A unique design feature of the VEST is two partial solenoid coils installed at both vertical ends of a center stack, which can provide sufficient magnetic fluxes to initiate tokamak plasmas while keeping a low aspect ratio configuration in the central region. According to initial double null merging start-up scenario using the partial solenoid coils, apprdpriate p6wer supplies for driving a toroidal field coil, outer poloidal field coils, and the partial solenoid Coils are fabricated and successfully commissioned. For reliable start-up, a pre-ionization system with two cost-effective homemade magnetron power supplies is also prepared. In addition, magnetic and spectroscopic diagnostics with appropriate data acquisition and control systems are well prepared for initial operation of the device. The VEST is ready for tokamak plasma operation by completing and commissioning most of the designed components.展开更多
Following its initial execution in November 2015,pure laparoscopic donor hepatectomy(PLDH)has gained acceptance as a conventional practice at Seoul National University Hospital(SNUH).It is noteworthy that a significan...Following its initial execution in November 2015,pure laparoscopic donor hepatectomy(PLDH)has gained acceptance as a conventional practice at Seoul National University Hospital(SNUH).It is noteworthy that a significant proportion of cases entail full right hepatectomies,which are acknowledged to be technically demanding.As expertise and knowledge have been accrued,the pure laparoscopic technique has been extended to encompass liver recipients as a viable option in SNUH.The aim of this review is to present the developmental progression of PLDH,with a focus on pure laparoscopic donor right hepatectomy(PLDRH),at SNUH.This includes the standardization process,which can be achieved by sharing the hospital’s accumulated experience and previous reports.Various types of graft,including full right,left,left lateral section,and monosegment,were procured by pure laparoscopic technique.The criteria for selection were expanded to include donors with variations in the anatomy of the portal vein and bile duct.Additionally,the procedure of PLDRH was determined to be safe and viable for donors with high body mass index and larger graft weight.In conclusion,this review demonstrates the alterations implemented throughout our evolution from restricted to inclusive criteria for donor selection,leading to a complete shift from open surgery to pure laparoscopic procedures in donor hepatectomy and eventually pure laparoscopic living donor liver transplantation(LDLT)in recipient.展开更多

Including some naturalized plants that had been contribute to land restoration, many naturalized plants have been invaded to original habitat or landscape for native plants. Once the plants were colonized, they extend their area and population size. Urban developed areas often give an important role of source habitat for naturalized plants and expanding their population size. In recent, this situation is appearing as one of environmental problems about the urban landscape management controlling the naturalized plants that invaded in the developed area and conserving the native vegetation. This paper is focusing on relationships between distribution of habitat of naturalized plants and landscape patch in urban area in Seoul. Gangdong-Gu, one of the administrative areas in Seoul was selected for this study. We examined the recent land use change using LANDSAT TM data and spreading of the representative naturalized plants (Robinia pseudoacacia and Eupatorium rugosum) by Seoul Biotope Mapping Project and field survey in 1999. As a result, these two species were often occurred in the same habitat and distributed in forest edge disturbed by man. Their distribution patterns were related to landscape indices (patch size and shape) in the forest edge.展开更多
BACKGROUND Periodontitis is a chronic inflammation of periodontal tissues.The effect of periodontitis on the development of inflammatory bowel disease(IBD)remains unclear.AIM To assessed the risk of IBD among patients...BACKGROUND Periodontitis is a chronic inflammation of periodontal tissues.The effect of periodontitis on the development of inflammatory bowel disease(IBD)remains unclear.AIM To assessed the risk of IBD among patients with periodontitis,and the risk factors for IBD related to periodontitis.METHODS A nationwide population-based cohort study was performed using claims data from the Korean National Healthcare Insurance Service.In total,9950548 individuals aged≥20 years who underwent national health screening in 2009 were included.Newly diagnosed IBD[Crohn’s disease(CD),ulcerative colitis(UC)]using the International Classification of Disease 10th revision and rare intractable disease codes,was compared between the periodontitis and nonperiodontitis groups until 2017.RESULTS A total of 1092825 individuals(11.0%)had periodontitis.Periodontitis was significantly associated with older age,male gender,higher body mass index,quitting smoking,not drinking alcohol,and regular exercise.The mean age was 51.4±12.9 years in the periodontitis group and 46.6±14.2 years in the nonperiodontitis group(P<0.01),respectively.The mean body mass index was 23.9±3.1 and 23.7±3.2 in the periodontitis and non-periodontitis groups,respectively(P<0.01).Men were 604307(55.3%)and 4844383(54.7%)in the periodontitis and non-periodontitis groups,respectively.The mean follow-up duration was 7.26 years.Individuals with periodontitis had a significantly higher risk of UC than those without periodontitis[adjusted hazard ratio:1.091;95%confidence interval(CI):1.008-1.182],but not CD(adjusted hazard ratio:0.879;95%confidence interval:0.731-1.057).The risks for UC were significant in the subgroups of age≥65 years,male gender,alcohol drinker,current smoker,and reduced physical activity.Current smokers aged≥65 years with periodontitis were at a 1.9-fold increased risk of UC than non-smokers aged≥65 years without periodontitis.CONCLUSION Periodontitis was significantly associated with the risk of developing UC,but not CD,particularly in current smokers aged≥65 years.展开更多
The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Base...The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Based on the foundation of the previous consensus statement,this new consensus document aimed to update clinical recommendations and provide guidance to improve the outcomes of RLR clinical practice.The guideline steering group and guideline expert group were formed by 29 international experts of liver surgery and evidence-based medicine(EBM).Relevant literature was reviewed and analyzed by the evidence evaluation group.According to the WHO Handbook for Guideline Development,the Guidance Principles of Development and Amendment of the Guidelines for Clinical Diagnosis and Treatment in China 2022,a total of 14 recommendations were generated.Among them were 8 recommendations formulated by the GRADE method,and the remaining 6 recommendations were formulated based on literature review and experts’opinion due to insufficient EBM results.This international experts consensus guideline offered guidance for the safe and effective clinical practice and the research direction of RLR in future.展开更多

