Adherence to medical treatment is a multifaceted issue that can substantially alter the outcomes of therapy. Patient non-adherence to chemotherapy is the ultimate barrier to the treatment effectiveness. Objective: This study was carried out to determine the relationship between cancer chemotherapy adherence and breast cancer staging, patient’s perception of cancer care and patient’s socio-demographic characteristics. Material and method: This was a cross sectional study selection of respondents and was based on simple random sampling technique, 184 patients were interviewed and data was collected using a semi-structured questionnaire to obtain socio-demographic data, adherence data, and facility-related information. Results: There was a significant association between marital status and non-adherence (P?= 0.013). Both separated and single subjects had higher proportion of non-adherence compared with married subjects. Analysis of perception of chemotherapy care revealed a significant association between the satisfaction score and non-adherence, with non-adherent patients showing higher scores or being less satisfied. The quality of service (P?= 0.0052);rating of needs been met (P?= 0.0079);rating on whether the services helped the subject (P?= 0.0405);rating on the general satisfaction of the services provided (P?= 0.0115);and rating on whether subject would seek help again (P?= 0.0320) all had a significant association with non-adherence. Conclusion: The awareness of oncologist and patient of the problem of non-adherence and communication regarding the importance of adherence to therapy may improve health outcomes.展开更多
Asymptomatic urinary tract infections (UTIs) of bacterial and parasitic origins occur in both males and females where bacteria and/or parasites are present in the urine, with the absence of clinical signs or symptoms ...Asymptomatic urinary tract infections (UTIs) of bacterial and parasitic origins occur in both males and females where bacteria and/or parasites are present in the urine, with the absence of clinical signs or symptoms in the host. Using microbiological methods, mid-stream urine collected from sixty (60) Redeemer’s University students comprising apparently healthy 30 male and 30 female undergraduate students were examined. The samples were cultured on CLED and MacConkey agar. Bacteriuria was observed in 15 (25%) of the samples while there was no significant growth in 34 (56.7%), and no growth in 11 (18.3%). Prevalence of significant bacteriuria was higher in females 11 (73.3%) than males 4 (26.7%). The bacteria isolated were Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Streptococcus spp, and Klebsiella spp. The isolates were resistant to amoxycillin, augumentin and cotrimoxazole but most were highly susceptible to ofloxacin. Schistosoma haematobium and Trichomonas vaginalis were the two asymptomatic UTI-causing parasites isolated from the samples used in this study. Out of the 30 males examined using the microscopy method, 1 (1.67%) had T. vaginalis while the females had higher prevalence of 2 (3.33%) out of 30 females examined. Out of the overall participants examined for S. haematobium eggs, 15 (25%) had Schistosomiasis in which the males had higher prevalence of 10 (33.33%) while the females had 5 (16.67%). These results are lower than those observed in similar rural communities in Nigeria possibly due to effective health education, availability of potable water, higher-than average income and hygienic practices on Redemption Camp, Ogun State, Nigeria.展开更多

Objective: To compare adherence of private and public providers of directly observed treatment short course (DOTS) in the Lagos State TB control program, Nigeria (LSTBLCP) with the national TB guidelines. Design: A retrospective review of treatment cards of TB patients managed within the first and second quarter of 2012 in 34 DOTS facilities {23 public, 7 private for profit (PFP), and 4 private not for profit (PNFP)} involved in the private public mix of the LSTBLCP. Results: Of the 1896 treatment cards reviewed, 1524 (80.4%), 132 (7.0%) and 240 (12.6%) were from public, PFP and PNFP DOTS facilities, respectively. About 19%, 25% and none of the patients managed at the public, PNFP, and PFP DOTS facilities were treated in full adherence with the national guidelines respectively. A significantly higher proportion of adults and sputum smear positive TB patients were treated in full adherence with the national guidelines (p < 0.05). Treatment success was associated with full adherence with the national guidelines. Conclusion: There is a need to reorient health care providers in public and private health facilities in Lagos State Nigeria to ensure full adherence with the national TB guidelines.展开更多
Federal government of Nigeria,in collaboration with Lagos State Government proposed development of a seaport in Badagry.This research examined and documented the current state of the ecosystem and livelihood of thirte...Federal government of Nigeria,in collaboration with Lagos State Government proposed development of a seaport in Badagry.This research examined and documented the current state of the ecosystem and livelihood of thirteen communities that will be impacted by the proposed deep-sea port.Qualitative and quantitative approaches were used for the study.Review of secondary data was used to investigate the demographic data of the community while Participatory Rural Appraisal was conducted for 300 households in the communities.Majority of the sampled respondents were in the age range of 45 years and above with females(60%)more than males(40%)in the entire population sampled.Educational levels of the respondents are relatively low.The majority of the sampled households have multiple livelihood systems that keep them engaged throughout all seasons of the year.Capture fisheries is the major occupation in the study area and it is complemented with aquaculture.Existing groups are not strong enough to operate as a pressure group to influence policies and regulate market prices,which has been identified as a major limitation in the study area.There was no regular training or capacity building.Hence the groups were not operating as a business enterprise and could not expand or increase capacity.Consequently not able to contribute significantly to poverty alleviation and increase employment opportunities in their localities.This document will serve as one of the guides to the government for decision-making and compensation to the communities.展开更多

The aetiology and pathogenesis are still poorly understood and substantial improvement has not been made in the prediction, prevention and treatment of the disease. Objective: To compare the frequency of activated protein C resistance (APC-R) in patients with pre-eclampsia to that of normotensive pregnant women and to determine the correlation between activated protein ratio (APC-ratio) and the severity of pre-eclampsia. Methodology: A cross-sectional study was carried out in 100 pre-eclamptic patients and 100 normotensive pregnant controls. The APC-ratio was determined using the modified activated partial thromboplastin time. Study participants with APC-ratio of less than 2.0 were defined as having APC-R. Data was analyzed using SPSS version 22.0. Results: Mean APC-ratio was significantly lower in pre-eclamptics (2.89 ± 1.70) compared to normotensive pregnant women (3.57 ± 1.06) (p = 0.0008) and the levels were also higher in mild (2.95 ± 1.15) compared to severe pre-eclamptics (2.62 ± 1.14). The frequency of APC-R was 26% among women with pre-eclampsia compared to 4% among normotensive controls (p = 0.000). Among 100 pre-eclamptic women 7 (21.2%) out of 33 with mild pre–eclampsia had APC-R, while 19 (28.4%) out of 67 with severe pre-eclampsia had APC-R. APC-ratio had a significant negative correlation with mean arterial blood pressure (r = −0.324;p = 0.000) and proteinuria (r = −0.379;p = 0.000) among study participants. Conclusion: The frequency of activated protein c resistance is significantly higher in pre-eclamptics compared to normotensive pregnant women and this is more pronounced in those with severe pre-eclampsia compared with those with mild disease. APC-R may therefore be used as a marker of severity in the disease.展开更多

