Treatment of a patient with aconitine poisoning using veno-arterial membrane oxygenation:A case report

BACKGROUND Aconitine poisoning is highly prone to causing malignant arrhythmias.The elimination of aconitine from the body takes a considerable amount of time,and during this period,patients are at a significant risk of death due to malignant arrhythmias associated with aconitine poisoning.CASE SUMMARY A 30-year-old male patient was admitted due to accidental ingestion of aconitinecontaining drugs.Upon arrival at the emergency department,the patient intermittently experienced malignant arrhythmias including ventricular tachycardia,ventricular fibrillation,ventricular premature beats,and cardiac arrest.Emergency interventions such as cardiopulmonary resuscitation and defibrillation were promptly administered.Additionally,veno-arterial extracorporeal membrane oxygenation(VA-ECMO)therapy was initiated.Successful resuscitation was achieved before ECMO placement,but upon initiation of ECMO,the patient experienced recurrent malignant arrhythmias.ECMO was utilized to maintain hemodynamics and respiration,while continuous blood purification therapy for toxin clearance,mechanical ventilation,and hypothermic brain protection therapy were concurrently administered.On the third day of VA-ECMO support,the patient’s respiratory and hemodynamic status stabilized,with only frequent ventricular premature beats observed on electrocardiographic monitoring,and echocardiography indicated recovery of cardiac contractile function.On the fourth day,a significant reduction in toxin levels was observed,along with stable hemodynamic and respiratory functions.Following a successful pump-controlled retrograde trial occlusion test,ECMO assistance was terminated.The patient gradually improved postoperatively and achieved recovery.He was discharged 11 days later.CONCLUSION VA-ECMO can serve as a bridging resuscitation technique for patients with reversible malignant arrhythmias.

Clinical Observation on the Common Acne Treated with the Five,ioint Therapy of the Traditional Medicine

Objective： The curative effect of the common acne treated by the five-joint therapy of traditional Chinese medicine was observed. Methods： 360 cases with the common acne were treated using five methods （embedding acupuncture point into traditional Chinese medicine soaked catgut; traditional Chinese medicine facial mask; taking traditional Chinese medicine orally; auricular point stimulation; discerning and cupping therapy） for 4-8 weeks, and then the improvements of the patients in clinical skin lesion were observed and also compared using the historical control method and the treatment method in the literature history. Results： The total effective rate was I00%, the cure rate was 56.39%, and the efficiency was 30.56%. Conclusion： The curative effect of the common ache treated by the five-joint therapy of traditional Chinese medicine is significant and also obviously better than that of the control method in this paper. Therefore, it is worthy of popularization and application.

The Practice Research of Task Driving Method in Basic Teaching of Information Technology

Purpose： It is used for judging the advantages and disadvantages of information technology foundation course teaching in health vocational colleges. Method： In teaching, it takes the two classes of 2012 grade nursing major as the experiment object. The comparison class adopts traditonal and speaking-practice combination teaching method and the experiment class adopts task-driving teaching method. When the semester finishes, it conducts testing andd questionnaire survey, collecting the relevant data, analyzing the changes of students in the aspects of performance, learning interest and attitude, autonomous learning consciousness and ability after experiment class adopting new teaching methods. Result： The exam performance of experiment class is obviously higher than the comparison class, and the experiment class has an obvious improvement in the aspects of learning interest, autonomous learning consciousness and ability, and the difference has statistical significance. Conclusion： The task driving teaching method is suitable for the status of information foundation teaching in health vocational colleges, which improves students＇ performance significantly and is good for students＇ learning interest and enthusiasm, obtaining good classroom effect. Also, it makes students＇ autonomous learning consciousness and ability improve greatly.

Control Experimental Study on the Application of Acupuncture Therapy to the Treatment of Peripheral Facial Neuritis in Acute Phase and Its Nursing

Objective： This study aimed to prove acupuncture therapy exerted an obvious clinical efficacy in the treatment of peripheral facial neuritis in acute phase. Methods： 60 clinically collected cases were chosen in the study and randomly divided into control group and treatment group. In control group, dexamethasone, vitamin B, vitamin B 12, and mecobalamine were applied in acute phase, and then acupuncture therapy was combined in remission. In treatment group, western medicine combining with acupuncture therapy was used in acute phase, and three courses of treatment were totally applied. Results： The final conditions of two groups were good, and the grouping was balanced, so the comparability between them was good （P〉0.05）; the curative effects of two groups were significantly different： the effect of treatment group was better than that of control group （P〈0.01）. Conclusion： The acute phase was necessarily grasped to apply acupuncture therapy to the treatment of peripheral facial neuritis, and the curative effect would be more significant if acupuncture therapy was more early used.

