The objective of this study is to visualize flooding risk of municipalities at the intersection of the coastal sedimentary zone and the crystalline surface. The methodology adopted is based on geomatic approach, which involves documentary research, processing and assisted classification using remote sensing images and multi-criteria analysis of the Geographic Information System (GIS). Flooding risk is very high at 8.85% in Djidja, Toffo, Zè and Bonou municipalities. In other municipalities such as Agbangnizoun, Abomey, Bohicon, Za-Kpota and Cove, it is high of 46.85%. To the Southeast of the study area, it is located on the eastern and western banks of Oueme Valley. The medium risk represents 26.35% and is located in the municipalities of Ouinhi and Adjohoun. The other municipalities have a low rate of 17.95%. Risk modeling has made it possible to access the various levels of rising water that can cause flooding. Land-use planning decisions can be influenced by the results of this study.展开更多

Due to lack of financial resources, but also because of poor management, everywhere in Niger we are witnessing an accelerated deterioration of the irrigation infrastructure of hydro-agricultural developments. Institutional studies carried out on this situation led the State of Niger to initiate a reform of the governance of hydro-agricultural developments, by streng-thening the status of ONAHA, by creating an Association of Irrigation Water Users (AUEI) and by restructuring the old cooperatives. Indeed, this research aims to analyze the creation of functional and sustainable Irrigation Water User Associations (AUEI) in Niger in a context of reform of the irrigation sector, and based on the experience of the Konni AHA. It is based on a methodological approach which takes into account documentary research and the collection of data from 115 farmers, selected by reasoned choice and directly concerned by the management of the irrigated area. The data collected was analyzed and the results were analyzed using the systemic approach and the diagnostic process. The results show that the main mission of the AUEI is to ensure better management of water, hydraulic equipment and infrastructure on the hydro-agricultural developments of Konni. The creation of the Konni AUEI was possible thanks to massive support from the populations and authorities in the implementation process. After its establishment, the AUEI experienced a certain lethargy for some time due to the rehabilitation work of the AHA but currently it is functional and operational in terms of associative life and governance. Thus, the constraints linked to the legal system, the delay in the completion of the work, the uncertainties of access to irrigation water but also the problems linked to the change in mentality of certain ONAHA agents constitute the challenges that must be resolved in the short term for the operationalization of the Konni AUEI.展开更多

The GERD-TEST and the Medical Outcome Study Short Form-8 Health Survey (SF-8) were administered at baseline and after 4 wk of treatment. The GERD-TEST contains three domains: the severity of GERD and functional dyspepsia (FD) symptoms (5 items), the level of dissatisfaction with daily life (DS) (4 items), and the therapeutic efficacy as assessed by the patients and medication compliance (4 items).RESULTSA total of 290 patients were eligible at baseline; 198 of these patients completed 4 wk of PPI therapy. The internal consistency reliability as evaluated using the Cronbach’s α values for the GERD, FD and DS subscales ranged from 0.75 to 0.82. The scores for the GERD, FD and DS items/subscales were significantly correlated with the physical and mental component summary scores of the SF-8. After 4 wk of PPI treatment, the scores for the GERD items/subscales were greatly reduced, ranging in value from 1.51 to 1.87 and with a large effect size (P < 0.0001, Cohen’s d; 1.29-1.63). Statistically significant differences in the changes in the scores for the GERD items/subscales were observed between treatment responders and non-responders (P < 0.0001).CONCLUSIONThe GERD-TEST has a good reliability, a good convergent and concurrent validity, and is responsive to the effects of treatment. The GERD-TEST is a simple, easy to understand, and multifaceted PRO instrument applicable to both clinical trials and the primary care of GERD patients.展开更多

It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by MRA.RESULTS The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL. As for the individual symptoms, meal-related distress, dumping, abdominal pain, and esophageal reflux significantly affected the postoperative QOL in that order, while the influence of indigestion, diarrhea and constipation was insignificant. CONCLUSION Several clinical factors such as the symptom severity(especially in meal-related distress and dumping), ability for working and necessity for additional meals were the main factors which affected the patients' wellbeing after gastrectomy.展开更多