The obj...Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and Plasmodium species are the causative agents of coronavirus disease 2019 (COVID-19) and malaria respectively with similar clinical presentations. The objective of this study is to determine the burden of co-infection of SARS-CoV-2 and malaria in the general population. Five (5 mLs) of blood samples were collected for SARS-CoV-2 and malaria parasite test. The malaria test was performed using a commercially available one-step malaria antigen Plasmodium falciparum histidine-rich protein 2 (Pf HRP-II) rapid test kit. The results of the study showed that the participants that were coinfected with SARS-CoV-2 IgG and malaria were 13 (2.5%) in Lagos, 1114 (39.1%) in Delta and 49 (2.3%) in Sokoto States. The prevalence of coinfection of SARS-CoV-2 and malaria in urban areas in Lagos, Delta and Sokoto States were 7 (2.2%), 1373 (48.1%), and 5 (0.2%) respectively. In rural areas, the prevalence of coinfection of SARS-CoV-2 and malaria in Lagos, Delta and Sokoto States were 6 (0.3%), 365 (12.8%), and 44 (2.1%) respectively in this study. This suggests that participants in the urban areas were more prone to co-infections than the rural areas in Lagos and Delta states, while it was otherwise in Sokoto State. In conclusion, the co-infection of SARS-CoV-2 and malaria was very high in Delta State compared to the other States. It is important for clinics to screen for both diseases when patients present with symptoms of malaria. This is because the infections have similar symptoms and the public is quick to assume malaria infection without diagnosing for COVID-19 and vice versa.展开更多

The incubation period is usually 6 to 13 days but can range from 5 to 21 days while symptoms and signs may persist f...Mpox disease is caused by a double-stranded DNA virus, genus Orthopoxvirus of the family Poxviridae. The incubation period is usually 6 to 13 days but can range from 5 to 21 days while symptoms and signs may persist for 2 to 5 weeks. Although, the clinical features are usually less severe when compared to the deadly smallpox, the disease can be fatal with case fatality rate between 1% and 10%. In Imo State, Nigeria, there has been a changing epidemiology of the disease in the last 6 years and the frequency and geographic distribution of cases have progressively increased. This study aims to conduct a review of the disease epidemiology between 2017 and 2023 and implications for surveillance in Imo State. Surveillance data from the Surveillance Outbreak Response and Management System (SORMAS) was extracted between January 2017 and December 2023 across the 27 Local Government Areas (LGAs) of Imo State. A line list of 231 suspected cases was downloaded into an excel template and analyzed using SPSS<sup>®</sup> version 20 software. Analysis was done using descriptive statistics and associations were tested using Fischer’s exact at 0.05 level of significance. Of the 231 suspected cases, 57.1% (132) were males, 42.9% (99) were females and the modal age group was between the ages of 0 - 4 (32.5%). Eight (8) LGAs (districts) accounted for 71% (n = 164) of all the suspected cases. 21.2% (49) were confirmed positive, 27 males (55.1%) and 22 females (44.9%) (p > 0.05). Modal age group was 20 - 24 (22.4%, n = 11), 18% (9) were children under 14 years, p > 0.05. Case fatality rate was 8% (n = 4). There was no significant association between mortality and age group. Five (5) LGAs accounted for about 60% (29) of all confirmed cases. These LGAs contribute only 20% to the total population in the State. Only 5.6% and 4% of suspected and confirmed cases, respectively, had knowledge of contact with an infectious source. The study described the epidemiology of Mpox outbreaks between 2017 and 2023 and the findings have significant implications on detection and outbreak response activities.展开更多

The emergence of multidrug resistant (MDR) Salmonella has become a serious problem globally. This study investigated the antibiotic resistance and plasmid profiles of Salmonella isolates from different sources. Methods: Seventy-three samples comprised of clinical (30), hand swab (15), food (10) and water (18) were analyzed bacteriologically. Salmonella isolates were identified and characterized by standard procedures. Isolates were subjected to antimicrobial susceptibility testing and were further screened for plasmid DNA by standard methods. Results: A total of 27 Salmonella isolates made up of 5 (18.5%) S. typhi, 6 (22.2%) S. enteritidis, 9 (33.3) S. typhimurium, 5 (18.5%) S. cholerasuis, and 1 (3.7%) each of S.arizonae and S. vichow were obtained in this study. All the isolates developed resistance to three or more antibiotics evaluated. Four distinct resistance profiles: TetAmpCol, TetAmpColCot, TetAmpColCip and TetAmpColCotCip were recorded with 63% of the isolates exhibiting resistance profile TetAmpColCot. Specifically 23 of 27 (85.2%) of the isolates harboured plasmid DNA comprised of 12 distinct plasmid profiles of different sizes ranging from 3.2 kb to 30.2kb. Salmonella isolates of the same species from different sources differed in plasmid profile. Plasmid profile was found to show good discriminatory capability compared to antibiotics resistance profile. Conclusion: This study revealed that both resistance antibiogram and plasmid profile are still viable epidemiological tools for tracing the source of Salmonella isolates. A need for prudent use of antibiotics is suggested.展开更多

Study Design: This was a pros...Objectives: To determine the seropositivity of Chlamydia antibody in patients with ruptured ectopic pregnancy compared to normal pregnant women and the risk factors for ectopic pregnancy. Study Design: This was a prospective case-control study of 85 cases of ruptured ectopic pregnancy and 100 cases of second trimester on-going intrauterine pregnant controls presenting in Lagos State University Teaching Hospital (LASUTH) between September 2009 and March 2010. Study Site: This was at the gynaecological emergency room and antenatal clinic in the Department of Obstetrics and Gynaecology. Ethical approval was sought and granted by the ethics review committee of LASUTH. Study Participants: Patients presenting with ruptured ectopic pregnancy were recruited as cases while the controls were made up of those with uncomplicated second trimester intrauterine pregnancy. A semi-structured questionnaire containing socio-demographic and clinical characteristics was administered following informed consent. Five milliliters of venous blood was taken from each participant and tested for?Lymphogranuloma Venerum?(LGV) type 2 broadly reacting antigen of?Chlamydia trachomatis.?Data Analysis: Data gathered from the case notes and laboratories were imputed into the computer and analyzed using the statistical package?Epi-Info 3.51, Atlanta, USA. Frequency tables were generated for continuous variables and?chi-square analysis used to determine association between variables, with p values <0.05 considered statistically significant. Results: There were 91 cases of ectopic pregnancy among a total of 2468 deliveries giving an incidence of 3.68% or 1 in 27 deliveries. Factors which significantly contributed to increased incidence of ectopic pregnancy in this study were: level of education (p = 0.001), socio-economic status (p = 0.001), parity (p = 0.005), early age of sexual debut (p = 0.001), multiple sexual partners (p = 0.001), previous pelvic inflammatory disease (p = 0.003), previous induced abortion (p = 0.013) and previous?postabortal/puerperal sepsis (p = 0.013). The seropositivity of?Chlamydia IgG (62.4%) in the cases was significantly higher than that of 29% in the control (p < 0.0001). Conclusion: There was a high incidence of ectopic during the period of study and the seropositivity of Chlamydia IgG antibody was significantly higher amongst the cases. Risk factors identified were low level of education, low socio-economic status, low parity, early age of sexual debut, multiple sexual partners, previous history of pelvic inflammatory disease, previous induced abortion and previous postabortal/puerperal sepsis.展开更多

Methods: A descriptive cross-sectional study was conducted. Four hundred and seventy new smear positive TB patients above 14 years of age were consecutively recruited between October 1 and December 31 2012 from 34 (23 public and 11 private) directly observed treatment short course (DOTS) facilities that offered TB treatment and microscopy services. They were followed up till treatment was completed. Logistic regression was used to assess the predictors of treatment interruption. Results: A significantly higher proportion of smokers (58.6% vs 38.3%, p = 0.030), patients supervised by treatment supporters (44.4% vs 34.7%, p = 0.032), patients not counselled before initiation of treatment (55.6% vs 38.2%, p = 0.041), patients managed at private DOTS facilities (50% vs 36.3%, p = 0.010) and TB/HIV co-infected patients (54.2% vs 38.6%, p = 0.038) had treatment interruption. Predictors of treatment interruption were supervision by treatment supporters, smoking, lack of pre-treatment counselling and TB/HIV co-infection. Conclusion: A higher proportion of patients supervised by treatment supporters had treatment interruption than those supervised by health care workers. There may be a need to review the concept of treatment supervision by treatment supporters in Lagos state Nigeria.展开更多
