Epidemiological and clinical features,treatment status,and economic burden of traumatic spinal cord injury in China:a hospital-based retrospective study

Traumatic spinal cord injury is potentially catastrophic and can lead to permanent disability or even death.China has the largest population of patients with traumatic spinal cord injury.Previous studies of traumatic spinal cord injury in China have mostly been regional in scope;national-level studies have been rare.To the best of our knowledge,no national-level study of treatment status and economic burden has been performed.This retrospective study aimed to examine the epidemiological and clinical features,treatment status,and economic burden of traumatic spinal cord injury in China at the national level.We included 13,465 traumatic spinal cord injury patients who were injured between January 2013 and December 2018 and treated in 30 hospitals in 11 provinces/municipalities representing all geographical divisions of China.Patient epidemiological and clinical features,treatment status,and total and daily costs were recorded.Trends in the percentage of traumatic spinal cord injuries among all hospitalized patients and among patients hospitalized in the orthopedic department and cost of care were assessed by annual percentage change using the Joinpoint Regression Program.The percentage of traumatic spinal cord injuries among all hospitalized patients and among patients hospitalized in the orthopedic department did not significantly change overall(annual percentage change,-0.5%and 2.1%,respectively).A total of 10,053(74.7%)patients underwent surgery.Only 2.8%of patients who underwent surgery did so within 24 hours of injury.A total of 2005(14.9%)patients were treated with high-dose(≥500 mg)methylprednisolone sodium succinate/methylprednisolone(MPSS/MP);615(4.6%)received it within 8 hours.The total cost for acute traumatic spinal cord injury decreased over the study period(-4.7%),while daily cost did not significantly change(1.0%increase).Our findings indicate that public health initiatives should aim at improving hospitals’ability to complete early surgery within 24 hours,which is associated with improved sensorimotor recovery,increasing the awareness rate of clinical guidelines related to high-dose MPSS/MP to reduce the use of the treatment with insufficient evidence. 展开更多

The first record of abnormal body coloration in Sebastes koreanus Kim and Lee,1994,from the Yellow Sea of China,was documented based on morphological characteristics and DNA barcoding.The two rockfish specimens were collected from the coastal waters of Qingdao,China,and the whole body and all fins of them were red.Of the two red-colored rockfish,there were tiny deep red spots on each fin,2 red radial stripes behind and below the eyes and 1 large deep red blotch on the opercula,while the similar stripe and spot patterns are also present in the S.koreanus specimens with normal body coloration.The countable characteristics of the two specimens are in the range of the morphometry of S.koreanus.To further clarify the species identity and taxonomic status of the two specimens,DNA barcode analysis was carried out.The genetic distance between the red-colored rockfish and S.koreanus was 0,and the minimum net genetic distances between the red-colored rockfish and other Sebastes species except for S.koreanus were 3.0%,which exceeds the threshold of species delimitation.The phylogenetic analysis showed that the DNA barcoding sequences of the two red-colored rockfish clustered with the S.koreanus sequences.The above results of DNA barcode analysis also support that the two red-colored rockfish could be identified as the species of S.koreanus.The mechanism of color variation in S.koreanus is desirable for further research and the species could be an ideal model to study the color-driven speciation of the rockfishes. 展开更多

Biomimetic materials have emerged as attractive and competitive alternatives for tissue engineering(TE)and regenerative medicine.In contrast to conventional biomaterials or synthetic materials,biomimetic scaffolds based on natural biomaterial can offer cells a broad spectrum of biochemical and biophysical cues that mimic the in vivo extracellular matrix(ECM).Additionally,such materials have mechanical adaptability,micro-structure interconnectivity,and inherent bioactivity,making them ideal for the design of living implants for specific applications in TE and regenerative medicine.This paper provides an overview for recent progress of biomimetic natural biomaterials(BNBMs),including advances in their preparation,functionality,potential applications and future challenges.We highlight recent advances in the fabrication of BNBMs and outline general strategies for functionalizing and tailoring the BNBMs with various biological and physicochemical characteristics of native ECM.Moreover,we offer an overview of recent key advances in the functionalization and applications of versatile BNBMs for TE applications.Finally,we conclude by offering our perspective on open challenges and future developments in this rapidly-evolving field. 展开更多