Investigation on Sharp Instrument Injury and Self-Protection of Internship Nursing Students

Objective： To understand the situation of nursing students was stabbed by sharp instrument,in order to provide a scientific basis to take protective measures.Method：By using a self-designed questionnaire.The 100 nursing students were randomly selected from a hospital for retrospective investigation.There are 98 valid data.Results： The data shows that sharp instrument injury is more common in nursing students. Nursing students＇ protection consciousness is low, and protective ability is poor.Conclusion： Nursing students awareness of self- protection to sharp instrument injury is not enough,in order to avoid the risk of infection of nursing students occupation.When they entered the clinics, practice of systematic pre occupation safety education is very necessary;and the school and the hospital should actively implement the management standardized operation regulations;and put up student occupation injury management regulations;to improve the safety performance of nursing equipment.

A Survey Report about the General Situation of the Students in Adult Higher Medical Education

This survey was implemented, so that the general situation of the students in adult higher medical education is more accurately and comprehensively known, and also adult education and teaching work can be developed in a targeted way. 193 students attending adult education junior college participated in this survey, and the surveyed details included the general situation of student sources, the ideological trends, and the degree of the cognition on the majors, the degree of the recognition on teaching, the employment concept, and the problems in the current jobs. Clinical Study on the Relationship between the HCG Levels during the Late Pregnancy and the Delivery Mechanism

Objective： The relationship between the HCG levels during the late pregnancy and the delivery mechanism was discussed. Method： If the HCG levels during the late pregnancy were related to the delivery mechanism was studied by using the β -HCG changes of 100 women pregnant for 36 weeks, 37 weeks, 38 weeks, 39 weeks, 40 weeks, and 41 weeks, and also the [3 -HCG changes when their uterine orifice was opened for 3cm near the time of labor as the clinical data. All these cases were found to suffer no clinical complications. Result： The difference in HCG changes during the late pregnancy was of no statistical significance （P〉0.05）. Conclusion： The β-HCG levels change during the late pregnancy is not significantly correlated with the labor onset time, and the labor onset time is unpredictable with the monitoring of the HCG levels change during the late pregnancy.

Analysis on the Correlation between Fasting Blood-glucose and Gestational Diabetes Mellitus

Objective： The correlation between the fasting blood-glucose （FBG） in early pregnancy and the gestational diabetes mellitus （GDM） was studied. Methods： The clinical data of 300 pregnant women receiving a treatment from Yongqing Hospital of Traditional Chinese Medicine and Langfang Health Vocational College from September 2012 to September 2013 was chosen; these cases were divided into group 6.9-6.0 mmol/L （35 cases）, group 5.9~5.0 mmol/L （58 cases）, group 4.9~4.5 mmol/L （120 cases）, and group 4.4~4.0mmol/L （108 cases） according to the range of the normal FBG. The proportional differences among the pregnant women diagnosed with GDM in different groups during the late pregnancy as well as the maternal and fetal prognosis were compared. Results： The proportion of the pregnant women diagnosed with GDM in group 6.9-6.0 mmol/L and group 5.9-5.0 mmol/L during the middle-late pregnancy was significantly higher than that of group 4.9-4.5 mmol/L and group 4.4-4.0mmol/L; there was no significant proportional difference among the pregnant women diagnosed with GDM in group 6.9~6.0 mmol/L and group 5.9-5.0 mmol/L during the middle-late pregnancy; there was no significant proportional difference among the pregnant women diagnosed with GDM in group 4.9-4.5 mmol/L and group 4.4-4.0mmol/L during the middle-late pregnancy. Conclusion： The risk of the pregnant women with FBG is higher than that of the normal pregnant women in suffering GDM during the early pregnancy, so attention should be paid to early blood glucose test and the blood glucose level should be positively controlled once they are defmitely diagnosed with GDM, and then the perinatal and maternal survival quality can be improved.

A Task-driven Medical Teaching Design under the Guidance of B-learning

A new challenge has been brought by the rapid development of information technology to medical teaching, and the teaching reform of the medical education in China should be deepened for attaining a development. The core of deepening medical teaching reform is to break the traditional teaching model （i.e. giving a lecture）, and create a better classroom teaching situation by making full use of the modern teaching methods. In B-learning, the effective combination of the traditional teaching method＇ s advantages with the E-learning in a pure technical environment is advocated. In the task-driven teaching, a teaching design is made by students in a real or simulated real environment. In this paper, the effective combination of B-learning with the task-driven teaching model is introduced, so that the better teaching effect is achieved.
