目的探讨高频重复经颅磁刺激(rTMS)对男性酒精依赖患者渴求及认知功能的影响。方法选取2019年3月—2019年7月新乡医学院第二附属医院住院的66例男性酒依赖患者。患者经急性期苯二氮卓类药物替代治疗后,根据随机数字表法随机分为研究组... 目的探讨高频重复经颅磁刺激(rTMS)对男性酒精依赖患者渴求及认知功能的影响。方法选取2019年3月—2019年7月新乡医学院第二附属医院住院的66例男性酒依赖患者。患者经急性期苯二氮卓类药物替代治疗后,根据随机数字表法随机分为研究组和对照组。对照组接受中等剂量选择性5羟色胺再摄取抑制药治疗,研究组另加用rTMS治疗。患者在治疗前后采用渴求自评量表及认知功能评估,后者包括简易精神状态检查量表、数字广度测验、词语流畅性测验及霍普金斯词语学习,比较两组治疗前后的改变。结果两组年龄、受教育年限、首次饮酒年龄、累计饮酒时间及每日饮用白酒量比较,差异无统计学意义(P>0.05)。两组治疗前渴求自评及认知功能比较,差异无统计学意义(P>0.05)。研究组、对照组治疗后的渴求自评及认知功能较治疗前有改善(P<0.05)。研究组渴求自评较对照组高(P<0.05),认知功能较对照组低(P<0.05)。结论rTMS可以降低酒依赖患者的渴求,改善认知功能。 展开更多

A 28-year-old woman was admitted to hospital with chief complains of cough, sputum, and dyspnea. The clinical laboratory tests for blood revealed an increased eosinophil percentage of 31.3%and significantly elevated total IgE. The chest computed tomography scan revealed that bilateral bronchial walls were thickening, accompanied with patchy spots scattered throughout bilateral lungs. A suspected multiflagellated protozoan was observed under a light microscope. But some different features were observed by electron microscopy, such as the orientation of flagella and nucleus. Besides, both bronchoalveolar lavage fluid and bronchoscopic brush smears underwent Gram staining and Pap staining, which revealed that numerous respiratory ciliated cells were scattered or accumulated in the sample.Finally, she was diagnosed with eosinophil pneumonia. Metronidazole,bronchodilators, and mucolytics were taken for 5 d and symptoms and pulmonary ventilation function improved. We herein report a case of chronic eosinophilic pneumonia, which was misdiagnosed as multiflagellated protozoan infection, and it is suggested that reliable diagnosis approaches are necessary,rather than clinical symptoms and morphological features. 展开更多

The fractions of post-binary-interaction stars and evolved blue straggler stars on the red giant branch of globular clusters

The red giant branch(RGB)of globular clusters(GCs)is home to some exotic stars,which may provide clues on the formation of multiple stellar populations in GCs.It is well known that binary interactions are responsible for many exotic stars.Thus,it is important to understand what fraction of stars on the RGB of GCs is the result of binary interactions.In this paper,we performed a binary population synthesis study to track the number of post-binary-interaction(post-BI)stars that appear on the RGB,with particular emphasis on the evolved blue straggler stars(E-BSSs).Assuming an initial binary fraction of nearly 50%,we find that about half of the objects on the RGB(called giants)underwent the binary interactions,and that E-BSSs account for around 10%of the giants in our standard simulation.We also compare the properties of post-BI giants that evolved from different channels.We find that the initial orbital period and mass ratio distributions significantly affect the fraction of post-BI giants.Our results imply that the non-standard stars from binary interactions provide a non-negligible contribution to the RGB stars in GCs,which should be considered in future investigations of the origin of multiple stellar populations. 展开更多

Antimicrobial resistance profiles of ESKAPE and Escherichia coli isolated from blood at a tertiary hospital in China

Bloodstream infections(BSIs)have emerged as a worldwide concern because of their increasing incidence,mortality,and associated healthcare cost.Strains in ESKAPE including Enterococcus faecium(E.faecium),Staphylococcus aureus(S.aureus),Klebsiella pneumonia(K.pneumonia),Acinetobacter baumannii(A.baumannii),Pseudomonas aeruginosa(P.aeruginosa),and Enterobacter spp.,as well as Escherichia coli(E.coli),account for more than half of the causative pathogens of BSI.[1]Moreover,the incidences and resistance rates of ESKAPE and E.coli have changed in recent years.A study in Rome found that E.coli,followed by S.aureus,was the most frequently identified microorganism;further,the study revealed that,regardless of hospital or community-onset,the number of extended-spectrumβ-lactamases-producing E.coli was increasing,and carbapenemase-producing K.pneumoniae had become more prevalent during the study period.[1]A recent study suggested upward resistance trends of ESKAPE and E.coli organisms isolated from blood cultures from 2014 to 2018 in a hospital in Italy.[2]The frequencies and resistance rates of ESKAPE and E.coli are most likely interrelated with the spectrum of disease,geographical situation,antimicrobial management,and hand hygiene.Since 2012,some requirements have been gradually executed according to the Measures for the Administration of Clinical Application of Antibiotics(Decree No.84 of the Ministry of Health),the Guidelines for Clinical Application of Antibiotics(2015 edition),and the Notice on Continuous Management of Clinical Application of Antibiotics(General Office of the National Health Commission,2018)at our hospital.However,it is unclear whether these measures are effective to minimize the antibiotic resistance rate.This study investigated the resistance rates and antimicrobial resistance indexes(ARIs)of ESKAPE and E.coli isolated from blood samples at our hospital,and aimed to contribute to the research on ESKAPE and E.coli in China as well as the availability of methods for antibiotic administration. 展开更多
