Treatment of Single Level Lumbar Spondylolisthesis with Lumbar Interbody Fusion via Oblique Lateral Approach (OLIF)

Objectives: To investigate the effect of lumbar interbody fusion via the oblique lateral approach (OLIF) in the treatment of single level lumbar spondylolisthesis. Methods: Retrospective analysis was made on 32 cases of single level lumbar spondylolisthesis treated by lumbar interbody fusion via the oblique lateral approach from July 2020 to July 2021. 14 males and 18 females;the age was (66.5 ± 11.5) years (55 - 82 years). 1) The operation time, intraoperative blood loss and complications were recorded;2) the scores of visual analog scale. VAS and Oswestry disability index (ODI) of low back pain and lower limb pain were collected before operation and at the last follow-up;by observing the imaging data, the height of the intervertebral space, the anterior convex angle of the intervertebral space, the anterior convex angle of the lumbar spine, the sagittal diameter of the dural sac and the spondylolisthesis were measured. Results: All patients successfully completed the operation, the average operation time was (103.9 ± 21.1) min, the average intraoperative bleeding volume was (72.3 ± 16.4) ml. There was no vascular injury during the operation, no infection occurred in all surgical incisions, and Class I/A healing was achieved. The VAS scores of low back pain and leg pain before operation and at the last follow-up were lower than those before operation, and the difference was statistically significant (P < 0.05);the ODI at the last follow-up was lower than that before operation, and the difference was statistically significant (P < 0.05). At the last follow-up, the height of intervertebral space, the height of intervertebral foramen and the sagittal diameter of dural sac were greater than those before operation, with statistically significant differences (P < 0.05);the spondylolisthesis rate at the last follow-up was lower than that before operation, with a statistically significant difference (P < 0.05). Left thigh surface numbness occurred in 2 cases (6.3%) and disappeared after 1 week;Hip flexion weakness occurred in 1 case (0.03%), which recovered after 12 days;there were no complications such as retroperitoneal hematoma, ureteral injury, retrograde ejaculation, intestinal and lumbar plexus injury. Conclusion: The early clinical effect of OLIF in the treatment of single level lumbar spondylolisthesis is significant. This surgical method is minimally invasive, safe and effective, which can significantly reduce the amount of intraoperative bleeding and reduce the risk of postoperative complications. Its main working principle is to make the annulus fibrosus, posterior longitudinal ligament and ligamentum flavum shrink and recover the height of the intervertebral space through decompression, loosening and stretching of the intervertebral space, so as to achieve the reduction of the slipped vertebral body, increase the height of the intervertebral foramen Enlarge the spinal canal volume and eliminate dynamic compression to play an indirect decompression role, improve the symptoms of low back and leg pain, and reconstruct the stability of the spine through interbody fusion. 展开更多

目的分析癌症患者疼痛程度、疲劳程度与生活质量之间的关系。方法采用横断面研究设计。应用便利抽样法于2019年5-11月选取两省两所三级甲等医院224例接受化疗且符合入选标准的癌症患者进行调查。采用一般资料调查表、简明疲劳量表(Brief... 目的分析癌症患者疼痛程度、疲劳程度与生活质量之间的关系。方法采用横断面研究设计。应用便利抽样法于2019年5-11月选取两省两所三级甲等医院224例接受化疗且符合入选标准的癌症患者进行调查。采用一般资料调查表、简明疲劳量表(Brief Fatigue Inventory,BFI)、疼痛数字评分量表(The Nume-rical Rating Scale,NRS)和由欧洲癌症治疗和研究组织编制的生活质量量表(The European Organization for Research and Treatment of Cancer Quality of Life Questionnaire,EORTCQLQ-C30)收集资料。结果在报告过去24 h最严重的疼痛情况时,85例(37.9%)有轻度疼痛,121例(54.0%)有中度疼痛,18例(8.0%)有重度疼痛。此外,92例(41.1%)患者有轻度疲劳,72例(32.1%)有中度疲劳,60例(26.8%)有重度疲劳。大多数轻度疼痛的患者仅经历轻度疲劳,其生活质量也处于中等水平。中度和重度疼痛患者大多有中度或更高程度的疲劳(r=0.296,P<0.01)和较低的生活质量(r=-0.141,P<0.01)。轻度疼痛患者中,疲劳与生活质量间不存在相关性(r=-0.179,P=0.104)。中、重度疼痛患者的疲劳与生活质量呈负相关(r=-0.537,P<0.01;r=-0.509,P<0.05)。结论中、重度疼痛患者的疲劳症状比轻度疼痛患者更重,生活质量也更低。护士应更关注中重度疼痛患者,探索症状间的相互作用机制,开展症状联合干预,以提高患者生活质量。 展开更多

