目的评价新辅助放化疗联合腹腔镜治疗中低位直肠癌的有效性和可行性。方法利用PubMed、Web of knowledge、Ovid Lww等英文数据库,全面检索新辅助放化疗联合腹腔镜与单独腹腔镜手术治疗中低位直肠癌的英文随机对照试验和回顾性分析文献,...目的评价新辅助放化疗联合腹腔镜治疗中低位直肠癌的有效性和可行性。方法利用PubMed、Web of knowledge、Ovid Lww等英文数据库,全面检索新辅助放化疗联合腹腔镜与单独腹腔镜手术治疗中低位直肠癌的英文随机对照试验和回顾性分析文献,利用RevMan5.2软件对2种治疗方法的有效性和可行性进行Meta分析。结果最终纳入7篇英文文献共1015例患者(新辅助治疗组472例,单独腹腔镜组543例),6篇文献为回顾性分析,1篇为前瞻性非随机实验。新辅助治疗组手术时间较长,术中切除淋巴结数目较少,中转开腹率、术中出血、术后并发症发生率、吻合口瘘发生率、切口感染及术后住院时间,两组相比较,差异无显著性。结论行新辅助放化疗的中低位直肠癌患者应用腹腔镜治疗,短期疗效是安全、有效的,但仍需大量随机对照试验及远期随访观察的支持。展开更多
