Against the backdrop of the global COVID-19 pandemic,the teaching and management of clinical medical interns have been facing tremendous challenges.When interns majoring in clinical medicine enter the internship position,they lack self-protection awareness and have limited ability to respond to unexpected public health events.This article explores the cognitive situation,existing problems,and improvement measures of clinical medical interns in the post-epidemic era.Therefore,this article proposes a series of improvement measures,including strengthening epidemic training and education for interns,enhancing personal protective awareness,and lastly achieving the role transition from intern to doctor. 展开更多

目的:阿片类药物是中、重度癌痛治疗的首选药物。目前以缓释阿片类药物为背景的滴定方法的剂量调整时机和方式还需进一步探索。本研究旨在评估盐酸羟考酮缓释片12 h滴定方案用于中重度癌痛患者的有效性和安全性。方法:选择2018年2月至2... 目的:阿片类药物是中、重度癌痛治疗的首选药物。目前以缓释阿片类药物为背景的滴定方法的剂量调整时机和方式还需进一步探索。本研究旨在评估盐酸羟考酮缓释片12 h滴定方案用于中重度癌痛患者的有效性和安全性。方法:选择2018年2月至2019年12月浙江省24家医院收治的中重度阿片类药物未耐受癌痛患者114例[数字分级法(numerical rating scale,NRS)≥4分]作为研究对象。筛选出合并1次以上爆发痛且存在中重度以上疼痛(NRS≥4分)患者87例,按盐酸羟考酮缓释片调整的时间不同分为试验组(12 h滴定组,n=45)和对照组(24 h滴定组,n=42)。试验组起始给予盐酸羟考酮缓释片10 mg并根据疼痛情况给予即释吗啡补救镇痛,12 h后根据即释吗啡量调整剂量,剂量调整为背景剂量+12 h内即释吗啡剂量。对照组起始剂量给予盐酸羟考酮缓释片10 mg q12h,并根据疼痛情况给予即释吗啡补救镇痛,24 h后剂量调整为背景剂量+24 h内即释吗啡剂量/2。比较两组24、48、72 h的疼痛缓解率、不良反应发生率、即释吗啡补救镇痛情况及使用量、生活质量评分,以及镇痛满意度。结果:给药后24、48、72 h,试验组和对照组均显示出较高的疼痛缓解率,组间比较差异无统计学意义(P>0.05)。相对于对照组,试验组在24、48及72 h补救镇痛次数及剂量均显著减少(P<0.05)。两组患者不良反应大多数为轻中度,发生率比较差异无统计学意义(P>0.05)。镇痛满意度水平均较高,组间比较差异无统计学意义(P>0.05)。结论:在中重度癌痛患者应用盐酸羟考酮缓释片为背景的滴定方案中,12 h调整剂量能有效减少补救镇痛次数及剂量,维持较高的镇痛缓解率和镇痛满意度,安全性良好。 展开更多

A biodegradable poly(lactic-co-glycolic acid) loading atorvastatin calcium(AC) nanoparticles(AC-PLGA-NPs) were prepared by probe ultrasonication and evaporation method aiming at improving the oral bioavailability of AC. The effects of experimental parameters, including stabilizer species, stabilizer concentration and pH of aqueous phase, on particle size were also evaluated. The resultant nanoparticles were in spherical shape with an average diameter of 174.7 nm and a narrow particle size distribution. And the drug loading and encapsulation efficiency were about 8% and 71%, respectively. The particle size and polydispersion were almost unchanged in 10 days. The release curves of AC-PLGA-NPs in vitro displaying sustained release characteristics indicated that its release mechanisms were matrix erosion and diffusion. The pharmacokinetic study in vivo revealed that the Cmax and AUC0-∞ of AC-PLGA-NPs in rats were nearly 3.7-fold and 4.7-fold higher than that of pure atorvastatin calcium suspension. Our results demonstrated that the delivery of AC-PLGANPs could be a promising approach for the oral delivery of AC for enhanced bioavailability. 展开更多

目的探讨睾丸结核的临床及超声影像学特征。方法回顾性分析2013年1月至2020年10月西藏自治区人民医院37例临床诊断为睾丸结核的患者的临床及超声影像学资料,总结其特征。结果37例患者中单侧睾丸结核26例,双侧睾丸结核11例。临床症状以... 目的探讨睾丸结核的临床及超声影像学特征。方法回顾性分析2013年1月至2020年10月西藏自治区人民医院37例临床诊断为睾丸结核的患者的临床及超声影像学资料,总结其特征。结果37例患者中单侧睾丸结核26例,双侧睾丸结核11例。临床症状以阴囊肿大、慢性肿胀疼痛为主。超声检查表现多样,以片状低回声及混合回声(17/37,45.9%)、粟粒样结节(18/37,48.6%)、结节或肿块(6/37,16.2%)、窦道形成(8/37,21.62%)为主要超声表现,部分患者可多种征象并存。32例患者同时伴发附睾结核声像图改变,表现为附睾弥漫性增大、结节或脓肿。结论睾丸结核的临床表现及超声声像图表现多样。详细了解相关病史,熟练掌握其超声声像图特征及鉴别诊断要点,有助于做出正确诊断。 展开更多

Background and purpose Stroke is the second leading cause of death worldwide and the leading cause of mortality and long-term disability in China,but its underlying risk genes and pathways are far from being comprehensively understood.We here describe the design and methods of whole genome sequencing(WGS)for 10914 patients with acute ischaemic stroke or transient ischaemic attack from the Third China National Stroke Registry(CNSR-III).Methods Baseline clinical characteristics of the included patients in this study were reported.DNA was extracted from white blood cells of participants.Libraries are constructed using qualified DNA,and WGS is conducted on BGISEQ-500 platform.The average depth is intended to be greater than 30×for each subject.Afterwards,Sentieon software is applied to process the sequencing data under the Genome Analysis Toolkit best practice guidance to call genotypes of single nucleotide variants(SNVs)and insertion-deletions.For each included subject,21 fingerprint SNVs are genotyped by MassARRAY assays to verify that DNA sample and sequencing data originate from the same individual.The copy number variations and structural variations are also called for each patient.All of the genetic variants are annotated and predicted by bioinformatics software or by reviewing public databases.Results The average age of the included 10914 patients was 62.2±11.3 years,and 31.4%patients were women.Most of the baseline clinical characteristics of the 10914 and the excluded patients were balanced.Conclusions The WGS data together with abundant clinical and imaging data of CNSR-III could provide opportunity to elucidate the molecular mechanisms and discover novel therapeutic targets for stroke. 展开更多

In this paper,pure titanium(Ti)thin films deposited by radio frequency sputtering were used as a diffusion barrier layer in a flexible copper indium gallium selenium(CIGS)solar cell on a stainless-steel foil and characterized by X-ray diffraction,scanning electron microscopy and second ion mass spectroscopy measurement methods.The influences of the magnetron sputtering pressure on the surface morphology and preferred crystal orientation of Ti films are discussed.It was found that the Ti film showed a(001)preferred orientation and smooth surface topography at lower deposition pressure,while(002)preferred orientation and relatively rough surface topography at higher deposition pressure.In addition,Ti films made with different process pressures were deposited as the barriers and the second ion mass spectroscopy results indicated that a Ti film with the thickness of 200 nm was able to effectively block Fe and Cr diffusion from the stainless-steel foil into the CIGS absorber across the molybdenum back contact.The Ti barrier significantly improved the conversion efficiency of the CIGS solar cell. 展开更多