BACKGROUND Acute pancreatitis(AP)encompasses a spectrum of pancreatic inflammatory conditions,ranging from mild inflammation to severe pancreatic necrosis and multisystem organ failure.Given the challenges associated with obtaining human pancreatic samples,research on AP predominantly relies on animal models.In this study,we aimed to elucidate the fundamental molecular mechanisms underlying AP using various AP models.AIM To investigate the shared molecular changes underlying the development of AP across varying severity levels.METHODS AP was induced in animal models through treatment with caerulein alone or in combination with lipopolysaccharide(LPS).Additionally,using Ptf1αto drive the specific expression of the hM3 promoter in pancreatic acinar cells transgenic C57BL/6J-hM3/Ptf1α(cre)mice were administered Clozapine N-oxide to induce AP.Subsequently,we conducted RNA sequencing of pancreatic tissues and validated the expression of significantly different genes using the Gene Expression Omnibus(GEO)database.RESULTS Caerulein-induced AP showed severe inflammation and edema,which were exacerbated when combined with LPS and accompanied by partial pancreatic tissue necrosis.Compared with the control group,RNA sequencing analysis revealed 880 significantly differentially expressed genes in the caerulein model and 885 in the caerulein combined with the LPS model.Kyoto Encyclopedia of Genes and Genomes enrichment analysis and Gene Set Enrichment Analysis indicated substantial enrichment of the TLR and NOD-like receptor signaling pathway,TLR signaling pathway,and NF-κB signaling pathway,alongside elevated levels of apoptosis-related pathways,such as apoptosis,P53 pathway,and phagosome pathway.The significantly elevated genes in the TLR and NOD-like receptor signaling pathways,as well as in the apoptosis pathway,were validated through quantitative real-time PCR experiments in animal models.Validation from the GEO database revealed that only MYD88 concurred in both mouse pancreatic tissue and human AP peripheral blood,while TLR1,TLR7,RIPK3,and OAS2 genes exhibited marked elevation in human AP.The genes TUBA1A and GADD45A played significant roles in apoptosis within human AP.The transgenic mouse model hM3/Ptf1α(cre)successfully validated significant differential genes in the TLR and NOD-like receptor signaling pathways as well as the apoptosis pathway,indicating that these pathways represent shared pathological processes in AP across different models.CONCLUSION The TLR and NOD receptor signaling pathways play crucial roles in the inflammatory progression of AP,notably the MYD88 gene.Apoptosis holds a central position in the necrotic processes of AP,with TUBA1A and GADD45A genes exhibiting prominence in human AP. 展开更多

Finesse is a critical parameter for describing the characteristics of an optical enhancement cavity(OEC). This paper first presents a review of finesse measurement techniques, including a comparative analysis of the advantages, disadvantages, and potential limitations of several main methods from both theoretical and practical perspectives. A variant of the existing method called the free spectral range(FSR) modulation method is proposed and compared with three other finesse measurement methods, i.e., the fast-switching cavity ring-down(CRD) method, the rapidly swept-frequency(SF) CRD method, and the ringing effect method. A high-power OEC platform with a high finesse of approximately 16000 is built and measured with the four methods. The performance of these methods is compared, and the results show that the FSR modulation method and the fast-switching CRD method are more suitable and accurate than the other two methods for high-finesse OEC measurements. The CRD method and the ringing effect method can be implemented in open loop using simple equipment and are easy to perform. Additionally, recommendations for selecting finesse measurement methods under different conditions are proposed, which benefit the development of OEC and its applications. 展开更多

Copper is a microelement with important physiological functions in the body.However,the excess copper ion(Cu^(2+))may cause severe health problems,such as hair cell apoptosis and the resultant hearing loss.Therefore,the assay of Cu^(2+)is important.We integrate ionic imprinting technology(IIT)and structurally colored hydrogel beads to prepare chitosan-based ionically imprinted hydrogel beads(IIHBs)as a low-cost and high-specificity platform for Cu^(2+)detection.The IIHBs have a macroporous microstructure,uniform size,vivid structural color,and magnetic responsiveness.When incubated in solution,IIHBs recognize Cu^(2+)and exhibit a reflective peak change,thereby achieving label-free detection.In addition,benefiting from the IIT,the IIHBs display good specificity and selectivity and have an imprinting factor of 19.14 at 100μmol·L^(-1).These features indicated that the developed IIHBs are promising candidates for Cu^(2+)detection,particularly for the prevention of hearing loss. 展开更多