Recently, in the Association of Medical Oncology of the German Cancer Society(AIO) 0604 trial, tri?weekly XELIRI plus bevacizumab, with reduced doses of irinotecan(200 mg/m^2 on day 1) and capecitabine(1600 mg/m^2 on days 1–14), repeated every 3 weeks, has shown favorable tolerability and eicacy which were comparable to those of capecitabine and oxaliplatin(XELOX) plus bevacizumab. The doses of capecit?abine and irinotecan in the AIO trial are considered optimal. In a phase I/II study, XELIRI plus bevacizumab(BIX) as second?line chemotherapy was well tolerated and had promising eicacy in Japanese patients.Methods: The Asian XELIRI Projec T(AXEPT) is an East Asian collaborative, open?labelled, randomized, phase Ⅲ clinical trial which was designed to demonstrate the non?inferiority of XELIRI with or without bevacizumab versus standard FOLFIRI(5?fluorouracil, leucovorin, and irinotecan combination) with or without bevacizumab as second?line chemo?therapy for patients with m CRC. Patients with 20 years of age or older, histologically conirmed m CRC, Eastern Coop?erative Oncology Group performance status 0–2, adequate organ function, and disease progression or intolerance of the irst?line regimen will be eligible. Patients will be randomized(1:1) to receive standard FOLFIRI with or with?out bevacizumab(5 mg/kg on day 1), repeated every 2 weeks(FOLIRI arm) or XELIRI with or without bevacizumab(7.5 mg/kg on day 1), repeated every 3 weeks(XELIRI arm). A total of 464 events were estimated as necessary to show non?inferiority with a power of 80% at a one?sided α of 0.025, requiring a target sample size of 600 patients. The 95% conidence interval(CI) upper limit of the hazard ratio was pre?speciied as less than 1.3.Conclusion: The Asian XELIRI Projec T is a multinational phase III trial being conducted to provide evidence for XELIRI with or without bevacizumab as a second?line treatment option of mCRC.展开更多
BACKGROUND Korean National Health Insurance(NHI)claims database provides large-cohort.However,studies regarding accuracy of administrative database for pancreatic cancer(PC)have not been reported.We aimed to identify ...BACKGROUND Korean National Health Insurance(NHI)claims database provides large-cohort.However,studies regarding accuracy of administrative database for pancreatic cancer(PC)have not been reported.We aimed to identify accuracy of NHI database regarding PC classified by international classification of disease(ICD)-10 codes.AIM To identify the accuracy and usefulness of administrative database in PC and the accurate ICD codes for PC with location.METHODS Study and control groups were collected from 2003 to 2016 at Seoul National University Bundang Hospital.Cases of PC were identified in NHI database by international classification of diseases,10th revision edition(ICD-10 codes)supported with V codes.V code is issued by medical doctors for covering 95%of medical cost by Korean government.According to pathologic reports,definite or possible diagnoses were defined using medical records,images,and pathology.RESULTS A total of 1846 cases with PC and controls were collected.Among PC,only 410(22.2%)cases were identified as specific cancer sites including head in 234(12.7%)cases,tail in 104(5.6%)cases and body in 72(3.9%)cases.Among PC,910(49.3%)cases were diagnosed by definite criteria.Most of these were adenocarcinoma(98.0%).The rates of definite diagnosis of PC were highest in head(70.1%)followed by body(47.2%)and tail(43.3%).False-positive cases were pancreatic cystic neoplasm and metastasis to the pancreas.In terms of the overall diagnosis of PC,sensitivity,specificity,positive predictive value,and negative predictive value were 99.95%,98.72%,98.70%,and 99.95%,respectively.Diagnostic accuracy was similar both in terms of diagnostic criteria and tumor locations.CONCLUSION Korean NHI claims database collected according to ICD-10 code with V code for PC showed good accuracy.展开更多

The purpose of this study is to clarify the vegetation structure in breeding area of Siberian rubythroat (Luscinia calliope). In Daecheongbong peak, Mt. Seoraksan national park, South Korea from May to August, 2001, breeding population of Siberian rubythroat and the dominant species in breeding area of Siberian rubythroat (Luscinia calliope) were surveyed by line transect method along the ridge in the Daecheongbong peak area. Number of individuals and location of song posts were observed and recorded. According to the survey results, the study area was classified into high, middle and low density areas. Those birds selected the forest area of dominant species for Ermans birch and dwarf Siberian pine as habitat and preferred the shrubs area with the lower height and higher coverage.展开更多

