Objective: To determine whether endovaginal ultrasound is a reliable measure in visualization of the sacrospinous ligament among women with prolapse versus women without prolapse, and thus might be clinically applicab...Objective: To determine whether endovaginal ultrasound is a reliable measure in visualization of the sacrospinous ligament among women with prolapse versus women without prolapse, and thus might be clinically applicable in the design of an ultrasound-guided device for performing sacrospinous ligament anchor placement as a treatment for pelvic organ prolapse. Methods: In the first phase of this study we performed a sacrospinous anchor placement in four normal fresh-frozen female pelves. Afterwards, an endovaginal ultrasound was performed to visualize the anchor localization which was validated by dissection of the cadaveric pelves. In the second phase of the study: two groups of volunteer females with and without pelvic organ prolapsed (POP-group, vs NON-POP group) were evaluated by endovaginal ultrasound to localize the sacrospinous ligament. Results: Cadaveric dissection demonstrated accurate anchor placement into the 8/8 sacrospinous ligament. We performed endovaginal ultrasound in a total of 17 N-POP and 10 (POP) patients. Among the N-POP group, the right and left ischial spines were visible in 6/17 (35%) and (6/17) 35% vs 0/10 (0%) for both right and left sides in POP group (p = 0.008). The right sacrospinous ligament was visualized in 4/17 (23%) N-POP subjects and 0/10 POP subjects (p = 0.27) and the left sacrospinous ligament was visualized in 7/17 (41%) N-POP subjects and 2/10 POP subjects (p = 0.48). Conclusions: Sacrospinous ligament and the ischial spines couldn’t be reliably visualized among women with or without pelvic organ prolapse using endovaginal ultrasound, although the structures are visualized more in some of the non-prolapsed women. The sacrospinous anchoring device demonstrated accurate placement by cadaveric dissections.展开更多

Using molecular profiling to target the molecular vulnerabilities of neuroblastoma with existing therapeutic agents may result in a rational, data-driven approach with potential to improve clinical outcomes. Methods: The primary objective of this pilot study was to evaluate the feasibility of supporting real-time treatment decisions through predictive modeling of genome-wide mRNA gene expression data from neuroblastoma tumor biopsies. Feasibility was defined as completion of tumor biopsy, histopathological evaluation, RNA extraction and quality control, gene expression profiling within a CLIA-certified laboratory, bioinformatic analysis, generation of a drug predicttion report, molecular tumor board review yielding a formulated treatment plan, and independent medical monitor review within a 2-week period. Results: Five patients with multiply relapsed or refractory neuroblastoma were enrolled between April and June 2010. All biopsies passed histopathology and RNA quality control. Generation of gene expression data and its analysis (3-7 days), reports which linked this data into medically actionable drug candidates (1-5 days), molecular tumor board (1-3 days) and independent medical monitor review (1 day) were all completed in real-time. The average time was 10.5 days for all patients. Conclusion: This study shows that it is feasible to create therapeutic treatment plans based on genomic profiling in less than 12 days. This warrants further testing in a Phase I study to determine safety of predicted treatments and evaluate whether the information obtained in these analyses would result in patient benefit.展开更多