Turkey ranks 18th place in the world based on the number of scholarly publications. The objective of this paper is to find out if the monetary support program initiated in 1993 by the Turkish Scientific and Technological Research Council (TUBITAK) to incentivize researchers and increase the number, impact, and quality of international publications has been effective in doing so.Design/methodology/approach: We analyzed some 390,000 publications with Turkish affiliations listed in the Web of Science (WoS) database between 1976 and 2015 along with about 157,000 supported ones between 1997 and 2015. We used the interrupted time series (ITS) analysis technique (also known as "quasi-experimental time series analysis" or "intervention analysis") to test if TOBITAK's support program helped increase the number of publications. We defined ARIMA (1,1,0) model for ITS data and observed the impact of TOBiTAK's support program in 1994, 1997, and 2003 (after one, four and 10 years of its start, respectively). The majority of publications (93%) were full papers (articles), which were used as the experimental group while other types of contributions functioned as the control group. We also carried out a multiple regression analysis.Findings: TUBITAK's support program has had negligible effect on the increase of the number of papers with Turkish affiliations. Yet, the number of other types of contributions continued to increase even though they were not well supported, suggesting that TUBITAK's support program is probably not the main factor causing the increase in the number of papers with Turkish affiliations. Research limitations: Interrupted time series analysis shows if the "intervention" has had any significant effect on the dependent variable but it does not explain what caused the increase in the number of papers if it was not the intervention. Moreover, except the"intervention", other "event(s)" that might affect the time series data (e.g., increase in the number of research personnel over the years) should not occur during the period of analysis, a prerequisite that is beyond the control of the researcher. Practical implications: TUBITAK's "cash-for-publication" program did not seem to have direct impact on the increase of the number of papers published by Turkish authors, suggesting that small amounts of payments are not much of an incentive for authors to publish more. It might perhaps be a better strategy to concentrate limited resources on a few high impact projects rather than to disperse them to thousands of authors as "micropayments." Originality/value: Based on 25 years' worth of payments data, this is perhaps one of the first large-scale studies showing that "cash-for-publication" policies or "piece rates" paid to researchers tend to have little or no effect on the increase of researchers' productivity. The main finding of this paper has some implications for countries wherein publication subsidies are used as an incentive to increase the number and quality of papers published in international journals. They should be prepared to consider reviewing their existing support programs (based usually on bibliometric measures such as journal impact factors) and revising their reward policies.展开更多

Recently, a nationwide PGS study was performed using the Postgastrectomy Syndrome Assessment Scale-45(PGSAS-45) questionnaire.AIM To determine the influence of each background factor on PGS for each gastrectomy type using PGS assessment study(PGSAS) data as an additional analysis. METHODS The data of 2368 patients were obtained from the PGSAS. This included patients undergoing distal gastrectomy(DG) with Billroth I reconstruction, DG with Roux-en-Y reconstruction, total gastrectomy with Roux-en-Y, proximal gastrectomy, pylorus-preserving gastrectomy(PPG), and local resection. Multiple regression analysis was performed to explore the independent effects of each background factor on the main outcome measures(MOMs) of PGSAS-45 for each gastrectomy type. The background factors included postoperative period, age, sex, surgical approach(laparoscopic or open), and the status of the celiac branch of the vagal nerve.RESULTS The MOMs of DG and PPG were highly affected by background factors, whereas those of total gastrectomy with Roux-en-Y, proximal gastrectomy, and local resection were not. Worse PGS were found in females, whereas a longer postoperative period alleviated some of the MOMs. For DG and PPG, a laparoscopic approach and preservation of the celiac branch improved several MOMs.CONCLUSION Various background factors affected PGS, and their influence varied with the type of gastrectomy performed. Laparoscopic surgery and celiac branch preservation can improve PGS in patients undergoing DG and PPG.展开更多