Controlled-release urea(CRU)is widely reported to supply crop nitrogen(N)demand with one basal application,thus effectively replacing split applications of urea without diminishing grain yield and N use efficiency(NUE).However,its use for replacement for high-yield split applications of urea(CK)for rice is untested.In addition,the degree to which greenhouse gas(GHG)emissions in rice systems are affected when CRU is substituted for CK remains unclear.During 2017 and 2018,we sampled plant growth and gas emissions in a rice paddy field treated with three CRU types(sulfur-coated urea[SCU],polymer-coated urea[PCU],and bulk blended CRU[BBU])applied via two methods(surface broadcasting on the soil and subsurface banding at 5 cm depth),with CK as a control.The three CRUs led to different soil NH_(4)^(+)-N dynamics,and the N supply pattern under BBU was more beneficial for rice seedling establishment than under SCU and PCU,resulting in grain yield and NUE comparable to those under CK.CRU type showed no significant effect on either CH_(4) emissions or N_(2)O emissions,and broadcast CRUs exhibited significantly higher total GHG emissions than CK.However,banded CRUs significantly reduced the total GHG emissions in comparison with broadcast CRUs,by 9.2%averaged across the two years.Reduced CH_(4) emissions,particularly during the period prior to the middle drainage,contributed largely to the GHG difference.With comparably high grain yield and low total GHG emissions,banded BBU showed a low yield-scaled GHG(GHG emissions divided by grain yield)comparable to that under CK in both years.Overall,our study suggested that N management synchronized with rice demand and contributing to a high NUE tended to minimize yield-scaled GHG.Broadcast CRU can hardly substitute for CK in terms of either grain yield or GHG emissions,but banded BBU is a promising N management strategy for sustaining rice production while minimizing environmental impacts. 展开更多

Comparison of DAA and PLA Approaches for Total Hip Replacement in the Treatment of Femoral Neck Fractures

Objectives: Femoral neck fractures are becoming more common within nowadays. This research is to explore the clinical effect of primary total hip replacement (THA) via direct anterior approach (DAA) and posterolateral approach (PLA) in the treatment of femoral neck fracture. Methods: Retrospective analysis of 100 cases of elderly patients with femoral neck fracture who underwent total hip arthroplasty admitted to Tianyou Hospital affiliated to Wuhan University of Science and Technology from January 2019 to January 2022. 50 patients treated with DAA approach were included in the observation group, and 50 patients treated with PLA approach were included in the control group. The operation indexes, postoperative acetabular abduction angle and anteversion angle, hip joint function, Harris score and complications were compared between the two groups. Result: The length of incision in the observation group was shorter than that in the control group, and the amount of intraoperative bleeding and postoperative hospital stay were shorter than those in the control group (P < 0.05);There was a statistically significant difference between the two groups in the ratio of acetabular abduction angle and its safe zone, and the length difference of both lower limbs (P < 0.05), while there was no statistically significant difference between the two groups in the ratio of acetabular anteversion angle and its safe zone, eccentricity, and its recovery rate (P > 0.05);Harris score of hip joint: 6 months after operation, the anterior approach group was significantly higher than the posterolateral approach group (P < 0.05), and there was no statistical difference between the two groups 12 months after operation (P > 0.05);The total incidence of complications in the observation group was lower than that in the control group, with a statistically significant difference (P Conclusion: DAA and PLA approaches for total hip replacement can restore the hip joint structure of patients with femoral neck fractures and achieve good results, but DAA approach has greater advantages in early postoperative recovery, improvement of hip joint function, small surgical injury, high application value, so it is recommended. 展开更多

