Background:The incidence of lumbar disc herniation(LDH)is notably high.Consensus among experts highlights non-surgical treatments as the primary therapeutic approach for LDH.Contemporary medicine frequently employs ph...Background:The incidence of lumbar disc herniation(LDH)is notably high.Consensus among experts highlights non-surgical treatments as the primary therapeutic approach for LDH.Contemporary medicine frequently employs pharmacotherapy and epidural injections in such treatments,which are associated with numerous adverse effects.Prolonged use can severely impair the liver and kidney functions of patients.Hence,the role of safe and effective traditional Chinese medicine techniques becomes pivotal.Among various traditional Chinese medicine approaches for treating LDH,fire dragon cupping,renowned for its remarkable efficacy,cost-effectiveness,and ease of application,is extensively utilised in clinical settings for managing LDH.Nonetheless,there is a scarcity of systematic and standardised evidence from evidence-based medicine studies.Therefore,conducting a meta-analysis is imperative.Methods:A comprehensive computerised search was conducted in databases including China National Knowledge Infrastructure,WanFang Data,VIP,China Biology Medicine disc,PubMed,EMbase,The Cochrane Library,Web of Science,and CINAHL.The search aimed to gather randomised controlled trials on fire dragon cupping therapy for LDH,spanning from the inception of these databases until December 2023.Two researchers independently screened the literature according to inclusion and exclusion criteria,extracted data,and assessed the methodological quality of the studies included,utilising RevMan 5.3 software for meta-analysis.Results:The results show that the fire dragon cupping therapy group had better clinical effectiveness(relative risk=1.23,95%confidence interval(CI)(1.14,1.33),P<0.00001),less pain(standardized mean difference=–1.33,95%CI(–1.49,–1.16),P<0.00001),and some improvement in lumbar function(Japanese Orthopaedic Association scores:mean difference=3.37,95%CI(2.31,4.43),P<0.00001.Conclusion:The fire dragon cupping therapy significantly alleviates LDH,warranting its extensive application.However,considering the limitations in the number and quality of studies included,the aforementioned conclusion necessitates further validation through more high-quality research.展开更多

When a patient has glaucoma, cataracts and irregular astigmatism at the same time, it is often difficult to achieve satisfactory results. Purpose: This study intends to describe a case of a patient with glaucoma, irregular astigmatism, and cataract who was successfully treated. Additionally, it can serve as a useful source of inspiration for the future care of patients like this. Case Presentation: A 24-year-old male with keratoconus in the past. He had undergone a corneal crosslinking operation in the right eye and a penetrating keratoplasty in the left eye due to his long history of keratoconus in both eyes. Right now, the keratoconus in that eye is stable. Unfortunately, he has now been diagnosed with open-angle glaucoma and complicated cataracts in both eyes, and he was admitted to our hospital for surgery. The right eye had significant irregular astigmatism, which was discovered during the preoperative assessment, and the implantation of a Toric intraocular lens was unable to produce good results. Finally, we chose to perform EXPRESS glaucoma drainage device implantation + phacoemulsification + intraocular lense (ZEISS CT ASPHINA 409MP) implantation. The patient had stable postoperative astigmatism with rule thanks to the traction effect of the scleral flap suture. Astigmatism was typically stable six months after surgery, and the corrected visual acuity with glasses had improved to 20/25. Conclusion: This patient suffers from cataracts, keratoconus, glaucoma, and irregular astigmatism. Due to the interconnectedness of these four disorders, simultaneous success is challenging. We realized that surgically induced astigmatism, frequently affects vision early after filtering surgery for glaucoma patients. In the instance of this patient, we tightly sutured the scleral flap and using tractive action, established regular astigmatism. After the patient’s astigmatism stabilized, optometry was given to correct the vision. This technique should result in much better visual acuity. Finally, it came to pass.展开更多