A q-exponential probability discounting model based on Tsallis’ statistics has been proposed in econophysics (Takahashi, 2007, Physica A). We examined (a) fitness of the models to behavioral data of probability discounting of both gain and loss;and (b) relationships between parameters in the q-exponential probability discounting model across gain and loss. Our results demonstrated that, for both gain and loss, the q-exponential model better fits the behavioral data than exponential and hyperbolic functions, and there is the sign effect in q-exponential probability discounting. Relationships between Kahneman-Tversky’s prospect theory in behavioral economics and the q-exponential probability discounting are high-lightened.展开更多

By adopting mathematical models of probabilistic punishment behavior (i.e., exponential, hyperbolic, and q-exponential probability discounting model based on Tsallis thermodynamics and neuroeconomics, Takahashi, 2007, Physica A;Takahashi et al., 2012, Applied Mathematics), we examined 1) fitness of the models to behavioral data of uncertain punishment, and 2) deviation from the socially optimal hyperbolic punishment function. Our results demonstrated that, the q-exponential punishment function best fits the behavioral data, and people overweigh the severity of punishment at small punishing probabilities and underweigh the severity of punishment at large punishing probabilities. In other words, people tend to punish crimes too severely and mildly with high and low arrest rate (e.g., homicide vs. excess of speed limit), respectively. Implications for neuroeconomics and neurolaw of crime and punishment (Takahashi, 2012, NeuroEndocrinology Letters) are discussed.展开更多

We have proposed that psychophysical time commonly explains anomalies in both decisions (Takahashi, 2011, Physica A;Takahashi et al., 2012, J Behav Econ & Finance). By adopting the q-exponential time and probability discounting models, our psychophysical and behavioral economic experiment confirmed that nonlinear distortion of psychophysical time is a common cause of the anomalies in decision both over time and under risk (i.e., intertemporal choice and decision under risk). Implications for psychophysical neuroeconomics and econophysics are discussed.展开更多

We examined whether the higher-order cognitive functions are involved in the activations in the mirror neuron area when we perceive simplified pseudo-postures. We measured 14 participants’ brain activation during the posture-recognition task using near-infrared spectroscopy. The participants’ task was to observe five sequentially presented target pseudo-postures and judge whether a test pseudo-posture was identical to one of the preceding five target pseudo-postures. The results in the majority of participants (n = 10/14) revealed that the activity in the inferior frontal mirror neuron area is modulated by perception of human-likeness, but not in the remaining four participants (n = 4/14). These results suggest that the degree of the activation of higher-order cognitive functions, which may be engaged in the inhibitory and/or facilitative processing of human body or bodily movement, leads to the distinctive activities in the inferior frontal mirror neuron area.展开更多

The objective of this research is to analyze the issues and challenges of capacity building for Konni producers in the context of AHA management reform in Niger. The methodological approach adopted is based on documentary research and field surveys with all the actors directly concerned with the management of the Konni irrigated perimeter. A total of 448 people were interviewed in all the localities selected. Direct observation and interview are the two field data collection techniques adopted. The data collected was processed and on the basis of the established diagnosis, the training needs were identified. The results show that there are several experiences in terms of training and capacity building of actors in charge of the management of irrigation systems at the Konni AHA level. However, the sustainability of the perimeter is threatened by various crises. The main themes that founded the first training were related to the mobilization, transport and distribution of irrigation water, operation and maintenance, and setting and collection of the fee. In view of the expectations of operators, future issues and challenges, the training modules are increasingly oriented towards social management, the integration of gender in management and environmental impacts. Even if technical progress is possible, the real lever is organizational and requires the professionalization of irrigators. This process requires a new pedagogical approach aimed at irrigators.展开更多

