文摘目的探讨福辛普利联合氯沙坦治疗2型糖尿病肾病(type 2 diabetic nephropathy,T2DN)的效果及对血管内皮功能的影响。方法选取2017年1月-2018年12月收治的T2DN患者110例,根据治疗方法分为观察组和对照组,每组55例。对照组采取常规治疗联合福辛普利治疗,观察组在对照组基础上加用氯沙坦治疗,疗程均为6个月。比较2组治疗后临床疗效,治疗前、治疗6个月后肾功能指标[尿素(BUN)、血肌酐(SCr)、24 h尿白蛋白排泄率(UAER)],血糖[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)],血压[收缩压(SBP)、舒张压(DBP)],血管内皮功能指标[血管紧张素Ⅱ(AngⅡ)、血管内皮生长因子(VEGF)、内皮素-1(ET-1)]水平变化,记录药物不良反应发生情况。结果治疗6个月后,观察组总有效率高于对照组(P<0.01)。治疗6个月后,2组BUN、SCr、24 h UAER、SBP、DBP、AngⅡ、VEGF、ET-1均低于治疗前,且观察组低于对照组(P<0.05)。2组间FPG、2 h PG、HbA1c治疗前后比较差异均无统计学意义(P>0.05)。2组均无严重不良反应发生。结论福辛普利联合氯沙坦治疗可有效提高T2DN治疗效果,更好地控制血压水平,改善患者肾功能和血管内皮功能,且治疗安全性良好。