This led to Japan’s invasion of the Ryukyu Islands, Taiwan, Korea, and Manchuria, which ultimately led to the Manchurian Incident of 1931, the Sino-Japanese War, and the Pacific War. Surprisingly, the subject and the timing of each and every one of these acts of war were in the same order of Yoshida Shoin’s proposal on preoccupancy. The Sino-Japanese war of 1894 was romanticized as clearing the barbaric culture by civilization, and the Russo-Japanese war of 1904 was romanticized as the realization of Eastern Peace. However, Japanese policies of aggressions were first deemed illegal by international law during the 1931 Manchurian Incident by the investigations of the League of Nations. The Japanese Empire received the recommendation by the League of Nations to restore to original state, but declined and exited from the League of Nations. Following their exit, they started the Sino-Japanese War and the Pacific War and eventually lost in 1945. The goal of the San Francisco Peace Treaty of 1951 was to punish Japan’s aggressions. However, as the Cold War between the East and the West started to arise in 1948, the punishment was eased, and their punishment for the aggressions on the Ryukyu Islands, Taiwan, and Korea was nearly unasked for. This paper examines the issues of the San Francisco Peace Treaty in the views of the international law of the League of Nations, established by Manley O. However, POPSs have been frequently criticized for failing to correspond with the ur...Seoul’s privately owned public space (POPS) initiative has achieved quantitative success since its introduction in the early 1990s. However, POPSs have been frequently criticized for failing to correspond with the urban context in which they are created and are thus often underused. Association between surrounding urban context and the use of POPS is acknowledged, but specific elements of the urban context associated with the use of POPS and the extent of association are unclear. In this study, the relationship between constituent elements of 48 POPSs including the surrounding ground floor facility use and the use of those spaces in Seoul’s central business district was measured by multiple regression analysis. The results present empirical evidence of a substantial relationship between assembly, mercantile, and business facilities on ground floors of surrounding buildings and the use of POPS. As the surrounding ground floor facility use showed an overriding relationship with the use of POPS that possibly overshadowed the influence of secondary determinants, the 48 POPSs were divided into two groups: those surrounded by commercial facilities and those surrounded by business facilities. The use of POPSs surrounded by commercial facilities was associated with additional variables including the proximity to transit stations and crosswalks. Models using the sample of POPSs surrounded by business facilities were not statistically significant. Based on these results, this paper emphasizes the importance of creating POPSs in connection with desirable surrounding facilities. This study also suggests that anticipation of the usage patterns of POPSs and establishment of guidelines supporting those patterns may be viable by investigating preexisting urban contexts.展开更多
BACKGROUND As people living with human immunodeficiency virus(HIV)(PLWH)enjoy longer life expectancy with highly effective antiretroviral therapy,they are encountering challenging cardiovascular health risks.AIM To re...BACKGROUND As people living with human immunodeficiency virus(HIV)(PLWH)enjoy longer life expectancy with highly effective antiretroviral therapy,they are encountering challenging cardiovascular health risks.AIM To retrospectively examine the increasing burden of cardiovascular diseases in PLWH over the past decade.METHODS All hospitalizations for heart failure(HF),ischemic heart disease(IHD),and cerebrovascular disease(CeVD)in PLWH were identified using the International Classification of Diseases,Ninth Revision,Clinical Modification(ICD-9-CM)and ICD-10-CM codes in the National Inpatient Sample from 2008 to 2018.Outcomes included number of hospitalizations,in-hospital mortality,length of stay,and total hospital charge.Trend of the outcomes from 2008 to 2018 were analyzed using Cochran-Armitage_(trend)test and simple linear regression.RESULTS The number of hospitalizations for HF in PLWH increased from 4212 in 2008 to 6700 in 2018(P_(trend)<0.01).Similar increasing_(trend)was seen with those for IHD and CeVD over the decade(P_(trend)<0.01).A decreasing_(trend)of in-hospital mortality was observed in all hospitalizations of PLWH(P_(trend)<0.01)and CeVD in PLWH(P_(trend)<0.01),but not in those for HF(P_(trend)=0.67)and IHD(P_(trend)=0.13).The_(trend)of length of stay was decreasing in all hospitalizations of PLWH(P_(trend)<0.01),but increasing in those for HF in PLWH(P_(trend)<0.01).An increasing_(trend)of total hospital charge was observed in hospitalizations for HF,IHD,and CeVD(P_(trend)<0.01).CONCLUSION The burden of cardiovascular diseases has significantly increased in hospitalizations of PLWH from 2008 to 2018.Continued efforts are needed to address the additional cardiovascular risks in this vulnerable population.展开更多

The amount of space serving as a nexus between universities and urban areas is gradually increasing. This study defines such intermediate spaces as “the university-city interface”—areas that influence the university’s physical and functional connection to the surrounding city. The research presents comparative case studies of three universities in urban contexts—Harvard University, University of Pennsylvania, and Université Catholique de Louvain— by analyzing plazas and boulevards. These representative open spaces provide interfaces for both the university campuses and their surrounding cities. This paper analyzes design elements from the perspective of campus spatial structure, as well as locations and functions, to develop a comparative checklist for plazas and boulevards. The results offer a set of urban design principles for university plazas and boulevards that could significantly improve the quality of the university-city relationship. While these checklists and principles may vary depending on circumstances, they can be useful starting points for initializing design processes.展开更多