The colour purple signifies a metaphysical, social and personal rebirth reflected into different shades by their inner self. The social oppression for black women in their quest for freedom is the main theme of Walker's novel The Color Purple, written in the epistolary technique of Samuel Richardson's in the XVIllth century English novel. In our attempt to analyse the theme and the female characters of the novel we are also trying to cross a bridge from the slave woman of the past belonging to a completely different culture and race to the contemporary paradigm of the liberated woman. The movement in time encompasses possible similitudes and differences.展开更多

The analysis of the hazard was determined by the field study coupled with the application of geophysical methods to locate and map the cavities and identify their dimensions and their positions in the subsoil. In the region of El Fahs (40 km NW of Tunis): we used a non-destructive method, georadar (GPR) with a 200 MHz antenna band-width, and an electric method, by using the electric tomography of which we have applicated the sequence of dipole-dipole measurement. The results ob-tained were examined and interpreted according to 2D profiles. The geophysi-cal methods of GPR and electrical tomography aim to detect many calvities in different depths. However, the geological radar was able to identify several cavities and the zones of dissolution whose investigation depth did not exceed the first 3 meters. On the contrary, the electric tomography method allowing the presence of several deeper underground cavities with larger dimensions ex-tended to 64 meters. To conclude, the geological and hydrogeological contexts of the terrain studied and on the state of the soil and subsoil allow us to better understanding the mechanisms of the establishment of the dissolution phe-nomenon and the appearance of cavities in the basement. The analysis of the predisposing factors present in the study area shows that the geodynamic con-text of the Triassic ascent takes place according to a precise thermodynamic process that favors the phenomenon of dissolution of the gypsum.展开更多

The diachronic study of beach profiles, before and after the seawalls construction showed that these protective structures accentuated the coastal erosion, with more important sedimentary deficits at the ends of the structures. Today, the Rufisque coast is characterized by a set of sedimentary cells whose morphological evolution and annual sedimentary dynamics are controlled by seasonal forcing: sedimentary deposits in dry season and erosion in wet season. However, this alternation of deposits and erosion phase shows spatiotemporal disparities. These disparities are very marked between a northwestern area characterized by concave profiles and a southeastern area (the bay of Rufisque) characterized by convex profiles and a much more important sedimentary deficit. The results also showed that these methods of shoreline protection are not durable measurements (degradation of the structures) and have exacerbated the vulnerability to coastal erosion as well as those of the coastal populations.展开更多

In this paper, we propose a framework called CRISE that allows studying and understanding the psychological effect of COVID-19 during the lockdown period. Mainly, CRISE consists of four data stages: Collection, tRansformation, reductIon, and cluStEring. The first stage collects data from more than 2000 participants through a questionnaire containing attributes related to psychological effect before and during the lockdown. The second stage aims to preprocess the data before performing the study stage. The third stage proposes a model that finds the similarities among the attributes, based on the correlation matrix, to reduce its number. Finally, the fourth stage introduces a new version of Kmeans algorithm, called as Jaccard-based Kmeans (JKmeans), that allows to group participants having similar psychological situation in the same cluster for a later analysis. We show the effectiveness of CRISE in terms of clustering accuracy and understanding the psychological effect of COVID-19.展开更多

