The manifestations inv...Hypophosphatasia (HPP) is an inherited skeletal disease caused by mutation of the gene encoding tissue non-specific alkaline phosphatase (TNSALP). Odonto-HPP is well known as the mildest of HPP. The manifestations involve only the teeth, such as premature primary teeth exfoliation caused by reduction of alveolar bone, enlarged dental pulp chamber, and dental defects. We report a case of a 9-years-old boy who developed HPP. He was observed from the primary dentition to the mixed dentition period. At initial presentation at our hospital, he had multiple premature exfoliation of primary teeth and reduction of the alveolar bone. HPP was suspected due to the low level of ALP activity in serum, his oral manifestation, and dental history. He was referred to a physician for the final diagnosis. Therefore his compound heterozygote mutations, c.1559 delT (T/delT) and c.407G > A (G/A), were found in TNSALP and he diagnosed with odonto-HPP. Even though these mutations were reported as being involved in odonto-HPP, his mineral densities tended to be lower than that of his age. It is therefore necessary to investigate the bone mineralization level in odonto-HPP without other bone symptoms. Moreover, ongoing enzyme-replacement therapy in odonto-HPP might improve dental abnormality and bone disorders.展开更多

Methods: We designed a prospective study to determine if VATS with mini-tho...Purpose: We report our experience of Video-assisted thoracic surgery (VATS) to treat acute blunt traumatic hemothorax using mini-thoracotomy. Methods: We designed a prospective study to determine if VATS with mini-thoracotomy benefits for patients with blunt traumatic hemothorax compared with conventional repair through full thoracotomy. Twenty-five patients underwent emergency operation for acute hemothorax from 2000. Five patients with less than 5% probability of survival (PS) were excluded, leaving 20 as the subjects. Results: Ten patients underwent conventional thoracotomy (conventional group) and the other 10 patients underwent VATS with mini-thoracotomy (VATS group). There was no difference between conventional group and VATS group in injury severity score (29.1 and 27.0) or PS (81.2% and 80.7%). Hospital mortality rates were 10% in conventional group and 0% in VATS group (N.S). Total amounts of intra-operative bleeding and post-operative transfusion until day 7 were 735 ml and 19.3 units in conventional group and 303 ml and 9.2 units in VATS group respectively (N.S). The length of ICU stay was 9.7 days in conventional group and 5.9 days in VATS group (N.S). Conclusion: VATS with mini-thoracotomy can be alternative for patients with blunt traumatic hemothorax in most emergency operations.展开更多

Gravi...A gravity survey on Mizuho Plateau was carried out in austral summer of 2001-2002 by the 43rd Japanese Antarctic Research Expedition (JARE-43), as one of the geophysical measurements of deep seismic exploration. Gravity measurements were conducted at 151 points in approximately 1 km interval along the 150 km profile. Considering the gravity effect from the ice-sheet, Free-air and Bouguer anomalies were calculated by using precise GPS locations. Furrowed and basin-like negative Free-air anomalies were observed in middle and northern parts of the profile. Bouguer anomalies were calculated by two bedrock elevation data obtained from seismic refraction and radio-echo sounding. High resolution model for bedrock elevation by radio-echo sounding was applied in determining the crustal thickness.展开更多

She underwent left upper lobectomy with mediastinal lymph node dissection and concomitant resection of the left phrenic nerve, vagus nerve and pericardium. She received adjuvant chemotherapy, but had tumor recurrence. We then performed left completion pneumonectomy, but could not remove the tumor completely because of rapid tumor invasion to the left atrium. Urgent surgery should be considered for recurrent resectable PPC.展开更多
