Encouraging and astonishing developments have recently been achieved in image-based diagnostic technology.Modern medical care and imaging technology are becoming increasingly inseparable.However,the current diagnosis pattern of signal to image to knowledge inevitably leads to information distortion and noise introduction in the procedure of image reconstruction(from signal to image).Artificial intelligence(AI)technologies that can mine knowledge from vast amounts of data offer opportunities to disrupt established workflows.In this prospective study,for the first time,we develop an AI-based signal-toknowledge diagnostic scheme for lung nodule classification directly from the computed tomography(CT)raw data(the signal).We find that the raw data achieves almost comparable performance with CT,indicating that it is possible to diagnose diseases without reconstructing images.Moreover,the incorporation of raw data through three common convolutional network structures greatly improves the performance of the CT models in all cohorts(with a gain ranging from 0.01 to 0.12),demonstrating that raw data contains diagnostic information that CT does not possess.Our results break new ground and demonstrate the potential for direct signal-to-knowledge domain analysis. 展开更多

Camouflaged people are extremely expert in actively concealing themselves by effectively utilizing cover and the surrounding environment. Despite advancements in optical detection capabilities through imaging systems, including spectral, polarization, and infrared technologies, there is still a lack of effective real-time method for accurately detecting small-size and high-efficient camouflaged people in complex real-world scenes. Here, this study proposes a snapshot multispectral image-based camouflaged detection model, multispectral YOLO(MS-YOLO), which utilizes the SPD-Conv and Sim AM modules to effectively represent targets and suppress background interference by exploiting the spatial-spectral target information. Besides, the study constructs the first real-shot multispectral camouflaged people dataset(MSCPD), which encompasses diverse scenes, target scales, and attitudes. To minimize information redundancy, MS-YOLO selects an optimal subset of 12 bands with strong feature representation and minimal inter-band correlation as input. Through experiments on the MSCPD, MS-YOLO achieves a mean Average Precision of 94.31% and real-time detection at 65 frames per second, which confirms the effectiveness and efficiency of our method in detecting camouflaged people in various typical desert and forest scenes. Our approach offers valuable support to improve the perception capabilities of unmanned aerial vehicles in detecting enemy forces and rescuing personnel in battlefield. 展开更多

Structural similarity of lithospheric velocity models of Chinese mainland

Existing lithospheric velocity models exhibit similar structures typically associated with the first-order tectonic features,with dissimilarities due to different data and methods used in model generation.The quantification of model structural similarity can help in interpreting the geophysical properties of Earth's interior and establishing unified models crucial in natural hazard assessment and resource exploration.Here we employ the complex wavelet structural similarity index measure(CW-SSIM)active in computer image processing to analyze the structural similarity of four lithospheric velocity models of Chinese mainland published in the past decade.We take advantage of this method in its multiscale definition and insensitivity to slight geometrical distortion like translation and scaling,which is particularly crucial in the structural similarity analysis of velocity models accounting for uncertainty and resolution.Our results show that the CW-SSIM values vary in different model pairs,horizontal locations,and depths.While variations in the inter-model CW-SSIM are partly owing to different databases in the model generation,the difference of tomography methods may significantly impact the similar structural features of models,such as the low similarities between the full-wave based FWEA18 and other three models in northeastern China.We finally suggest potential solutions for the next generation of tomographic modeling in different areas according to corresponding structural similarities of existing models. 展开更多

Complex plasma widely exists in thin film deposition,material surface modification,and waste gas treatment in industrial plasma processes.During complex plasma discharge,the configuration,distribution,and size of particles,as well as the discharge glow,strongly depend on discharge parameters.However,traditional manual diagnosis methods for recognizing discharge parameters from discharge images are complicated to operate with low accuracy,time-consuming and high requirement of instruments.To solve these problems,by combining the two mechanisms of attention mechanism(strengthening the extraction of the channel feature)and shortcut connection(enabling the input information to be directly transmitted to deep networks and avoiding the disappearance or explosion of gradients),the network of squeeze and excitation convolution with shortcut(SECS)for complex plasma image recognition is proposed to effectively improve the model performance.The results show that the accuracy,precision,recall and F1-Score of our model are superior to other models in complex plasma image recognition,and the recognition accuracy reaches 97.38%.Moreover,the recognition accuracy for the Flowers and Chest X-ray publicly available data sets reaches 97.85%and 98.65%,respectively,and our model has robustness.This study shows that the proposed model provides a new method for the diagnosis of complex plasma images and also provides technical support for the application of plasma in industrial production. 展开更多