This research focuses on the effects of climate variability on yam production in central and northern Benin. Daily climate data such as temperatures, precipitation, potential evapotranspiration, relative humidity and insolation from 1981 to 2016 were collected at the direction of METEO-Benin. A survey was then conducted with 351 producers to collect their perceptions of the effect of climate variability on the yams production. Descriptive statistics, principal component analysis (PCA) followed by trend analyses, Lamb index calculations and the agro-climatic stress index were carried out. The distribution of rain during the yam cycle (91.5% of those surveyed), the ambient temperature (84.9%), insolation (83.5%), the amount of rainwater that fell during the rainy season (82.2%) and the harmattan (53.3%) are perceived by sociolinguistic groups as climatic factors affecting the growth, the tuberization and yield of yams according to the producers. The results indicate an upward trend in temperature over the period 2001 to 2016 in the transition zone (28.4<span style="white-space:nowrap;">°</span>C) and in the Northeast (27.76<span style="white-space:nowrap;">°</span>C) and over the period from 2001 to 2011 in the North-West of Benin (27.71<span style="white-space:nowrap;">°</span>C), where the average annual temperature during these periods is higher than normal (27.82<span style="white-space:nowrap;">°</span>C in the center, 27.44<span style="white-space:nowrap;">°</span>C in the Northeast and 27.42<span style="white-space:nowrap;">°</span>C to the northwest). This research also shows an instability of the rains with a regular decrease in the rainfall in the Centre and the North-East of the country. The agro-climatic stress index (ISA) and above all the annual rainfall constitute the main climatic factors which determine the yield of yam in the various growing areas in Benin. The average annual temperature and that of January in the transition zone, the rains of January and April in the North-East zone and the relative humidity in the North-West zone also probability determine the yield of the yam.展开更多
文摘Climate change and population growth have led to the increase and/or intensification of flooding becoming a major issue. Land-use planning decisions can be influenced by the results of this study.
文摘During the 1980s, as part of a policy of liberalization, following budgetary cuts linked to the implementation of structural adjustment programs, management responsibilities for AHAs were transferred from ONAHA to cooperatives concerned. Thus, the constraints linked to the legal system, the delay in the completion of the work, the uncertainties of access to irrigation water but also the problems linked to the change in mentality of certain ONAHA agents constitute the challenges that must be resolved in the short term for the operationalization of the Konni AUEI.

文摘To evaluate the psychometric properties of a newly developed questionnaire, known as the gastroesophageal reflux and dyspepsia therapeutic efficacy and satisfaction test (GERD-TEST), in patients with GERD.METHODSJapanese patients with predominant GERD symptoms recruited according to the Montreal definition were treated for 4 wk using a standard dose of proton pump inhibitor (PPI). The GERD-TEST is a simple, easy to understand, and multifaceted PRO instrument applicable to both clinical trials and the primary care of GERD patients.

文摘AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after gastrectomy.METHODS The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. CONCLUSION Several clinical factors such as the symptom severity(especially in meal-related distress and dumping), ability for working and necessity for additional meals were the main factors which affected the patients' wellbeing after gastrectomy.
文摘Purpose: One of the main indicators of scientific production is the number of papers published in scholarly journals. They should be prepared to consider reviewing their existing support programs (based usually on bibliometric measures such as journal impact factors) and revising their reward policies.

文摘Since the 1980 s,Japan has witnessed an unprecedented decline in agriculture chiefly due to farmers’aging, depopulation, and unfavorable socio-economic conditions. This development has resulted in an increase of farmland abandonment(FLA)across the country. However,it remains unclear as to how and to what extent FLA is influenced by intraregional agricultural characteristics. As such, this article discusses the issue of FLA by taking a closer look at the Chugoku and Shikoku region,as it has experienced the highest FLA rates in Japan in recent years. For this analysis,a total of 25 indicators retrieved from the census of agriculture and forestry at the former municipalities scale were selected to describe intraregional agricultural characteristics. We employed principal component analysis(PCA)to evaluate agricultural characteristics, while multiple linear regressions(MLR)was applied to explore their correlations with FLA and spatial variations.First, there are strong intraregional differences in the agricultural characteristics across the Chugoku and Shikoku region, with eight different principle components(PCs)describing their characteristics.Second, variables measuring agricultural characteristics explain nearly 52.8%of the variation in FLA in our sample. The sales orientation and scale of agriculture have the strongest negative correlation to FLA in the region, while the status of agricultural succession displays the strongest positive correlation to FLA.Third,in areas where agriculture is more stable and easier to maintain,FLA is more strongly influenced by changes in agricultural characteristics than by geographical variations. We argue that localized approaches and policies for future management need to take intraregional differences in agricultural characteristics and FLA into account. Our findings help to explain spatial variations in agricultural characteristics and FLA in regional contexts, suggesting the need for better-informed farmland use policies to mitigate further abandonment.