The impetus for buildings to decarbonize and move towards radical energy and water efficiency is increasingly strong and identified as a priority within the green building sector.The tiny house movement offers an oppo...The impetus for buildings to decarbonize and move towards radical energy and water efficiency is increasingly strong and identified as a priority within the green building sector.The tiny house movement offers an opportunity to both address the challenges of affordable housing and contribute to residential building decarbonization.Tiny houses de-emphasize mass consumption and excessive belongings and have potential to address equity issues such as gentrification by providing living spaces to low-income residents in desirable housing locations.This paper analyzes the Tiny House in My Backyard(THIMBY)project,investigating building sustainability concepts through the design-build-occupy process in a three-year-old structure.THIMBY demonstrates energy and water efficiency technologies inside an award-winning small living space(18.5 m^(2)).THIMBY was designed to reduce energy and water use by 87 and 82%compared to California residential averages.In practice,it has reduced site energy by 88%and has emitted 96%fewer carbon emissions than a 2100 square foot California Energy Commission 2016 Title 24 minimally compliant home.We discuss the differences between design and performance of energy and water systems,which we find offer important lessons for the further expansion of the tiny house movement and other alternative and micro green housing types.We find that optimizing such houses through integration of energy and water saving technologies,home energy management systems,and strong communication between modelers,builders and occupants will be essential to achieving dramatic energy(87%),water(82%),and carbon(96%)savings.展开更多
AIM: To report the indications for and techniques of corneal transplantation at Vietnam National Institute of Ophthalmology (VNIO) over a period of 12y (2002-2013), METHODS: Records of patients who had undergone...AIM: To report the indications for and techniques of corneal transplantation at Vietnam National Institute of Ophthalmology (VNIO) over a period of 12y (2002-2013), METHODS: Records of patients who had undergone corneal transplantation at VNIO from January 1, 2002 to January 1, 2014 were reviewed to determine the indication for and type of corneal transplant performed. Patient age, gender, indication for corneal transplantation and surgical technique were recorded and analyzed. RESULTS: Corneal transplantation were underwent in 1390 eyes of 1278 patients with a mean age of 44.9±18.1y during the period under review. The most common indication was infectious corneal ulcer (n=670; 48.2%), followed by corneal scar (n=333, 24.0% ), corneal dystrophy (n=138, 9.9%) and failed graft (n=112, 8.1%). Nearly all procedures performed were penetrating keratoplasty (n=1300, 93.5% ), with a few lamellar keratoplasty procedures performed: lamellar keratoplasty (n=52, 3.7%), Descemet's stripping automated endothelial keratoplasty (n =27, 1.9% ) and deep anterior lamellar keratoplasty (n =11, 0.8%).CONCLUSION: While the most common indication for keratoplasty was infectious keratitis, nearly all indications for corneal transplantation were managed with penetrating keratoplasty, However, lamellar keratoplasty techniques, including deep anterior lamellar keratoplasty and Descemet's stripping automated endothelial keratoplasty, are being performed with increasing frequency for isolated stromal and endothelial disorders, respectively,展开更多

Breakthroughs in the natural and behavioural sciences, political ideology, and education come from evidence for {1} the Mebir, {2} the theorem, humans are loving, non-competitive, and non-aggressive (in part from two new theories of evolution), and {3} the Mebir providing social species with cultural and evolutionary freedom (e.g. the social brain hypothesis is substantiated). Other social species (including humans and songbirds), and possibly “non-social” species (e.g., including domestic cats) likely undergo a Mebir. Social species more frequently undergo social interactions suggesting they more frequently undergo the Mebir, and therefore have greater cultural and evolutionary freedom. Two hypotheses were tested and proven invalid. Friendly greetings do not make dogs more effective in behaviour (nor do they invoke innate hunting behaviours as a displaced innate response (that in wolves improve fitness)) (N = 50), and do not make dogs walk faster (N = 29). Data are inclusive of various dog breeds, mixed breeds, both sexes, and all ages from chance encounters with dogs being walked on a sidewalk in a naturalistic setting. Three synchronous greetings were used to elicit the Mebir in dogs, eye contact, friendly verbal greetings, and handwaves. Behavioural responses were commonly exaggerated but weak for certain individuals. Responses included heightened curiosity (about objects and the environment), courageous exploration, creative and playful behaviour, assertiveness (e.g., in pulling on the leash), “pronounced social affiliation seeking” with dogs, pedestrians, and the dog’s owner, heightened aggressiveness (when territorial), and perhaps heightened concentration. Past research indicates that dog learning is improved from social interactions with humans.展开更多