目的:探讨食管静脉曲张择期采用静脉曲张套扎术(EVL)和硬化术(EIS)治疗后的临床疗效及预后。方法:对126例肝硬化合并食管静脉曲张择期行EVL及EIS二级预防患者进行回顾性研究,通过对比分析两组患者的一般资料以及治疗后随访情况对治疗效... 目的:探讨食管静脉曲张择期采用静脉曲张套扎术(EVL)和硬化术(EIS)治疗后的临床疗效及预后。方法:对126例肝硬化合并食管静脉曲张择期行EVL及EIS二级预防患者进行回顾性研究,通过对比分析两组患者的一般资料以及治疗后随访情况对治疗效果进行总结。结果:术前两组患者一般资料无显著性差异,术后2周内套扎组有3例出现并发症,硬化组有9例出现并发症,6周内套扎组有2例出现再出血,硬化组有6例出现再出血,两组在6周内出现并发症及再出血率无显著性差异(P>0.05)。随访6个月结果显示静脉曲张复发率套扎组为66.67%(28/42)、硬化组为38.10%(32/84),随访至12个月结果显示静脉曲张复发率套扎组为83.33%(35/42)、硬化组为51.19%(43/84)。6个月后两组食管静脉曲张的复发率有显著性差异(P<0.05)。结论:EVL及EIS治疗食管胃底静脉曲张短期疗效较好,术后并发症和早期再出血率无显著差异,EIS治疗静脉曲张的再发生率明显低于EVL。 展开更多

目的探讨标准化病人(SP)在消化内科住院医师出科考核中应用的可行性。方法选择2016年9月—2017年9月在苏州大学附属第一医院消化科轮转出科的住院医师60人作为研究对象。将其分为两组,分别设为对照组(即传统考核组)30人,SP组30人。考核... 目的探讨标准化病人(SP)在消化内科住院医师出科考核中应用的可行性。方法选择2016年9月—2017年9月在苏州大学附属第一医院消化科轮转出科的住院医师60人作为研究对象。将其分为两组,分别设为对照组(即传统考核组)30人,SP组30人。考核结束后,比较两组学生的成绩。结果 SP组学员考核成绩(74.370±3.560)分,低于传统考核组的(79.420±3.424)分,两组数据对比,差异具有显著统计学意义(P<0.001)。SP组学生医患沟通考核成绩为(12.800±1.014)分,高于传统考核组的(12.200±1.141)分,两组数据对比,差异具有显著统计学意义(P<0.001)。结论标准化病人考核模式可尝试应用于出科考核过程中,可有效提高学员对常见疾病病种的诊治能力的重视度。 展开更多

背景:胃癌是常见的消化道恶性肿瘤,早期诊断可显著降低其死亡率。2017年发布的我国专家共识推荐使用新型胃癌筛查评分系统进行胃癌风险分层。目的:初步探讨新型胃癌筛查评分系统在社区无症状人群中对早期胃癌的筛查价值。方法:在苏州市... 背景:胃癌是常见的消化道恶性肿瘤,早期诊断可显著降低其死亡率。2017年发布的我国专家共识推荐使用新型胃癌筛查评分系统进行胃癌风险分层。目的:初步探讨新型胃癌筛查评分系统在社区无症状人群中对早期胃癌的筛查价值。方法:在苏州市相城区部分社区招募自愿接受血清学检测的社区居民,对其中符合筛选标准的无症状个体采用问卷调查形式采集胃癌高危因素信息,同时进行包括胃蛋白酶原(PG)Ⅰ、PGⅡ、胃泌素17(G-17)和幽门螺杆菌(Hp)IgG在内的血清学检测。使用新型胃癌筛查评分系统进行胃癌风险分层,推荐中、高危个体行胃镜精查。结果:共540例无症状个体完成研究,其中高危11例(2.0%),中危168例(31.1%),低危361例(66.9%)。64例中、高危个体完成胃镜精查,精查率为35.8%。共检出4例(6.2%)癌前病变,包括胃低级别上皮内瘤变3例,十二指肠腺瘤1例,未检出胃癌。结论:新型胃癌筛查评分系统有助于对无症状人群进行胃癌风险分层,为进一步的内镜精查提供依据。但在胃癌低风险地区,其筛查价值有待进一步验证。 展开更多

