Urodynamic examination was performed 2 weeks later to determine neurogenic bladder status. The rabbits were treated with anodal block stimulation at sacral anterior roots for 4 weeks. Electrical stimulation of sacral anterior roots improved urodynamic parameters of neurogenic bladder in rabbit models of complete spinal cord injury, effectively promoted urinary function, and relieved urinary retention. Immunohistochemistry results showed that a balance was achieved among expression of muscarinic receptor subunits M2, M3, ATP-gated ion channel P2X3 receptors, and 132-adrenergic receptor, and nerve growth factor expression decreased. These results suggested that long-term sacral anterior root stimulation of anodal block could'be used to treat neurogenic bladder in a rabbit model of complete spinal cord injury.展开更多

Due to the lack of characteristic pathological features, it is often underdiagnosed and misdiagnosed. A variety of methods can be used to diagnose NMC, including immunohistochemistry, karyotyping, fluorescence in situ hybridization, reverse transcription-polymerase chain reaction, and next-generation sequencing. So far, there is no standard treatment plan for NMC and the prognosis is poor, related to its rapid progression, easy recurrence, and unsatisfactory treatment outcome.CASE SUMMARY A 58-year-old female came to our hospital with a complaint of eye swelling and pain for 8 d. The diagnosis of NMC was confirmed after postoperative pathology and genetic testing. The patient developed nausea and vomiting, headache, and loss of vision in both eyes to blindness after surgery. Magnetic resonance imaging(MRI) and positron emission tomography/computed tomography(PET/CT) performed after 1.5 mo postoperatively suggested tumor recurrence. The patient obtained remission after radiation therapy to some extent and after initial treatment with anti-angiogenic drugs and sonodynamic therapy(SDT), but cannot achieve long-term stability and eventually developed distant metastases, with an overall survival of only 17 mo.CONCLUSION For patients with rapidly progressing sinus tumors and poor response to initial treatment, the possibility of NMC should be considered and immunohistochemical staining with anti-NUT should be performed as soon as possible, combined with genetic testing if necessary. CT, MRI, and PET/CT imaging are essential for the staging, management, treatment response assessment and monitoring of NMC. This case is the first attempt to apply heat therapy and SDT in the treatment of NMC, unfortunately, the prognosis remained poor.展开更多

Howe...BACKGROUND: Vascular endothelial growth factor (VEGF) induces bone marrow-derived mesenchymal stem cell (BMSC) differentiation into vascular endothelial-like cells and promotes BMSC migration toward gliomas. However, the molecular mechanisms by which VEGF induces BMSC differentiation and migration remain poorly understood. OBJECTIVE; To investigate the role of platelet-derived growth factor (PDGF) receptor (PDGFR) in BMSC differentiation and migration induced by VEGE DESIGN, TIME AND SETTING: A parallel, controlled, in vitro experiment was performed at the Molecular Neurobiology & Neural Regeneration and Repairing Laboratory, Anhui Provincial Hospital of Anhui Medical University, China from June 2008 to March 2009. MATERIALS: U87 glioma cells were purchased from Shanghai Institutes for Biological Sciences; mouse anti-human PDGFR and VEGF receptor (VEGFR) monoclonal antibodies were purchased from Peprotech, USA. METHODS: Isolated BMSCs were precultured with neutralizing antibody for VEGFR-1, VEGFR-2, PDGFR-α, and PDGFR-β to block biological activity of related receptors, followed by induced differentiation with 50μg/L VEGF. BMSCs induced with 50μg/L VEGF alone served as the VEGF-induced group. The control group remained untreated. MAIN OUTCOME MEASURES: Cell surface markers were identified by flow cytometry; BMSC surface cytokine receptor expression was detected by reverse transcription-polymerase chain reaction; the Transwell model was used to observe cell migration. RESULTS: After blocking the PDGFR, VEGF did not induce BMSC cell surface marker CD-31 or von Willebrand factor (vWF) expression. However, inhibition with VEGF receptor blocking agents, VEGF induced BMSCs to express CD-31 and vWE Following inhibition of the PDGFR, the number of cells migrating through the polycarbonate membrane Transwell chamber was decreased, as well as the number of BMSCs migrating to glioma cells. However, through the use of VEGF receptor blocking agents, the number of migrating cells remained unchanged. VEGF preculture increased the number of BMSCs migrating to gliomas. CONCLUSION: VEGF interacts with PDGFRs on the BMSC surface to attract BMSC directional migration and induce BMSC differentiation. The VEGF/PDGFR pathway participates in BMSC directional migration to glioma. VEGF pretreatment increased efficiency of BMSC migration to glioma.展开更多

To improve the quality of the power supply system for RSSMs, an improved synchronous control strategy is suggested for the superconducting magnet power supply system, which comprises four series-connected six-pulse converters fed by a phase-shifting transformer, respectively. According to the proposed control strategy, the basic unit is two 12-pulse converters and the control method will be changed in terms of loadfluctuations which are represented by the per unit value of converter output voltage. As a result, harmonic is greatly reduced but the power factor is also high.展开更多