Mo_(2)C is an excellent electrocatalyst for hydrogen evolution reaction(HER).However,Mo_(2)C is a poor electrocatalyst for oxygen evolution reaction(OER).Herein,two different elements,namely Co and Fe,are incorporated...Mo_(2)C is an excellent electrocatalyst for hydrogen evolution reaction(HER).However,Mo_(2)C is a poor electrocatalyst for oxygen evolution reaction(OER).Herein,two different elements,namely Co and Fe,are incorporated in Mo_(2)C that,therefore,has a finely tuned electronic structure,which is not achievable by incorporation of any one of the metals.Consequently,the resulting electrocatalyst Co_(0.8)Fe_(0.2)-Mo_(2)C-80 displayed excellent OER catalytic performance,which is evidenced by a low overpotential of 214.0(and 246.5)mV to attain a current density of 10(and 50)mA cm^(-2),an ultralow Tafel slope of 38.4 mV dec^(-1),and longterm stability in alkaline medium.Theoretical data demonstrates that Co_(0.8)Fe_(0.2)-Mo_(2)C-80 requires the lowest overpotential(1.00 V)for OER and Co centers to be the active sites.The ultrahigh catalytic performance of the electrocatalyst is attributed to the excellent intrinsic catalytic activity due to high Brunauer-Emmett-Teller specific surface area,large electrochemically active surface area,small Tafel slope,and low chargetransfer resistance.展开更多
Background Genome editing has been considered as powerful tool in agricultural fields.However,genome editing progress in cattle has not been fast as in other mammal species,for some disadvantages including long gestat...Background Genome editing has been considered as powerful tool in agricultural fields.However,genome editing progress in cattle has not been fast as in other mammal species,for some disadvantages including long gestational periods,single pregnancy,and high raising cost.Furthermore,technically demanding methods such as microinjection and somatic cell nuclear transfer(SCNT)are needed for gene editing in cattle.In this point of view,electroporation in embryos has been risen as an alternative.Results First,editing efficiency of our electroporation methods were tested for embryos.Presence of mutation on embryo was confirmed by T7E1 assay.With first combination,mutation rates for MSTN and PRNP were 57.6%±13.7%and 54.6%±13.5%,respectively.In case of MSTN/BLG,mutation rates were 83.9%±23.6%for MSTN,84.5%±18.0%for BLG.Afterwards,the double-KO embryos were transferred to surrogates and mutation rate was identified in resultant calves by targeted deep sequencing.Thirteen recipients were transferred for MSTN/PRNP,4 calves were delivered,and one calf underwent an induction for double KO.Ten surrogates were given double-KO embryos for MSTN/BLG,and four of the six calves that were born had mutations in both genes.Conclusions These data demonstrated that production of genome edited cattle via electroporation of RNP could be effectively applied.Finally,MSTN and PRNP from beef cattle and MSTN and BLG from dairy cattle have been born and they will be valuable resources for future precision breeding.展开更多
Artificial intelligence(AI)models have significantly impacted various areas of the atmospheric sciences,reshaping our approach to climate-related challenges.Amid this AI-driven transformation,the foundational role of ...Artificial intelligence(AI)models have significantly impacted various areas of the atmospheric sciences,reshaping our approach to climate-related challenges.Amid this AI-driven transformation,the foundational role of physics in climate science has occasionally been overlooked.Our perspective suggests that the future of climate modeling involves a synergistic partnership between AI and physics,rather than an“either/or”scenario.Scrutinizing controversies around current physical inconsistencies in large AI models,we stress the critical need for detailed dynamic diagnostics and physical constraints.Furthermore,we provide illustrative examples to guide future assessments and constraints for AI models.Regarding AI integration with numerical models,we argue that offline AI parameterization schemes may fall short of achieving global optimality,emphasizing the importance of constructing online schemes.Additionally,we highlight the significance of fostering a community culture and propose the OCR(Open,Comparable,Reproducible)principles.Through a better community culture and a deep integration of physics and AI,we contend that developing a learnable climate model,balancing AI and physics,is an achievable goal.展开更多
The electrochemical reduction of carbon dioxide offers a sound and economically viable technology for the electrification and decarbonization of the chemical and fuel industries.In this technology,an electrocatalytic ...The electrochemical reduction of carbon dioxide offers a sound and economically viable technology for the electrification and decarbonization of the chemical and fuel industries.In this technology,an electrocatalytic material and renewable energy-generated electricity drive the conversion of carbon dioxide into high-value chemicals and carbon-neutral fuels.Over the past few years,single-atom catalysts have been intensively studied as they could provide near-unity atom utilization and unique catalytic performance.Single-atom catalysts have become one of the state-of-the-art catalyst materials for the electrochemical reduction of carbon dioxide into carbon monoxide.However,it remains a challenge for single-atom catalysts to facilitate the efficient conversion of carbon dioxide into products beyond carbon monoxide.In this review,we summarize and present important findings and critical insights from studies on the electrochemical carbon dioxide reduction reaction into hydrocarbons and oxygenates using single-atom catalysts.It is hoped that this review gives a thorough recapitulation and analysis of the science behind the catalysis of carbon dioxide into more reduced products through singleatom catalysts so that it can be a guide for future research and development on catalysts with industry-ready performance for the electrochemical reduction of carbon dioxide into high-value chemicals and carbon-neutral fuels.展开更多
Inspired by the overlapping structure of snake scales,a reinforced scale-like knitted fabric(R-SLKF)was created in this work.To achieve this,short carbon fibers in an epoxy resin(ER)matrix were incorporated into the s...Inspired by the overlapping structure of snake scales,a reinforced scale-like knitted fabric(R-SLKF)was created in this work.To achieve this,short carbon fibers in an epoxy resin(ER)matrix were incorporated into the scales of an SLKF.The resulting textile is a highly stable protective composite that is flexible,warm,and thermally insulated.In addition,supe-rior stab-resistance is ensured through rigid protective blocks in the R-SLKF,making up a hard overlapping scale region,besides satisfactory flexibility via soft twisted ultra-high-molecular-weight polyethylene yarn-based textiles.The R-SLKF achieves high stab resistance(peak load of approximately 600 N for a single scale thickness of 2 mm),good flexibility(~290 mN cm),and breathability(100 MPa,423 mm/s),coupled with good warmth retention and thermal insulation prop-erties(0.28℃/s),which are superior to previously reported protective composite textiles.From the results,the combination of desirable individual protection,excellent wearability and comfort enables human beings to survive in extremely danger-ous environments.Finite element simulations provided valuable insights into the factors influencing the stab resistance of R-SLKF and elucidated the underlying anti-puncture mechanism in accordance with the experimental findings.This study presents a novel strategy for the facile industrial fabrication of flexible and lightweight protective composite textiles,which is expected to enhance the structure and material design for future innovations and provide advantages for personal protec-tive equipment in various industrial fields.展开更多
BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however,its incidence is increasing in the East.The epidemiology of IBD in Asia differs significantly from the patterns in the Wes...BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however,its incidence is increasing in the East.The epidemiology of IBD in Asia differs significantly from the patterns in the West.AIM To comprehensively investigate the epidemiology of IBD in South Korea,inclu-ding its incidence,prevalence,medication trends,and outcomes.METHODS We analyzed claims data from the Health Insurance Review and Assessment Service and Rare and Intractable Diseases(RIDs),operated by the National Health Insurance Service of South Korea.Patients with IBD were identified based on the International Classification of Diseases,Tenth Revision,and RID diagnostic codes for Crohn’s disease(CD)and ulcerative colitis(UC)from 2010 to 2018.RESULTS In total,14498 and 31409 patients were newly diagnosed with CD and UC,respectively,between 2010 and 2018.The annual average incidence of CD was 3.11 cases per 105 person-years,and that of UC was 6.74 cases per 10^(5) person-years.Since 2014,the incidence rate of CD has been stable,while that of UC has steadily increased,shifting the peak age group from 50-year-olds in 2010 to 20-year-olds in 2018.The CD and UC prevalence increased consistently over the study period;the use of 5-aminosali-cylates and corticosteroids gradually decreased,while that of immunomodulators and biologics steadily increased in both CD and UC.The clinical outcomes of IBD,such as hospitalization and surgery,decreased during the study period.CONCLUSION The CD incidence has been stable since 2014,but that of UC has increased with a shift to a younger age at peak incidence between 2010 and 2018.IBD clinical outcomes improved over time,with increased use of immunomodu-lators and biologics.展开更多