Direct epoxidation of propylene with H_(2)/O_(2),being the dream reaction for propylene oxide(PO)production,has raised wide scientific and industrial interests.Fundamentally understanding the formation mechanism of ac...Direct epoxidation of propylene with H_(2)/O_(2),being the dream reaction for propylene oxide(PO)production,has raised wide scientific and industrial interests.Fundamentally understanding the formation mechanism of acrolein,as the main by-product of this epoxidation process,is very important to achieve the high yield of PO.In this study,we perform the spin-polarized density functional theory(DFT)calculations to investigate the reaction pathway from propylene to acrolein over two representative Au surfaces,that is,Au(111)and Au(100),which incorporates propylene adsorption,methyl hydrogen activation and acrolein formation.The results show that the oxygenated species(mainly O^(*),OH^(*)and OOH^(*))are able to stabilize the adsorption of propylene to decrease the energy barrier for its activation.It is demonstrated that the OOH^(*)on Au(111)surface emerges as the most easily formed oxygenated species via the H-assisted O_(2) dissociation,which is also the most active for the cleavage of methyl CAH bond in propylene.Furthermore,three pathways of acrolein formation activated by O^(*)/OH^(*)/OOH^(*)are analyzed,in which O^(*)is found as the key species to form acrolein.Finally,Bader charge analysis was conducted to explore the reasons behind the promotion effect of the oxygenated species.The insights reported here could be valuable in the design and optimization of gold catalysts for the direct epoxidation of propylene.展开更多
Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision...Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision setting.Despite massive studies of the geology,chronology,petrogenesis,and ore-related fluids and their sources in Jiama,there is a lack of systematic summaries and reviews of this system.In contrast to traditional porphyry copper systems in a subduction setting,recent studies and exploration suggest that the Jiama deposit includes porphyry-type Mo-Cu,skarn-type Cu polymetallic,vein-type Au and manto orebodies.This paper reviews the latest studies on the geology,chronology,petrogenesis,fluid inclusions,and isotopic geochemistry(hydrogen,oxygen,sulfur,and lead)of the Jiama deposit.Accordingly,a multi-center complex mineralization model was constructed,indicating that multi-phase intrusions from the same magma reservoir can form multiple hydrothermal centers.These centers are mutually independent and form various orebodies or are superimposed on each other and form thick,high-grade orebodies.Finally,a new comprehensive exploration model was established for the Jiama porphyry copper system.Both models established in this study help to refine the theories on continental-collision metallogeny and porphyry copper systems.展开更多
The further development of catalytic elements has been plagued by activation and binary problems.The automatic shift model that has emerged in recent years helps components achieve full range.However,the detection dat...The further development of catalytic elements has been plagued by activation and binary problems.The automatic shift model that has emerged in recent years helps components achieve full range.However,the detection data still remains unstable in the shift area(7%∼13%).This paper proposes a Catalytic Combustion and Thermal Conductivity(CCTC)model for the specified range,which can be explained fromtwo aspects based on the existing methods.On the one hand,it uses iterative location search to process heterogeneous data,judges the prediction position of data points,and then givesweight evaluation.On the other hand,it corrects the abnormal points,determines the abnormal points in the horizontal direction,and gives the replacement value through the data of adjacent points.The experimental results show that the CCTC model reduces the sum of variance from 17 of the automatic shift model to 13,and the comparison of experimental variance is reduced by 23%.In the full-scale real-time data,the experimental variance of CCTC model and automatic shift model is reduced by 18%.In conclusion,CCTC is a cross section stability framework for full-scale methane measurement,in which the specified heterogeneous combination and anomaly point correction methods improve the stability.展开更多
BACKGROUND Gastric cancer(GC)is a malignant tumor with a high incidence and mortality rate worldwide for which acute bleeding is a common clinical complication.Gastro-scopic hemostasis is an important method for treat...BACKGROUND Gastric cancer(GC)is a malignant tumor with a high incidence and mortality rate worldwide for which acute bleeding is a common clinical complication.Gastro-scopic hemostasis is an important method for treating acute bleeding in GC;however,its efficacy and safety remain controversial.AIM To systematically analyze the efficacy and safety of gastroscopic hemostasis for the treatment of acute gastric hemorrhage.METHODS The PUBMED,Web of Science,Wiley Library,EMBASE,Wanfang,CNKI,and VIP databases were searched for studies related to gastroscopic hemostatic treatment for acute GC published through February 20,2023.The literature was screened according to the inclusion and exclusion criteria,data were extracted,and lite-rature quality was evaluated.The meta-analysis was performed using RevMan software(version 5.3),while Begg’s test for publication bias was performed using Stata 13.0 software.RESULTS Six randomized controlled trials and two retrospective analyses were retrieved.Five studies had a low,two had an uncertain,and one had a high risk of bias.Compared with the control group,the hemostatic rate of gastroscopic hemostasis was increased[relative risk(RR)=1.24;95%confidence interval(CI):1.08 to 1.43;P=0.003];the rate of rebleeding(RR=0.27;95%CI:0.09 to 0.80;P=0.02),rate of surgery transfer(RR=0.16;95%CI:0.06 to 0.43;P=0.0003),serum C-reactive protein level[mean difference(MD)=-5.16;95%CI:-6.11 to 4.21;P<0.00001],interleukin-6 level(MD=-6.37;95%CI:-10.33 to-2.42;P=0.002),and tumor necrosis factor-αlevel(MD=-2.29;95%CI:-4.06 to-0.52;P=0.01)were decreased;and the quality of life improvement rate was increased(RR=1.95;95%C I=1.41-2.71;P<0.0001).Begg’s test revealed no significant publication bias.CONCLUSION The efficacy and safety of endoscopic hemostasis were higher than those of the control group,suggesting that it is an effective treatment for acute GC hemorrhage.展开更多
The current adversarial attacks against deep learning models have achieved incredible success in the white-box scenario.However,they often exhibit weak transferability in the black-box scenario,especially when attacki...The current adversarial attacks against deep learning models have achieved incredible success in the white-box scenario.However,they often exhibit weak transferability in the black-box scenario,especially when attacking those with defense mechanisms.In this work,we propose a new transfer-based blackbox attack called the channel decomposition attack method(CDAM).It can attack multiple black-box models by enhancing the transferability of the adversarial examples.On the one hand,it tunes the gradient and stabilizes the update direction by decomposing the channels of the input example and calculating the aggregate gradient.On the other hand,it helps to escape from local optima by initializing the data point with random noise.Besides,it could combine with other transfer-based attacks flexibly.Extensive experiments on the standard ImageNet dataset show that our method could significantly improve the transferability of adversarial attacks.Compared with the state-of-the-art method,our approach improves the average success rate from 88.2%to 96.6%when attacking three adversarially trained black-box models,demonstrating the remaining shortcomings of existing deep learning models.展开更多

