A complete case of a deep excavation was explored. According to the practical working conditions, a 3D non-linear finite element procedure is used to simulate a deep excavation supported by the composite soil nailed wall with bored piles in soft soil. The modified cam clay model is employed as the constitutive relationship of the soil in the numerical simulation. Results from the numerical analysis are fitted well with the field data, which indicate that the research approach used is reliable. Based on the field data and numerical results of the deep excavation supported by four different patterns of the composite soil nailed wall, the significant corner effect is founded in the 3D deep excavation. If bored piles or soil anchors are considered in the composite soil nailed wall, they are beneficial to decreasing deformations and internal forces of bored piles, cement mixing piles, soil anchors, soil nailings and soil around the deep excavation. Besides, the effects due to bored piles are more significant than those deduced from soil anchors. All mentioned above prove that the composite soil nailed wall with bored piles is feasible in the deep excavation. 展开更多

Reinforcement of slopes using soil nailing can effectively improve slope stability, and it has been widely used in upgrading cut slopes. Based on the assumptions of stresses on the slip surface, a new method for analyzing the stability of a slope reinforced with soil nails was established in the limit equilibrium theory framework, by considering that slope sliding occurs owing to shear failure of the slip surface, which subjects to Mohr–Coulomb(M–C) strength criterion. Meanwhile, in order to easily analyze the stability of a soil nailed slope in actual engineering and facilitate optimum design of parameters for soil nailing, factor of safety(FOS) contour curve charts were drawn on the basis of the established linear proportional relationship between the spacing of soil nails and slope height, and the length of soil nails and slope height. Then, by analyzing and verifying the results obtained from classic examples, some conclusions can be got as follows: 1) The results obtained from the current method are close to those obtained from the traditional limit equilibrium methods, and the current method can provide a strict solution for the slope FOS as it satisfies all the static equilibrium conditions of a sliding body, thus confirming the feasibility of the current method; 2) The slope FOS contour curve charts can be used not only to reliably analyze the stability of a soil nailed slope, but also to design optimally the parameters of soil nailing for the slope with a certain safety requirement. 展开更多

An attempt has been made to study the behavior of nailed vertical excavations in medium dense to dense cohesionless soil under seismic conditions using a pseudo-dynamic approach. The effect of several parameters such as angle of internal friction of soil (φ), horizontal (kh) and vertical (kv) earthquake acceleration coefficients, amplification factor (fa), length of nails (L), angle of nail inclination (a) and vertical spacing of nails (S) on the stability of nailed vertical excavations has been explored. The limit equilibrium method along with a planar failure surface is used to derive the formulation involved with the pseudo-dynamic approach, considering axial pullout of the installed nails. A comparison of the pseudo-static and pseudo-dynamic approaches has been established in order to explore the effectiveness of the pseudo-dynamic approach over pseudo-static analysis, since most of the seismic stability studies on nailed vertical excavations are based on the latter. The results are expressed in terms of the global factor of safety (FOS). Seismic stability, i.e., the FOS of nailed vertical excavations is found to decrease with increase in the horizontal and vertical earthquake forces. The present values of FOS are compared with those available in the literature. 展开更多

Traditional diagnostic techniques including visual examination, ultrasound(US),and magnetic resonance imaging(MRI)have limitations of in-depth information for the detection of nail disorders, resolution, and practicality. This pilot study, for thefirst time, evaluates a dualmodality imaging system that combines photoacoustic tomography(PAT)with the US for the multiparametric quantitative assessment of human nail. The study involved a small cohort offive healthy volunteers who underwent PAT/US imaging for acquiring the nail unit data. The PAT/US dual-modality imaging successfully revealed thefine anatomical structures and microvascular distribution within the nail and nail bed. Moreover, this system utilized multispectral PAT to analyze functional tissue parameters, including oxygenated hemoglobin, deoxyhemoglobin, oxygen saturation, and collagen under tourniquet and cold stimulus tests to evaluate changes in the microcirculation of the nail bed.The quantitative analysis of multispectral PAT reconstructed images demonstrated heightened sensitivity in detecting alterations in blood oxygenation levels and collagen content within the nail bed, under simulated different physiological conditions. This pilot study highlights the potential of PAT/US dual-modality imaging as a real-time,noninvasive diagnostic modality for evaluating human nail health and for early detection of nail bed pathologies. 展开更多