Background and Aims: Food is of paramount importance to the sustenance of human health, on that basis this study was initiated with a view to determining bacterial agents associated with the vended foods and to invest...Background and Aims: Food is of paramount importance to the sustenance of human health, on that basis this study was initiated with a view to determining bacterial agents associated with the vended foods and to investigating antibacterial resistance of the isolates. Methods: Twelve food samples from vendors were randomly collected from ten locations within Lagos environs. The food samples were analyzed bacteriologically by standard methods. The isolates were identified and subjected to antimicrobial susceptibility testing using standard procedures. Results: The total plate counts of bacteria recorded was between 2.98 × 103 cfu/g in Jollofrice and 4.09 × 104 cfu/g in Salad. Seventy-six bacterial isolates made up of six genera;Escherichia coli, Salmonella Typhimurium, Alcaligenes spp., Klebsiella pneumoniae, Enterobacter spp., Serratia marcensens and Proteus spp. were identified. Most of the bacterial isolates were moderately sensitive to cotrimoxazole, nalidixic acid, amoxicillin and nitrofurantion except in S. Typhimurium with less than 50% susceptibility. Also over 60% of the pathogens isolated were inhibited by ceftazidime, cefpodoxime, and levofloxacin antibiotics and most strains of E. coli, S. Typhimurium and K. pneumoniae were resistant to ofloxacin, ciprofloxacin and ceftriazone. Conclusion: The study revealed gross contamination of various vended foods. It also indicated that vended foods are potential reservoir for the spread of emerging multiple drug resistant bacteria strains that pose threat to public health. There is a need for intensive surveillance to avoid possible outbreak of food- borne diseases resulting from consumption of contaminated vended foods.展开更多
The present study is on the haematological and hepatic responses of the African catfish fingerlings Clarias gariepinus to sublethal toxicity of industrial effluents(IE)from the environment of Ologe Lagoon,Lagos,Nigeri...The present study is on the haematological and hepatic responses of the African catfish fingerlings Clarias gariepinus to sublethal toxicity of industrial effluents(IE)from the environment of Ologe Lagoon,Lagos,Nigeria.The fish were cultured in five concentrations of industrial effluents:0%(control),5%,15%,25%,and 35%.Trials were carried out in triplicates for twelve(12)weeks.The culture system was a static renewable bioassay and was carried out in the fisheries laboratory of the Lagos State University,Ojo-Lagos.Weekly physico-chemical parameters:Temperature(°C),pH,conductivity(ppm)and dissolved oxygen(DO in mg/L)were measured in each treatment tank.Haematological parameters:packed cell volume(PCV),red blood cells(RBC),white blood cell(WBC),neutrophil and lymphocytes etc.,and hepatological alterations were measured after 12 weeks.The physico-chemical parameters showed that the pH ranged from 7.82±0.25~8.07±0.02.DO ranged from 1.92±0.66~4.43±1.24 mg/L.The conductivity values increased with increase in concentration of I.E.While the temperature difference remained insignificant with mean value range between 26.08±2.14~26.38±2.28.The DO showed significant differences at p<0.05.Though survival was 100%during the sublethal study,haematological results showed that C.gariepinus had PCV ranging from 13.0±1.7~27.7±0.6,RBC ranged from 4.7±0.6~9.1±0.1,and neutrophil ranged from 26.7±4.6~61.0±1.0 amongst others.The highest values of these parameters were obtained in the control and lowest at 35%.While the reverse effects were observed for WBC and lymphocytes,the liver shows normal liver cells in the control(0%),but at higher toxic levels,there were:vacoulation,destruction of the hepatic parenchyma,tissue becoming eosinophilic(i.e.tending towards Carcinogenicity)and severe disruption of the hepatic cord architecture.This study therefore shows that disposal of effluents into the aquatic environment affects the health of fishes by impairing normal hepatic functions and hindering vital physiological processes if exposure continues for a long period of time(sublethal effect).展开更多

Asymptomatic bacteriuria is associated with an increased risk of symptomatic urinary tract infection and the latter being an important contributor to development of chronic renal failure, hypertension and toxaemia of pregnancy. The aim of this study was to determine the prevalence of asymptomatic bacteriuria in HIV-infected patients and proffer a recommendation on the need or otherwise to screen. Methods: This was a cross sectional study of treatment-naive HIV-infected patients attending the HIV clinics of Lagos State University Teaching Hospital (LASUTH), Ikeja. A single voided aseptically collected mid-stream urine (MSU) was obtained from each patient and all samples processed immediately, were sent for urinalysis and culture. Isolates were considered significant if there were ≥105 colony forming unit/mL (CFU/mL) with 2 or less isolates, doubtful significance if 5 CFU/mL. Significant isolates were selected for identification. Data were analyzed using SPSS version 16.0 (Statistical Package for Social Sciences, Inc., Chicago, Ill). Results: A total of 156 consenting participants were recruited into the study. The mean age was 36.45 ± 8.65 years. There were 60 of 156 (38.4%) males and 96 of 156 (61.56%) females. Only 33 of 156 (21.2%) had significant bacteriuria, out of the 33 participants, 19 (57.8%) were asymptomatic, while 14 of 33 (42.4%) had significant growth but were symptomatic or on antibiotics. E. coli was isolated in 9 of 19 (47.3%), followed by Staph aureus 4 of 19 (21.05%). Conclusion: More than half of participants who had significant growth had asymptomatic bacteriuria, while one-fifth of all patients had significant growth. Considering this statistics, screening for or treatment of asymptomatic bacteriuria may be recommended in all HIV-infected patients.展开更多

Only 29% of those who had unprotected sexual intercourse used emergency contraceptives. Lack of knowledge and promotion of sexual promiscuity were identified as the main reasons for not using emergency contraceptives. Previous use of contraceptives, married status, increasing age and year of study were positive predictors for the use of emergency contraceptives while poor knowledge was a significant predictor of non-use. </span><b><span style="font-family:Verdana;">Conclusion: </span></b><span style="font-family:Verdana;">There was poor knowledge and low utilization of emergency contraceptives among respondents. Information on contraceptives should be introduced in secondary schools and in general studies courses in tertiary institutions while parents and caregivers should discuss issues relating to sex and contraceptives with adolescents.</span></span></span></span>展开更多

Safety, delay and predictability metrics contribute to the analytical framework. From the findings, the occurrence of air incidence has a high severity level at departure, and arrival phases of flight, higher likelihood at the radar room and much of the incidences were as a result of faulty equipment and inherent absence of modern airspace infrastructure. Also, in Lagos terminal airspace, the number of incidences has no close correlation with the level of traffic complexity. Total schedule arrival delay ranges from 1 - 392 minutes representing an average of 7.8 - 17.9 minutes per aircraft that arrived Lagos airport at that period. The causation of suicidality is multi-dimensional, however, there are insufficient studies which have examined the burden and risk factors of suicidality in Nigerian police officers. Objective: To determine the prevalence and predictors of suicidality among police officers in Lagos, Nigeria. Methods: This was a cross-sectional study of 600 police officers who were selected using a non-probability sampling method. Questionnaires used were: Suicide Behaviour Questionnaire-Revised (SBQ-R), State-Trait Anxiety Inventory (STAI), and a Socio-Demographic Questionnaire. Results: Prevalence of suicidality was 14%, and the predictors of suicidality were: state anxiety (p 0.001), trait anxiety (p 0.001), substance use (p = 0.03), being unmarried (p = 0.03), and female gender (p = 0.02). Conclusion: Suicidality is prevalent among Nigerian police officers, with a higher risk in those who are vulnerable. The creation of government policies and infrastructure which promote mental health in police officers is necessary.展开更多