Lung cancer is a malady of the lungs that gravely jeopardizes human health.Therefore,early detection and treatment are paramount for the preservation of human life.Lung computed tomography(CT)image sequences can explicitly delineate the pathological condition of the lungs.To meet the imperative for accurate diagnosis by physicians,expeditious segmentation of the region harboring lung cancer is of utmost significance.We utilize computer-aided methods to emulate the diagnostic process in which physicians concentrate on lung cancer in a sequential manner,erect an interpretable model,and attain segmentation of lung cancer.The specific advancements can be encapsulated as follows:1)Concentration on the lung parenchyma region:Based on 16-bit CT image capturing and the luminance characteristics of lung cancer,we proffer an intercept histogram algorithm.2)Focus on the specific locus of lung malignancy:Utilizing the spatial interrelation of lung cancer,we propose a memory-based Unet architecture and incorporate skip connections.3)Data Imbalance:In accordance with the prevalent situation of an overabundance of negative samples and a paucity of positive samples,we scrutinize the existing loss function and suggest a mixed loss function.Experimental results with pre-existing publicly available datasets and assembled datasets demonstrate that the segmentation efficacy,measured as Area Overlap Measure(AOM)is superior to 0.81,which markedly ameliorates in comparison with conventional algorithms,thereby facilitating physicians in diagnosis. 展开更多

In view of the problem that a single modeling method cannot predict the distribution of microfacies, a new idea of coupling modeling method to comprehensively predict the distribution of sedimentary microfacies was proposed, breaking the tradition that different sedimentary microfacies used the same modeling method in the past. Because different sedimentary microfacies have different distribution characteristics and geometric shapes, it is more accurate to select different simulation methods for prediction. In this paper, the coupling modeling method was to establish the distribution of sedimentary microfacies with simple geometry through the point indicating process simulation, and then predict the microfacies with complex spatial distribution through the sequential indicator simulation method. Taking the DC block of Bohai basin as an example, a high-precision reservoir sedimentary microfacies model was established by the above coupling modeling method, and the model verification results showed that the sedimentary microfacies model had a high consistency with the underground. The coupling microfacies modeling method had higher accuracy and reliability than the traditional modeling method, which provided a new idea for the prediction of sedimentary microfacies. 展开更多

There is a recent increase in the worldwide prevalence of both obesity and diabetes.In this review we assessed insulin signaling,genetics,environment,lipid metabolism dysfunction and mitochondria as the major determinants in diabetes and to identify the potential mechanism of gut microbiota in diabetes diseases.We searched relevant articles,which have key information from laboratory experiments,epidemiological evidence,clinical trials,experimental models,metaanalysis and review articles,in PubMed,MEDLINE,EMBASE,Google scholars and Cochrane Controlled Trial Database.We selected 144 full-length articles that met our inclusion and exclusion criteria for complete assessment.We have briefly discussed these associations,challenges,and the need for further research to manage and treat diabetes more efficiently.Diabetes involves the complex network of physiological dysfunction that can be attributed to insulin signaling,genetics,environment,obesity,mitochondria and stress.In recent years,there are intriguing findings regarding gut microbiome as the important regulator of diabetes.Valid approaches are necessary for speeding medical advances but we should find a solution sooner given the burden of the metabolic disorder―What we need is a collaborative venture that may involve laboratories both in academia and industries for the scientific progress and its application for the diabetes control. 展开更多

Background During the transition period,the insufficient dry matter intake and a sharply increased in energy consumption to produce large quantities of milk,high yielding cows would enter a negative energy balance(NEB)that causes an increase in ketone bodies(KBs)and decrease in reproduction efficiency.The excess concentrations of circulating KBs,represented byβ-hydroxybutyric acid(BHBA),could lead to oxidative damage,which potentially cause injury to follicular granulosa cells(fGCs)and delayed follicular development.Sirtuin 3(Sirt3)regulates mitochondria reactive oxygen species(mitoROS)homeostasis in a beneficial manner;however,the molecular mechanisms underlying its involvement in the BHBA-induced injury of fGCs is poorly understood.The aim of this study was to explore the protection effects and underlying mechanisms of Sirt3 against BHBA overload-induced damage of fGCs.Results Our findings demonstrated that 2.4 mmol/L of BHBA stress increased the levels of mitoROS in bovine fGCs.Further investigations identified the subsequent mitochondrial dysfunction,including an increased abnormal rate of mitochondrial architecture,mitochondrial permeability transition pore(MPTP)opening,reductions in mitochondrial membrane potential(MMP)and Ca^(2+)release;these dysfunctions then triggered the caspase cascade reaction of apoptosis in fGCs.Notably,the overexpression of Sirt3 prior to treatment enhanced mitochondrial autophagy by increasing the expression levels of Beclin-1,thus preventing BHBA-induced mitochondrial oxidative stress and mitochondrial dysfunction in fGCs.Furthermore,our data suggested that the AMPK-mTOR-Beclin-1 pathway may be involved in the protective mechanism of Sirt3 against cellular injury triggered by BHBA stimulation.Conclusions These findings indicate that Sirt3 protects fGCs from BHBA-triggered injury by enhancing autophagy,attenuating oxidative stress and mitochondrial damage.This study provides new strategies to mitigate the fGCs injury caused by excessive BHBA stress in dairy cows with ketosis. 展开更多

