目的明确绝经期女性冠心病(CAD)发病的影响因素,并基于上述影响因素构建绝经期女性冠心病风险预测模型,同时验证其预测效能。方法通过计算机检索国外4个电子数据库(Cochrane Library、Embase、PubMed、Web of Science)和4个中文电子数据...目的明确绝经期女性冠心病(CAD)发病的影响因素,并基于上述影响因素构建绝经期女性冠心病风险预测模型,同时验证其预测效能。方法通过计算机检索国外4个电子数据库(Cochrane Library、Embase、PubMed、Web of Science)和4个中文电子数据库(中国知网、万方数据库、中国生物医学文献数据库、维普数据库),收集建库到2022年12月公开发表的文献。使用RevMan5.4软件运用Meta分析方法对纳入文献5次或以上的影响因素进行频次统计与分析,计算影响绝经期女性CAD发病的相关因素的综合危险度OR,将有统计学意义(P<0.05)的因素纳入构建Logistic回归预测模型并进行外部验证;同时使用2017年9月-2022年7月在广西中医药大学附属瑞康医院心内科住院的303例绝经期女性患者的临床资料绘制列线图模型并验证。结果Meta分析结果显示,合并糖尿病、合并高血压、吸烟、体质量指数(BMI)、冠心病家族史、合并血脂异常、总胆固醇(TC)、低密度脂蛋白(LDL)、血小板平均容积(MPV)、血小板分布宽度(PDW)等10项因素(X_(1)~X_(10))是绝经期女性CAD发病的独立危险因素(P<0.05),构建绝经期女性CAD发病风险预测模型:Logit(P)=-0.26+0.61X_(1)+0.65X_(2)+0.88X_(3)+0.7X_(4)+0.87X_(5)+1.26X_(6)+0.56X_(7)+0.27X_(8)+0.94X_(9)+1.13X_(10),经验证该模型有一定的准确性(AUC=0.630),列线图模型准确度较高(AUC=0.732),经校准度检验及决策曲线分析后均提示模型具有较高的临床应用价值。结论该预测模型具有较好的准确度,可以为临床早期筛查绝经期女性CAD发生风险提供循证依据。展开更多
Objective To evaluate the economy of domestic bivalent human papilloma virus(HPV)vaccine,imported 9-valent HPV vaccine and 5 cervical cancer screening programs,and to provide a reference for relevant decision-making.M...Objective To evaluate the economy of domestic bivalent human papilloma virus(HPV)vaccine,imported 9-valent HPV vaccine and 5 cervical cancer screening programs,and to provide a reference for relevant decision-making.Methods A Markov model of the natural disease development of cervical cancer was constructed to simulate the cumulative long-term cost and quality-adjusted life years(QALYs)of one hundred thousand healthy women after they received different interventions for cost-effectiveness analysis.Results and Conclusion Compared with the non-intervention group,the cost of per QALY obtained by two HPV vaccines and 5 screening programs ranged from 1117.56 yuan to 71660.48 yuan.Taking China’s GDP per capita in 2020 as the threshold,two HPV vaccines and 5 screening programs are cost-effective.Domestic bivalent vaccine is cost-effective and it should be introduced to the national immunization program in the future.Different screening programs are all cost-effective,too.Among them,careHPV test once every 5 years has the lowest ICER value and it can be used as the first choice for cervical cancer screening in rural areas or resource-limited areas in China.展开更多
Objective To study the relationship between essential drug list and the provisions on the drug coverage in tertiary hospitals,and put forward the corresponding countermeasures and suggestions for the adjustment of ess...Objective To study the relationship between essential drug list and the provisions on the drug coverage in tertiary hospitals,and put forward the corresponding countermeasures and suggestions for the adjustment of essential drug list and the improvement of relevant policies.Methods Expert consultation and statistical methods were used to classify the drugs in the list of essential drugs according to different diseases.Besides,the