A study on seasonal abundance,morphometric and meristic data,growth pattern,condition fector,sex ratio and gonadosomatic index of Liza falcip-innis(Wlenciennes,1836)from the Ojo axis of Badagry creek,Nigeria was condu...A study on seasonal abundance,morphometric and meristic data,growth pattern,condition fector,sex ratio and gonadosomatic index of Liza falcip-innis(Wlenciennes,1836)from the Ojo axis of Badagry creek,Nigeria was conducted from May 2019 to March 2020.A total of 1012 species were randomly selected,having 499 females and 513 males.The length frequency analysis and length-weight relationships(LWR)were determined.Sex ratio was determined by Chi-square analysis.The results showed that morphometric data are:0.5-2.5 mm for ED,2.1-12 mm for HL,1.7-8.1 mm for HD,2.5-11.7 for BD,2.6-233.3 mm for TL and 9.23-1006 g for BW for the combined sexes.The slope(b)shows an allometric growth pattern.The intercept'a'and slope'b'of the LWR(LogW=a+bLogL)were Log W=15.39+0.34 LogL(r=0.54)for combined sexes,Log W=12.49+0.02 log L(r=0.38)for males and Log W=18.23+0.01 log L(r=0.16)for females.The length frequency distribution indicated that species were dominated by two year classes(Ages 1 and 2).Condition factors were generally low.The values ranged between 0.68-0.85 for combined sexes.The gonadosomatic index for female was highest in August,2019(17.77%)with Mean±SD of 2.88±0.75;which indicated the peak of spawning period in the study area.Sex ratio difference was significant(P<0.05).Sexual differences were significant;the females are phenotypically larger than the male.展开更多
Prostate cancer(PCa)symptoms are commonly confused with benign prostate hyperplasia(BPH),particularly in the early stages due to similarities between symptoms,and in some instances,underdiagnoses.Clinical methods have...Prostate cancer(PCa)symptoms are commonly confused with benign prostate hyperplasia(BPH),particularly in the early stages due to similarities between symptoms,and in some instances,underdiagnoses.Clinical methods have been utilized to diagnose PCa;however,at the full-blown stage,clinical methods usually present high risks of complicated side effects.Therefore,we proposed the use of support vector machine for early differential diagnosis of PCa(SVM-PCa-EDD).SVM was used to classify persons with and without PCa.We used the PCa dataset from the Kaggle Healthcare repository to develop and validate SVM model for classification.The PCa dataset consisted of 250 features and one class of features.Attributes considered in this study were age,body mass index(BMI),race,family history,obesity,trouble urinating,urine stream force,blood in semen,bone pain,and erectile dysfunction.The SVM-PCa-EDD was used for preprocessing the PCa dataset,specifically dealing with class imbalance,and for dimensionality reduction.After eliminating class imbalance,the area under the receiver operating characteristic(ROC)curve(AUC)of the logistic regression(LR)model trained with the downsampled dataset was 58.4%,whereas that of the AUC-ROC of LR trained with the class imbalance dataset was 54.3%.The SVM-PCa-EDD achieved 90%accuracy,80%sensitivity,and 80%specificity.The validation of SVM-PCa-EDD using random forest and LR showed that SVM-PCa-EDD performed better in early differential diagnosis of PCa.The proposed model can assist medical experts in early diagnosis of PCa,particularly in resource-constrained healthcare settings and making further recommendations for PCa testing and treatment.展开更多

This investigation involved data validation analysis, evaluation of site suitability for CO_(2) storage, and compositional simulation of hydrocarbon components. The primary objective was to determine the initial components and behavior of the hydrocarbon system required to optimize the injection of CO_(2) and accompanying impurities, establishing a robust basis for subsequent sequestration efforts in the six wells in the depleted KOKA AA-01 reservoir. The process, simulated using industry software such as ECLIPSE, PVTi, SCAL, and Petrel, included a compositional fluid analysis to confirm the pressure volume temperature(PVT) hydrocarbon phases and components. This involved performing a material balance on the quality of the measured data and matching the initial reservoir pressure with the supplied data source. The compositional PVT analysis adopted the Peng–Robinson equation of state to model fluid flow in porous media and estimate the necessary number of phases and components to describe the system accurately. Results from this investigation indicate that the KOKA AA-01 reservoir is suitable for CO_(2)sequestration. This conclusion is based on the reservoir's good quality, evidenced by an average porosity of 0.21 and permeability of 1 111.0 mD, a measured lithological depth of 9 300 ft, and characteristic reservoir – seal properties correlated from well logs. The study confirmed that volumetric behavior predictions are directly linked to compositional behavior predictions, which are essential during reservoir initialization and data quality checks. Additionally, it highlighted that a safe design for CO_(2) storage relies on accurately representing multiphase behaviour across wide-ranging pressure–temperature–composition conditions.展开更多
Background: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic breast cancer and to examine the relationship between received dose intensity (RDI) and survival in pati...Background: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic breast cancer and to examine the relationship between received dose intensity (RDI) and survival in patients with breast cancer Nigeria. Methods: Our study was a retrospective analysis of patients with breast cancer recruited from 2012 and 2015. A total of 204 patients were initially entered into the study, 102 were lost to follow-up leaving 102 patients who were suitable for the survival analysis. Survival time was calculated from 106 days, the scheduled end of chemotherapy. Results: The total average RDI for patients was 74%. Over the 204 patients that were reviewed, 144 (70.6%) had some reduction of RDI. This subgroup had an average RDI of 63%. On average, 79% of the intended dose of chemotherapy was given. The time to completion of chemotherapy was 1.33 times that specified by the protocol. Dose delays an overall reduction was mainly attributed to intolerability and financial constraints. Survival by RDI showed a significant decrease in survival rate for patients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, P = 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, P = 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, P = 0.003) compared with patients who received an RDI of 100%. Although associated with poorer prognosis, there were no significant changes in the survival rate for patients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, P = 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, P = 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, P = 0.405) compared with patients who received an RDI of 100%. Conclusion: This study provides evidence that decreased RDI of <70% in non-metastatic breast cancer patients is strongly associated with decreased overall survival.展开更多
文摘Background: Breast cancer is one of the most common malignant diseases in women and adjuvant combination chemotherapy has been shown to reduce mortality from this disease. Adherence to medical treatment is a multifaceted issue that can substantially alter the outcomes of therapy. Patient non-adherence to chemotherapy is the ultimate barrier to the treatment effectiveness. Objective: This study was carried out to determine the relationship between cancer chemotherapy adherence and breast cancer staging, patient’s perception of cancer care and patient’s socio-demographic characteristics. Material and method: This was a cross sectional study selection of respondents and was based on simple random sampling technique, 184 patients were interviewed and data was collected using a semi-structured questionnaire to obtain socio-demographic data, adherence data, and facility-related information. Results: There was a significant association between marital status and non-adherence (P?= 0.013). Both separated and single subjects had higher proportion of non-adherence compared with married subjects. Analysis of perception of chemotherapy care revealed a significant association between the satisfaction score and non-adherence, with non-adherent patients showing higher scores or being less satisfied. The quality of service (P?= 0.0052);rating of needs been met (P?= 0.0079);rating on whether the services helped the subject (P?= 0.0405);rating on the general satisfaction of the services provided (P?= 0.0115);and rating on whether subject would seek help again (P?= 0.0320) all had a significant association with non-adherence. Conclusion: The awareness of oncologist and patient of the problem of non-adherence and communication regarding the importance of adherence to therapy may improve health outcomes.
