Background:The Victorian Institute of Sport Assessment–Patella(VISA-P) scale is the most condition-specific patient-reported outcome measure used to assess symptom severity in athletes with patellar tendinopathy.Prev...Background:The Victorian Institute of Sport Assessment–Patella(VISA-P) scale is the most condition-specific patient-reported outcome measure used to assess symptom severity in athletes with patellar tendinopathy.Previous exploratory factor analyses have been conducted to evaluate the scale's dimensionality,with inconsistent results,and the factor structure of the scale remains unclear.The aims of the present study were to determine the factorial structure of the VISA-P scale using confirmatory factor analysis(CFA) and test measurement invariance across sexes.Methods:The study included a convenience sample of 249 Spanish athletes with patellar tendinopathy.CFA was performed to assess factorial validity.Hypothesized 1-and 2-factor models were tested.Measurement invariance across sexes was evaluated via multi-group CFA with several fit indices using EQS 6.1 software.Results:The internal consistency coefficient was 0.74.Several CFA models were examined and the 1-factor model in which errors for Items 7 and8 were correlated showed acceptable fit in terms of comparative fit index(CFI) and goodness-of-fit index(GFI) statistics(CFI = 0.93;GFI = 0.94;standardized root mean square residual = 0.06;root mean square error of approximation = 0.10;90% confidence interval:0.08–0.13).This model was invariant across sexes.Conclusion:The 1-factor model of the Spanish version of the VISA-P scale(VISA-P-Sp) in which errors for Items 7 and 8 were correlated demonstrated relative fit in CFA.Scores obtained via VISA-P-Sp can be compared between men and women without sexes bias.Further studies should examine the VISA-P scale and other single-score patient-reported outcome measures concurrently.展开更多

METHODS: Specimens were collected surgically from seven patients with gastric adenocarcinomas. Immu- nostaining for C-MYC and dual-color fluorescence in situ hybridization (FISH) for C-MYC gene and chromosome 8 centromere were performed. RESULTS: All the cases showed chromosome 8 aneu- ploidy and C-MYC amplification, in both the diffuse and intestinal histopathological types of Lauren. No significant difference (P < 0.05) was observed between the level ofchromosome 8 ploidy and the site, stage or histological type of the adenocarcinomas. C-MYC high amplification, like homogeneously stained regions (HSRs) and double minutes (DMs), was observed only in the intestinal-type. Structural rearrangement of C-MYC, like translocation, was observed only in the diffuse type. Regarding C-MYC gene, a significant difference (P < 0.05) was observed between the two histological types. The C-MYC protein was expressed in all the studied cases. In the intestinal- type the C-MYC immunoreactivity was localized only in the nucleus and in the diffuse type in the nucleus and cytoplasm. CONCLUSION: Distinct patterns of alterations between intestinal and diffuse types of gastric tumors support the hypothesis that these types follow different genetic path- ways.展开更多

This study evaluated the clinical outcomes in keratoconus corneas following accelerated transepithelial corneal collagen cross-linking(CXL)(Avedro KXL system,Waltham,MA,USA) over one year of follow-up.The mean de...This study evaluated the clinical outcomes in keratoconus corneas following accelerated transepithelial corneal collagen cross-linking(CXL)(Avedro KXL system,Waltham,MA,USA) over one year of follow-up.The mean depth of the demarcation line measured by optical coherence tomography(OCT) was 205.19 μm.One month after surgery,a non-statistically significant change was noted in sphere(P= 0.18) and in spherical equivalent(P= 0.17),whereas a significant improvement was observed in corrected distance visual acuity(P=0.04).A significant change was observed in topographic astigmatism(P= 0.03) and posterior corneal a sphericity(P= 0.04).Accelerated transepithelial CXL may be a useful technique for the management of progressive keratoconus.展开更多
Purpose It is well recognised that injury prevention training can reduce injury incidence,however current coach education pathways do not provide grass-root coaches with the knowledge and confidence to deliver such tr...Purpose It is well recognised that injury prevention training can reduce injury incidence,however current coach education pathways do not provide grass-root coaches with the knowledge and confidence to deliver such training to youth players.The aim of this study was to explore differences in knowledge,understanding,attitude and confidence to deliver such injury prevention training in three European countries.Methods A total of 269 grass-root soccer coaches from 3 European countries(Czech Republic,UK,Spain)were recruited for this study.A validated questionnaire exploring knowledge,understanding,attitude towards and confidence to deliver youth injury prevention training was completed prior to a 2 h workshop on injury prevention training.Differences between countries was examined using Bayesian factors to quantify the evidence for and against the hypothesis of independence(H0)by assuming a Poisson sampling scheme(as there was no a priori restriction on any cell count,nor on the grand total)(BF10 Poisson).Results Current knowledge,attitude and confidence to deliver injury prevention training to youth players was poor across all three European countries.Relatively few coaches were currently using injury prevention training in their coaching sessions(23%).There were some country specific differences for attitude towards injury prevention training and confidence to deliver injury training,with Spanish coaches reporting a more positive attitude and confidence to deliver such training.Significantly fewer coaches in the UK were using injury prevention training compared to coaches in Spain and the Czech Republic.Conclusion As coaches identified a need for coach education and few were delivering injury prevention training,there is a clear need to embed and implement this programme into the grassroots coaching framework of sports governing bodies to improve adoption,implementation and maintenance.展开更多

The tumor is indolent, usually with long survival, even in the presence of exte...Solid pseudopapillary tumor of the pancreas (SPT) is an uncommon neoplasm of low malignant potential, generally occurring in young women. The tumor is indolent, usually with long survival, even in the presence of extension into adjacent organs and metastases. Pathological features include solid, cellular,and cystic regions and degenerative pseudopapillae formation.Despite its distinctive morphology and cytological features, the cell lineage of this entity is unclear. Here we report a case of solid pseudopapillary tumor in a 48-year-old man with 10-year follow-up in which melanin pigment was found within the tumor cells. The tumor cells stained positive not only for melanocytic markers including S-100, HMB-45, and Fontana, but also other well-established markers for this kind of neoplasm such as alpha-antitrypsin (α-AT), anti-alpha-chymotrypsin (AACT) ,NSE, CD10, cyclin D1, and beta-catenin. Electron microscopy confirmed the formation of premelanosomes and melanosome granules in the tumor cells. To our knowledge, this is the first report in which melanosomes were produced by SPT. Because melanocytes are derived from neurocrest, we hypothesize that the histogenesis of SPT is of neurocrest origin. This phenomenon may also be explained by ongoing research in which it has been shown that Wnt signaling/beta-catenin intra-nuclear localization promotes pigment cell formation by medial crest cells in embryos.展开更多