文摘A new spherical torus called VEST (Versatile Experiment Spherical Torus) is designed, constructed and successfully commissioned at Seoul National University. A unique design feature of the VEST is two partial solenoid coils installed at both vertical ends of a center stack, which can provide sufficient magnetic fluxes to initiate tokamak plasmas while keeping a low aspect ratio configuration in the central region. According to initial double null merging start-up scenario using the partial solenoid coils, apprdpriate p6wer supplies for driving a toroidal field coil, outer poloidal field coils, and the partial solenoid Coils are fabricated and successfully commissioned. For reliable start-up, a pre-ionization system with two cost-effective homemade magnetron power supplies is also prepared. In addition, magnetic and spectroscopic diagnostics with appropriate data acquisition and control systems are well prepared for initial operation of the device. The VEST is ready for tokamak plasma operation by completing and commissioning most of the designed components.
文摘Following its initial execution in November 2015,pure laparoscopic donor hepatectomy(PLDH)has gained acceptance as a conventional practice at Seoul National University Hospital(SNUH).It is noteworthy that a significant proportion of cases entail full right hepatectomies,which are acknowledged to be technically demanding.As expertise and knowledge have been accrued,the pure laparoscopic technique has been extended to encompass liver recipients as a viable option in SNUH.The aim of this review is to present the developmental progression of PLDH,with a focus on pure laparoscopic donor right hepatectomy(PLDRH),at SNUH.This includes the standardization process,which can be achieved by sharing the hospital’s accumulated experience and previous reports.Various types of graft,including full right,left,left lateral section,and monosegment,were procured by pure laparoscopic technique.The criteria for selection were expanded to include donors with variations in the anatomy of the portal vein and bile duct.Additionally,the procedure of PLDRH was determined to be safe and viable for donors with high body mass index and larger graft weight.In conclusion,this review demonstrates the alterations implemented throughout our evolution from restricted to inclusive criteria for donor selection,leading to a complete shift from open surgery to pure laparoscopic procedures in donor hepatectomy and eventually pure laparoscopic living donor liver transplantation(LDLT)in recipient.

文摘During land transformation process in the human history, naturalized plants were introduced to several land use patterns by the different ways of plant itself. Including some naturalized plants that had been contribute to land restoration, many naturalized plants have been invaded to original habitat or landscape for native plants. Once the plants were colonized, they extend their area and population size. Urban developed areas often give an important role of source habitat for naturalized plants and expanding their population size. In recent, this situation is appearing as one of environmental problems about the urban landscape management controlling the naturalized plants that invaded in the developed area and conserving the native vegetation. This paper is focusing on relationships between distribution of habitat of naturalized plants and landscape patch in urban area in Seoul. Gangdong-Gu, one of the administrative areas in Seoul was selected for this study. We examined the recent land use change using LANDSAT TM data and spreading of the representative naturalized plants (Robinia pseudoacacia and Eupatorium rugosum) by Seoul Biotope Mapping Project and field survey in 1999. As a result, these two species were often occurred in the same habitat and distributed in forest edge disturbed by man. Their distribution patterns were related to landscape indices (patch size and shape) in the forest edge.
文摘BACKGROUND Periodontitis is a chronic inflammation of periodontal tissues.The effect of periodontitis on the development of inflammatory bowel disease(IBD)remains unclear.AIM To assessed the risk of IBD among patients with periodontitis,and the risk factors for IBD related to periodontitis.METHODS A nationwide population-based cohort study was performed using claims data from the Korean National Healthcare Insurance Service.In total,9950548 individuals aged≥20 years who underwent national health screening in 2009 were included.Newly diagnosed IBD[Crohn’s disease(CD),ulcerative colitis(UC)]using the International Classification of Disease 10th revision and rare intractable disease codes,was compared between the periodontitis and nonperiodontitis groups until 2017.RESULTS A total of 1092825 individuals(11.0%)had periodontitis.Periodontitis was significantly associated with older age,male gender,higher body mass index,quitting smoking,not drinking alcohol,and regular exercise.The mean age was 51.4±12.9 years in the periodontitis group and 46.6±14.2 years in the nonperiodontitis group(P<0.01),respectively.The mean body mass index was 23.9±3.1 and 23.7±3.2 in the periodontitis and non-periodontitis groups,respectively(P<0.01).Men were 604307(55.3%)and 4844383(54.7%)in the periodontitis and non-periodontitis groups,respectively.The mean follow-up duration was 7.26 years.Individuals with periodontitis had a significantly higher risk of UC than those without periodontitis[adjusted hazard ratio:1.091;95%confidence interval(CI):1.008-1.182],but not CD(adjusted hazard ratio:0.879;95%confidence interval:0.731-1.057).The risks for UC were significant in the subgroups of age≥65 years,male gender,alcohol drinker,current smoker,and reduced physical activity.Current smokers aged≥65 years with periodontitis were at a 1.9-fold increased risk of UC than non-smokers aged≥65 years without periodontitis.CONCLUSION Periodontitis was significantly associated with the risk of developing UC,but not CD,particularly in current smokers aged≥65 years.
文摘The robotic liver resection(RLR)has been increasingly applied in recent years and its benefits shown in some aspects owing to the technical advancement of robotic surgical system,however,controversies still exist.Based on the foundation of the previous consensus statement,this new consensus document aimed to update clinical recommendations and provide guidance to improve the outcomes of RLR clinical practice.The guideline steering group and guideline expert group were formed by 29 international experts of liver surgery and evidence-based medicine(EBM).Relevant literature was reviewed and analyzed by the evidence evaluation group.According to the WHO Handbook for Guideline Development,the Guidance Principles of Development and Amendment of the Guidelines for Clinical Diagnosis and Treatment in China 2022,a total of 14 recommendations were generated.Among them were 8 recommendations formulated by the GRADE method,and the remaining 6 recommendations were formulated based on literature review and experts’opinion due to insufficient EBM results.This international experts consensus guideline offered guidance for the safe and effective clinical practice and the research direction of RLR in future.

