Rotationplasty type BIIIb as an effective alternative to limb salvage procedure in adults:Two case reports

BACKGROUND Rotationplasty is often performed for malignant tumors,but type BIIIb rotationplasty is rarely reported,and there needs to be more evidence of the procedure and treatment.The purpose of this case study was to report a new direction in the use of type BIIIb rotationplasty in treating patients with limb salvage and longterm non-healing infections.CASE SUMMARY Case 1:A 47-year-old man underwent radiotherapy for hemangioendothelioma in his left thigh,resulting in a femoral fracture.Despite the use of plates,intramedullary nailing,and external fixators,the femoral bone failed to unite due to infectious nonunion.Multiple operations were unable to control the infection,leaving the patient immobile.We performed a modified tibia-pelvic-constrained hip rotationplasty,utilizing a constrained prosthetic hip between the tibia and pelvis following a femur resection.Two years post-surgery,the patient was able to walk with the prosthetic device without any signs of recurring infection.The corresponding functional scores were 72 points for the Musculoskeletal Tumor Society(MSTS),53 for the Functional Mobility Assessment(FMA),93 for the Toronto Extremity Salvage Score(TESS),and 56 for the MOS 36-item short form health survey(SF-36).Case 2:A 59-year-old woman presented with liposarcoma in her left thigh.Magnetic resonance imaging revealed tumors in the medial,anterior,and posterior femur muscles,encircling the femoral vessels and nerves.Fortunately,there were no symptoms of sciatic dysfunction,and the tumor had not invaded the sciatic nucleus.After one year of follow-up,the patient expressed satisfaction with limb preservation post-type BIIIb rotationplasty.The corresponding functional scores were 63 points for the MSTS,47 for the FMA,88 for the TESS,and 52 for the SF-36.CONCLUSION Our study suggests that type BIIIb rotationplasty may be an alternative to amputation in patients with incurable infections.For malignant tumors of the lower extremities without invasion of the sciatic nerve,type BIIIb rotationplasty remains an excellent alternative to amputation.This surgical method may prevent amputation,improve functional outcomes,and facilitate biological reconstruction. 展开更多

Argonaute(AGO)proteins are the core of the RNA-induced gene silencing complex which regulate a wide variety of processes in plants,from organ development to abiotic stress responses.They have been identified in many plants,but little is known in longan(Dimocarpus longan Lour.),and how AGO functions in the signaling pathways in plant embryos in response to changing environmental stimuli remains unclear.In the present research,a genome-wide analysis of the AGO gene family members and their roles in somatic embryogenesis(SE),zygotic embryogenesis(ZE),tissue developmental processes,and responses to hormones,light and abiotic stress in longan were conducted.Ten longan AGO genes were identified genome-wide and divided into four clades.They were distributed on chromosomes 1,4,8,10,12,13,14,and 15,and had 2–23 introns.The expression profiling implied that DlAGOs regulated early and middle embryogenesis,as well as developmental processes of seed,flower,and stem in longan.In addition,the transcript levels of DlAGOs in response to exogenous hormones,light and abiotic stress showed differences in expression patterns.These results provide the useful information for further elucidation of RNAi-mediated gene silencing in longan embryogenic callus(EC). 展开更多

Objective To predict the molecular mechanism of Dihuang(Rehmanniae Radix)in the treatment of diabetic nephropathy(DN)complicated with depression based on network pharmacology.Methods The components of Dihuang(Rehmanniae Radix)were identified from the Integrated Pharmacology-based Research Platform of Traditional Chinese Medicine(TCMIP),Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP),and relevant literature.The component targets were detected by combining the SwissTargetPrediction and Pub Chem databases.Disease targets were collected from the Therapeutic Target Database(TTD),Dis Ge NET,and Ensembl databases with“diabetic nephropathy”and“depression”as keywords.The disease-component targets were mapped using Venny 2.1.0 to obtain potential targets.A protein-protein interaction(PPI)network was constructed using the Search Tool for the Retrieval of Interacting Genes/Proteins(STRING)database and Cytoscape 3.7.2.The co-expression genes of the key targets were collected based on the COXPRESdb 7.3.Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis were performed for potential targets using R language.Target-component docking was verified and evaluated using Discovery Studio 4.5.Results According to the databases and literature reports,Dihuang(Rehmanniae Radix)contained 65 active components,and had 155 related targets for the treatment of DN complicated with depression.PPI screening showed that the key targets included serine/threonine protein kinase 1(AKT1),signal transducer and activator transcription 3(STAT3),interleukin 6(IL-6),mitogen-activated protein kinase 1(MAPK1),and vascular endothelial growth factor A(VEGFA),etc.GO enrichment analysis mainly involved biological processes,such as lipid metabolism,protein secretion regulation,cell homeostasis,and phosphatidylinositol 3 kinase activity.KEGG pathway enrichment analysis included the role of the AGE-RAGE signaling pathway in diabetic complements,insulin resistance(IR),neurotrophin signal path,Toll-like receptor signaling pathway,relaxin signaling pathway,epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKIs),etc.Molecular docking showed that the target had high affinity for stachyose,manninotriose,verbascose,nigerose,etc.Conclusion Based on network parmacology,this study preliminarily predict the effects of Dihuang(Rehmanniae Radix)in treating DN complicated with depression by regulating inflammation,glucose metabolism,nution nerve,etc. 展开更多

The title compound phenyl(6-phenyl-3-p-tolyl-6,7-dihydro-5 H-[1,2,4]triazolo[3,4-b][1,3,4]thiadiazin-7-yl)methanone(PTM),designed using 4-amino-3-p-tolyl-1H-1,2,4-triazole-5(4 H)-thione as the starting material,was successfully synthesized via a two-step synthesis route and finally characterized by NMR,FT-IR,single-crystal X-ray diffraction and mass spectrometry techniques.The crystal structure of trans-PTM was obtained from X-ray diffraction:C24H20N4OS,Mr=412.52,monoclinic system,space group P21/c,a=16.650(3),b=13.876(3),c=8.812(2)A,β=100.340(3)°,V=2002.8(7)A^3,F(000)=865,Z=4,Dc=1.3680 g/cm^3,λ=0.71073A,μ=0.186 mm^-1 and the final R=0.0786 for 3514 unique reflections with 2044 observed ones(Ⅰ>2σ(Ⅰ)).The biological activities of antimicrobial and regulation of plant growth of PTM were investigated.The results indicated that PTM showed weak antimicrobial activity on Bacillus pumilus,while promoted the growth of radish and inhibited that of wheat in a dose-dependent manner.Therefore,PTM may be developed as a potential drug to promote the growth of dicotyledonous plants or as an herbicide to inhibit that of monocotyledonous plants in the future. 展开更多
