Treatment outcomes and adverse drug reactions among patients with drug-resistant tuberculosis receiving all-oral,long-term regimens:First record viewing report from Pakistan

Objective:To assess the effectiveness and adverse drug reactions of all-oral regimens for patients with multidrug-resistant tuberculosis.Methods:This retrospective study was conducted at 10 Programmatic Management of Drug Resistant Tuberculosis sites in Punjab province of Pakistan.Patients receiving treatment for drug resistant tuberculosis from July 2019 to December 2020 with at least interim result i.e.6th month culture conversion or final outcomes(cured,complete,lost to follow-up,failure,death)available,were included in the study.Data was extracted from electronic data management system.For the reporting and management of adverse drug events,active tuberculosis drug safety monitoring and management was implemented across all sites.All the data was analyzed using SPSS version 22.Results:Out of 947 drug resistant tuberculosis patients included in this study,579(68%)of the patients had final outcomes available.Of these,384(67.9%)successfully completed their treatment.Out of 368(32%)patients who had their interim results available,all had their 6th month culture negative.Combining new medications was thought to result in serious adverse outcomes such as QT prolongation.However,this study did not record any severe adverse events among patients.Conclusions:All-oral regimens formulation guided by overall treatment effectiveness resulted in treatment outcomes comparable to those obtained with traditional injectable treatment. 展开更多

Objective Robotic-assisted live donor nephrectomy(LDN)is being gradually adopted across transplant centers.The left donor kidney is preferred over right due to anatomical factors and ease of procurement.We aimed to study donor and recipient outcomes after robotic procurement and subsequent open implantation of right and left kidneys.Methods All fully robotic LDNs and their corresponding open kidney transplants performed at our center between February 2016 and December 2021 were retrospectively analyzed.Results Out of 196 robotic LDN(49[right]vs.147[left]),10(5.1%)donors had intra-operative events(6.1%[right]vs.4.8%[left],p=0.71).None of the LDN required conversion to open surgery.The operative times were comparable for the two groups.Nausea(13.3%)was the most common post-operative complication.There was no mortality in either LDN group.Herein,we report our outcomes on 156 recipients(39 right and 117 left allografts)excluding robotic implants,exports,and pediatric recipients.There were no significant differences between right and left kidney recipients with respect to 1-year post-transplant patient survival(100.0%vs.98.1%,p=0.45)or graft survival(93.9%vs.97.1%,p=0.11),or delayed graft function(7.7%vs.5.1%,p=0.55).Conclusion Non-hand-assisted robotic live donor nephrectomies can be safely performed with excellent outcomes.Right LDN was not associated with higher incidence of complications compared to left LDN.Open implantation of robotically procured right renal allografts was not associated with higher risk of recipient complications. 展开更多

Phosphorus Solubility from Rock Phosphate Mixed Compost with Sulphur Application and Its Effect on Yield and Phosphorus Uptake of Wheat Crop

A field experiment was conducted to determine the effect of sulphur application with Rock phosphate mixed compost on phosphorus (P) solubility and its effect on yield and P uptake of wheat crop. The experiment was laid out in randomized complete block design (RCBD) with three replications at the research farm of The University of Agriculture Peshawar. The experiment was conducted during rabi 2015-16 with plot size of 3 m × 5 m. Nitrogen, phosphorus and potassium were applied at the rate of 120, 90 and 60 kg&#183ha-1 in the form of urea, compost, or single super phosphate and potassium sulphate, respectively. Elemental sulphur was applied at the rate of 10, 20 and 30 kg&#183ha-1 at the time of sowing. Results showed that sulphur applied with compost significantly improved wheat yield and yield components, soil organic matter, soil total N and AB-DTPA extractable P contents, plant N and P concentrations and their uptake, plant micronutrients concentration and their uptakes. No significant changes were noted in soil pH, ECe and lime contents. Maximum grain yield of 4076 kg&#183ha-1, total dry matter yield 9721 kg&#183ha-1, straw yield 5644 kg&#183ha-1, plant height 98.3 cm, spike length 11.2 cm, grain per spike 61.0, thousand grain weight 50.2 g were recorded on the application of S at the rate 20 kg&#183ha-1 with compost. The highest soil organic matter content of 1.41% was found for the application of S at the rate of 10 kg&#183ha-1 with compost. Maximum soil total N content of 1756 mg&#183kg-1 and P 5.7 mg&#183kg-1 were observed by the application of double recommended S with compost. Plant N uptakes of 125.7 kg&#183ha-1, and P uptake of 17.5 kg&#183ha-1, were maximum with application of compost and S @ 20 kg&#183ha-1. Highest plant uptake of Fe 0.56 kg&#183ha-1, Zn 0.41 kg&#183ha-1, Cu 0.16 kg&#183ha-1 and Mn 0.93 kg&#183ha-1 were found by the application of full recommended S with compost. Results suggested that S at the rate of 20 kg&#183ha-1 application with compost prepared from farm yard manure and rock phosphate proved better combination to enhance wheat yield, yield components and nutrients uptakes of wheat crop. 展开更多

Nitrification inhibitors are widely used in agriculture to mitigate nitrous oxide(N_(2)O)emission and increase crop yield.However,no concrete information on their mitigation of N_(2)O emission is available under soil and environmental conditions as in Pakistan.A field experiment was established using a silt clay loam soil from Peshawar,Pakistan,to study the effect of urea applied in combination with a nitrification inhibitor,nitrapyrin(2-chloro-6-tri-chloromethyl pyridine),and/or a plant growth regulator,gibberellic acid(GA_3),on N_(2)O emission and the nitrogen(N)uptake efficiency of maize.The experimental design was a randomized complete block with five treatments in four replicates:control with no N(CK),urea(200 kg N ha^(-1))alone,urea in combination with nitrapyrin(700 g ha^(-1)),urea in combination with GA_3(60 g ha^(-1)),and urea in combination with nitrapyrin and GA_3.The N_(2)O emission,yield,N response efficiency,and total N uptake were measured during the experimental period.The treatment with urea and nitrapyrin reduced total N_(2)O emission by 39%–43%and decreased yield-scaled N_(2)O emission by 47%–52%,relative to the treatment with urea alone.The maize plant biomass,grain yield,and total N uptake increased significantly by 23%,17%,and 15%,respectively,in the treatment with urea and nitrapyrin,relative to the treatment with urea alone,which was possibly due to N saving,lower N loss,and increased N uptake in the form of ammonium;they were further enhanced in the treatment with urea,nitrapyrin,and GA_3 by 27%,36%,and 25%,respectively,probably because of the stimulating effect of GA_3 on plant growth and development and the reduction in biotic and abiotic stresses.These results suggest that applying urea in combination with nitrapyrin and GA_3 has the potential to mitigate N_(2)O emission,improve N response efficiency,and increase maize yield. 展开更多