文摘Asymptomatic urinary tract infections (UTIs) of bacterial and parasitic origins occur in both males and females where bacteria and/or parasites are present in the urine, with the absence of clinical signs or symptoms in the host. Using microbiological methods, mid-stream urine collected from sixty (60) Redeemer’s University students comprising apparently healthy 30 male and 30 female undergraduate students were examined. The samples were cultured on CLED and MacConkey agar. Bacteriuria was observed in 15 (25%) of the samples while there was no significant growth in 34 (56.7%), and no growth in 11 (18.3%). Prevalence of significant bacteriuria was higher in females 11 (73.3%) than males 4 (26.7%). The bacteria isolated were Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Streptococcus spp, and Klebsiella spp. The isolates were resistant to amoxycillin, augumentin and cotrimoxazole but most were highly susceptible to ofloxacin. Schistosoma haematobium and Trichomonas vaginalis were the two asymptomatic UTI-causing parasites isolated from the samples used in this study. Out of the 30 males examined using the microscopy method, 1 (1.67%) had T. vaginalis while the females had higher prevalence of 2 (3.33%) out of 30 females examined. Out of the overall participants examined for S. haematobium eggs, 15 (25%) had Schistosomiasis in which the males had higher prevalence of 10 (33.33%) while the females had 5 (16.67%). These results are lower than those observed in similar rural communities in Nigeria possibly due to effective health education, availability of potable water, higher-than average income and hygienic practices on Redemption Camp, Ogun State, Nigeria.
文摘Setting: Private and public tuberculosis (TB) treatment centers in Lagos State, Nigeria. Objective: To compare adherence of private and public providers of directly observed treatment short course (DOTS) in the Lagos State TB control program, Nigeria (LSTBLCP) with the national TB guidelines. Design: A retrospective review of treatment cards of TB patients managed within the first and second quarter of 2012 in 34 DOTS facilities {23 public, 7 private for profit (PFP), and 4 private not for profit (PNFP)} involved in the private public mix of the LSTBLCP. Results: Of the 1896 treatment cards reviewed, 1524 (80.4%), 132 (7.0%) and 240 (12.6%) were from public, PFP and PNFP DOTS facilities, respectively. About 19%, 25% and none of the patients managed at the public, PNFP, and PFP DOTS facilities were treated in full adherence with the national guidelines respectively. A significantly higher proportion of adults and sputum smear positive TB patients were treated in full adherence with the national guidelines (p < 0.05). Treatment success was associated with full adherence with the national guidelines. Conclusion: There is a need to reorient health care providers in public and private health facilities in Lagos State Nigeria to ensure full adherence with the national TB guidelines.

文摘Federal government of Nigeria,in collaboration with Lagos State Government proposed development of a seaport in Badagry.This research examined and documented the current state of the ecosystem and livelihood of thirteen communities that will be impacted by the proposed deep-sea port.Qualitative and quantitative approaches were used for the study.Review of secondary data was used to investigate the demographic data of the community while Participatory Rural Appraisal was conducted for 300 households in the communities.Majority of the sampled respondents were in the age range of 45 years and above with females(60%)more than males(40%)in the entire population sampled.Educational levels of the respondents are relatively low.The majority of the sampled households have multiple livelihood systems that keep them engaged throughout all seasons of the year.Capture fisheries is the major occupation in the study area and it is complemented with aquaculture.Existing groups are not strong enough to operate as a pressure group to influence policies and regulate market prices,which has been identified as a major limitation in the study area.There was no regular training or capacity building.Hence the groups were not operating as a business enterprise and could not expand or increase capacity.Consequently not able to contribute significantly to poverty alleviation and increase employment opportunities in their localities.This document will serve as one of the guides to the government for decision-making and compensation to the communities.
文摘Background: Preeclampsia is reported to complicate 2% - 8% of pregnancies globally and is an important cause of maternal and perinatal morbidity and mortality. The aetiology and pathogenesis are still poorly understood and substantial improvement has not been made in the prediction, prevention and treatment of the disease. Objective: To compare the frequency of activated protein C resistance (APC-R) in patients with pre-eclampsia to that of normotensive pregnant women and to determine the correlation between activated protein ratio (APC-ratio) and the severity of pre-eclampsia. Methodology: A cross-sectional study was carried out in 100 pre-eclamptic patients and 100 normotensive pregnant controls. The APC-ratio was determined using the modified activated partial thromboplastin time. Study participants with APC-ratio of less than 2.0 were defined as having APC-R. Data was analyzed using SPSS version 22.0. Results: Mean APC-ratio was significantly lower in pre-eclamptics (2.89 ± 1.70) compared to normotensive pregnant women (3.57 ± 1.06) (p = 0.0008) and the levels were also higher in mild (2.95 ± 1.15) compared to severe pre-eclamptics (2.62 ± 1.14). The frequency of APC-R was 26% among women with pre-eclampsia compared to 4% among normotensive controls (p = 0.000). Among 100 pre-eclamptic women 7 (21.2%) out of 33 with mild pre–eclampsia had APC-R, while 19 (28.4%) out of 67 with severe pre-eclampsia had APC-R. APC-ratio had a significant negative correlation with mean arterial blood pressure (r = −0.324;p = 0.000) and proteinuria (r = −0.379;p = 0.000) among study participants. Conclusion: The frequency of activated protein c resistance is significantly higher in pre-eclamptics compared to normotensive pregnant women and this is more pronounced in those with severe pre-eclampsia compared with those with mild disease. APC-R may therefore be used as a marker of severity in the disease.
文摘Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and Plasmodium species are the causative agents of coronavirus disease 2019 (COVID-19) and malaria respectively with similar clinical presentations. The objective of this study is to determine the burden of co-infection of SARS-CoV-2 and malaria in the general population. Five (5 mLs) of blood samples were collected for SARS-CoV-2 and malaria parasite test. The malaria test was performed using a commercially available one-step malaria antigen Plasmodium falciparum histidine-rich protein 2 (Pf HRP-II) rapid test kit. The results of the study showed that the participants that were coinfected with SARS-CoV-2 IgG and malaria were 13 (2.5%) in Lagos, 1114 (39.1%) in Delta and 49 (2.3%) in Sokoto States. The prevalence of coinfection of SARS-CoV-2 and malaria in urban areas in Lagos, Delta and Sokoto States were 7 (2.2%), 1373 (48.1%), and 5 (0.2%) respectively. In rural areas, the prevalence of coinfection of SARS-CoV-2 and malaria in Lagos, Delta and Sokoto States were 6 (0.3%), 365 (12.8%), and 44 (2.1%) respectively in this study. This suggests that participants in the urban areas were more prone to co-infections than the rural areas in Lagos and Delta states, while it was otherwise in Sokoto State. In conclusion, the co-infection of SARS-CoV-2 and malaria was very high in Delta State compared to the other States. It is important for clinics to screen for both diseases when patients present with symptoms of malaria. This is because the infections have similar symptoms and the public is quick to assume malaria infection without diagnosing for COVID-19 and vice versa.