The antimicrobial properties of host-derived derived lipids have become increasingly recognized and evidence is mounting that antimicrobial lipids (AMLs), like antimicrobial peptides, are effector molecules of the i...The antimicrobial properties of host-derived derived lipids have become increasingly recognized and evidence is mounting that antimicrobial lipids (AMLs), like antimicrobial peptides, are effector molecules of the innate immune system and are regulated by its conserved pathways. This review, with primary focus on the human body, provides some background on the biochemistry of lipids, summarizes their biological functions, expands on their antimicrobial properties and site-specifc composition, presents modes of synergism with antimicrobial peptides, and highlights the more recent reports on the regulation of AML production as well as bacterial resistance mechanisms. Based on extant data a concept of innate epithelial defense is proposed where epithelial cells, in response to microbial products and proinflammatory cytokines and through activation of conserved innate signaling pathways, increase their lipid uptake and up-regulate transcription of enzymes involved in antimicrobial lipid biosynthesis, and induce transcription of antimicrobial peptides as well as cytokines and chemokines. The subsequently secreted antimicrobial peptides and lipids then attack and eliminate the invader, assisted by or in synergism with other antimicrobial molecules delivered by other defense cells that have been recruited to the site of infection, in most of the cases. This review invites reconsideration of the interpretation of cholesteryl ester accumulation in macrophage lipid droplets in response to infection as a solely proinflammatory event, and proposes a direct antimicrobial role of lipid droplet- associated cholesteryl esters. Finally, for the interested, but new- to- the-feld investigator some starting points for the characterization of AMLs are provided. Before it is possible to utilize AMLs for anti-infectious therapeutic and prophylactic approaches, we need to better understand pathogen responses to these lipids and their role in the pathogenesis of chronic infectious disease.展开更多
A fluid-structure interaction approach is proposed in this paper based onNon-Ordinary State-Based Peridynamics(NOSB-PD)and Updated Lagrangian Particle Hydrodynamics(ULPH)to simulate the fluid-structure interaction pro...A fluid-structure interaction approach is proposed in this paper based onNon-Ordinary State-Based Peridynamics(NOSB-PD)and Updated Lagrangian Particle Hydrodynamics(ULPH)to simulate the fluid-structure interaction problem with large geometric deformation and material failure and solve the fluid-structure interaction problem of Newtonian fluid.In the coupled framework,the NOSB-PD theory describes the deformation and fracture of the solid material structure.ULPH is applied to describe the flow of Newtonian fluids due to its advantages in computational accuracy.The framework utilizes the advantages of NOSB-PD theory for solving discontinuous problems and ULPH theory for solving fluid problems,with good computational stability and robustness.A fluidstructure coupling algorithm using pressure as the transmission medium is established to deal with the fluidstructure interface.The dynamic model of solid structure and the PD-ULPH fluid-structure interaction model involving large deformation are verified by numerical simulations.The results agree with the analytical solution,the available experimental data,and other numerical results.Thus,the accuracy and effectiveness of the proposed method in solving the fluid-structure interaction problem are demonstrated.The fluid-structure interactionmodel based on ULPH and NOSB-PD established in this paper provides a new idea for the numerical solution of fluidstructure interaction and a promising approach for engineering design and experimental prediction.展开更多

文摘Natural convection is a heat transfer mechanism driven by temperature or density differences,leading to fluid motion without external influence.It occurs in various natural and engineering phenomena,influencing heat transfer,climate,and fluid mixing in industrial processes.This work aims to use the Updated Lagrangian Particle Hydrodynamics(ULPH)theory to address natural convection problems.The Navier-Stokes equation is discretized using second-order nonlocal differential operators,allowing a direct solution of the Laplace operator for temperature in the energy equation.Various numerical simulations,including cases such as natural convection in square cavities and two concentric cylinders,were conducted to validate the reliability of the model.The results demonstrate that the proposed model exhibits excellent accuracy and performance,providing a promising and effective numerical approach for natural convection problems.
文摘The impetus for buildings to decarbonize and move towards radical energy and water efficiency is increasingly strong and identified as a priority within the green building sector.The tiny house movement offers an opportunity to both address the challenges of affordable housing and contribute to residential building decarbonization.Tiny houses de-emphasize mass consumption and excessive belongings and have potential to address equity issues such as gentrification by providing living spaces to low-income residents in desirable housing locations.This paper analyzes the Tiny House in My Backyard(THIMBY)project,investigating building sustainability concepts through the design-build-occupy process in a three-year-old structure.THIMBY demonstrates energy and water efficiency technologies inside an award-winning small living space(18.5 m^(2)).THIMBY was designed to reduce energy and water use by 87 and 82%compared to California residential averages.In practice,it has reduced site energy by 88%and has emitted 96%fewer carbon emissions than a 2100 square foot California Energy Commission 2016 Title 24 minimally compliant home.We discuss the differences between design and performance of energy and water systems,which we find offer important lessons for the further expansion of the tiny house movement and other alternative and micro green housing types.We find that optimizing such houses through integration of energy and water saving technologies,home energy management systems,and strong communication between modelers,builders and occupants will be essential to achieving dramatic energy(87%),water(82%),and carbon(96%)savings.
文摘AIM: To report the indications for and techniques of corneal transplantation at Vietnam National Institute of Ophthalmology (VNIO) over a period of 12y (2002-2013), METHODS: Records of patients who had undergone corneal transplantation at VNIO from January 1, 2002 to January 1, 2014 were reviewed to determine the indication for and type of corneal transplant performed. Patient age, gender, indication for corneal transplantation and surgical technique were recorded and analyzed. RESULTS: Corneal transplantation were underwent in 1390 eyes of 1278 patients with a mean age of 44.9±18.1y during the period under review. The most common indication was infectious corneal ulcer (n=670; 48.2%), followed by corneal scar (n=333, 24.0% ), corneal dystrophy (n=138, 9.9%) and failed graft (n=112, 8.1%). Nearly all procedures performed were penetrating keratoplasty (n=1300, 93.5% ), with a few lamellar keratoplasty procedures performed: lamellar keratoplasty (n=52, 3.7%), Descemet's stripping automated endothelial keratoplasty (n =27, 1.9% ) and deep anterior lamellar keratoplasty (n =11, 0.8%).CONCLUSION: While the most common indication for keratoplasty was infectious keratitis, nearly all indications for corneal transplantation were managed with penetrating keratoplasty, However, lamellar keratoplasty techniques, including deep anterior lamellar keratoplasty and Descemet's stripping automated endothelial keratoplasty, are being performed with increasing frequency for isolated stromal and endothelial disorders, respectively,