背景:幽门螺杆菌(Hp)与消化性溃疡、胃癌等胃肠道疾病密切相关,根除Hp是预防和治疗Hp相关疾病的重要手段,而患者管理是提高根除率的关键环节之一。目的:探索全程管理模式对Hp感染患者根除治疗服药依从性和复查率的影响。方法:连续收集于... 背景:幽门螺杆菌(Hp)与消化性溃疡、胃癌等胃肠道疾病密切相关,根除Hp是预防和治疗Hp相关疾病的重要手段,而患者管理是提高根除率的关键环节之一。目的:探索全程管理模式对Hp感染患者根除治疗服药依从性和复查率的影响。方法:连续收集于2020年6月-2020年11月在苏州大学附属第一医院Hp专病门诊接受根除治疗的患者。2020年6月-2020年8月接受根除治疗者仅在就诊时被告知服药方法和复查时间,作为对照组(n=112);2020年9月-2020年11月接受根除治疗者实施“告知服药方法和复查时间+微信平台线上随诊+微信和电话提醒复查”的全程管理模式,作为观察组(n=112)。两组均使用14 d铋剂四联方案根除Hp,并被告知在服药结束后4~8周完成^(13)C-尿素呼气试验复查,服药结束后第9周通过电话确认服药和复查情况。结果:对照组治疗完成率为91.1%(102/112),观察组为98.2%(110/112),观察组服药依从性显著优于对照组(P=0.018),复查率亦显著高于对照组(69.6%对50.9%,P=0.004);观察组不良反应停药率低于对照组,但差异无统计学意义(8.3%对16.7%,P=0.636)。结论:全程管理模式可有效提高Hp感染患者根除治疗的服药依从性和复查率。 展开更多

目的 初步评价含阿莫西林的四联方案根除幽门螺杆菌的疗效以及安全性.方法 纳入幽门螺杆菌感染的患者共159例,患者均来自苏州大学附属第一医院幽门螺杆菌专病门诊.159例患者被随机分配到A组(含阿莫西林)和B组(不含阿莫西林).两组均采用1... 目的 初步评价含阿莫西林的四联方案根除幽门螺杆菌的疗效以及安全性.方法 纳入幽门螺杆菌感染的患者共159例,患者均来自苏州大学附属第一医院幽门螺杆菌专病门诊.159例患者被随机分配到A组(含阿莫西林)和B组(不含阿莫西林).两组均采用14 d四联方案,即艾司奥美拉唑镁肠溶片20 mg bid+枸橼酸铋钾胶囊220 mg bid+2种抗生素,A组的抗生素组合为阿莫西林胶囊1 g bid+克拉霉素缓释片500 mg bid,B组的抗生素组合为克拉霉素缓释片500 mg bid+替硝唑片500 mg bid.治疗结束后4周复查尿素呼气试验,比较两组的根除率和不良反应发生率.结果 在ITT分析中,A组的根除率为87.2%,B组的根除率为56.8%,两组间的根除率差异具有统计学意义(P<0.05).在PP分析中,A组的根除率为93.2%,B组的根除率为62.2%,两组间的根除率差异具有统计学意义(P<0.05).主要的不良反应有皮疹、恶心呕吐、腹痛腹泻和头晕头痛等,两组的不良反应发生率差异无统计学意义(P=0.746).结论 含阿莫西林的四联方案根除幽门螺杆菌更有优势,值得进一步推广应用. 展开更多