By modeling the impulsive noise as α-stable distribution, new methods which combine the sparse signal representation technique and fractional lower order statistics theory are proposed. In the new algorithms, the fractional lower order statistics vectors of the array output signal are sparsely represented on an overcomplete basis and the DOAs can be effectively estimated by searching the sparsest coefficients. To enhance the robustness performance of the proposed algorithms,the improved algorithms are advanced by eliminating the fractional lower order statistics of the noise from the fractional lower order statistics vector of the array output through a linear transformation. Simulation results have shown the effectiveness of the proposed methods for a wide range of highly impulsive environments.展开更多
BACKGROUND Microvascular invasion(MVI)of small hepatocellular carcinoma(sHCC)(≤3.0 cm)is an independent prognostic factor for poor progression-free and overall survival.Radiomics can help extract imaging information ...BACKGROUND Microvascular invasion(MVI)of small hepatocellular carcinoma(sHCC)(≤3.0 cm)is an independent prognostic factor for poor progression-free and overall survival.Radiomics can help extract imaging information associated with tumor pathophysiology.AIM To develop and validate radiomics scores and a nomogram of gadolinium ethoxybenzyl-diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)-enhanced magnetic resonance imaging(MRI)for preoperative prediction of MVI in sHCC.METHODS In total,415 patients were diagnosed with sHCC by postoperative pathology.A total of 221 patients were retrospectively included from our hospital.In addition,we recruited 94 and 100 participants as independent external validation sets from two other hospitals.Radiomics models of Gd-EOB-DTPA-enhanced MRI and diffusion-weighted imaging(DWI)were constructed and validated using machine learning.As presented in the radiomics nomogram,a prediction model was developed using multivariable logistic regression analysis,which included radiomics scores,radiologic features,and clinical features,such as the alpha-fetoprotein(AFP)level.The calibration,decision-making curve,and clinical usefulness of the radiomics nomogram were analyzed.The radiomic nomogram was validated using independent external cohort data.The areas under the receiver operating curve(AUC)were used to assess the predictive capability.RESULTS Pathological examination confirmed MVI in 64(28.9%),22(23.4%),and 16(16.0%)of the 221,94,and 100 patients,respectively.AFP,tumor size,non-smooth tumor margin,incomplete capsule,and peritumoral hypointensity in hepatobiliary phase(HBP)images had poor diagnostic value for MVI of sHCC.Quantitative radiomic features(1409)of MRI scans)were extracted.The classifier of logistic regression(LR)was the best machine learning method,and the radiomics scores of HBP and DWI had great diagnostic efficiency for the prediction of MVI in both the testing set(hospital A)and validation set(hospital B,C).The AUC of HBP was 0.979,0.970,and 0.803,respectively,and the AUC of DWI was 0.971,0.816,and 0.801(P<0.05),respectively.Good calibration and discrimination of the radiomics and clinical combined nomogram model were exhibited in the testing and two external validation cohorts(C-index of HBP and DWI were 0.971,0.912,0.808,and 0.970,0.843,0.869,respectively).The clinical usefulness of the nomogram was further confirmed using decision curve analysis.CONCLUSION AFP and conventional Gd-EOB-DTPA-enhanced MRI features have poor diagnostic accuracies for MVI in patients with sHCC.Machine learning with an LR classifier yielded the best radiomics score for HBP and DWI.The radiomics nomogram developed as a noninvasive preoperative prediction method showed favorable predictive accuracy for evaluating MVI in sHCC.展开更多