Although great accomplishments of functional material synthesis have been achieved in sodium ion batteries(SIBs)recently,there are still numerous challenges and problems in preparing carbon-based materials with porous architectures and enough lattice distance for Na^+insertion.Herein we report a templated strategy to synthesize 3D porous graphene girds(PGGs)consisting of several stacking graphene structure with ultrahigh surface area and hierarchical connected structure by employing Ag nanoparticles(NPs).The Ag NPs will regenerate for decreasing the experimental cost,also in line with principles of green chemistry and environmentally friendly strategy.The PGGs obtain advanced specific capacity of160 m A h g^(-1)at current density of 50 m A h g^(-1).Moreover,112 mA h g^(-1)capacity can be gained at 1 A h g^(-1)during 1000 cycles.Due to their porous architecture,ultrahigh surface area and low amorphous graphited structure,PGGs electrode showed the excellent electrochemical performance in high rate capability. 展开更多

Objectives: To explore the treatment and related prognosis of elderly patients with Anderson II odontoid fracture with posterior pedicle screw fixation combined with iliac bone grafting. Methods: Retrospective analysis of 17 cases of elderly patients with Anderson II odontoid fracture who underwent posterior pedicle screw fixation combined with iliac bone grafting from January 2013 to December 2016. 17 patients had a history of trauma before surgery, and they all have varying degrees of atlantoaxial instability or subluxation and varying degrees of neck occipital pain and limited mobility. Result: No spinal cord or vertebral artery injury occurred during surgery. Follow-up information is complete. The follow-up period was 6 to 48 months (mean 27.4 ± 12.4 months). Postoperative imaging review prompted a good reduction of cervical spine, stable sequence;no pedicle screw loosening, fracture, iliac bone graft at the location of the situation, odontoid fracture and bone healing at the good, the patient after cervical rotation are limited to varying degrees. Conclusion: Posterior pedicle screw fixation combined with iliac bone grafting in elderly patients with Anderson II odontoid fracture can achieve good stability, and the prognosis is good, but long-term cervical rotation function may be affected to varying degrees. 展开更多

BACKGROUND The outcomes of the use of commercial in vitro maturation(IVM)medium to culture immature oocytes obtained from conventional ovulation induction,followed by rescue intracytoplasmic sperm injection(RICSI),are not ideal.It is thus difficult to widely adopt this approach in clinical practice.Therefore,it is necessary to explore methods for improving the clinical outcome of IVM.AIM To study the effect of sperm on the developmental potential of in vitro-matured oocytes in conventional culture.METHODS This was a retrospective study of patients whose immature oocytes were harvested from conventional oocyte stimulation cycles and underwent ICSI at our hospital between June 2018 and August 2020.RICSI was performed using sperm collected on the day of oocyte harvest(old)and sperm collected on the day of RICSI(fresh)and oocytes matured in vitro after 24 h of culture in conventional medium.The rates of in vitro oocyte maturation,normal fertilization,normal cleavage,day-3 top-quality embryos,and useful blastocyst formation were compared between the two groups.RESULTS In total,102 germinal vesicle(GV)-stage immature oocytes were cultured in the old sperm group.In the fresh sperm group,122 GV-stage immature oocytes were collected and cultured in vitro for 24 h.There were no significant differences in the general conditions of males and females between the two groups(P>0.05).The oocyte maturation,normal fertilization,and normal cleavage rates of the old and fresh groups were 51.0%vs 55.7%,61.5%vs 64.7%,and 93.8%vs 93.2%,respectively.None of the rates differed significantly(P>0.05)between the two groups.However,the day-3 top-quality embryo and useful blastocyst rates of the old and fresh sperm groups were 16.6%vs 63.4%;6.67%vs 34.6%,respectively.The day-3 top-quality embryos and useful blastocyst rates of the old sperm group were significantly lower than those of the fresh group(P<0.05).CONCLUSION In vitro maturation with conventional culture medium combined with the use of fresh sperm collected on the day of RICSI is an easy-to-implement strategy for patients whose oocytes are completely or mostly immature. 展开更多