With the aging population, intertrochanteric femur fracture in the elderly has become one of the most serious public health issues and a hot topic of research in trauma orthopedics.Due to the limitations of internal fixation techniques and the insufficient mechanical design of nails, the occurrence of complications delays patient recovery after surgical treatment.Design of a proximal femur bionic nail(PFBN)based on Zhang’s N triangle theory provides triangular supporting fixation, which dramatically decreases the occurrence of complications and has been widely used for clinical treatment of unstable intertrochanteric femur fracture worldwide. In this work,we developed an equivalent biomechanical model to analyze improvement in bone remodeling of unstable intertrochanteric femur fracture through PFBN use.The results show that compared with proximal femoral nail antirotation(PFNA)and InterTan,PFBN can dramatically decrease the maximum strain in the proximal femur.Based on Frost’s mechanostat theory, the local mechanical environment in the proximal femur can be regulated into the medium overload region by using a PFBN,which may render the proximal femur in a state of physiological overload, favoring post-operative recovery of intertrochanteric femur fracture in the elderly. This work shows that PFBN may constitute a panacea for unstable intertrochanteric femur fracture and provides insights into improving methods of internal fixation. 展开更多

Background: Fractures of humeral shaft in adults are common injuries. Humeral shafts non-union either from late presentation after initial treatment by traditional bone setters or failed non-operative orthodox care is a major problem in this part of the world. This non-union is a major treatment challenge with increased cost of care and morbidity in this part of the world. Humeral shaft non-union can be treated with locked intra-medullary nailing (LIMN) or dynamic compression plating (DCP). Study on comparison of these methods of fixation in this part of the world is scarce in literature search, hence the reason for this study. Objective: The objectives of this study are: (1) to compare early clinical outcome following fixation of humeral shaft fracture nonunion with DCP versus LIMN;(2) to compare the time of radiologic fracture union of DCP with LIMN;(3) to compare complications following fixation of humeral shaft fracture nonunion with DCP versus LIMN. Patients and Methods: This was a randomized control study done for 2 years in which fifty adult patients with humeral shaft non-union were recruited. The patients were grouped into 2 (P = DCP & N = LIMN). Forty five of the patients completed the follow up periods of the study and then analyzed. The P group had ORIF with DCP while the N group had ORIF with LIMN. Both groups had grafting with cancellous bones. Each patient was followed up for a period of 6 months at the time which radiographic union is expected. Any patient without clinical and/or radiographic evidence of union after six months of surgery was diagnosed as having recurrent non-union. The data generated was analyzed using SPSS Version 23. The results were presented in charts and tables. The paired t-test was used while considering p value Result: Forty five patients completed follow up. There was a male preponderance (4:1), right humerus predominated (3:2). Motor vehicular accidents were the commonest cause of the fractures (62%). Most non-union fractures occurred at the level of the middle 3rd of the humeral shaft (60%). Failed TBS treatment was the commonest indication for the osteosynthesis (71%). More patients had plating (53%) compared to 47% who had LIMN. Most patients (93.4%) had union between 3 to 6 months irrespective of fixation type with no significant statistical difference between the union rate of DCP and LIMN (p value 0.06) with similar functional outcome and complication rates irrespective of the type of fixation. Conclusion: This study showed that the success rates in term of fracture union, outcome functional grades and complication rates were not directly dependent on the types of the fixation: plating or locked intra-medullary nailing. 展开更多

