目的,探讨高龄男性原发性高血压患者高尿酸血症的患病率及相关因素。方法从广东省人民医院管理的社区高血压数据库中选取资料完整的高龄男性原发性高血压患者200例,其中80~90岁原发性高血压患者185例,年龄〉90岁原发性高血压患者15... 目的,探讨高龄男性原发性高血压患者高尿酸血症的患病率及相关因素。方法从广东省人民医院管理的社区高血压数据库中选取资料完整的高龄男性原发性高血压患者200例,其中80~90岁原发性高血压患者185例,年龄〉90岁原发性高血压患者15例。抽取空腹血检测尿酸、血生化等指标,分析高龄男性原发性高血压患者高尿酸血症患病率及分布情况,同时用logistic回归进行高尿酸血症相关因素分析。结果高龄男性原发性高血压患者尿酸为(412.21±105.23)μmol/L,总患病率为49.5%。80~90岁与年龄〉90岁高龄男性原发性高血压患者高尿酸血症患病率比较,差异无统计学意义(49.7%72546.7%,P〉0.05)。logistic回归分析显示,TC、TG、体质量指数、腰臀比、腰围为高龄男性原发性高血压患者高尿酸血症的相关影响因素(P〈0.05,P〈0.01)。结论高龄男性原发性高血压患者高尿酸血症患病率高,且高尿酸血症相关因素多为代谢综合征相关因紊。 展开更多

目的探讨高龄老年患者血清胆红素水平与颈动脉粥样硬化的相关性。方法选择2014年9月~2016年2月从社区收集临床资料完整的高龄老年患者400例,其中男性201例,女性199例,年龄80~100(85.88±4.05)岁。根据彩色多普勒超声诊断仪测定颈动... 目的探讨高龄老年患者血清胆红素水平与颈动脉粥样硬化的相关性。方法选择2014年9月~2016年2月从社区收集临床资料完整的高龄老年患者400例,其中男性201例,女性199例,年龄80~100(85.88±4.05)岁。根据彩色多普勒超声诊断仪测定颈动脉内膜中层厚度,分为硬化组200例和非硬化组200例,所有患者进行相关生化检测,Spearman相关分析和多因素logistic回归分析。结果非硬化组血清总胆红素、直接胆红素和间接胆红素明显高于硬化组,差异有统计学意义(P<0.05,P<0.01)。Spearman相关分析显示,血清总胆红素、直接胆红素和间接胆红素与颈动脉内膜中层厚度呈负相关(r=-0.799,r=-0.785,r=-0.781,P=0.000)。多因素logistic回归分析显示,血清总胆红素是颈动脉粥样硬化的保护因素(OR=0.48,95%CI:0.29~0.68,P=0.000),而年龄和收缩压是颈动脉粥样硬化的危险因素(OR=1.16,95%CI:1.08~1.31,P=0.002;OR=1.23,95%CI:1.18~1.42,P=0.004)。结论高龄老年患者血清胆红素水平与颈动脉粥样硬化呈负相关,并且是颈动脉粥样硬化的保护因素。 展开更多