文摘BACKGROUND Postgastrectomy syndromes(PGS) after curative gastrectomy for gastric cancer are influenced by not only gastrectomy type but also by background factors. Laparoscopic surgery and celiac branch preservation can improve PGS in patients undergoing DG and PPG.
文摘Probability discounting is defined as the devaluation of outcomes as the probability of receiving or paying those decreases. Relationships between Kahneman-Tversky’s prospect theory in behavioral economics and the q-exponential probability discounting are high-lightened.
文摘BACKGROUND The effects of various gastrectomy procedures on the patient’s quality of life(QOL)are not well understood. Thus, this nationwide multi-institutional crosssectional study using the Postgastrectomy Syndrome Assessment Scale-45(PGSAS-45),a well-established questionnaire designed to clarify the severity and characteristics of the postgastrectomy syndrome, was conducted.AIM To compare the effects of six main gastrectomy procedures on the postoperative QOL.METHODS Eligible questionnaires retrieved from 2368 patients who underwent either of six gastrectomy procedures[total gastrectomy with Roux-en-Y reconstruction(TGRY;n=393),proximal gastrectomy(PG;n=193),distal gastrectomy with Roux-en-Y reconstruction(DGRY;n=475),distal gastrectomy with Billroth-I reconstruction(DGBI;n=909),pylorus-preserving gastrectomy(PPG;n=313),and local resection of the stomach(LR;n=85)]were analyzed.Among the 19 main outcome measures of PGSAS-45,the severity and characteristics of postgastrectomy syndrome were compared for the aforementioned six gastrectomy procedures using analysis of means.RESULTS TGRY and PG significantly impaired the QOL of postoperative patients.Postoperative QOL was excellent in LR(cardia and pylorus were preserved with minimal resection). In procedures removing the distal stomach, diarrhea subscale(SS)and dumping SS were less frequent in PPG than in DGBI and DGRY.However, there was no difference in the postoperative QOL between DGBI and DGRY.The most noticeable adverse effects caused by gastrectomy were mealrelated distress SS,dissatisfaction at the meal, and weight loss, with significant differences among the surgical procedures.CONCLUSION Postoperative QOL greatly differed among six gastrectomy procedures. The severity and characteristics of postgastrectomy syndrome should be considered to select gastrectomy procedures, overcome surgical shortcomings, and enhance postoperative care.
文摘Whether people tend to punish criminals in a socially-optimal manner (i.e., hyperbolic punishment) or not is unknown. Implications for neuroeconomics and neurolaw of crime and punishment (Takahashi, 2012, NeuroEndocrinology Letters) are discussed.
文摘Anomalies in decision over time (e.g., “hyperbolic time discounting”) and under risk (e.g., Allais paradox and hyperbolic probability discounting) have been attracting attention in behavioral and neuroeconomics. Implications for psychophysical neuroeconomics and econophysics are discussed.
文摘Previous studies have reported that the mirror neuron system plays a crucial role in social cognition. These results suggest that the degree of the activation of higher-order cognitive functions, which may be engaged in the inhibitory and/or facilitative processing of human body or bodily movement, leads to the distinctive activities in the inferior frontal mirror neuron area.
文摘Following the reform of the governance of irrigation schemes in Niger, producer cooperatives are encountering real difficulties in ensuring effective and efficient management of irrigation water, infrastructure maintenance and renewal of hydromechanical equipment. This process requires a new pedagogical approach aimed at irrigators.
文摘The paper traces back women like Celie, Shug, Sofia or Nettie engaged in journeys of self-discovery and development, ones that follow predicted feminist patterns, who try to get free from any dominance, either be it male, social or cultural. The movement in time encompasses possible similitudes and differences.
文摘The Tunisian territory (area of diapirs) is exposed to the risks of ground movements linked to water, some of which are related to the phenomenon of dissolution of gypsum, allowing the appearance of underground cavities which present natural risks and set people in danger. The analysis of the predisposing factors present in the study area shows that the geodynamic con-text of the Triassic ascent takes place according to a precise thermodynamic process that favors the phenomenon of dissolution of the gypsum.
文摘Protective structures, built in the 1980's in order to reduce or stop coastal erosion in Rufisque, involved a modification of the coastal morpho-sedimentary landscape and have intensified the coastal dynamics. The results also showed that these methods of shoreline protection are not durable measurements (degradation of the structures) and have exacerbated the vulnerability to coastal erosion as well as those of the coastal populations.