Heteroatom-doped carbon-based transition-metal single-atom catalysts(SACs) are promising electrocatalysts for oxygen reduction reaction(ORR). Herein, with the aid of hierarchically porous silica as hard template, a facile and general melting perfusion and mesopore-confined pyrolysis method was reported to prepare single-atomic Fe/N–S-doped carbon catalyst(FeNx/NC-S) with hierarchically porous structure and well-defined morphology. The FeNx/NC-S exhibited excellent ORR activity with a half-wave potential(E_(1/2)) of 0.92 V, and a lower overpotential of 320 mV at a current density of 10 mA cm^(-2)for OER under alkaline condition. The remarkable electrocatalysis performance can be attributed to the hierarchically porous carbon nanospheres with S doping and high content of Fe-Nx sites(up to 3.7 wt% of Fe), resulting from the nano-confinement effect of the hierarchically porous silica spheres(NKM-5) during the pyrolysis process. The rechargeable Zn-air battery with FeNx/NC-S as a cathode catalyst demonstrated a superior power density of 194.5 mW cm-2charge–discharge stability. This work highlights a new avenue to design advanced SACs for efficient sustainable energy storage and conversion. 展开更多

The clinical scenario of pediatric liver disease is becoming more intricate due to changes in the disease spectrum,in which an increasing number of inherited/metabolic liver diseases are reported,while infectious diseases show a decreasing trend.The similar clinical manifestations caused by inherited/metabolic diseases might be under-recognized or misdiagnosed due to nonspecific characteristics.A delayed visit to a doctor due to a lack of symptoms or mild symptoms at an early stage will result in late diagnosis and treatment.Moreover,limited diagnostic approaches,especially liver biopsy,are not easily accepted by pediatric patients,leading to challenges in etiological diagnosis.Liver dysfunction due to inherited/metabolic diseases is often caused by a variety of metabolites,so precision treatment is difficult;symptomatic treatment is a compelling option for inherited disorders. 展开更多

目的:探讨根治性肾输尿管切除术(radical nephroureterectomy,RNU)后高级别且淋巴结阴性上尿路尿路上皮癌(upper tract urothelial carcinoma,UTUC)患者的不同转移部位对预后的影响。方法:回顾性分析2010年2月至2020年12月355例哈尔滨... 目的:探讨根治性肾输尿管切除术(radical nephroureterectomy,RNU)后高级别且淋巴结阴性上尿路尿路上皮癌(upper tract urothelial carcinoma,UTUC)患者的不同转移部位对预后的影响。方法:回顾性分析2010年2月至2020年12月355例哈尔滨医科大学附属肿瘤医院行RNU后病理证实为高级别且淋巴结阴性UTUC患者的临床资料,行预后相关因素的单因素及Cox比例风险回归模型多因素分析。结果:355例患者中77例(21.7%)出现转移,93例(26.1%)死亡,中位随访时间为45个月,中位年龄为67岁。肝脏、骨骼和多部位转移的中位生存时间(overall survival,OS)分别为6、6和5个月。Cox比例风险回归模型多因素分析显示,膀胱内复发(intravesical recurrence,IVR)、肿瘤大小≥2 cm、肿瘤病理分期>T2期是患者转移的独立预测因素。结论:UTUC患者出现肝脏转移、骨转移和多部位转移相对较快,预后较差,而出现肺转移和淋巴结转移的预后相对较好。发生转移的晚期UTUC患者行同步放化疗、化疗联合免疫治疗的疗效优于未治疗、单独放疗和单独化疗者。 展开更多

Early fault diagnosis of bearings is crucial for ensuring safe and reliable operations.Convolutional neural networks(CNNs)have achieved significant breakthroughs in machinery fault diagnosis.However,complex and varying working conditions can lead to inter-class similarity and intra-class variability in datasets,making it more challenging for CNNs to learn discriminative features.Furthermore,CNNs are often considered“black boxes”and lack sufficient interpretability in the fault diagnosis field.To address these issues,this paper introduces a residual mixed domain attention CNN method,referred to as RMA-CNN.This method comprises multiple residual mixed domain attention modules(RMAMs),each employing one attention mechanism to emphasize meaningful features in both time and channel domains.This significantly enhances the network’s ability to learn fault-related features.Moreover,we conduct an in-depth analysis of the inherent feature learning mechanism of the attention module RMAM to improve the interpretability of CNNs in fault diagnosis applications.Experiments conducted on two datasets—a high-speed aeronautical bearing dataset and a motor bearing dataset—demonstrate that the RMA-CNN achieves remarkable results in diagnostic tasks. 展开更多