We realized that surgically induced astigmatism, frequently affects vision early after filtering surgery for glaucoma patients. In the instance of this patient, we tightly sutured the scleral flap and using tractive action, established regular astigmatism. After the patient’s astigmatism stabilized, optometry was given to correct the vision. This technique should result in much better visual acuity. Finally, it came to pass.

文摘BACKGROUND Eosinophilic granuloma(EG)is a proliferative condition that affects the cells of bone tissue.There are no specific clinical signs or imaging manifestations in the early stages of the disease,making it simple to overlook and misdiagnose.Because of the disease's rarity,there is presently no standardized treatment principle.There are few accounts of such occurrences affecting the axis among children.We discovered a case of a child whose EG resulted in atlantoaxial joint dislocation and destruction of the axial bone.CASE SUMMARY After having pharyngeal discomfort for more than six months without a clear explanation,a 6-year-old boy was brought to our hospital.Following a careful evaluation,the pathology indicated a strong likelihood of an axial EG.Ultimately,we decided to treat the boy with posterior pedicle screw fixation and local steroid injections.CONCLUSION EGs of the upper cervical spine are quite uncommon in children,and they are exceedingly easy to overlook or misdiagnose.Posterior pedicle screw fixation and local steroid injections are effective treatments for patients with axial EGs affecting the atlantoaxial junction.