文摘BACKGROUND Defecation disorders are obscure sequelae that occurs after gastrectomy, and its implication on daily lives of patients have not been sufficiently investigated.AIM To examine the features of defecation disorders after gastrectomy and to explore its implication on daily lives of patients in a large cohort using the Postgast rectomy Syndrome Assessment Scale(PGSAS)-45.METHODS We conducted a nationwide multi-institutional study using PGSAS-45 to examine the prevalence of postgastrectomy syndrome and its impact on daily lives of patients after various types of gastrectomy.Data were obtained from 2368 eligible patients at 52 institutions in Japan.Of these,1777 patients who underwent total gastrectomy(TG;n=393)or distal gastrectomy(DG;n=1384)were examined. The severity of defecation disorder symptoms, such as diarrhea and constipation, and their correlation with other postgastrectomy symptoms were examined. The importance of defecation disorder symptoms on the living states and quality of life(QOL)of postgastrectomy patients, and those clinical factors that affect the severity of defecation disorder symptoms were evaluated using multiple regression analysis.RESULTS Among seven symptom subscales of PGSAS-45,the ranking of diarrhea was 4th in TG and 2nd in DG.The ranking of constipation was 5th in TG and 1st in DG.The symptoms that correlated well with diarrhea were dumping and indigestion in both TG and DG;while those with constipation were abdominal pain and mealrelated distress in TG,and were meal-related distress and indigestion in DG.Among five main outcome measures(MOMs)of living status domain, constipation significantly impaired four MOMs,while diarrhea had no effect in TG.Both diarrhea and constipation impaired most of five MOMs in DG.Among six MOMs of QOL domain, diarrhea impaired one MOM,whereas constipation impaired all six MOMs in TG.Both diarrhea and constipation equally impaired all MOMs in DG.Male sex, younger age, division of the celiac branch of vagus nerve, and TG,independently worsened diarrhea, while female sex worsened constipation.CONCLUSION Defecation disorder symptoms, particularly constipation, impair the living status and QOL of patients after gastrectomy;therefore,we should pay attention and adequately treat these relatively modest symptoms to improve postoperative QOL.
文摘Specimens of the forest species such as Pentaclethra macrophylla, Petersianthus macrocarpus, Pycnanthus angolensis and Terminalia superba have been sampled from LUKI Biosphere reserve in the Democratic Republic of the Congo in order to determine their wood density with the perspective to decarbonisation. These parameters have been found out experimentally utilizing a drying technique in an oven including techniques of immersion in an Erlenmeyer full of water. The corresponding results indicated that the four species wood density is respectively 0.85, 0.80, 0.77 and 0.51. These preliminary results will be useful in our ongoing project on carbon dioxide absorption capacity of Congo rainforest tree species.
文摘Recently, the COVID-19 emerged in China and propagated around all the world has threatened millions of people and affected most countries and governments at several sides such as economical, educational, tourism, healthcare, etc. Indeed, one of the most important challenges that directly affect the people is the psychological side due to the harsh policies imposed by public authorities in most countries. We show the effectiveness of CRISE in terms of clustering accuracy and understanding the psychological effect of COVID-19.
文摘Climate change poses an important constraint to agricultural sector and food security in tropical African countries dominated by rain-fed agriculture. The average annual temperature and that of January in the transition zone, the rains of January and April in the North-East zone and the relative humidity in the North-West zone also probability determine the yield of the yam.