Wound healing has become one of the basic issues faced by the medical community because of the susceptibility of skin wounds to bacterial infection.As such,it is highly desired to design a nanocomposite hydrogel with excellent antibacterial activity to achieve high wound closure effectiveness.Here,based on ultrasound-triggered piezocatalytic therapy,a multifunctional hydrogel is designed to promote bacteria-infected wound healing.Under ultrasonic vibration,the surface of barium titanate(BaTiO_(3),BT)nanoparticles embedded in the hydrogel rapidly generate reactive oxygen species(ROS)owing to the established strong built-in electric field,endowing the hydrogel with superior antibacterial efficacy.This modality shows intriguing advantages over conventional photodynamic therapy,such as prominent soft tissue penetration ability and the avoidance of serious skin phototoxicity after systemic administration of photosensitizers.Moreover,the hydrogel based on N-[tris(hydroxymethyl)methyl]acrylamide(THM),N-(3-aminopropyl)methacrylamide hydrochloride(APMH)and oxidized hyaluronic acid(OHA)exhibits outstanding self-healing and bioadhesive properties able to accelerate full-thickness skin wound healing.Notably,compared with the widely reported mussel-inspired adhesive hydrogels,OHA/THM-APMH hydrogel due to the multiple hydrogen bonds from unique tri-hydroxyl structure overcomes the shortage that catechol groups are easily oxidized,giving it long-term and repeatable adhesion performance.Importantly,this hybrid hydrogel confines BT nanoparticles to wound area and locally induced piezoelectric catalysis under ultrasound to eradicate bacteria,markedly improving the therapeutic biosafety and exhibits great potential for harmless treatment of bacteria-infected tissues. 展开更多

Flat optics has been considered promising for constructions of spaceborne imaging systems with apertures in excess of 10 m.Despite recent advances,there are long-existing challenges to perform in-phase stitching of multiple flat optical elements.Phasing the segmented planar instrument has remained at the proof of concept.Here,we achieve autonomous system-level cophasing of a 1.5-m stitching flat device,bridging the gap between the concept and engineering implementation.To do so,we propose a flat element stitching scheme,by manipulating the point spread function,which enables our demonstration of automatically bringing seven flat segments'tip/tilt and piston errors within the tolerance.With phasing done,the 1.5-m system has become the largest phased planar instrument ever built in the world,to our knowledge.The first demonstration of phasing the large practical flat imaging system marks a significant step towards fielding a 10-m class one in space,also paving the way for ultrathin flat imaging in various remote applications. 展开更多

Microbial communities in sediment are an important indicator linking to environmental pollution in urban river systems.However,how the diversity and structure of bacterial communities in sediments from an urban river network respond to different environmental factors has not been well studied.The goal of this study was to understand the patterns of bacterial communities in sediments from a highly dense urbanized river network in the lower Yangtze River Delta by Illumina MiSeq sequencing.The correlations between bacterial communities,the environmental gradient and geographical distance were analyzed by redundancy analysis(RDA)and network methods.The diversity and richness of bacterial community in sediments increased from upstream to downstream consistently with the accumulation of nutrient in the urban river network.Bacterial community composition and structure showed obvious spatial changes,leading to two distinct groups,which were significantly related to the characteristics of nutrient and heavy metal in sediments.Humic substance,available nitrogen,available phosphorus,Zn,Cu,Hg and As were selected as the key environmental factors shaping the bacterial community in sediments based on RDA.The co-occurrence patterns of bacterial networks showed that positive interaction between bacterial communities increased but the connectivity among bacterial genera and stability of sediment ecosystem reduced under a higher content of nutrient and heavy metal in average.The sensitive and ubiquitous taxa with an overproportional response to key environmental factors were detected as indicator species,which provided a novel method for the prediction of the pollution risk of sediment in an urban river network. 展开更多

Landfill leachate is a complex effluent and it is difficult to deal with. Electrochemical methods have been considered as a promising alternative technology for treatment of landfill leachate with refractory organic contaminants and heavy metals. Peroxi-coagulation(PC)process with iron anode and modified graphite felt cathode was developed for efficient landfill leachate concentrate treatment. Compared to electro-Fenton(EF) and electrocoagulation(EC) processes, the PC process was more cost-effective due to the combined action of·OH oxidation and iron hydroxides coagulation. A maximal TOC removal of 77.2% ± 1.4% was obtained after 360 min at initial p H = 5.0 and current density of 10 m A/cm^(2). After the PC process, concentrations of all seven heavy metals in the final effluents were below the allowable emission limits given by the present regulatory standards. The method preference for heavy metal removal was PC > EC > EF. Based on the three-dimensional fluorescence spectroscopy coupled with regional integration analysis during the PC treatment, the florescence peaks of both humic acids and fulvic acids disappeared after treatment for 360min. Decreasing trends were observed in the fluorescent regional standard volumes for aromatic protein Ⅰ(31.4%), aromatic protein Ⅱ(63.7%), fulvic acid-like(69.5%), soluble microbial by-product-like(75%) and humic acid-like regions(76.3%). The results indicate that comparing to the EF and EC process, the PC process provide a promising and more cost-effective alternative for the treatment of landfill leachate concentrate. 展开更多

