目的:探讨术前血小板和淋巴细胞比值(PLR)联合CA19-9在胆囊癌患者术后预后评估中的价值。方法:回顾性分析2015年12月至2020年12月华中科技大学同济医学院附属同济医院收治的67例胆囊癌术后患者临床资料。其中男28例,女39例;平均年龄(58&... 目的:探讨术前血小板和淋巴细胞比值(PLR)联合CA19-9在胆囊癌患者术后预后评估中的价值。方法:回顾性分析2015年12月至2020年12月华中科技大学同济医学院附属同济医院收治的67例胆囊癌术后患者临床资料。其中男28例,女39例;平均年龄(58±10)岁。患者均签署知情同意书,符合医学伦理学规定。生存分析采用Kaplan-Meier法和Log-rank检验;Cox多因素回归分析影响胆囊癌患者术后预后的独立危险因素;采用ROC曲线下面积(AUC)分析PLR +CA19-9预测胆囊癌患者术后预后的能力。结果:PLR、CA19-9、PLR + CA19-9对胆囊癌患者术后3年总体生存期(OS)预测的ROC曲线分析显示,最佳界值分别为162、38、0.65。Cox多因素回归分析显示,PLR≥162和CA19-9≥38 kU/L是胆囊癌患者术后预后的独立危险因素(HR=3.093,3.852;P<0.05)。低水平和高水平PLR + CA19-9组中位OS分别为43、15个月,差异有统计学意义(χ^(2)=44.342,P<0.05)。PLR预测胆囊癌患者术后1、3年无进展生存期的AUC分别为0.741、0.777,CA19-9相应为0.838、0.780,PLR + CA19-9相应为0.901、0.882。结论:术前PLR联合CA19-9在胆囊癌患者术后预后评估中的预测价值大于PLR或CA19-9单独评估,有一定的临床应用价值。 展开更多

Although various anti-osteoporosis drugs are available,the limitations of these therapies,including drug resistance and collateral responses,require the development of novel anti-osteoporosis agents.Rhizoma Drynariae displays a promising anti-osteoporosis effect,while the effective component and mechanism remain unclear.Here,we revealed the therapeutic potential of Rhizoma Drynariae-derived nanovesicles(RDNVs)for postmenopausal osteoporosis and demonstrated that RDNVs potentiated osteogenic differentiation of human bone marrow mesenchymal stem cells(hBMSCs)by targeting estrogen receptor-alpha(ERα).RDNVs,a natural product isolated from fresh Rhizoma Drynariae root juice by differential ultracentrifugation,exhibited potent bone tissue-targeting activity and anti-osteoporosis efficacy in an ovariectomized mouse model.RDNVs,effectively internalized by hBMSCs,enhanced proliferation and ERαexpression levels of hBMSC,and promoted osteogenic differentiation and bone formation.Mechanistically,via the ERαsignaling pathway,RDNVs facilitated mRNA and protein expression of bone morphogenetic protein 2 and runt-related transcription factor 2 in hBMSCs,which are involved in regulating osteogenic differentiation.Further analysis revealed that naringin,existing in RDNVs,was the active component targeting ERαin the osteogenic effect.Taken together,our study identified that naringin in RDNVs displays exciting bone tissue-targeting activity to reverse osteoporosis by promoting hBMSCs proliferation and osteogenic differentiation through estrogen-like effects. 展开更多

监狱医疗保障是监狱的一项重要工作。由于监管对象的特殊性,监狱的医疗保障不仅是一个公共卫生管理问题,更是直接关系到服刑人员的生命健康与教育矫正质量、刑罚执行的公平正义、监狱的安全稳定等,是国家治理体系和治理能力现代化背景... 监狱医疗保障是监狱的一项重要工作。由于监管对象的特殊性,监狱的医疗保障不仅是一个公共卫生管理问题,更是直接关系到服刑人员的生命健康与教育矫正质量、刑罚执行的公平正义、监狱的安全稳定等,是国家治理体系和治理能力现代化背景下监狱公共管理的重要维度。进一步研究监狱医疗保障模式,既是推进国家治理体系和治理能力现代化的客观要求,同时也对保障服刑人员的生命健康权、保证服刑人员的教育矫正质量、维护刑罚执行的公平正义与监狱安全稳定等具有重要意义。构建监狱医疗保障新模式可以从以下三个方面着手:一是各地进一步推广监狱系统内大病统筹制度。二是在监狱医疗实践中坚持关注服刑人员在狱期间的全周期和健康全过程。三是不断加强与社会医疗部门的协作。 展开更多

This work investigated the evolution of multiphase microstructure and impact fracture behavior of medium carbon high silicon high strength steel subjected to the austempering treatment at 240,360,and400℃.The results show that martensite,bainite,and retained austenite(RA)are the main microstructural phases.The austempering treatments at 360 and 400℃ caused the formation of carbon-poor ferrite in the matrix,and the transformation of ultrafine bainite into coarse lath bainite and granular bainite,respectively.Thick filmy RA was distributed between bainite laths.The polygonal martensiteaustenite islands and blocky RA formed along the grain boundaries.The average carbon concentration in the matrix decreased with the temperature increase,while the impact toughness initially increased and then dropped with temperature.The quasi-cleavage brittle fracture dominated the impact fracture mechanism of the sample austempered at 240℃ by forming tearing surfaces and tearing steps.The microcracks disappeared in the RA on the prior austenite grain boundaries.On the other side,the fracture surface of the sample austempered at 360℃ exhibited ductile fracture with deep dimples and brittle fracture with cleavage river patterns.The polygonal martensite-austenite islands or blocky RA constrained the microcracks.After austempered at 400℃,the brittle fracture was dominant,showing river patterns,and the microcracks propagated through the granular bainite without any resistance. 展开更多