文摘Mpox disease is caused by a double-stranded DNA virus, genus Orthopoxvirus of the family Poxviridae. The incubation period is usually 6 to 13 days but can range from 5 to 21 days while symptoms and signs may persist for 2 to 5 weeks. Although, the clinical features are usually less severe when compared to the deadly smallpox, the disease can be fatal with case fatality rate between 1% and 10%. In Imo State, Nigeria, there has been a changing epidemiology of the disease in the last 6 years and the frequency and geographic distribution of cases have progressively increased. This study aims to conduct a review of the disease epidemiology between 2017 and 2023 and implications for surveillance in Imo State. Surveillance data from the Surveillance Outbreak Response and Management System (SORMAS) was extracted between January 2017 and December 2023 across the 27 Local Government Areas (LGAs) of Imo State. A line list of 231 suspected cases was downloaded into an excel template and analyzed using SPSS<sup>®</sup> version 20 software. Analysis was done using descriptive statistics and associations were tested using Fischer’s exact at 0.05 level of significance. Of the 231 suspected cases, 57.1% (132) were males, 42.9% (99) were females and the modal age group was between the ages of 0 - 4 (32.5%). Eight (8) LGAs (districts) accounted for 71% (n = 164) of all the suspected cases. 21.2% (49) were confirmed positive, 27 males (55.1%) and 22 females (44.9%) (p > 0.05). Modal age group was 20 - 24 (22.4%, n = 11), 18% (9) were children under 14 years, p > 0.05. Case fatality rate was 8% (n = 4). There was no significant association between mortality and age group. Five (5) LGAs accounted for about 60% (29) of all confirmed cases. These LGAs contribute only 20% to the total population in the State. Only 5.6% and 4% of suspected and confirmed cases, respectively, had knowledge of contact with an infectious source. The study described the epidemiology of Mpox outbreaks between 2017 and 2023 and the findings have significant implications on detection and outbreak response activities.
文摘Introduction: Salmonella enterica Serovars remains one of the leading pathogens that cause diarrhoea and bloodstream infections in developing countries. The emergence of multidrug resistant (MDR) Salmonella has become a serious problem globally. This study investigated the antibiotic resistance and plasmid profiles of Salmonella isolates from different sources. Methods: Seventy-three samples comprised of clinical (30), hand swab (15), food (10) and water (18) were analyzed bacteriologically. Salmonella isolates were identified and characterized by standard procedures. Isolates were subjected to antimicrobial susceptibility testing and were further screened for plasmid DNA by standard methods. Results: A total of 27 Salmonella isolates made up of 5 (18.5%) S. typhi, 6 (22.2%) S. enteritidis, 9 (33.3) S. typhimurium, 5 (18.5%) S. cholerasuis, and 1 (3.7%) each of S.arizonae and S. vichow were obtained in this study. All the isolates developed resistance to three or more antibiotics evaluated. Four distinct resistance profiles: TetAmpCol, TetAmpColCot, TetAmpColCip and TetAmpColCotCip were recorded with 63% of the isolates exhibiting resistance profile TetAmpColCot. Specifically 23 of 27 (85.2%) of the isolates harboured plasmid DNA comprised of 12 distinct plasmid profiles of different sizes ranging from 3.2 kb to 30.2kb. Salmonella isolates of the same species from different sources differed in plasmid profile. Plasmid profile was found to show good discriminatory capability compared to antibiotics resistance profile. Conclusion: This study revealed that both resistance antibiogram and plasmid profile are still viable epidemiological tools for tracing the source of Salmonella isolates. A need for prudent use of antibiotics is suggested.
文摘Objectives: To determine the seropositivity of Chlamydia antibody in patients with ruptured ectopic pregnancy compared to normal pregnant women and the risk factors for ectopic pregnancy. Study Design: This was a prospective case-control study of 85 cases of ruptured ectopic pregnancy and 100 cases of second trimester on-going intrauterine pregnant controls presenting in Lagos State University Teaching Hospital (LASUTH) between September 2009 and March 2010. Study Site: This was at the gynaecological emergency room and antenatal clinic in the Department of Obstetrics and Gynaecology. Ethical approval was sought and granted by the ethics review committee of LASUTH. Study Participants: Patients presenting with ruptured ectopic pregnancy were recruited as cases while the controls were made up of those with uncomplicated second trimester intrauterine pregnancy. A semi-structured questionnaire containing socio-demographic and clinical characteristics was administered following informed consent. Five milliliters of venous blood was taken from each participant and tested for?Lymphogranuloma Venerum?(LGV) type 2 broadly reacting antigen of?Chlamydia trachomatis.?Data Analysis: Data gathered from the case notes and laboratories were imputed into the computer and analyzed using the statistical package?Epi-Info 3.51, Atlanta, USA. Frequency tables were generated for continuous variables and?chi-square analysis used to determine association between variables, with p values <0.05 considered statistically significant. Results: There were 91 cases of ectopic pregnancy among a total of 2468 deliveries giving an incidence of 3.68% or 1 in 27 deliveries. Factors which significantly contributed to increased incidence of ectopic pregnancy in this study were: level of education (p = 0.001), socio-economic status (p = 0.001), parity (p = 0.005), early age of sexual debut (p = 0.001), multiple sexual partners (p = 0.001), previous pelvic inflammatory disease (p = 0.003), previous induced abortion (p = 0.013) and previous?postabortal/puerperal sepsis (p = 0.013). The seropositivity of?Chlamydia IgG (62.4%) in the cases was significantly higher than that of 29% in the control (p < 0.0001). Conclusion: There was a high incidence of ectopic during the period of study and the seropositivity of Chlamydia IgG antibody was significantly higher amongst the cases. Risk factors identified were low level of education, low socio-economic status, low parity, early age of sexual debut, multiple sexual partners, previous history of pelvic inflammatory disease, previous induced abortion and previous postabortal/puerperal sepsis.
文摘Background: This study assessed treatment interruption of tuberculosis (TB) patients managed by treatment supporters and health care workers and other predictors of treatment interruption. Methods: A descriptive cross-sectional study was conducted. Four hundred and seventy new smear positive TB patients above 14 years of age were consecutively recruited between October 1 and December 31 2012 from 34 (23 public and 11 private) directly observed treatment short course (DOTS) facilities that offered TB treatment and microscopy services. They were followed up till treatment was completed. Logistic regression was used to assess the predictors of treatment interruption. Results: A significantly higher proportion of smokers (58.6% vs 38.3%, p = 0.030), patients supervised by treatment supporters (44.4% vs 34.7%, p = 0.032), patients not counselled before initiation of treatment (55.6% vs 38.2%, p = 0.041), patients managed at private DOTS facilities (50% vs 36.3%, p = 0.010) and TB/HIV co-infected patients (54.2% vs 38.6%, p = 0.038) had treatment interruption. Predictors of treatment interruption were supervision by treatment supporters, smoking, lack of pre-treatment counselling and TB/HIV co-infection. Conclusion: A higher proportion of patients supervised by treatment supporters had treatment interruption than those supervised by health care workers. There may be a need to review the concept of treatment supervision by treatment supporters in Lagos state Nigeria.