The full aperture complex amplitude transmittance function of a multi-level diffraction lens with mask- alignment errors was derived based on scalar diffraction theory. The point spread function (PSF) was calculated by the Kirchhoff diffraction integral. It is found that the radius of the Airy disk increases with the increase of the error in the direction of misalignment, and the image center shifts along the direction of misalignment. A fourlevel diffractive lens with a diameter of 80 mm was fabricated, and its PSF and diffraction efficiency of +1st order were calculated and measured. Naturally occurring arsenic enrichment in groundwater poses a huge threat to human health.Air cathode electrocoagulation(ACEC)has recently been proposed to enhance As(Ⅲ)oxidation and lower energy consumption.In this study,ACEC,EC/O_(2) and EC/N_(2) were evaluated with different current densities from 1 to 8 mA/cm^(2) to investigate the effect on As(Ⅲ)removal in different redox environments.Current density had no appreciable effect on arsenic removal efficiency given the same charge in ACEC because the concentration ratio of Fe/H2O_(2) under different current densities remained stable.However,in EC/O_(2) and EC/N_(2),As(Ⅲ)removal was inhibited at higher current densities(4–8 mA/cm^(2)),likely because more Fe(Ⅱ)competed with As(Ⅲ)for the oxidant,leading to less effective oxidation of As(Ⅲ).In all EC systems,the$OH units generated per power consumption reached the highest value at the lowest current density.Compared with other EC systems,the ACEC system showed lower energy consumption at all current densities due to the low energy consumption of the electrode reaction and more free radical generation.A lower current density saved more energy at the expense of time,showing the trade-off relationship between energy consumption and removal time.The operation costs for As(Ⅲ)removal under optimal conditions were calculated as 0.028$/m^(3) for ACEC,0.030$/m^(3) for EC/O_(2),and 0.085$/m^(3) for EC/N_(2) 展开更多

Effects of maturity of citrus fruits on their stalks cutting force

When the citrus harvesting robot harvests citruses,the mechanical properties of citrus stalks have an important influence on the success rate of the citrus harvesting robot.During the harvesting,the maturity of citrus fruits not only determined the harvesting time of citrus fruits but also affected the mechanical properties of citrus fruit stalks.In this study,the changes in the cutting force of citrus fruit stalks were described during the maturity of citrus fruits,and the effect of the maturity on the cutting force of stalks was clarified,so as to determine the harvesting time with the minimum cutting force required for harvesting citrus fruits by the harvesting robot.During the maturity,the relevant parameters of fruit maturity,such as the hardness,pH,and solid solution content of citrus fruits,were monitored.The results showed that there is a significant correlation between the hardness,pH,the solid solution content of citrus fruits,and the cutting force of citrus fruit stalks during maturity.The single-factor mechanical model of hardness,pH,solid solution content of citrus fruits,and the cutting force of citrus fruit stalks were established based on the data of 2019,which were verified through tests in 2020.The test results are as follows:during the ripening period of citrus fruits,the fruit hardness varies in the range of 0.13-0.31 MPa,the hardness changes by 0.02 MPa,and the cutting force changes by about 2.0-6.0 N;the pH of the citrus fruits changes in the range of 2.8-4.0,and the cutting force changes by about 1.5-2.2 N for every 0.1 change in the pH;the variation range of fruit solid solution content is 6.5%-9.0%,and for every 0.2%change in solid solution,the cutting force of citrus fruit stalks changes by about 1.25-2.0 N.The mechanical models can predict the cutting force required to cut off citrus fruit stalks according to the relevant parameters of citrus fruit maturity and can provide a reference for effectively evaluating the required cutting force. 展开更多