背景:结肠镜检查通过检测和切除息肉,可显著降低结直肠癌发病率和死亡率,但存在一定的息肉漏诊率。目的:探讨基于人工智能(AI)的结直肠息肉辅助诊断系统的临床应用价值。方法:根据纳入和排除标准,选取2021年9月—11月在苏州大学附属第... 背景:结肠镜检查通过检测和切除息肉,可显著降低结直肠癌发病率和死亡率,但存在一定的息肉漏诊率。目的:探讨基于人工智能(AI)的结直肠息肉辅助诊断系统的临床应用价值。方法:根据纳入和排除标准,选取2021年9月—11月在苏州大学附属第一医院接受结肠镜检查的患者400例,随机分为2组,每组200例,分别接受常规结肠镜检查和AI辅助结肠镜检查。采用Boston肠道准备量表(BBPS)评估肠道准备质量。主要结局指标为息肉检出率(PDR),次要结局指标为平均息肉检出数(PPC)。结果:AI系统可显著提高PDR和PPC(37.0%对23.0%,0.775对0.495,P均<0.05),尤其是对于微小息肉和小息肉(微小息肉:23.0%对13.0%,0.410对0.295;小息肉:16.0%对8.0%,0.255对0.095;P均<0.05),两组间大息肉检出情况无明显差异(P均>0.05)。在肠道准备较差(BBPS 0~5分)或优秀(BBPS 8~9分)的情况下,两组间息肉检出情况无明显差异(P均>0.05);在肠道准备合格(BBPS 6~7分)的情况下,AI系统可显著提高PDR和PPC(33.0%对20.0%,0.670对0.450,P均<0.05)。结论:基于AI的结直肠息肉辅助诊断系统对微小息肉和小息肉的检出有较大优势,从而显著提高结肠镜检查的PDR和PPC。该系统在肠道准备质量为合格时能发挥更大优势。 展开更多

METHODS: In vivo cytotoxicity was first screened by 3-[4,5-dimethylthiazol-2-yl]-2, 5-diphenyltetrazolium bromide (MTT) assay. Morphological observation, DNA ladder, flow cytometry analysis, Western blot and real time PCR were employed to elucidate the cytotoxic mechanism of UA. Implanted mouse hepatoma H22 was used to evaluate the growth inhibitory effect of UA in vivo . RESULTS: UA could significantly inhibit the proliferation of HepG2 and its drug-resistance strain, R-HepG2 cells, but had no inhibitory effect on primarily cultured normal mouse hepatocytes whereas all the six derivatives of UA could not inhibit the growth of all tested cell lines. Further study on mechanism demonstrated that apoptosis and G0/G1 arrest were involved in the cytotoxicity and cleavage of poly-(ADP-ribose)- polymerase (PARP). Downregulation of cyclooxygenase-2 (COX-2) protein and upregulation of heat shock protein (HSP) 105 mRNA correlated to the apoptosis of HepG2 cells treated with UA. In addition, UA also could inhibit the growth of H22 hepatoma in vivo. CONCLUSION: UA is a promising anti-tumor agent, but further work needs to be done to improve its solubility. 展开更多

