43岁男性,诊断为占位性左侧枕叶出血,血肿破入整个脑室系统。入院时处于深昏迷状态,有脑疝的临床体征,Glasgow昏迷量表评分4分。由于患者年龄较轻且无高血压史,因此高度怀疑存在血管畸形。鉴于患者存在生命危险,医生建议将开颅和血肿清... 43岁男性,诊断为占位性左侧枕叶出血,血肿破入整个脑室系统。入院时处于深昏迷状态,有脑疝的临床体征,Glasgow昏迷量表评分4分。由于患者年龄较轻且无高血压史,因此高度怀疑存在血管畸形。鉴于患者存在生命危险,医生建议将开颅和血肿清除术作为首选治疗方案,但被其家属以非医疗原因拒绝。为挽救患者的生命,决定行立体定向抽吸术,并得到患者家属认可。在操作过程中和操作后5d内应用了大剂量尿激酶(50 000IU)治疗。1个月后,该患者转归良好,Glasgow转归量表评分为5分。 展开更多

模糊建筑整体水雾弥漫,是自然与人类的合力之作。纳沙泰尔湖(Lake Neuchatel)湖水经水泵抽出并过滤后,再由3.5万个高压喷嘴喷射成绵绵细雾。智能天气系统随时检测温度、湿度、风速、风向等不断变化的气象条件,调节各个区域的水压压力。... 模糊建筑整体水雾弥漫,是自然与人类的合力之作。纳沙泰尔湖(Lake Neuchatel)湖水经水泵抽出并过滤后,再由3.5万个高压喷嘴喷射成绵绵细雾。智能天气系统随时检测温度、湿度、风速、风向等不断变化的气象条件,调节各个区域的水压压力。一踏入"模糊建筑",眼前雾气蔼蔼,耳旁淅淅沥沥,视觉与听觉也会变得模糊起来。沉浸式环境通过越发精湛的科学技术追求高清高保真的视觉体验。而"模糊建筑"逆道而行,极尽朦胧之所能。该展馆设计尝试摆脱周边环境的束缚。游客置身其中,活动无拘无束,目之所及似有似无,宛如梦幻。层层阶梯在水雾中若隐若现,通向蓝天。公众踏着阶梯,可登上天使甲板(Angel Deck)。这座建筑选址湖边。水不仅是其主要材料,也令游客置身其中,宛若步入瑶池,仿佛畅饮琼浆玉液。艺术大师克里斯蒂安·马克雷(Christian Marclay)还采用轻质张拉整体结构,为其内部打造了沉浸式的声学环境。该结构由四根立柱支撑,75英尺高、300英尺宽、200英尺深。 展开更多

目的探讨脑转移瘤同期推量适形放疗与调强放疗的剂量学差异。方法选择在本院放疗的41例脑转移瘤患者为对象,分别设计全脑照射联合局部同期推量治疗的2种放疗计划,即三维适形放疗计划(3DCRT)、调强放疗计划(IMRT),处方给量为全脑3 000cGy... 目的探讨脑转移瘤同期推量适形放疗与调强放疗的剂量学差异。方法选择在本院放疗的41例脑转移瘤患者为对象,分别设计全脑照射联合局部同期推量治疗的2种放疗计划,即三维适形放疗计划(3DCRT)、调强放疗计划(IMRT),处方给量为全脑3 000cGy/10次+转移瘤同期推量4 000cGy/10次;比较并分析2种放疗计划的靶区适形度(CN)、靶区均匀指数(HI)和机器跳数(MU)。结果 2种放疗计划均能满足临床要求;在CN方面,IMRT明显优于3DCRT(P<0.05);在HI、MU方面,3DCRT明显优于IMRT(均P<0.05)。2个转移瘤且无共层和3□4个转移瘤且有共层的情况下,IMRT与3DCRT在CN方面比较差异无统计学意义(P>0.05),3DCRT在HI、MU方面均优于IMRT(均P<0.05);3个转移瘤且无共层的情况下,3DCRT与IMRT在CN、HI方面比较差异均无统计学意义(均P>0.05);3DCRT在MU方面优于IMRT(P<0.05)。结论 2种放疗计划均能满足临床要求。在某些特定情况下,如2□3个转移瘤且无共层或3□4个转移瘤且有共层,3DCRT较优。 展开更多

The cause of glaucoma,a disease defined by degeneration of retinal ganglion cells associated with cupping of the optic nerve head(ONH)and progressive vision loss,continues to be debated despite numerous advances in ophthalmic imaging and diagnostics.Although elevated intraocular pressure is often implicated,many studies,using a variety of imaging techniques including plain fundus photography,fluorescein angiography,laser Doppler flowmetry,and color Doppler ultrasound(1-4),have suggested that insufficiencies of ONH circulation may play a role in glaucoma pathogenesis.While all of these imaging modalities have been informative,each has a set of limitations,including low spatial resolution,invasiveness,restriction to superficial vessels,lack of repeatability,and modality specific artifacts.Optical coherence tomography angiography(OCTA),an imaging technology that can produce high resolution,three-dimensional images of fundus microcirculation,is well suited to overcome many of these limitations(5). 展开更多

Peptides are increasingly important resources for biological and therapeutic development,however,their intrinsic susceptibility to proteolytic degradation represents a big hurdle.As a natural agonist for GLP-1R,glucagon-like peptide 1(GLP-1)is of significant clinical interest for the treatment of type-2 diabetes mellitus,but its in vivo instability and short half-life have largely prevented its therapeutic application.Here,we describe the rational design of a series of a/sulfono-γ-AA peptide hybrid analogues of GLP-1 as the GLP-1R agonists.Certain GLP-1 hybrid analogues exhibited enhanced stability(t_(1/2)>14 days)compared to t_(1/2)(<1 day)of GLP-1 in the blood plasma and in vivo.These newly developed peptide hybrids may be viable alternative of semaglutide for type-2 diabetes treatment.Additionally,our findings suggest that sulfono-γ-AA residues could be adopted to substitute canonical amino acids residues to improve the pharmacological activity of peptide-based drugs. 展开更多