文摘Background: Capecitabine and irinotecan combination therapy(XELIRI) has been examined at various dose levels to treat metastatic colorectal cancer(m CRC). Recently, in the Association of Medical Oncology of the German Cancer Society(AIO) 0604 trial, tri?weekly XELIRI plus bevacizumab, with reduced doses of irinotecan(200 mg/m^2 on day 1) and capecitabine(1600 mg/m^2 on days 1–14), repeated every 3 weeks, has shown favorable tolerability and eicacy which were comparable to those of capecitabine and oxaliplatin(XELOX) plus bevacizumab. The doses of capecit?abine and irinotecan in the AIO trial are considered optimal. In a phase I/II study, XELIRI plus bevacizumab(BIX) as second?line chemotherapy was well tolerated and had promising eicacy in Japanese patients.Methods: The Asian XELIRI Projec T(AXEPT) is an East Asian collaborative, open?labelled, randomized, phase Ⅲ clinical trial which was designed to demonstrate the non?inferiority of XELIRI with or without bevacizumab versus standard FOLFIRI(5?fluorouracil, leucovorin, and irinotecan combination) with or without bevacizumab as second?line chemo?therapy for patients with m CRC. Patients with 20 years of age or older, histologically conirmed m CRC, Eastern Coop?erative Oncology Group performance status 0–2, adequate organ function, and disease progression or intolerance of the irst?line regimen will be eligible. Patients will be randomized(1:1) to receive standard FOLFIRI with or with?out bevacizumab(5 mg/kg on day 1), repeated every 2 weeks(FOLIRI arm) or XELIRI with or without bevacizumab(7.5 mg/kg on day 1), repeated every 3 weeks(XELIRI arm). A total of 464 events were estimated as necessary to show non?inferiority with a power of 80% at a one?sided α of 0.025, requiring a target sample size of 600 patients. The 95% conidence interval(CI) upper limit of the hazard ratio was pre?speciied as less than 1.3.Conclusion: The Asian XELIRI Projec T is a multinational phase III trial being conducted to provide evidence for XELIRI with or without bevacizumab as a second?line treatment option of mCRC.

文摘BACKGROUND Korean National Health Insurance(NHI)claims database provides large-cohort.However,studies regarding accuracy of administrative database for pancreatic cancer(PC)have not been reported.We aimed to identify accuracy of NHI database regarding PC classified by international classification of disease(ICD)-10 codes.AIM To identify the accuracy and usefulness of administrative database in PC and the accurate ICD codes for PC with location.METHODS Study and control groups were collected from 2003 to 2016 at Seoul National University Bundang Hospital.Cases of PC were identified in NHI database by international classification of diseases,10th revision edition(ICD-10 codes)supported with V codes.V code is issued by medical doctors for covering 95%of medical cost by Korean government.According to pathologic reports,definite or possible diagnoses were defined using medical records,images,and pathology.RESULTS A total of 1846 cases with PC and controls were collected.Among PC,only 410(22.2%)cases were identified as specific cancer sites including head in 234(12.7%)cases,tail in 104(5.6%)cases and body in 72(3.9%)cases.Among PC,910(49.3%)cases were diagnosed by definite criteria.Most of these were adenocarcinoma(98.0%).The rates of definite diagnosis of PC were highest in head(70.1%)followed by body(47.2%)and tail(43.3%).False-positive cases were pancreatic cystic neoplasm and metastasis to the pancreas.In terms of the overall diagnosis of PC,sensitivity,specificity,positive predictive value,and negative predictive value were 99.95%,98.72%,98.70%,and 99.95%,respectively.Diagnostic accuracy was similar both in terms of diagnostic criteria and tumor locations.CONCLUSION Korean NHI claims database collected according to ICD-10 code with V code for PC showed good accuracy.