Piezocatalytic materials have been widely used for catalytic hydrogen evolution and purification of organic contaminants.However,most studies focus on nano-size and/or polycrystalline catalysts,suffering from aggregation and neutralization of internal piezoelectric field caused by polydomains.Here we report a single crystal ZnO of large size and few bulk defects crafted by a hydrothermal method for piezocatalytic hydrogen generation from pure water.It is noteworthy that single-side surface areas of both original as-prepared ZnO and Ga-doped ZnO bulk crystals are larger than 30 cm^(2).The high quality of ZnO and Ga-doped ZnO bulks are further uncovered by high-resolution transmission electron microscope(HRTEM),photoluminescence(PL)and X-ray diffraction(XRD).Remarkably,an outstanding hydrogen production rate of co-catalyst-free Ga-doped ZnO bulk crystal(i.e.,a maximum rate of 5915μmol h^(-1) m^(-2))is observed in pure water triggered by ultrasound in dark,which is over 100 times higher than that of its powder counterpart(i.e.,52.54μmol h^(-1) m^(-2)).The piezocatalytic performance of ZnO bulk crystal is systematically studied in terms of varied exposed crystal facet,thickness and conductivity.Different piezocatalytic performances are attributed to magnitude and distribution of piezoelectric potential,revealed by the finite element method(FEM)simulation.The density functional theory(DFT)calculations are employed to investigate the piezocatalytic hydrogen evolution process,indicating a strong H_(2)O adsorption and a low energy barrier for both H_(2)O dissociation and H2 generation on the stressed Znterminated(0001)ZnO surface. 展开更多

Modified Child-Pugh grade vs albumin-bilirubin grade for predicting prognosis of hepatocellular carcinoma patients after hepatectomy 被引量:6

BACKGROUND Hepatectomy is the main treatment for patients with hepatocellular carcinoma(HCC)and it has a high possibility for long-term cure potential.But the postoperative mortality and recurrence rates remain high.Since the long-term prognosis of HCC patients is strongly linked to liver function,preoperative assessment of liver function is very important for HCC patients.AIM To compare the predictive power of the modified Child-Pugh(MCP)and albumin-bilirubin(ALBI)grades for the long-term outcome of HCC.METHODS From January 2010 to June 2017,a total of 204 patients with HCC who underwent surgery at the Second Affiliated Hospital of Chongqing Medical University were enrolled in this retrospective study.Multivariate Cox regression analysis was used to determine the independent predictive factors of survival and relapse.The area under the curve(AUC)was used to evaluate the discriminative performance of the MCP grade and ALBI grade to predict the postoperative overall survival(OS)time and recurrence-free survival(RFS)time.RESULTS The median OS and RFS times were 44.0 mo(range:22.0-74.0 mo)and 22.0 mo(range:5.0-45.0 mo),respectively.The median OS and RFS times of MCP grades 1,2,and 3 patients were 60.0,39.0,and 18.0 mo(P<0.001)and 36.0,15.0,and 7.0 mo(P<0.001),respectively.The median OS and RFS times of ALBI grades 1,2,and 3 patients were 56.0,26.0,and 6.0 mo(P<0.001)and 25.0,10.0,and 3.0 mo(P=0.003),respectively.Both the MCP and ALBI grades were more accurate than the Child-Pugh grade for predicting long-term prognosis.Further analysis demonstrated that for both predicting OS and RFS,the MCP grade performed better than the ALBI grade(AUC:0.642 vs 0.605 for OS;0.659 vs 0.594 for RFS).CONCLUSION The MCP grade is more accurate than the ALBI grade for predicting long-term outcome of patients with HCC. 展开更多

The effects of Ga N/In Ga N asymmetric lower waveguide(LWG)layers on photoelectrical properties of In Ga N multiple quantum well laser diodes(LDs)with an emission wavelength of around 416 nm are theoretically investigated by tuning the thickness and the indium content of In Ga N insertion layer(In Ga N-IL)between the Ga N lower waveguide layer and the quantum wells,which is achieved with the Crosslight Device Simulation Software(PIC3D,Crosslight Software Inc.).The optimal thickness and the indium content of the In Ga N-IL in lower waveguide layers are found to be 300 nm and 4%,respectively.The thickness of In Ga N-IL predominantly affects the output power and the optical field distribution in comparison with the indium content,and the highest output power is achieved to be 1.25 times that of the reference structure(symmetric Ga N waveguide),which is attributed to the reduced optical absorption loss as well as the concentrated optical field nearby quantum wells.Furthermore,when the thickness and indium content of In Ga N-IL both reach a higher level,the performance of asymmetric quantum wells LDs will be weakened rapidly due to the obvious decrease of optical confinement factor(OCF)related to the concentrated optical field in the lower waveguide. 展开更多