文摘Casein kinase I is a group of ubiquitous Serine/Threonine kinases that have been implicated in both normal cellular functions and several pathological conditions including Alzheimer's disease and cancer.Recent findings in colon and pancreatic cancer have brought tremendous attention to these molecules as potential therapeutic targets in treatment of digestive cancers.In this review,we summarize up to date what is known about this family of kinases and their involvement in carcinogenesis and other pathological conditions.Our emphasis is on their implications in digestive cancers and their potential for cancer screening and therapy.

文摘The microstructure of crystal defects,e.g.,dislocation patterns,are not arbitrary,and it is possible that some of them may be related to the microstructure of crystals itself,i.e.,the lattice structure.We call those dislocation patterns or substructures that are related to the corresponding crystal microstructure as the Geometrically Compatible Dislocation Patterns(GCDP).Based on this notion,we have developed a Multiscale Crystal Defect Dynamics(MCDD)to model crystal plasticity without or with minimum empiricism.In this work,we employ the multiscale dislocation pattern dynamics,i.e.,MCDD,to simulate crystal plasticity in body-centered cubic(BCC)single crystals,mainlyα-phase Tantalum(α-Ta)single crystals.The main novelties of the work are:(1)We have successfully simulated crystal plasticity at micron scale without any empirical parameter inputs;(2)We have successfully employed MCDD to perform direct numerical simulation of inelastic hysteresis of the BCC crystal;(3)We have used MCDD crystal plasticity model to demonstrate the size-effect of crystal plasticity and(4)We have captured cross-slip which may lead to size-effect.

文摘Platelet inhibition with aspirin is the cornerstone of medical therapy for coronary artery disease.In the era of percutaneous coronary intervention with drug-eluting stents,dual anti-platelet therapy with the addition of clopidogrel(Plavix,Bristol-Myers Squibb,New York,NY)became the mainstay of therapy.However,with the advent of newer oral antiplatelet medications,including prasugrel(Effient?,Eli Lilly,Indianapolis,Indiana)and ticagrelor(Brilinta?,Astra-Zeneca,Cambridge,United Kingdom),choosing the appropriate platelet inhibitor has become more nuanced.The optimal regimen differs based on patient characteristics,clinical situation,and the condition being treated,with differing risk and safety profiles for these medications.This review explores the appropriate use of antiplatelet therapy for stable ischemic heart disease,acute coronary syndrome,stroke,and peripheral vascular disease.Furthermore,we evaluate the data behind the use of antiplatelet therapy in patients on oral anticoagulation.By understanding the biochemistry of platelet aggregation,the pharmacology of platelet inhibitors,and the extensive clinical trial data that informs current guideline recommendations we aim to better understand the role of clopidogrel in patients with cardiovascular disease.