文摘Background and Aims: Food is of paramount importance to the sustenance of human health, on that basis this study was initiated with a view to determining bacterial agents associated with the vended foods and to investigating antibacterial resistance of the isolates. Methods: Twelve food samples from vendors were randomly collected from ten locations within Lagos environs. The food samples were analyzed bacteriologically by standard methods. The isolates were identified and subjected to antimicrobial susceptibility testing using standard procedures. Results: The total plate counts of bacteria recorded was between 2.98 × 103 cfu/g in Jollofrice and 4.09 × 104 cfu/g in Salad. Seventy-six bacterial isolates made up of six genera;Escherichia coli, Salmonella Typhimurium, Alcaligenes spp., Klebsiella pneumoniae, Enterobacter spp., Serratia marcensens and Proteus spp. were identified. Most of the bacterial isolates were moderately sensitive to cotrimoxazole, nalidixic acid, amoxicillin and nitrofurantion except in S. Typhimurium with less than 50% susceptibility. Also over 60% of the pathogens isolated were inhibited by ceftazidime, cefpodoxime, and levofloxacin antibiotics and most strains of E. coli, S. Typhimurium and K. pneumoniae were resistant to ofloxacin, ciprofloxacin and ceftriazone. Conclusion: The study revealed gross contamination of various vended foods. It also indicated that vended foods are potential reservoir for the spread of emerging multiple drug resistant bacteria strains that pose threat to public health. There is a need for intensive surveillance to avoid possible outbreak of food- borne diseases resulting from consumption of contaminated vended foods.
文摘The present study is on the haematological and hepatic responses of the African catfish fingerlings Clarias gariepinus to sublethal toxicity of industrial effluents(IE)from the environment of Ologe Lagoon,Lagos,Nigeria.The fish were cultured in five concentrations of industrial effluents:0%(control),5%,15%,25%,and 35%.Trials were carried out in triplicates for twelve(12)weeks.The culture system was a static renewable bioassay and was carried out in the fisheries laboratory of the Lagos State University,Ojo-Lagos.Weekly physico-chemical parameters:Temperature(°C),pH,conductivity(ppm)and dissolved oxygen(DO in mg/L)were measured in each treatment tank.Haematological parameters:packed cell volume(PCV),red blood cells(RBC),white blood cell(WBC),neutrophil and lymphocytes etc.,and hepatological alterations were measured after 12 weeks.The physico-chemical parameters showed that the pH ranged from 7.82±0.25~8.07±0.02.DO ranged from 1.92±0.66~4.43±1.24 mg/L.The conductivity values increased with increase in concentration of I.E.While the temperature difference remained insignificant with mean value range between 26.08±2.14~26.38±2.28.The DO showed significant differences at p<0.05.Though survival was 100%during the sublethal study,haematological results showed that C.gariepinus had PCV ranging from 13.0±1.7~27.7±0.6,RBC ranged from 4.7±0.6~9.1±0.1,and neutrophil ranged from 26.7±4.6~61.0±1.0 amongst others.The highest values of these parameters were obtained in the control and lowest at 35%.While the reverse effects were observed for WBC and lymphocytes,the liver shows normal liver cells in the control(0%),but at higher toxic levels,there were:vacoulation,destruction of the hepatic parenchyma,tissue becoming eosinophilic(i.e.tending towards Carcinogenicity)and severe disruption of the hepatic cord architecture.This study therefore shows that disposal of effluents into the aquatic environment affects the health of fishes by impairing normal hepatic functions and hindering vital physiological processes if exposure continues for a long period of time(sublethal effect).
文摘Background: People living with Human Immunodeficiency Virus (HIV) are more predisposed to urinary tract infections due to suppression of their immunity by the virus. Asymptomatic bacteriuria is associated with an increased risk of symptomatic urinary tract infection and the latter being an important contributor to development of chronic renal failure, hypertension and toxaemia of pregnancy. The aim of this study was to determine the prevalence of asymptomatic bacteriuria in HIV-infected patients and proffer a recommendation on the need or otherwise to screen. Methods: This was a cross sectional study of treatment-naive HIV-infected patients attending the HIV clinics of Lagos State University Teaching Hospital (LASUTH), Ikeja. A single voided aseptically collected mid-stream urine (MSU) was obtained from each patient and all samples processed immediately, were sent for urinalysis and culture. Isolates were considered significant if there were ≥105 colony forming unit/mL (CFU/mL) with 2 or less isolates, doubtful significance if 5 CFU/mL. Significant isolates were selected for identification. Data were analyzed using SPSS version 16.0 (Statistical Package for Social Sciences, Inc., Chicago, Ill). Results: A total of 156 consenting participants were recruited into the study. The mean age was 36.45 ± 8.65 years. There were 60 of 156 (38.4%) males and 96 of 156 (61.56%) females. Only 33 of 156 (21.2%) had significant bacteriuria, out of the 33 participants, 19 (57.8%) were asymptomatic, while 14 of 33 (42.4%) had significant growth but were symptomatic or on antibiotics. E. coli was isolated in 9 of 19 (47.3%), followed by Staph aureus 4 of 19 (21.05%). Conclusion: More than half of participants who had significant growth had asymptomatic bacteriuria, while one-fifth of all patients had significant growth. Considering this statistics, screening for or treatment of asymptomatic bacteriuria may be recommended in all HIV-infected patients.

Eighty-two percent of those pregnancies were unintended. Only 29% of those who had unprotected sexual intercourse used emergency contraceptives. Lack of knowledge and promotion of sexual promiscuity were identified as the main reasons for not using emergency contraceptives. Previous use of contraceptives, married status, increasing age and year of study were positive predictors for the use of emergency contraceptives while poor knowledge was a significant predictor of non-use. </span><b><span style="font-family:Verdana;">Conclusion: </span></b><span style="font-family:Verdana;">There was poor knowledge and low utilization of emergency contraceptives among respondents. Information on contraceptives should be introduced in secondary schools and in general studies courses in tertiary institutions while parents and caregivers should discuss issues relating to sex and contraceptives with adolescents.</span></span></span></span>
文摘This paper outlines a multi-dimensional user-oriented performance metrics approach in evaluating the operation of the terminal airspace system to aid in the airport and airspace planning and decision making. Safety, delay and predictability metrics contribute to the analytical framework. From the findings, the occurrence of air incidence has a high severity level at departure, and arrival phases of flight, higher likelihood at the radar room and much of the incidences were as a result of faulty equipment and inherent absence of modern airspace infrastructure. Also, in Lagos terminal airspace, the number of incidences has no close correlation with the level of traffic complexity. Total schedule arrival delay ranges from 1 - 392 minutes representing an average of 7.8 - 17.9 minutes per aircraft that arrived Lagos airport at that period. Be</span><span style="font-family:Verdana;">sides, the total approach contact time ranges from 1 - 57 minutes, translating to 4.6 - 7.1 minutes per aircraft. However, variability in arrival time of 1 - 5 minutes is common from published airline arrival scheduled time. In the same vein, the variability of 1 - 5 minutes is common from approach contact times of aircraft. These figures indicate sound arrival predictability signature for Lagos airport. Also, departure time variability above 30 mi</span><span style="font-family:Verdana;">nutes is familiar from the ATC clearance time for the various routes under study. However, there is about or more 25% variability of more than 15</span> <span style="font-family:Verdana;">minutes, and this indicates possible inconsistency of predicting departure times from the times Air Traffic Control</span><b> </b><span style="font-family:Verdana;">(ATC) clearance was acquired. Above all, the predictability of departure times in Lagos airport is weak compared to those of the arrival. Taken by it, this may be a sign of airspace congestion or ATC deficiencies at the Lagos airport. This is an indication of the lack of users’ confidence in Nigeria’s air transport industry to deliver just-in-time service.
