats were used in this study, three for pre-testing and four for the formal experiments. According to the surgical requirements for axial-flow blood pump implantation into the cardiac apex, we creatively designed and made a series of highly practical animal surgical instruments including a composite disassemblable bed for experimental animal transferring and monitoring, a multifunctional animal surgery bed, and portable medical supporting equipment. We also applied for two national invention patents and one utility model patent. Active measures were taken to ensure careful preparation before surgery, close collaboration during surgery, and effective management of complications after surgery. Results: Two of the four experimental goats died during surgery because of a massive hemorrhage caused by distal anastomotic failure and air embolism-induced cardiac arrest caused by air leakage from the outlet into the heart due to poor connection of the auxiliary pressure tap (used to measure left ventricular pressure). The mean survival time of the remaining three experimental goats was 22.7 hours. Conclusion: This study was the first to systematically and comprehensively investigate the perioperative nursing management of axial-flow blood pump implantation using animal models. These findings could greatly promote further clinical applied nursing research of self-made artificial cardiac pump implantation in experimental goats.

Application Value of Evidence-Based Nursing in the Perioperative Care of Ankle Fracture Patients

Objective:To analyze the value of applying evidence-based nursing in the perioperative care of patients with ankle fractures and propose corresponding measures to improve the quality of patient care.Methods:The study started in May 2022 and was terminated in March 2023.During this period,50 patients with ankle fractures treated by our hospital were selected as research subjects.They were divided into a control group and a research group.The control group had 25 patients who received routine care,and the research group had 25 who received evidence-based care.The nursing effects of the two groups were observed and compared.Results:The pain scores,quality of life,incidence of adverse reactions,and nursing satisfaction of patients in the research group were all significantly better than those in the control group(P<0.05).Conclusion:Evidence-based nursing can effectively improve the quality of care,reduce the risk of adverse reactions,and promote patient recovery,so it is suitable for clinical implementation.

Analysis of the Effect of Comfort Care in The Perioperative Care of Patients with Hysteromyomectomy and its Influence on Self-Care Ability

Objective:To analyze the effect of perioperative comfort care for patients with hysteromyomectomy(HYS).Methods:Seventy cases of HYS patients admitted to our hospital from October 2022 to October 2023 were randomly selected and divided into Group A(control group,35 cases,conventional care)and Group B(observation group 35 cases,comfort care).The effects of the two groups before and after nursing care were compared.Results:The self-rating anxiety(SAS)scores,exercise of self-care agency(ESCA)scores,and nursing satisfaction scores at the time of discharge of Group B were better than in Group A after nursing care(P<0.05).Conclusion:Perioperative comfort care for HYS patients improved the patient’s adverse emotions,self-care ability,and nursing satisfaction.

Application of multidisciplinary collaborative nursing with family care for enhanced recovery after surgery in children with inguinal hernia

BACKGROUND Perioperative nursing can reduce the stress reaction and improve the prognosis of children.AIM To elucidate the influence of multidisciplinary collaborative nursing for enhanced recovery after surgery(ERAS)with family care in perioperative nursing children with an inguinal hernia and its impact on the prognosis.METHODS The data of 100 children with inguinal hernia were retrospectively analyzed.The participants were divided into three groups according to different nursing methods:Groups A(n=38),B(n=32),and C(n=30).Group A received multidisciplinary collaborative ERAS nursing combined with family care nursing;Group B received multidisciplinary collaborative nursing for ERAS;and Group C received routine nursing.The postoperative recovery results of the three groups were compared,including intraoperative blood loss and postoperative feeding time,time of getting out of bed,hospitalization time,and defecation time.Furthermore,the incidence of common complications was also compared between the three groups.RESULTS There was less intraoperative blood loss in Groups A and B than in Group C(P<0.05),and the time of getting out of bed and postoperative hospitalization and defecation times were also decreased in Group C(P<0.05).There was no significant difference in postoperative feeding time among the three groups(P>0.05).Each index had no statistical significance between Groups A and B(P>0.05).The incidence of urinary retention,infection,hematoma,and hernia recurrence in Group A was less than that in Group C(P<0.05).No significant difference was observed in the overall complication rate between Groups A and B and between Groups B and C(P>0.05).CONCLUSION The application of multidisciplinary collaborative nursing combined with family care in the perioperative care of children with an inguinal hernia for ERAS may promote postoperative rehabilitation for children and reduce the incidence of complications.

Nursing cooperation related to thoracoscopic surgery in children

Objective:To explore the nursing cooperation of pediatric cardiac surgery by thoracoscopy.Methods:30 patients undergoing thoracoscopy in a third-grade hospital in Yantai were observed and their nursing process was recorded and analyzed.Results:The good cooperation of operating room nurses and equipment nurses can relax the children and increase the success rate of surgery and reduce the occurrence of complications.Conclusion:Full preoperative preparation,close intraoperative cooperation,strict instrument maintenance,and cleaning and disinfection are the guaranteeof pediatric thoracic surgery.