Methods: From January 1, 2000 to August 31, 2022, we searched PubMed, Science Citation Index Database, China National Knowledge Infrastructure, and Wanfang Database for pertinent material about the treatment of AACG with various operations. The imported literature was carefully vetted using the inclusion and exclusion criteria, assessed for quality, and the raw data were retrieved and integrated into EndNoteX9. For the meta-analysis, STATA 16.0 and RevMan 5.3 were used as the tools. Results: The meta-analysis includes 20 clinical investigations in all, involving 1463 eyes. The quality of the literature was rated as excellent and the data homogeneity among the studies was excellent based on the various study types that were included in the literature. According to a meta-analysis, phacoemulsification with goniosynechialysis is superior to trabeculectomy for treating acute angle-closure glaucoma because it results in improved postoperative visual acuity, lower intraocular pressure, a broader anterior chamber depth, and fewer complications. Conclusion: If conditions allow, phacoemulsification in conjunction with goniosynechialysis performs better than trabeculectomy in terms of visual acuity, intraocular pressure, anterior chamber depth, and comorbidities.

文摘With the rapid development of maritime activities,there has been a growing demand for high data rate and ultra-reliable maritime communications.Traditionally,maritime communication services are provided mostly by maritime satellites.In some places,shore and island-based base stations(BSs)can also be built to extend the coverage of terrestrial networks,providing the fourth-generation(4G)or even the fifth-generation(5G)services.To further build up communication networks over the ocean for better coverage,unmanned aerial vehicles(UAVs)-aided and ship-borne BSs can be exploited to serve as supplementary relaying nodes.
文摘We develop an exponential spline interpolation method to solve the nonlinear Schrödinger equation. The truncation error and stability analysis of the method are investigated and the method is shown to be unconditionally stable. The conservation quantities are computed to determine the conservation properties of the problem. We will describe the method and present numerical tests by two problems. The numerical simulations results demonstrate the well performance of the proposed method.
文摘A numerical method based on septic B-spline function is presented for the solution of linear and nonlinear fifth-order boundary value problems. The method is fourth order convergent. We use the quesilinearization technique to reduce the nonlinear problems to linear problems and use B-spline collocation method, which leads to a seven nonzero bands linear system. Illustrative example is included to demonstrate the validity and applicability of the proposed techniques.

We use B-spline collocation method, which leads to a tri-diagonal linear system. The accuracy of the proposed method is demonstrated by test problems. The numerical results are found in good agreement with exact solutions.

文摘Direct epoxidation of propylene with H_(2)/O_(2),being the dream reaction for propylene oxide(PO)production,has raised wide scientific and industrial interests.Fundamentally understanding the formation mechanism of acrolein,as the main by-product of this epoxidation process,is very important to achieve the high yield of PO.In this study,we perform the spin-polarized density functional theory(DFT)calculations to investigate the reaction pathway from propylene to acrolein over two representative Au surfaces,that is,Au(111)and Au(100),which incorporates propylene adsorption,methyl hydrogen activation and acrolein formation.The results show that the oxygenated species(mainly O^(*),OH^(*)and OOH^(*))are able to stabilize the adsorption of propylene to decrease the energy barrier for its activation.It is demonstrated that the OOH^(*)on Au(111)surface emerges as the most easily formed oxygenated species via the H-assisted O_(2) dissociation,which is also the most active for the cleavage of methyl CAH bond in propylene.Furthermore,three pathways of acrolein formation activated by O^(*)/OH^(*)/OOH^(*)are analyzed,in which O^(*)is found as the key species to form acrolein.Finally,Bader charge analysis was conducted to explore the reasons behind the promotion effect of the oxygenated species.The insights reported here could be valuable in the design and optimization of gold catalysts for the direct epoxidation of propylene.

