目的比较不同内固定方式联合椎管扩大成形术治疗颈椎后纵韧带骨化症(Ossification of posterior longitudinal ligament,OPLL)的效果。方法于2014年3月~2017年5月收治35例OPLL患者,均采用行椎管扩大成形术治疗。其中19例给予侧块螺钉内...目的比较不同内固定方式联合椎管扩大成形术治疗颈椎后纵韧带骨化症(Ossification of posterior longitudinal ligament,OPLL)的效果。方法于2014年3月~2017年5月收治35例OPLL患者,均采用行椎管扩大成形术治疗。其中19例给予侧块螺钉内固定,为A组;16例给予ARCH钛板内固定治疗,为B组。观察2组患者手术情况及相关疗效指标。结果2组手术时间、术中出血量比较差异不大(P>0.05);B组术后6个月JOA评分改善率及椎管矢状径改善率明显高于A组(P<0.05);B组术后3 d、术后6个月椎管开门角度均明显高于A组(P<0.05),开门角度丢失情况明显低于A组(P<0.05);A组1例患者在随访期间出现再关门现象,4例出现轴性症状,B组患者未见上述现象及症状;术后1年A组颈椎活动度明显减小(P<0.05),B组颈椎活动度明显高于A组(P<0.05);术后1年B组患者健康调查简表(SF-36)评分明显高于A组(P<0.05),上述差异均有统计学意义。结论采用ARCH钛板固定联合椎管扩大成形术治疗OPLL的疗效明显优于侧块螺钉固定,可明显提高JOA改善率及椎管矢状径改善率,降低再关门现象及轴性症状发生率,且保留颈椎活动度,使患者术后健康状况得到明显改善。展开更多