The quasi-snowflake(QSF) divertor configuration is studied in this paper. Starting from a standard QSF plasma equilibrium, a new QSF equilibrium with 300 kA total plasma current is designed. In order to reduce the currents of PF6 and PF14, the influence of plasma shape on PF coil current distribution is analyzed. A fixed boundary equilibrium solver based on a non-rigid plasma model is used to calculate the flux distribution and PF coil current distribution. Then the plasma shape parameters are studied by the orthogonal method. According to the result, the plasma shape is redefined, and the calculated equilibrium shows that the currents of PF6 and PF14 are reduced by 3.592 kA and 2.773 kA, respectively.展开更多

文摘To improve aneurysm treatment, this study examined the influence of clip locations on hemodynamic factors in patient-specific anterior communicating artery(ACoA)aneurysms with different aneurysmal angle. We proposed a simplified classification of ACoA aneurysms using aneurysmal angle, defined by the angle of pivot of the aneurysmal dome and the virtual two-dimensional plane created by both proximal A2 segments of anterior cerebral artery(ACA).ACoA aneurysms with three different aneurysmal angles, which are 15°,80°and 120°,were analyzed in our study. In this work,we obtained hemodynamics before and after clipping surgery with three clip locations based on clinical clipping strategies in three ACoA aneurysms with different aneurysm angles. Results showed that local high pressure occurs at impingement region of the ACoA aneurysm before clipping and new impingement region close to the clipping location after clipping treatment. For clipping the aneurysm with aneurysmal angle 15°and a wide neck, wall shear stress(WSS)distribution is more uniform when the clipping angle of two clips close to 180°comparing with other two angles. In addition, for clipping the aneurysm with aneurysmal angle 80°and 120°,local high pressure appears on new impingement region and high WSS distributes around the clipping location when the clip plane is normal to the direction of inflow of aneurysm from the dominance of A1 segment of ACA.Hence,we should avoid the impingement of inflow from the A1 segment and choose a favorable clipping location for the fastness of clip. The results of our study could preoperatively give a useful information to the decision of surgical plan.

文摘BACKGROUND Nuclear protein in testis(NUT) carcinoma is a rare aggressive malignant epithelial cell tumor, previously known as NUT midline carcinoma(NMC), characterized by an acquired rearrangement of the gene encoding NUT on chromosome 15q14. This case is the first attempt to apply heat therapy and SDT in the treatment of NMC, unfortunately, the prognosis remained poor.

文摘BACKGROUND: Vascular endothelial growth factor (VEGF) induces bone marrow-derived mesenchymal stem cell (BMSC) differentiation into vascular endothelial-like cells and promotes BMSC migration toward gliomas. VEGF pretreatment increased efficiency of BMSC migration to glioma.

文摘BACKGROUND Malignant giant cell tumor of the tendon sheath(MGCTTS)is an extremely rare malignant tumor originating from synovial and tendon sheath tissue with highly aggressive biological behavior and a high rate of local recurrence and distant metastasis which should be considered a highly malignant sarcoma and managed aggressively. How to systemically treat MGCTTS remains a challenge. In this case,a patient with MGCTTS suffered a recurrence after 2 surgical resections received adjuvant chemotherapy and radiation therapy, but the treatment outcome remained poor.More clinical trials and better understanding of the biology and molecular aspects of this subtype of sarcoma are needed while novel medicines should be developed to efficiently target particular pathways.CASE SUMMARY A 52-year-old man presented with persistent dull pain in the right groin accompanied by limited right hip motion starting 6 mo ago.Two months before his attending to hospital, the patient’s pain worsened, presenting as severe pain when standing or walking, limping, and inability to straighten or move the right lower extremity.Surgical excision was performed and MGCTTS was confirmed by pathology examination. Two recurrences occurred after surgical resection, moreover, the treatment outcome remained poor after adjuvant chemotherapy and radiation therapy. The patient died only 10 mo after the initial diagnosis.CONCLUSION MGCTTS is characterized by a joint mass with pain and limited motion. It typically grows along the tendons and infiltrated into the surrounding muscle and bone tissue, with a stubborn tendency to relapse,as well as pulmonary metastasis.Radically surgical resection provides a choice of treatment whereas post-operation care should be taken to preserve the function of the joint.Chemotherapy and radiotherapy can be used as alternative treatments when radical resection cannot be performed.

文摘To provide electrical power for the Research System of Superconducting Magnets(RSSMs)including the background field superconducting magnet and the tested superconducting objects, the high power phase-controlled converter will be used to develop the power supply system. However, because of its inner nonlinear feature, the current harmonics and the reactive power are injected into the AC power supply system. As a result, harmonic is greatly reduced but the power factor is also high.

文摘It is necessary to reduce the currents of poloidal field(PF) coils as small as possible, during the static equilibrium design procedure of Experimental Advanced Superconductive Tokamak(EAST). According to the result, the plasma shape is redefined, and the calculated equilibrium shows that the currents of PF6 and PF14 are reduced by 3.592 kA and 2.773 kA, respectively.