背景低密度脂蛋白胆固醇(LDL-C)是冠心病的一个重要危险因素,多数国际指南一致推荐将LDL-C作为高胆固醇血症患者的主要治疗目标,但是否应同时使用LDL-C和非高密度脂蛋白胆固醇(non-HDL-C)作为心血管疾病二级预防的指标在国内外仍有争议... 背景低密度脂蛋白胆固醇(LDL-C)是冠心病的一个重要危险因素,多数国际指南一致推荐将LDL-C作为高胆固醇血症患者的主要治疗目标,但是否应同时使用LDL-C和非高密度脂蛋白胆固醇(non-HDL-C)作为心血管疾病二级预防的指标在国内外仍有争议。目的探讨non-HDL-C达标情况与冠心病患者经皮介入冠状动脉治疗(PCI)术后不良心血管事件再发的关系,并比较non-HDL-C和LDL-C达标情况对冠心病患者远期预后的预测价值。方法于2019年4月对2002—2008年因冠心病行PCI治疗的665例患者随访3年的资料进行分析,根据二级预防6个月后的血脂(non-HDL-C、LDL-C)达标情况进行分组,采用Kaplan-Meier生存曲线、Log-Rank检验和Cox比例风险回归模型分析血脂达标情况与终点事件〔主要心血管不良事件(MACE)〕的关系。结果在真实世界里,665例冠心病患者在PCI术后经过6个月的二级预防后,分别有25.71%(171/665)、27.07%(180/665)的患者达到了指南推荐LDL-C或non-HDL-C的标准,有17.29%(115/665)的患者LDL-C和non-HDL-C水平同时达标。LDL-C达标组与不达标组MACE发生率分别为32.7%(56/171)、35.8%(177/494);non-HDL-C达标组与不达标组的MACE发生率分别为25.6%(46/180)、38.6%(187/485)。LDL-C达标组与不达标组二者风险曲线未见明显差异(P=0.545);non-HDL-C达标组的MACE累积发病率明显低于non-HDL-C不达标组(P=0.010);仅non-HDL-C达标组与仅LDL-C达标组相比,差异无统计学意义(P=0.127)。采用Cox比例风险回归分析校正年龄、性别、BMI、吸烟史、冠心病类型、共病情况等传统风险因子后发现,与LDL-C达标组相比,LDL-C不达标组的HR为1.16〔95%CI(0.83,1.61),P=0.386〕;与non-HDL-C达标组相比,non-HDL-C不达标组远期MACE发生风险较高,HR为1.61〔95%CI(1.12,2.30),P=0.009〕。结论冠心病PCI术后患者远期心血管不良事件发生风险与non-HDL-C达标与否有关,而与LDL-C治疗达标情况无关,因此,non-HDL-C可能是较LDL-C更适合的冠心病PCI术后血脂异常治疗监测靶点。 展开更多

目的探讨miR-133在白大衣高血压(white-coat hypertension,WCH)诊断中的价值。方法高血压病患者35例(高血压组),WCH患者35例(WCH组),体检健康者35例(对照组).采用荧光定量PCR法检测3组miR-133表达水平,绘制ROC曲线分析miR-133诊断WCH的... 目的探讨miR-133在白大衣高血压(white-coat hypertension,WCH)诊断中的价值。方法高血压病患者35例(高血压组),WCH患者35例(WCH组),体检健康者35例(对照组).采用荧光定量PCR法检测3组miR-133表达水平,绘制ROC曲线分析miR-133诊断WCH的效能。结果 WCH组、高血压组miR-133表达水平(29.78±2.01、31.38±2.08)低于对照组(34.30±1.62),且WCH组低于高血压组,差异均有统计学意义(P<0.05):miR-133诊断WCH的AUC为0.949,最佳截断值为31.51,灵敏性为94.30%,特异性为42.90%;miR-133诊断高血压的AUC为0.713,最佳截断值为29.94,灵敏性为85.70%,特异性为60.00%;miR-133诊断WCH的灵敏性高于高血压.特异性低于高血压(P<0.05)。结论 miR-133可能在WCH发生中起一定作用,有可能作为诊断WCH的分子标志物。 展开更多

