487份病历抗感染药物使用情况调查报告

通过对医院感染危险因素的流行病学调查分析表明,抗菌药物应用为医院感染的主要危险因素.因此抗菌药物应用与医院感染密切相关;医院制定应用抗菌药物的合理使用制度,已成为控制医院感染的主要措施之一,同时也是提高抗菌药物疗效必不可少的步骤.因此,我们对南阳市中心医院2001年1～6月份出院的上案病历进行了抗菌药物使用率的调查分析并评价其使用的合理性.

Reconstruction of trachea-esophagus vocalization by tunnel method in 12 cases

Objective To study the therapeutic effect of re construction in improving trachea-esophagus vocalization after total laryngecto-my.Method We reviewed the reconstruction of vo calization with trachea-esophagus tunnel in 12cases patients of total l aryngectomy,i.e.a triangular mucous valve was made by i ncision of the posterior wall of the t rachea with the correlative anterio r wall of the esophagus made into a tunnel and the mucous valve would vocalize.Result11of the 12cases obtained successfu l vocalization,counting for 91%of t he total.Atresia of the vocalizing valve occurred in o ne case.Conclusion Trachea-esophagus tunnel vocaliza tion reconstruction has better therapeutic effect with good vocalization effect and fewer c omplications and easily performed.It avoided the shortcomings of necro sis and atresia of the vocalization v alve and pharyngeal fistula formation,a nd the complexity of exchange of artificial vocalization tube again and a gain.

4479例病案检查结果分析

目的为了科学地对住院病案的质量进行评价,找出影响病案质量的主要因素,以寻求提高病案质量的最佳方法。方法选取近半年(2003年11月至2004年4月)的出院病案4479份,依照《病历书与基本规范》进行评价,并使用计算机进行分类统计。结果占不合格率前四位的内容是:入院七天内副主任医师以上医师查房不合格率为4.27%;入院3天内主治医师查房不合格率为2.26%;科主任主或主任医师签名不合格率为1.07%;主治医师签名不合格率为1.03%。结论提高病案书写质量应注重提高上级医师的专业水平及管理水平。