Introduction: The management of fractures of the tibia shaft is an important aspect of orthopaedic care, and the selection of the surgical method for fixation can substantially impact patient outcomes. The current review aims to compare the outcomes of adult tibia fractures treated with solid nails to those treated with hollow nails. Methods: A search on Scopus, PubMed, and Cochrane Library, using three keywords (Outcome, Tibia shaft fractures, Nail) was conducted in April 2023. Results were compiled and two independent reviewers screened and selected eligible articles After removing duplicates, titles and abstracts were read to exclude ineligible studies. Full-text articles of the remaining papers were read to select eligible studies which were further critically appraised to ascertain their methodological quality. The data extracted from the selected papers were synthesized using a combination of pooling of results, tests of statistical difference (t-test and chi-square) and narrative synthesis methods. Results: A total of 2295 articles were obtained from the databases and citation searching. A total of 9 papers were identified as eligible and included in the review. Findings revealed that there is no statistical difference in the outcomes of tibia fractures treated with either solid or hollow nail groups such as duration of surgery (p = 0.541), rate of delayed and non-union (p = 0.342), and rate of surgical site infections (p = 0.395). Conclusion: Intramedullary nailing of tibia shaft fractures with either solid or hollow nails have similar functional outcomes. 展开更多

Introduction: Pseudarthrosis (PSA) of the diaphysis of long bones still remains a current problem, despite improvements in the treatment of these fractures. Our study aims to study the epidemiological and therapeutic aspects of PSA of the diaphysis of long bones. Method: This retrospective work concerns 30 cases of non-union of the diaphysis of long bones treated in the orthopedic and trauma surgery department at Donka National Hospital, during a period of 18 months from January 1, 2019 to June 30, 2020. Results: We recruited 30 patients, 80% of whom were male, with an average age of 39.9 years. Public road accidents (AVP) represented the main cause of fractures of the diaphysis of long bones 87%, they were open in 25 cases or 83%. The fractures were located in the middle 1/3 of the diaphysis of the long bones in 50% of cases. Treatment of initial fractures was traditional in 21 cases, orthopedic in 2 cases and surgical in 7 cases. It was aseptic nonunion in 28 cases (93%) and septic nonunion in 2 cases. They were hypertrophic in 7 cases, slightly hypertrophic in 5 cases, oligotrophic in 11 cases, atrophic in 6 cases and with bone defect in 1 case. The treatment was based on osteosynthesis including 16 cases of screwed “PV” plate: 7 cases of centromedullary “ECM” nailing, 2 cases of external fixator, 1 case of broaching and 4 cases of Plastering. The results according to ASAMI criteria on an anatomical level were excellent in 19 cases, good in 3 cases and poor in 3 cases, with a union rate of 76%. And 5 patients undergoing consolidation. Conclusion: Based on the literature data and the experience of our department, the true treatment of PSA requires correct management of the initial fracture without forgetting the interest in preventing AVP which appears to be an element essential, making it possible to reduce the incidence of fractures of the diaphysis. 展开更多