matching degree of basic drug allocation and prescription policy was analyzed.Results and Conclusion Among the 685 drugs in the national essential drug list of 2018 edition,453 drugs were used to treat minor illness,accounting for 66.1%of the total number of essential drug list.232 drugs were for serious illness or fatal illness,accounting for 33.9%of the total number of essential drug list.The current essential drug list does not match the national requirements for drug plan coverage in tertiary hospitals,which needs further adjustment and improvement.展开更多
Objective To understand the real-world study(RWS)and its application in domestic and foreign pharmaceutical device regulatory decision-making,so as to promote its further application in the decision-making of medical ...Objective To understand the real-world study(RWS)and its application in domestic and foreign pharmaceutical device regulatory decision-making,so as to promote its further application in the decision-making of medical devices supervision in China.Methods Literature analysis and comparative analysis were used to compare the real-world evidence(RWE)and randomized controlled trials to obtain the development status of medical devices in China and the United States.Results and Conclusion It is found that the application of RWS in the field of medical devices has many advantages,which can make up for the deficiencies of traditional clinical data.In addition,the RWE has a wide range of applications in the field of medical devices in China.In recent years,the RWE has received extensive attention in the medical field in the United States.The FDA has issued related guidelines,and its applications have gradually formed a complete system.Research on RWE will become the development trend of clinical evaluation of medical devices in the future.While attaching importance to the development of the RWE,China should learn from the specifications of relevant foreign organizations to improve its development in the field of health.展开更多

文摘目的采用Meta分析方法评价血清同型半胱氨酸(Hcy)水平与心力衰竭(HF)相关指标的相关性。方法计算机检索Pub Med、Embase、Cochrane Library、中国知网、维普网、万方数据知识服务平台和中国生物医学文献数据库公开发表的血清Hcy水平与HF相关指标相关性的研究,检索时限为建库至2022年3月。由2名研究者根据纳入与排除标准独立筛选文献、提取资料并交叉核对,采用纽卡斯尔-渥太华量表(NOS)独立评价纳入文献的质量,采用Rev Man 5.4软件进行Meta分析。结果最终纳入文献68篇,其中中文文献66篇、英文文献2篇,总样本量为7058例。Meta分析结果显示,HF患者血清Hcy水平与N末端脑钠肽前体(NT-proBNP)、脑钠肽(BNP)、左心室舒张末期内径(LVEDD)、左心室舒张末期容积(LVEDV)、左心室质量指数(LVMI)、左心室后壁厚度(LVPWT)、E/A比值、室间隔基底段厚度(IVS)、纽约心脏病协会(NYHA)分级呈正相关,与左心室射血分数(LVEF)呈负相关(P<0.05)。敏感性分析结果显示,逐一剔除文献后Meta分析结果基本一致,表明结果基本可靠。根据患者年龄进行亚组分析,结果显示,<60岁、≥60岁HF患者血清Hcy水平与NT-proBNP、BNP、LVEDD均呈正相关,与LVEF均呈负相关(P<0.05)。根据患者来源(地区)进行亚组分析,结果显示:我国南方、我国北方、国外HF患者血清Hcy水平与NT-proBNP均呈正相关(P<0.05);我国南方、我国北方HF患者血清Hcy水平与BNP、LVEDD均呈正相关,与LVEF均呈负相关(P<0.05)。发表偏倚分析结果显示,报道HF患者血清Hcy水平与NT-proBNP、BNP、LVEF、LVEDD、NYHA分级相关性文献的漏斗图散点分布基本对称,提示不存在发表偏倚。结论HF患者血清Hcy水平与NT-proBNP、BNP、LVEDD、LVEDV、LVMI、LVPWT、E/A比值、IVS、NYHA分级呈正相关,与LVEF呈负相关;且血清Hcy水平与NT-proBNP、BNP、LVEF、LVEDD之间的关系不受年龄、地区影响。