文摘Most of forest birds have the characteristics for habitat selection. The purpose of this study is to clarify the vegetation structure in breeding area of Siberian rubythroat (Luscinia calliope). In Daecheongbong peak, Mt. Seoraksan national park, South Korea from May to August, 2001, breeding population of Siberian rubythroat and the dominant species in breeding area of Siberian rubythroat (Luscinia calliope) were surveyed by line transect method along the ridge in the Daecheongbong peak area. Number of individuals and location of song posts were observed and recorded. According to the survey results, the study area was classified into high, middle and low density areas. Those birds selected the forest area of dominant species for Ermans birch and dwarf Siberian pine as habitat and preferred the shrubs area with the lower height and higher coverage.
文摘The leaders of the Meiji Restoration believed in their master, Yoshida Shoin (吉田松陰), who claimed that in order for the islands of Japan not to be a colony of the powerful Western states, Japan had to conquer nearby countries. This led to Japan’s invasion of the Ryukyu Islands, Taiwan, Korea, and Manchuria, which ultimately led to the Manchurian Incident of 1931, the Sino-Japanese War, and the Pacific War. Surprisingly, the subject and the timing of each and every one of these acts of war were in the same order of Yoshida Shoin’s proposal on preoccupancy. The Sino-Japanese war of 1894 was romanticized as clearing the barbaric culture by civilization, and the Russo-Japanese war of 1904 was romanticized as the realization of Eastern Peace. However, Japanese policies of aggressions were first deemed illegal by international law during the 1931 Manchurian Incident by the investigations of the League of Nations. The Japanese Empire received the recommendation by the League of Nations to restore to original state, but declined and exited from the League of Nations. Following their exit, they started the Sino-Japanese War and the Pacific War and eventually lost in 1945. The goal of the San Francisco Peace Treaty of 1951 was to punish Japan’s aggressions. However, as the Cold War between the East and the West started to arise in 1948, the punishment was eased, and their punishment for the aggressions on the Ryukyu Islands, Taiwan, and Korea was nearly unasked for. This paper examines the issues of the San Francisco Peace Treaty in the views of the international law of the League of Nations, established by Manley O. Hudson of Harvard University and others in U.S. academia and judiciary.
文摘Seoul’s privately owned public space (POPS) initiative has achieved quantitative success since its introduction in the early 1990s. However, POPSs have been frequently criticized for failing to correspond with the urban context in which they are created and are thus often underused. Association between surrounding urban context and the use of POPS is acknowledged, but specific elements of the urban context associated with the use of POPS and the extent of association are unclear. In this study, the relationship between constituent elements of 48 POPSs including the surrounding ground floor facility use and the use of those spaces in Seoul’s central business district was measured by multiple regression analysis. The results present empirical evidence of a substantial relationship between assembly, mercantile, and business facilities on ground floors of surrounding buildings and the use of POPS. As the surrounding ground floor facility use showed an overriding relationship with the use of POPS that possibly overshadowed the influence of secondary determinants, the 48 POPSs were divided into two groups: those surrounded by commercial facilities and those surrounded by business facilities. The use of POPSs surrounded by commercial facilities was associated with additional variables including the proximity to transit stations and crosswalks. Models using the sample of POPSs surrounded by business facilities were not statistically significant. Based on these results, this paper emphasizes the importance of creating POPSs in connection with desirable surrounding facilities. This study also suggests that anticipation of the usage patterns of POPSs and establishment of guidelines supporting those patterns may be viable by investigating preexisting urban contexts.
文摘BACKGROUND As people living with human immunodeficiency virus(HIV)(PLWH)enjoy longer life expectancy with highly effective antiretroviral therapy,they are encountering challenging cardiovascular health risks.AIM To retrospectively examine the increasing burden of cardiovascular diseases in PLWH over the past decade.METHODS All hospitalizations for heart failure(HF),ischemic heart disease(IHD),and cerebrovascular disease(CeVD)in PLWH were identified using the International Classification of Diseases,Ninth Revision,Clinical Modification(ICD-9-CM)and ICD-10-CM codes in the National Inpatient Sample from 2008 to 2018.Outcomes included number of hospitalizations,in-hospital mortality,length of stay,and total hospital charge.Trend of the outcomes from 2008 to 2018 were analyzed using Cochran-Armitage_(trend)test and simple linear regression.RESULTS The number of hospitalizations for HF in PLWH increased from 4212 in 2008 to 6700 in 2018(P_(trend)<0.01).Similar increasing_(trend)was seen with those for IHD and CeVD over the decade(P_(trend)<0.01).A decreasing_(trend)of in-hospital mortality was observed in all hospitalizations of PLWH(P_(trend)<0.01)and CeVD in PLWH(P_(trend)<0.01),but not in those for HF(P_(trend)=0.67)and IHD(P_(trend)=0.13).The_(trend)of length of stay was decreasing in all hospitalizations of PLWH(P_(trend)<0.01),but increasing in those for HF in PLWH(P_(trend)<0.01).An increasing_(trend)of total hospital charge was observed in hospitalizations for HF,IHD,and CeVD(P_(trend)<0.01).CONCLUSION The burden of cardiovascular diseases has significantly increased in hospitalizations of PLWH from 2008 to 2018.Continued efforts are needed to address the additional cardiovascular risks in this vulnerable population.
文摘It has become an increased challenge for designers to define the boundaries between the university and its surrounding city. The amount of space serving as a nexus between universities and urban areas is gradually increasing. This study defines such intermediate spaces as “the university-city interface”—areas that influence the university’s physical and functional connection to the surrounding city. The research presents comparative case studies of three universities in urban contexts—Harvard University, University of Pennsylvania, and Université Catholique de Louvain— by analyzing plazas and boulevards. These representative open spaces provide interfaces for both the university campuses and their surrounding cities. This paper analyzes design elements from the perspective of campus spatial structure, as well as locations and functions, to develop a comparative checklist for plazas and boulevards. The results offer a set of urban design principles for university plazas and boulevards that could significantly improve the quality of the university-city relationship. While these checklists and principles may vary depending on circumstances, they can be useful starting points for initializing design processes.

文摘Point-of-care testing(POCT)is the practice of diagnosing and monitoring diseases where the patient is located,as opposed to traditional treatment conducted solely in a medical laboratory or other clinical setting.POCT has been less common in the recent past due to a lack of portable medical devices capable of facilitating effective medical testing.However,recent growth has occurred in this field due to advances in diagnostic technologies,device miniaturization,and progress in wearable electronics.Among these developments,electrochemical sensors have attracted interest in the POCT field due to their high sensitivity,compact size,and affordability.They are used in various applications,from disease diagnosis to health status monitoring.In this paper we explore recent advancements in electrochemical sensors,the methods of fabricating them,and the various types of sensing mechanisms that can be used.Furthermore,we delve into methods for immobilizing specific biorecognition elements,including enzymes,antibodies,and aptamers,onto electrode surfaces and how these sensors are used in real-world POCT settings.

文摘Mo_(2)C is an excellent electrocatalyst for hydrogen evolution reaction(HER).However,Mo_(2)C is a poor electrocatalyst for oxygen evolution reaction(OER).Herein,two different elements,namely Co and Fe,are incorporated in Mo_(2)C that,therefore,has a finely tuned electronic structure,which is not achievable by incorporation of any one of the metals.Consequently,the resulting electrocatalyst Co_(0.8)Fe_(0.2)-Mo_(2)C-80 displayed excellent OER catalytic performance,which is evidenced by a low overpotential of 214.0(and 246.5)mV to attain a current density of 10(and 50)mA cm^(-2),an ultralow Tafel slope of 38.4 mV dec^(-1),and longterm stability in alkaline medium.Theoretical data demonstrates that Co_(0.8)Fe_(0.2)-Mo_(2)C-80 requires the lowest overpotential(1.00 V)for OER and Co centers to be the active sites.The ultrahigh catalytic performance of the electrocatalyst is attributed to the excellent intrinsic catalytic activity due to high Brunauer-Emmett-Teller specific surface area,large electrochemically active surface area,small Tafel slope,and low chargetransfer resistance.