目的 先前国内外实验研究均表明miRNA-133 可通过调控RhoA 蛋白、细胞分化周期蛋白-42、参与心脏发生的核因子基因和结缔组织生长因子的表达而影响高血压左心室肥厚,但是上述实验并没有在临床上验证miRNA-133 对高血压左心室肥厚的预测... 目的 先前国内外实验研究均表明miRNA-133 可通过调控RhoA 蛋白、细胞分化周期蛋白-42、参与心脏发生的核因子基因和结缔组织生长因子的表达而影响高血压左心室肥厚,但是上述实验并没有在临床上验证miRNA-133 对高血压左心室肥厚的预测价值,因此本研究旨在验证miRNA-133 对高血压左心室肥厚的预测价值.方法 收集未服用降压药物且发生左心室肥厚的单纯高血压患者和年龄性别相匹配的正常人外周血浆-80 ℃保存,采用RT-PCR 技术验证差异表达miRNA 的稳定性;组间比较用t 检验,采用受试者工作特征(ROC)曲线验证miRNA-133 对高血压左心室肥厚的预测价值.结果 66 例高血压左心室肥厚和33 例正常人入选,血压和左心室质量指数有统计学差异(P〈0.05),而年龄、血脂等基线资料无统计学差异(P〉0.05);RT-PCR 结果显示miRNA-133 在正常人和高血压左心室肥厚组相对表达量分别为0.04±0.005、0.30±0.44,差异有统计学意义(P〈0.05);调整年龄、性别和体质量指数后,miRNA-133 表达水平与左心室肥厚具有相关性(ORs=1.654,95% CI 1.273~2.146,P〈0.05);miRNA-133 对高血压心肌肥厚预测的ROC曲线下面积(AUC)为0.860(P=0.000),95% CI 0.786~0.934(P〈0.05).结论 miRNA-133 对高血压左心室肥厚有较好的预测价值. 展开更多

目的探讨男性原发性高血压患者血清尿酸与左心房增大的关系。方法选取2015年6月至2016年12月广东省人民医院收治的男性原发性高血压患者298例,测量患者肝肾功能、血脂、血压、血糖和尿酸,根据超声心动图检查结果分为左心房增大组及左... 目的探讨男性原发性高血压患者血清尿酸与左心房增大的关系。方法选取2015年6月至2016年12月广东省人民医院收治的男性原发性高血压患者298例,测量患者肝肾功能、血脂、血压、血糖和尿酸,根据超声心动图检查结果分为左心房增大组及左心房正常组,血清尿酸与左心房增大的关系采用多变量logistic回归分析。结果298例男性原发性高血压患者平均年龄(55.29±8.48)岁,平均尿酸值(488.93±112.46)μmol/L,其中左心房增大患者47例,左心房增大组血清尿酸水平[(512.21±90.82)μmol/L比(474.38±94.26)μmol/L]明显高于左心房正常组(t=9.34,P〈0.001),差异有统计学意义。Pearson相关分析表明血清尿酸与左心房内径呈正相关(r:0.589,P〈0.001);多因素logistic回归分析血清尿酸是左心房增大的危险因素(OR=1.894,95%CI:1.13,2.69;P〈0.001)。结论男性原发性高血压患者血清尿酸与左心房内径呈正相关,并且是左心房增大的危险因素。 展开更多

The relationship of body mass index and hyperuricemia in hypertensive adults over 80 years

Background Hyperuricemia (HUA) and hypertension are associated with the increasing risk and mortality of cardiovascular disease (CVD). However, the relationship between body mass index (BMI) and HUA in hypertens... Background Hyperuricemia (HUA) and hypertension are associated with the increasing risk and mortality of cardiovascular disease (CVD). However, the relationship between body mass index (BMI) and HUA in hypertensive adults over 80 years remains uncertain. Methods Observational trial was designed and 308 patients who were newly diagnosed as essential hypertension without anti-hypertensive therapy were enrolled into our study. According to the cut-off value of serum UA, participants were stratified into normal ( 〈420μmol/L for men and 〈360 μmol/L for women) and increased UA groups (≥420 μmol/L for men and ≥360 μmol/L for women). Serum UA level, blood pressure and other baseline characteristics were compared, logistic regression analysis and receiver operating characteristic curve (ROC) were performed. Results The mean(SD) serum uric level was 382.2 (108.7) μmol/L and the prevalence of hyperuricemia was 45% among men and 50% among women. BMI was significantly higher (22.6 vs. 24.0 kg/m^2, P=0.003) and FBG was lower (5.13 vs. 4.98 mmol/L, P=0.025) in increased UA group among aged women, and BMI and FBG were found the independent determinants for UA increase in female subjects according to logistic regression analysis, and the odd ratio were 1.154(95% interval confidence 1.058- 1.259, P=0.001) and 0.646 (95% interval confidence 0.44-0.949, P=0.026) , respectively. Moreover, evaluation of receiver operating characteristic curve (ROC)showed that area under the curve for BMI to predict UA increase was 0.627±0.039, P=0.001 in women. However, the results mentioned above were only found in elderly women, not in men. Conclusions Our study indicates that aged women have higher prevalence of HUA than men, and that BMI is independently associated with serum uric acid level for hypertensive women but not for men over 80 years old. Therefore, BMI is a useful predictor of serum uric acid level in el- derly women with hypertension. 展开更多