Foxtail millet(Setaria italica)is an important C4 model crop;however,due to its high-density planting and high stature,lodging at the filling stage resulted in a serious reduction in yield and quality.Therefore,it is imperative to identify and deploy the genes controlling foxtail millet plant height.In this study,we used a semi-dwarf line 263A and an elite high-stalk breeding variety,Chuang 29 to construct an F2 population to identify dwarf genes.We performed transcriptome analysis(RNA-seq)using internode tissues sampled at three jointing stages of 263A and Chuang 29,as well as bulk segregant analysis(BSA)on their F2 population.A total of 8918 differentially expressed genes(DEGs)were obtained from RNA-seq analysis,and GO analysis showed that DEGs were enriched in functions such as‘‘gibberellin metabolic process”and‘‘oxidoreductase activity”,which have previously been shown to be associated with plant height.A total 593 mutated genes were screened by BSA-seq method.One hundred and seventy-six out of the 593 mutated genes showed differential expression levels between the two parental lines,and seven genes not only showed differential expression in two or three internode tissues but also showed high genomic variation in coding regions,which indicated they play a crucial role in plant height determination.Among them,we found a gibberellin biosynthesis related GA20 oxidase gene(Seita.5G404900),which had a single-base at the third exon,leading to the frameshift mutation at 263A.Cleaved amplified polymorphic sequence assay and association analysis proved the single-base in Seita.5G404900 co-segregated with dwarf phenotype in two independent F2 populations planted in entirely different environments.Taken together,the candidate genes identified in this study will help to elucidate the genetic basis of foxtail millet plant height,and the molecular marker will be useful for marker-assisted dwarf breeding. 展开更多

The higher capacity of CO_(2)adsorption on the surface of magnesium oxide(MgO)with low-coordination O^(2-)sites would effectively enhance the catalytic reduction of CO_(2).Herein,a series of copper oxide(CuO)and MgO composites with different mass ratios have been prepared by hydrothermal method and used for photothermal synergistic catalytic reduction of CO_(2)to ethanol.The catalyst with CuO mass ratio of 1.6% shows the best yield(15.17μmol·g^(-1)·h^(-1))under 3 h Xenon lamp illumination.The improved performance is attributable to the loose nano-sheet structure,uniform dispersion of active sites,the increased specific surface area,medium-strength basicity,the high separation efficiency of electrons and holes,and the formation of Mg-O-Cu species.The synthesized CuO and MgO composites with loose nano-sheet structure facilitate the diffusion of reactants CO_(2),so an excellent CO_(2)adsorption performance can be obtained.Meanwhile,the introduction of CuO in the form of bivalence provides higher specific surface area and porosity,thus obtaining more active sites.More importantly,the Mg-O-Cu species make the donation of electrons from MgO to CO_(2)easier,resulting in the breaking of the old Mg-O bond and the formation of C-O bond,thus promoting the adsorption and conversion of CO_(2)to ethanol. 展开更多

Mg_(3)Sb_(2)has attracted intensive attention as a typical Zintl-type thermoelectric material.Despite the exceptional thermoelectric performance in n-type Mg_(3)Sb_(2),the dimensionless figure of merit(zT)of p-type Mg_(3)Sb_(2)remains lower than 1,which is mainly attributed to its inferior electrical properties.Herein,we synergistically optimize the thermoelectric properties of p-type Mg_(3)Sb_(2)materials via codoping of Cd and Ag,which were synthesized by high-energy ball milling combined with hot pressing.It is found that Cd doping not only increases the carrier mobility of p-type Mg_(3)Sb_(2),but also diminishes its thermal conductivity(κ_(tot)),with Mg_(2.85)Cd_(0.5)Sb_(2)achieving a lowκtot value of∼0.67 W m^(−1)K^(−1)at room temperature.Further Ag doping elevates the carrier concentration,so that the power factor is optimized over the entire temperature range.Eventually,a peak zT of∼0.75 at 773 K and an excellent average zT of∼0.41 over 300−773 K are obtained in Mg_(2.82)Ag_(0.03)Cd_(0.5)Sb_(2),which are∼240%and∼490%higher than those of pristine Mg_(3.4)Sb_(2),respectively.This study provides an effective pathway to synergistically improve the thermoelectric performance of p-type Mg_(3)Sb_(2)by codoping Cd and Ag,which is beneficial to the future applications of Mg_(3)Sb_(2)-based thermoelectric materials. 展开更多