文摘Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision setting.Despite massive studies of the geology,chronology,petrogenesis,and ore-related fluids and their sources in Jiama,there is a lack of systematic summaries and reviews of this system.In contrast to traditional porphyry copper systems in a subduction setting,recent studies and exploration suggest that the Jiama deposit includes porphyry-type Mo-Cu,skarn-type Cu polymetallic,vein-type Au and manto orebodies.This paper reviews the latest studies on the geology,chronology,petrogenesis,fluid inclusions,and isotopic geochemistry(hydrogen,oxygen,sulfur,and lead)of the Jiama deposit.Accordingly,a multi-center complex mineralization model was constructed,indicating that multi-phase intrusions from the same magma reservoir can form multiple hydrothermal centers.These centers are mutually independent and form various orebodies or are superimposed on each other and form thick,high-grade orebodies.Finally,a new comprehensive exploration model was established for the Jiama porphyry copper system.Both models established in this study help to refine the theories on continental-collision metallogeny and porphyry copper systems.

文摘The further development of catalytic elements has been plagued by activation and binary problems.The automatic shift model that has emerged in recent years helps components achieve full range.However,the detection data still remains unstable in the shift area(7%∼13%).This paper proposes a Catalytic Combustion and Thermal Conductivity(CCTC)model for the specified range,which can be explained fromtwo aspects based on the existing methods.On the one hand,it uses iterative location search to process heterogeneous data,judges the prediction position of data points,and then givesweight evaluation.On the other hand,it corrects the abnormal points,determines the abnormal points in the horizontal direction,and gives the replacement value through the data of adjacent points.The experimental results show that the CCTC model reduces the sum of variance from 17 of the automatic shift model to 13,and the comparison of experimental variance is reduced by 23%.In the full-scale real-time data,the experimental variance of CCTC model and automatic shift model is reduced by 18%.In conclusion,CCTC is a cross section stability framework for full-scale methane measurement,in which the specified heterogeneous combination and anomaly point correction methods improve the stability.
文摘BACKGROUND Gastric cancer(GC)is a malignant tumor with a high incidence and mortality rate worldwide for which acute bleeding is a common clinical complication.Gastro-scopic hemostasis is an important method for treating acute bleeding in GC;however,its efficacy and safety remain controversial.AIM To systematically analyze the efficacy and safety of gastroscopic hemostasis for the treatment of acute gastric hemorrhage.METHODS The PUBMED,Web of Science,Wiley Library,EMBASE,Wanfang,CNKI,and VIP databases were searched for studies related to gastroscopic hemostatic treatment for acute GC published through February 20,2023.The literature was screened according to the inclusion and exclusion criteria,data were extracted,and lite-rature quality was evaluated.The meta-analysis was performed using RevMan software(version 5.3),while Begg’s test for publication bias was performed using Stata 13.0 software.RESULTS Six randomized controlled trials and two retrospective analyses were retrieved.Five studies had a low,two had an uncertain,and one had a high risk of bias.Compared with the control group,the hemostatic rate of gastroscopic hemostasis was increased[relative risk(RR)=1.24;95%confidence interval(CI):1.08 to 1.43;P=0.003];the rate of rebleeding(RR=0.27;95%CI:0.09 to 0.80;P=0.02),rate of surgery transfer(RR=0.16;95%CI:0.06 to 0.43;P=0.0003),serum C-reactive protein level[mean difference(MD)=-5.16;95%CI:-6.11 to 4.21;P<0.00001],interleukin-6 level(MD=-6.37;95%CI:-10.33 to-2.42;P=0.002),and tumor necrosis factor-αlevel(MD=-2.29;95%CI:-4.06 to-0.52;P=0.01)were decreased;and the quality of life improvement rate was increased(RR=1.95;95%C I=1.41-2.71;P<0.0001).Begg’s test revealed no significant publication bias.CONCLUSION The efficacy and safety of endoscopic hemostasis were higher than those of the control group,suggesting that it is an effective treatment for acute GC hemorrhage.