Previous studies have shown that miR- NAs are related to atherosclerosis. However, the relationship between miR-21 and atherosclerosis remains un- known. Therefore, this study aimed to explore the relationship between plasma miR-21 expression level and subclinical atherosclerosis in patients with hypertension. Methods We assessed the plasma level of miR-21 in 50 hypertension patients and in 30 age- and sexmatched healthy individuals. All patients underwent brachial-ankle pulse wave velocity (baPWV), office and ambulatory blood pressure monitoring. Quantitative reverse transcrip- tase polymerase chain reaction (qRT-PCR) was used to measure miR-21. The correlations between miR-21 level and blood pressure parameters and baPWV were assessed using the Pearson correlation coefficient. Receiver op- erating characteristic (ROC) curve was constructed for distinguishing from atherosclerosis and healthy. Results The baseline data of all the participants such as fasting blood glucose, body mass index, serum lipid, hepatic and renal function were not significantly different between hypertension and control groups, as well as atherosclerosis and non-atherosclerosis groups (P〉0.05). The hypertensive patients showed higher plasma miR-21 (32.99±3.01 vs. 26.14±1.03; P〈0.001) level as compared to controls, and in atherosclerosis group (33.53±2.86 vs. 28.56±3.66; P=0.002) than that in non-atherosclerosis group. MiR-21 level was significant positively correlated with 24h mean SBP (r=0.718, P〈0.001), 24 h mean DBP (r=0.247 P=0.027), office SBP (r=0.644, P〈0.001), office DBP (r=0.316, P=0.004) and baPWV (r=0.714, P〈0.001), respectively. The miR-21 for distinguishing atherosclerosis from healthy yielded an area under the ROC curve of 0.844 (95%CI: 0.758, 0.930; P〈0.001). Conclusion Our data provide significant evidence that circulating miR-21 may represent potential non-invasive marker of athero-sclerosis in essential hypertensive patients. 展开更多

Background MicroRNA (miR)-92a has been demonstrated to be associated with atherosclerosis through different ways. However, there are limited date about the association of miR-92a with arteriosclerosis in hypertensive patients. Methods We assessed the plasma level of miR-92a in 110 patients with untreated essential hypertension and 65 healthy individuals. All patients underwent the measurement of brachial-ankle pulse wave velocity (baPWV) and ambulatory blood pressure. Quantitative reverse transcriptase polymerase chain reaction (qRT- PCR) was used to evaluate level of miR-92a. The correlations of miR-92a with blood pressure parameters and baPWV were assessed using the Spearman correlation coefficient. Receiver operating characteristic (ROC) curve was constructed and the area under the ROC curve (AUC) was generated to assess the diagnostic value of miR- 92a for arteriosclerosis. Results The plasma level of miR-92a was significantly higher in hypertensive patients than in control individuals(31.03±3.13 vs. 25.00±2.99; P 〈0.001),and higher in arteriosclerosis group than in unarteriosclerosis group(33.74±2.43 vs. 27.51±3.28; P〈0.001). There were significantly positive correlations between MiR-92a level with 24 h mean SBP(r=0.874, P〈0.001), 24h mean DBP(r=0.680, P〈0.001) and baPWV(r=0.685, P〈0.001). ROC curve analyses revealed that the AUC value for miR-92a was 0.929 (95%CI: 0.886, 0.973; P〈 0.001). Conclusions The level of miR-92a is positively related to baPWV, and holds great potential to be a noninvasive screening marker for arteriosclerosis in essential hypertensive patients. 展开更多