文摘Background Genome editing has been considered as powerful tool in agricultural fields.However,genome editing progress in cattle has not been fast as in other mammal species,for some disadvantages including long gestational periods,single pregnancy,and high raising cost.Furthermore,technically demanding methods such as microinjection and somatic cell nuclear transfer(SCNT)are needed for gene editing in cattle.In this point of view,electroporation in embryos has been risen as an alternative.Results First,editing efficiency of our electroporation methods were tested for embryos.Presence of mutation on embryo was confirmed by T7E1 assay.With first combination,mutation rates for MSTN and PRNP were 57.6%±13.7%and 54.6%±13.5%,respectively.In case of MSTN/BLG,mutation rates were 83.9%±23.6%for MSTN,84.5%±18.0%for BLG.Afterwards,the double-KO embryos were transferred to surrogates and mutation rate was identified in resultant calves by targeted deep sequencing.Thirteen recipients were transferred for MSTN/PRNP,4 calves were delivered,and one calf underwent an induction for double KO.Ten surrogates were given double-KO embryos for MSTN/BLG,and four of the six calves that were born had mutations in both genes.Conclusions These data demonstrated that production of genome edited cattle via electroporation of RNP could be effectively applied.Finally,MSTN and PRNP from beef cattle and MSTN and BLG from dairy cattle have been born and they will be valuable resources for future precision breeding.

文摘Artificial intelligence(AI)models have significantly impacted various areas of the atmospheric sciences,reshaping our approach to climate-related challenges.Amid this AI-driven transformation,the foundational role of physics in climate science has occasionally been overlooked.Our perspective suggests that the future of climate modeling involves a synergistic partnership between AI and physics,rather than an“either/or”scenario.Scrutinizing controversies around current physical inconsistencies in large AI models,we stress the critical need for detailed dynamic diagnostics and physical constraints.Furthermore,we provide illustrative examples to guide future assessments and constraints for AI models.Regarding AI integration with numerical models,we argue that offline AI parameterization schemes may fall short of achieving global optimality,emphasizing the importance of constructing online schemes.Additionally,we highlight the significance of fostering a community culture and propose the OCR(Open,Comparable,Reproducible)principles.Through a better community culture and a deep integration of physics and AI,we contend that developing a learnable climate model,balancing AI and physics,is an achievable goal.

文摘The electrochemical reduction of carbon dioxide offers a sound and economically viable technology for the electrification and decarbonization of the chemical and fuel industries.In this technology,an electrocatalytic material and renewable energy-generated electricity drive the conversion of carbon dioxide into high-value chemicals and carbon-neutral fuels.Over the past few years,single-atom catalysts have been intensively studied as they could provide near-unity atom utilization and unique catalytic performance.Single-atom catalysts have become one of the state-of-the-art catalyst materials for the electrochemical reduction of carbon dioxide into carbon monoxide.However,it remains a challenge for single-atom catalysts to facilitate the efficient conversion of carbon dioxide into products beyond carbon monoxide.In this review,we summarize and present important findings and critical insights from studies on the electrochemical carbon dioxide reduction reaction into hydrocarbons and oxygenates using single-atom catalysts.It is hoped that this review gives a thorough recapitulation and analysis of the science behind the catalysis of carbon dioxide into more reduced products through singleatom catalysts so that it can be a guide for future research and development on catalysts with industry-ready performance for the electrochemical reduction of carbon dioxide into high-value chemicals and carbon-neutral fuels.

文摘Inspired by the overlapping structure of snake scales,a reinforced scale-like knitted fabric(R-SLKF)was created in this work.To achieve this,short carbon fibers in an epoxy resin(ER)matrix were incorporated into the scales of an SLKF.The resulting textile is a highly stable protective composite that is flexible,warm,and thermally insulated.In addition,supe-rior stab-resistance is ensured through rigid protective blocks in the R-SLKF,making up a hard overlapping scale region,besides satisfactory flexibility via soft twisted ultra-high-molecular-weight polyethylene yarn-based textiles.The R-SLKF achieves high stab resistance(peak load of approximately 600 N for a single scale thickness of 2 mm),good flexibility(~290 mN cm),and breathability(100 MPa,423 mm/s),coupled with good warmth retention and thermal insulation prop-erties(0.28℃/s),which are superior to previously reported protective composite textiles.From the results,the combination of desirable individual protection,excellent wearability and comfort enables human beings to survive in extremely danger-ous environments.Finite element simulations provided valuable insights into the factors influencing the stab resistance of R-SLKF and elucidated the underlying anti-puncture mechanism in accordance with the experimental findings.This study presents a novel strategy for the facile industrial fabrication of flexible and lightweight protective composite textiles,which is expected to enhance the structure and material design for future innovations and provide advantages for personal protec-tive equipment in various industrial fields.

文摘BACKGROUND Inflammatory bowel disease(IBD)was previously regarded as a Western disease;however,its incidence is increasing in the East.The epidemiology of IBD in Asia differs significantly from the patterns in the West.AIM To comprehensively investigate the epidemiology of IBD in South Korea,inclu-ding its incidence,prevalence,medication trends,and outcomes.METHODS We analyzed claims data from the Health Insurance Review and Assessment Service and Rare and Intractable Diseases(RIDs),operated by the National Health Insurance Service of South Korea.Patients with IBD were identified based on the International Classification of Diseases,Tenth Revision,and RID diagnostic codes for Crohn’s disease(CD)and ulcerative colitis(UC)from 2010 to 2018.RESULTS In total,14498 and 31409 patients were newly diagnosed with CD and UC,respectively,between 2010 and 2018.The annual average incidence of CD was 3.11 cases per 105 person-years,and that of UC was 6.74 cases per 10^(5) person-years.Since 2014,the incidence rate of CD has been stable,while that of UC has steadily increased,shifting the peak age group from 50-year-olds in 2010 to 20-year-olds in 2018.The CD and UC prevalence increased consistently over the study period;the use of 5-aminosali-cylates and corticosteroids gradually decreased,while that of immunomodulators and biologics steadily increased in both CD and UC.The clinical outcomes of IBD,such as hospitalization and surgery,decreased during the study period.CONCLUSION The CD incidence has been stable since 2014,but that of UC has increased with a shift to a younger age at peak incidence between 2010 and 2018.IBD clinical outcomes improved over time,with increased use of immunomodu-lators and biologics.