文摘The current adversarial attacks against deep learning models have achieved incredible success in the white-box scenario.However,they often exhibit weak transferability in the black-box scenario,especially when attacking those with defense mechanisms.In this work,we propose a new transfer-based blackbox attack called the channel decomposition attack method(CDAM).It can attack multiple black-box models by enhancing the transferability of the adversarial examples.On the one hand,it tunes the gradient and stabilizes the update direction by decomposing the channels of the input example and calculating the aggregate gradient.On the other hand,it helps to escape from local optima by initializing the data point with random noise.Besides,it could combine with other transfer-based attacks flexibly.Extensive experiments on the standard ImageNet dataset show that our method could significantly improve the transferability of adversarial attacks.Compared with the state-of-the-art method,our approach improves the average success rate from 88.2%to 96.6%when attacking three adversarially trained black-box models,demonstrating the remaining shortcomings of existing deep learning models.

文摘This paper is mainly to deal with the problem of direction of arrival(DOA) estimations of multiple narrow-band sources impinging on a uniform linear array under impulsive noise environments. By modeling the impulsive noise as α-stable distribution, new methods which combine the sparse signal representation technique and fractional lower order statistics theory are proposed. In the new algorithms, the fractional lower order statistics vectors of the array output signal are sparsely represented on an overcomplete basis and the DOAs can be effectively estimated by searching the sparsest coefficients. To enhance the robustness performance of the proposed algorithms,the improved algorithms are advanced by eliminating the fractional lower order statistics of the noise from the fractional lower order statistics vector of the array output through a linear transformation. Simulation results have shown the effectiveness of the proposed methods for a wide range of highly impulsive environments.

文摘BACKGROUND Microvascular invasion(MVI)of small hepatocellular carcinoma(sHCC)(≤3.0 cm)is an independent prognostic factor for poor progression-free and overall survival.Radiomics can help extract imaging information associated with tumor pathophysiology.AIM To develop and validate radiomics scores and a nomogram of gadolinium ethoxybenzyl-diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)-enhanced magnetic resonance imaging(MRI)for preoperative prediction of MVI in sHCC.METHODS In total,415 patients were diagnosed with sHCC by postoperative pathology.A total of 221 patients were retrospectively included from our hospital.In addition,we recruited 94 and 100 participants as independent external validation sets from two other hospitals.Radiomics models of Gd-EOB-DTPA-enhanced MRI and diffusion-weighted imaging(DWI)were constructed and validated using machine learning.As presented in the radiomics nomogram,a prediction model was developed using multivariable logistic regression analysis,which included radiomics scores,radiologic features,and clinical features,such as the alpha-fetoprotein(AFP)level.The calibration,decision-making curve,and clinical usefulness of the radiomics nomogram were analyzed.The radiomic nomogram was validated using independent external cohort data.The areas under the receiver operating curve(AUC)were used to assess the predictive capability.RESULTS Pathological examination confirmed MVI in 64(28.9%),22(23.4%),and 16(16.0%)of the 221,94,and 100 patients,respectively.AFP,tumor size,non-smooth tumor margin,incomplete capsule,and peritumoral hypointensity in hepatobiliary phase(HBP)images had poor diagnostic value for MVI of sHCC.Quantitative radiomic features(1409)of MRI scans)were extracted.The classifier of logistic regression(LR)was the best machine learning method,and the radiomics scores of HBP and DWI had great diagnostic efficiency for the prediction of MVI in both the testing set(hospital A)and validation set(hospital B,C).The AUC of HBP was 0.979,0.970,and 0.803,respectively,and the AUC of DWI was 0.971,0.816,and 0.801(P<0.05),respectively.Good calibration and discrimination of the radiomics and clinical combined nomogram model were exhibited in the testing and two external validation cohorts(C-index of HBP and DWI were 0.971,0.912,0.808,and 0.970,0.843,0.869,respectively).The clinical usefulness of the nomogram was further confirmed using decision curve analysis.CONCLUSION AFP and conventional Gd-EOB-DTPA-enhanced MRI features have poor diagnostic accuracies for MVI in patients with sHCC.Machine learning with an LR classifier yielded the best radiomics score for HBP and DWI.The radiomics nomogram developed as a noninvasive preoperative prediction method showed favorable predictive accuracy for evaluating MVI in sHCC.