Background Studies have shown that mi RNA-155 played an important role in the process of development of hypertension.However,there is no date about mi RNA-155 and blood pressure monitoring parameters.Therefore,we examined whether in hypertensive patients the expression level of plasma mi RNA-155 related to 24-h ambulatory blood pressure monitoring(ABPM) parameters.Methods A cohort of adult patients scheduled to receive physical examination,office and ambulatory blood pressure monitoring.Quantitative reverse transcriptase polymerase chain reaction(q RT-PCR) was used to evaluate the expression of selected mi RNA-155.The mi RNA-155 expression level correlation between blood pressure parameters was assessed using the Spearman correlation coefficient.Results Fifty four essential hypertension patients(25 men;mean age,53.28 ± 9.52 years) and thirty healthy volunteers(15 men;mean age,53.03 ± 5.87 years) were included.We observed higher expression level of mi RNA-155(32.31 ± 2.85 vs 27.21 ± 1.59,P 〈 0.001) in hypertensive patients compared to healthy control individuals.MiRNA- 155 expression level showed significant positive correlation with 24 h Daytime SBP(r =0.681,P 〈 0.001),24 h Daytime DBP(r = 0.473,P 〈 0.001),24 h Daytime PP(r = 0.565,P 〈 0.001) and dipping(r = 0.257,P = 0.018),respectively.Conclusions Our study showed that mi RNA-155 expression level was associated positively with daytime blood pressure monitoring parameters,as well as blood pressure variability,indicating a possible implication of miRNA- 155 in the pathogenesis of hypertension. 展开更多

A pilot study of microRNAs as a new potential biomarkers for hypertension

Background Micro RNAs have recently been considered as biomarkers in several different cardiovascular diseases,however,so far there are no circulating miRNAs data about hypertension.Therefore,the aim of the present pilot study was to identify circulating miRNAs for hypertension biomarkers.Methods Using an Agilent microarray,plasma miRNAs were profiled from plasma samples of 10 patients with untreated essential hypertension and 10 healthy controls.Candidate biomarkers identified in the profiles were subjected to validation by using quantitative PCR in an independent sample set of 20 patients with untreated essential hypertension and 20 healthy controls.Then,we assessed the selected miRNAs for the detection and diagnosis of hypertension from plasma samples of 70 patients with untreated essential hypertension and 20 healthy controls.The Spearman correlation coefficient was used to assessed the selected miRNAs correlations with blood pressure.The area under the receiver operating characteristic curve(AUC) was used to evaluate diagnostic accuracy.Results The expressions of selected 8 miRNAs were investigated independently in plasma samples from 10 hypertension patients and 10 healthy subjects.The levels of circulating miR-30c-5p,miR-133 b,miR-29b-3p,miR-29a-3p,miR-29c-3p,miR-30a-3p,miR-let7b-3p expression were significantly down regulated in hypertension group compared with healthy group and the level of hsa-miR-92b-3p was significantly unregulated between the groups.We used q RT-PCR assay to confirm the expression of 8 candidate miRNAs,miR-30c-5p(P 〈 0.001),miR-29b-3p(P 〈 0.001),miR-29a-3p(P = 0.027),miR-29c-3p(P 〈 0.001),miR-92b-3p(P = 0.003),miR-30a-3p(P = 0.704),miR-133b(P =0.346),andmiR-let7b(P = 0.161).The diagnostic accuracy of miR-30c-5p,miR-29b-3p,miR-29a-3p,miR-29c-3p and miR-92b-3p,as measured by AUC,were 0.897,0.90,0.829,0.825 and 0.832,respectively,with all P 〈0.001.Conclusions The plasma levels of miR-30c-5p,miR-29b-3p,miR-29a-3p,miR-29c-3p and miR-92b-3p associated with hypertension which provide an impetus for future evaluations of hypertension development,progression,therapeutic efficacy and prognosis. 展开更多