文摘Background: The many dangers and stressors associated with police work often place police officers at a great risk of a variety of mental health morbidity, including suicidality. The causation of suicidality is multi-dimensional, however, there are insufficient studies which have examined the burden and risk factors of suicidality in Nigerian police officers. Objective: To determine the prevalence and predictors of suicidality among police officers in Lagos, Nigeria. Methods: This was a cross-sectional study of 600 police officers who were selected using a non-probability sampling method. Questionnaires used were: Suicide Behaviour Questionnaire-Revised (SBQ-R), State-Trait Anxiety Inventory (STAI), and a Socio-Demographic Questionnaire. Results: Prevalence of suicidality was 14%, and the predictors of suicidality were: state anxiety (p 0.001), trait anxiety (p 0.001), substance use (p = 0.03), being unmarried (p = 0.03), and female gender (p = 0.02). Conclusion: Suicidality is prevalent among Nigerian police officers, with a higher risk in those who are vulnerable. The creation of government policies and infrastructure which promote mental health in police officers is necessary.
文摘A study on seasonal abundance,morphometric and meristic data,growth pattern,condition fector,sex ratio and gonadosomatic index of Liza falcip-innis(Wlenciennes,1836)from the Ojo axis of Badagry creek,Nigeria was conducted from May 2019 to March 2020.A total of 1012 species were randomly selected,having 499 females and 513 males.The length frequency analysis and length-weight relationships(LWR)were determined.Sex ratio was determined by Chi-square analysis.The results showed that morphometric data are:0.5-2.5 mm for ED,2.1-12 mm for HL,1.7-8.1 mm for HD,2.5-11.7 for BD,2.6-233.3 mm for TL and 9.23-1006 g for BW for the combined sexes.The slope(b)shows an allometric growth pattern.The intercept'a'and slope'b'of the LWR(LogW=a+bLogL)were Log W=15.39+0.34 LogL(r=0.54)for combined sexes,Log W=12.49+0.02 log L(r=0.38)for males and Log W=18.23+0.01 log L(r=0.16)for females.The length frequency distribution indicated that species were dominated by two year classes(Ages 1 and 2).Condition factors were generally low.The values ranged between 0.68-0.85 for combined sexes.The gonadosomatic index for female was highest in August,2019(17.77%)with Mean±SD of 2.88±0.75;which indicated the peak of spawning period in the study area.Sex ratio difference was significant(P<0.05).Sexual differences were significant;the females are phenotypically larger than the male.
文摘Prostate cancer(PCa)symptoms are commonly confused with benign prostate hyperplasia(BPH),particularly in the early stages due to similarities between symptoms,and in some instances,underdiagnoses.Clinical methods have been utilized to diagnose PCa;however,at the full-blown stage,clinical methods usually present high risks of complicated side effects.Therefore,we proposed the use of support vector machine for early differential diagnosis of PCa(SVM-PCa-EDD).SVM was used to classify persons with and without PCa.We used the PCa dataset from the Kaggle Healthcare repository to develop and validate SVM model for classification.The PCa dataset consisted of 250 features and one class of features.Attributes considered in this study were age,body mass index(BMI),race,family history,obesity,trouble urinating,urine stream force,blood in semen,bone pain,and erectile dysfunction.The SVM-PCa-EDD was used for preprocessing the PCa dataset,specifically dealing with class imbalance,and for dimensionality reduction.After eliminating class imbalance,the area under the receiver operating characteristic(ROC)curve(AUC)of the logistic regression(LR)model trained with the downsampled dataset was 58.4%,whereas that of the AUC-ROC of LR trained with the class imbalance dataset was 54.3%.The SVM-PCa-EDD achieved 90%accuracy,80%sensitivity,and 80%specificity.The validation of SVM-PCa-EDD using random forest and LR showed that SVM-PCa-EDD performed better in early differential diagnosis of PCa.The proposed model can assist medical experts in early diagnosis of PCa,particularly in resource-constrained healthcare settings and making further recommendations for PCa testing and treatment.
文摘This study characterized the AA-01 depleted hydrocarbon reservoir in the KOKA field, Niger Delta, using a multidimensional approach. This investigation involved data validation analysis, evaluation of site suitability for CO_(2) storage, and compositional simulation of hydrocarbon components. The primary objective was to determine the initial components and behavior of the hydrocarbon system required to optimize the injection of CO_(2) and accompanying impurities, establishing a robust basis for subsequent sequestration efforts in the six wells in the depleted KOKA AA-01 reservoir. The process, simulated using industry software such as ECLIPSE, PVTi, SCAL, and Petrel, included a compositional fluid analysis to confirm the pressure volume temperature(PVT) hydrocarbon phases and components. This involved performing a material balance on the quality of the measured data and matching the initial reservoir pressure with the supplied data source. The compositional PVT analysis adopted the Peng–Robinson equation of state to model fluid flow in porous media and estimate the necessary number of phases and components to describe the system accurately. Results from this investigation indicate that the KOKA AA-01 reservoir is suitable for CO_(2)sequestration. This conclusion is based on the reservoir's good quality, evidenced by an average porosity of 0.21 and permeability of 1 111.0 mD, a measured lithological depth of 9 300 ft, and characteristic reservoir – seal properties correlated from well logs. The study confirmed that volumetric behavior predictions are directly linked to compositional behavior predictions, which are essential during reservoir initialization and data quality checks. Additionally, it highlighted that a safe design for CO_(2) storage relies on accurately representing multiphase behaviour across wide-ranging pressure–temperature–composition conditions.
文摘Background: This study was initiated to determine practices patterns in adjuvant chemotherapy for non-metastatic breast cancer and to examine the relationship between received dose intensity (RDI) and survival in patients with breast cancer Nigeria. Methods: Our study was a retrospective analysis of patients with breast cancer recruited from 2012 and 2015. A total of 204 patients were initially entered into the study, 102 were lost to follow-up leaving 102 patients who were suitable for the survival analysis. Survival time was calculated from 106 days, the scheduled end of chemotherapy. Results: The total average RDI for patients was 74%. Over the 204 patients that were reviewed, 144 (70.6%) had some reduction of RDI. This subgroup had an average RDI of 63%. On average, 79% of the intended dose of chemotherapy was given. The time to completion of chemotherapy was 1.33 times that specified by the protocol. Dose delays an overall reduction was mainly attributed to intolerability and financial constraints. Survival by RDI showed a significant decrease in survival rate for patients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, P = 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, P = 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, P = 0.003) compared with patients who received an RDI of 100%. Although associated with poorer prognosis, there were no significant changes in the survival rate for patients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, P = 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, P = 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, P = 0.405) compared with patients who received an RDI of 100%. Conclusion: This study provides evidence that decreased RDI of <70% in non-metastatic breast cancer patients is strongly associated with decreased overall survival.