文摘Canus lupus familiais (domestic dogs) tend to elicit from (the sign stimulus of) a friendly greeting, improved effectiveness of behaviour, an (observable) innate, likely hormonal, (possibly oxytocin) mediated response (a Mebir). Breakthroughs in the natural and behavioural sciences, political ideology, and education come from evidence for {1} the Mebir, {2} the theorem, humans are loving, non-competitive, and non-aggressive (in part from two new theories of evolution), and {3} the Mebir providing social species with cultural and evolutionary freedom (e.g. the social brain hypothesis is substantiated). Other social species (including humans and songbirds), and possibly “non-social” species (e.g., including domestic cats) likely undergo a Mebir. Social species more frequently undergo social interactions suggesting they more frequently undergo the Mebir, and therefore have greater cultural and evolutionary freedom. Two hypotheses were tested and proven invalid. Friendly greetings do not make dogs more effective in behaviour (nor do they invoke innate hunting behaviours as a displaced innate response (that in wolves improve fitness)) (N = 50), and do not make dogs walk faster (N = 29). Data are inclusive of various dog breeds, mixed breeds, both sexes, and all ages from chance encounters with dogs being walked on a sidewalk in a naturalistic setting. Three synchronous greetings were used to elicit the Mebir in dogs, eye contact, friendly verbal greetings, and handwaves. Behavioural responses were commonly exaggerated but weak for certain individuals. Responses included heightened curiosity (about objects and the environment), courageous exploration, creative and playful behaviour, assertiveness (e.g., in pulling on the leash), “pronounced social affiliation seeking” with dogs, pedestrians, and the dog’s owner, heightened aggressiveness (when territorial), and perhaps heightened concentration. Past research indicates that dog learning is improved from social interactions with humans.

文摘The antimicrobial properties of host-derived derived lipids have become increasingly recognized and evidence is mounting that antimicrobial lipids (AMLs), like antimicrobial peptides, are effector molecules of the innate immune system and are regulated by its conserved pathways. This review, with primary focus on the human body, provides some background on the biochemistry of lipids, summarizes their biological functions, expands on their antimicrobial properties and site-specifc composition, presents modes of synergism with antimicrobial peptides, and highlights the more recent reports on the regulation of AML production as well as bacterial resistance mechanisms. Based on extant data a concept of innate epithelial defense is proposed where epithelial cells, in response to microbial products and proinflammatory cytokines and through activation of conserved innate signaling pathways, increase their lipid uptake and up-regulate transcription of enzymes involved in antimicrobial lipid biosynthesis, and induce transcription of antimicrobial peptides as well as cytokines and chemokines. The subsequently secreted antimicrobial peptides and lipids then attack and eliminate the invader, assisted by or in synergism with other antimicrobial molecules delivered by other defense cells that have been recruited to the site of infection, in most of the cases. This review invites reconsideration of the interpretation of cholesteryl ester accumulation in macrophage lipid droplets in response to infection as a solely proinflammatory event, and proposes a direct antimicrobial role of lipid droplet- associated cholesteryl esters. Finally, for the interested, but new- to- the-feld investigator some starting points for the characterization of AMLs are provided. Before it is possible to utilize AMLs for anti-infectious therapeutic and prophylactic approaches, we need to better understand pathogen responses to these lipids and their role in the pathogenesis of chronic infectious disease.

文摘A fluid-structure interaction approach is proposed in this paper based onNon-Ordinary State-Based Peridynamics(NOSB-PD)and Updated Lagrangian Particle Hydrodynamics(ULPH)to simulate the fluid-structure interaction problem with large geometric deformation and material failure and solve the fluid-structure interaction problem of Newtonian fluid.In the coupled framework,the NOSB-PD theory describes the deformation and fracture of the solid material structure.ULPH is applied to describe the flow of Newtonian fluids due to its advantages in computational accuracy.The framework utilizes the advantages of NOSB-PD theory for solving discontinuous problems and ULPH theory for solving fluid problems,with good computational stability and robustness.A fluidstructure coupling algorithm using pressure as the transmission medium is established to deal with the fluidstructure interface.The dynamic model of solid structure and the PD-ULPH fluid-structure interaction model involving large deformation are verified by numerical simulations.The results agree with the analytical solution,the available experimental data,and other numerical results.Thus,the accuracy and effectiveness of the proposed method in solving the fluid-structure interaction problem are demonstrated.The fluid-structure interactionmodel based on ULPH and NOSB-PD established in this paper provides a new idea for the numerical solution of fluidstructure interaction and a promising approach for engineering design and experimental prediction.