Purpose: The proximal femoral nail anti-rotation (PFNA) is known to have advantages in enhancing the anchorage ability of internal fixation in elderly unstable osteoporotic intertrochanteric fracture patients. However whether it is superior to condylar blade fixation is not clear. This study aimed to determine which treatment has better clinical outcomes in older patients. Materials and Methods: A total of 86 patients over the age of 60 with unstable trochanteric fractures within the past 3 weeks, were included in this prospective study conducted from June 1, 2018, to May 31, 2021. All the intertrochanteric fractures were classified according to AO/OTA classification. Among them, 44 cases were treated with the Proximal Femoral Nail (PFNA2) with or without an augmentation screw, and 42 cases were treated with the Condylar Blade Plate. In addition, the operative time, intraoperative blood loss, intraoperative and postoperative blood transfusion, postoperative weight-bearing time, hospitalization time, Harris score of hip function, Kyle’s criteria and postoperative complications were compared between the two groups. Results: The mean duration of surgery for the PFN group was 66.8 minutes (on average), whereas for the condylar blade plate group, it was 99.30 minutes (on average). The PFNA2 group experienced less blood loss (average of 80 mL) compared to the condylar blade plate group (average of 120 mL). Union and partial weight-bearing occurred earlier in the PFNA2 group (14.1 weeks and 10.6 weeks, respectively) compared to the Condylar blade plate group (18.7 weeks and 15.8 weeks). In two patients from the PFNA2 group, screw backing out and varus collapse complications were encountered;however, these patients remained asymptomatic and did not require revision surgery. In two other patients, screw cut out and breakage of the nail at the helical screw hole leading to non-union of the proximal femur were observed during the nine-month follow-up, necessitating revision surgery with prosthetic replacement. Among the condylar blade plate group, three patients experienced complications, including blade breakage at the blade and plate junction. In two cases, the fracture united in varus, and in one case, the blade cut through, resulting in non-union of the femoral head, which required revision surgery. According to the Harris hip score and Kyle’s criteria, a good-excellent outcome was observed in 92.85% of cases in the PFNA2 group and 90.90% of cases in the condylar blade plate group. Conclusion: Both the Proximal Femoral Nail A2 and Condylar blade plate are effective implants for the treatment of unstable trochanteric fractures. The intramedullary implant promotes biological healing and allows for early ambulation with minimal complications. Similarly satisfactory restoration of anatomy and favorable radiological and functional results can be achieved with the biological fixation provided by the 95-degree condylar blade plate. However, the use of PFNA2 internal fixation technique has the advantage of less trauma in elderly patients than the 95-degree condylar blade plate. 展开更多

[Objective] The aim of this study was to investigate the effects of salt stress on cell expansion in Arabidopsis thaliana rosette leaves.[Method] Arabidopsis seedlings were treated by sodium chloride at the concentration of 0,100 or 150 mmol/L. At the 7th and 14th d of treatment, with nail enamel printing mark method and computer software, the leaf blades area and abaxial epidermal pavement cells area was measured and compared using statistical analysis in Excel. [Result] The growth of Arabidopsis rosette leaves was inhibited under salt stress. Leaves treated for 7 or 14 d expanded less compared with controls. The salt-mediated decrease in leaf expansion is associated with a decrease in abaxial pavement cell expansion. [Conclusion] The decreased leaf and epidermal cell expansion under salt stress is the most important characteristic of plant physiological response to salt stress. 展开更多

The roof system of social housing in Brazil generally consists of components made out of native forest lumber of high market value. Taking into account the increasing number of planted forests and the need to develop new products and to add value to this timber, this work deals with the development and structural analysis of a roof system using yellow pine plantation wood (Pinus spp), a sustainable material which however presents many defects. The NLT (laminated nailed timber) technology was chosen as it allows the use of shorter length and smaller cross section pieces, eliminating major defects. Seven samples of structural trussed rafters in NLT were tested; six made out of graded timber and one ungraded in order to verify the impact of the wood grading in the structural performance of the model. The results showed that the trussed rafter system in NLT meets the necessary structural performance requiring poor conditions of infrastructure for manufacture process, and that the graded wood samples showed better performance than the ungraded one. 展开更多

The microstructure, mechanical properties and corrosion behavior of quaternary degradable Mg−1Zn−0.2Ca−xAg(x=1,2,4 wt.%)alloy wires, intended as anastomotic nails, were investigated. It was found that these Ag-containing alloy wires mainly consist of Mg matrix and Ag17Mg54 phase, characterized by SEM,EDS,XRD and TEM.Tensile and knotting tests results demonstrate the superior mechanical properties of these alloy wires.Especially,Mg−1Zn−0.2Ca−4Ag alloy exhibits the highest mechanical properties,i.e.an ultimate tensile strength of 334 MPa and an elongation of 8.6%.Moreover,with increasing Ag content, the corrosion rates of these alloy wires remarkably increase due to the formation of more micro-galvanic coupling between Mg matrix and Ag17Mg54 phase, shown by mass loss and scanning Kelvin probe force microscopy(SKPFM)results. The present alloy can be completely degraded within 28 d,satisfying the property requirements of anastomotic nails. 展开更多