Nursing practices for accelerated perioperative recovery of patients undergoing esophageal ultrasound-guided mitral valve chordae tendineae repair at the cardiac apex

Summary of the Experience in Perioperative Accelerated Recovery Care of a Patient with Severe Mitral Valve Regurgitation,Mitral Valve Prolapse,Coronary Heart Disease,and Hypertension Undergoing Transesophageal Echocardiography-Guided Mitral Valve Chord Repair System and Artificial Chord Implantation through the Apex of the Heart The patient had a history of coronary heart disease and hypertension,and was diagnosed with mitral valve prolapse and severe mitral regurgitation at admission.According to the comprehensive evaluation of the patient's condition by the cardiac surgery and MDT(Multidisciplinary Team)team of our hospital before operation,the patient had severe degenerative mitral regurgitation.Considering the high risk of surgical valvuloplasty,the treatment plan of transapical mitral valve artificial chordae implantation with mitral chordae repair system under the guidance of esophageal ultrasound was formulated.Following the guidelines for accelerated recovery,a multidisciplinary approach was taken during the surgery to monitor the patient's condition.Preoperative waiting periods,the postoperative critical period,and the stabilization period were considered,and a rehabilitation program that included exercise,nutritional support,health education,and psychological care was developed for the entire perioperative accelerated recovery process.Nutritional support,health education and psychological nursing were throughout the whole process of perioperative enhanced recovery.The patient was implanted with 4 artificial chordae tendineae.Transesophageal echocardiography monitoring showed that the surgical treatment effect was satisfactory,and no complications occurred during the perioperative period.

Robotic nurse duties in the urology operative room:11 years of experience

The robotic nurse plays an essential role in a successful robotic surgery.As part of the robotic surgical team,the robotic nurse must demonstrate a high level of professional knowledge,and be an expert in robotic technology and dealing with robotic malfunctions.Each one of the robotic nursing team“nurse coordinator,scrub-nurse and circulating-nurse”has a certain job description to ensure maximum patient’s safety and robotic surgical efficiency.Well-structured training programs should be offered to the robotic nurse to be well prepared,feel confident,and maintain high-quality of care.

Operating room nurses’lived experiences of ethical codes:A phenomenological study in Iran

Objective:Operating room nurses,as essential members of health care teams,often face ethical challenges in the operating room.By using the ethical experiences of operating room nurses,a better understanding of ethics in the operating room can be achieved,which can lead to better nursing decisions in the face of these challenges.Therefore,this study was conducted to investigate operating room nurses’lived experiences of ethical codes.Methods:A hermeneutic phenomenological study was performed in Hamadan(Iran)from February 2019 to November 2020.Ten operating room nurses were selected as participants by purposive sampling.Data were collected through in-depth and semi-structured interviews.Data analysis was performed based on Van Manen methodology.Results:Data analysis revealed three main themes and 11 sub-themes representing the operating room nurses experience of the ethical code.The main themes were;adherence to professional commitments,preserving patient dignity,and respect to colleagues.Conclusion:The results underlined ethics and ethical values in the operating room.Due to the intense interactions between operating room nurses with the patient and surgical team,commitment to ethics by nurses can lead to improving quality of care and interactions among members of the surgical team.It is suggested that using these codes as a guideline and a framework could be developed to improve the ethical and professional performance of operating room nurses.

Nutritional Evaluation by Nurses and Physical Recovery of Malnourished Patients after Esophagectomy

Following an esophagectomy, nurses’ patient nutrition observations are important for physical recovery and quality of life. The study proposed to identify the factors associated with physical recovery among esophageal cancer patients after surgery based on nurses’ observations and nutritional evaluations. Data were collected from the nursing records of 21 esophagectomy patients. The data included patients’ rate of weight loss, body mass index (BMI), energy intake, and postoperative complications. Patients were divided into two groups according to their weight loss rate: <8% and ≥8%. Patient outcomes were compared between the two groups. There was no difference in average BMI before or after surgery between the two groups. There was no difference in albumin levels during hospitalization;however, after esophagectomy, energy intake at discharge was insufficient in both groups. The ≥8% weight loss may be facilitated, or at least confounded, by the presence of recurrent nerve paralysis and aspiration pneumonia. Thus, the nursing records typically identified the need to check for recurrent laryngeal nerve palsy and aspiration pneumonia in conjunction with nutritional assessment to enhance patients’ physical recovery. When weight loss was significant, nurses provided oral intake support with the NST, and patients maintained oral intake levels similar to the group with weight loss under 8%. In order to better support physical recovery among malnourished, post esophagectomy patients, nurses need to monitor for signs of recurrent nerve palsy and aspiration pneumonia in addition to nutritional intake.
