文摘目的 探讨分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)伴左心室功能下降患者的影响因素及远期随访情况。方法 选取130例OSAHS患者,根据是否出现左心室功能下降分为轻度OSAHS组和中重度OSAHS组,分析影响OSAHS伴左心室功能下降患者的影响因素,分析OSAHS患者远期随访情况。结果 中重度OSAHS组体质量指数(BMI)高患者比例、完全阻塞的患者数量均高于轻度OSAHS组,鼾声评分(snore scale,SS)、微觉醒指数(microarousal index,MI)高于轻度OSAHS组,差异有统计学意义(P<0.05);呼吸暂停低通气指数(AHI)与舒张末室间隔厚度(interventricular septum thickness,IVST)、A、E峰比值(E/A)、左心室侧壁舒张早期运动峰速度(e’)、心输出量(cardiac output,CO)、心脏指数(cardiac index,CI)、左心室质量分数(left ventricular mass index,LVMI)及左心室射血分数(left ventricular ejection fraction,LVEF)存在相关性(P<0.05);BMI、AHI、阻塞情况、阻塞平面、血氧饱和度小于90%时间占监测总时间的百分比(the percent of the total record time spent below 90% oxygen saturation,TS90%)、最低血氧饱和度(low oxygen saturation,LSaO_(2))及MI是OSAHS伴左心室功能下降的影响因素。Cox风险比例模型分析显示,AHI、完全阻塞、BMI是OSAHS患者远期发生心脏功能下降的影响因素。结论AHI高、多平面阻塞、气道完全阻塞及低通气是OSAHS患者伴左心室功能下降的影响因素,AHI、气道完全阻塞及高BMI是OSAHS患者远期发生心脏功能下降的预测因素。
