上消化道出血病因分析的单中心回顾性研究

目的研究上消化道出血的病因及不同年龄段患者病因构成比情况。方法回顾性分析2012年1月至2018年1月于首都医科大学附属北京友谊医院诊断上消化道出血并行胃镜检查的住院患者的临床资料,包括性别、年龄、住院天数、胃镜检查结果及是否镜下治疗、是否入住重症监护室、是否输血及院内死亡等;并按年龄分组对上消化道出血病因构成比进行分析。结果共有525例住院患者因上消化道出血行胃镜检查,胃镜结果提示上消化道出血的前三位病因分别为胃十二指肠溃疡出血、胃恶性肿瘤及食管胃底静脉曲张破裂出血;消化性溃疡是各个年龄段的首要病因,随着年龄增长,胃恶性肿瘤所致的上消化道出血比率逐渐升高。结论消化性溃疡始终是各个年龄段患者上消化道出血的首要原因,胃恶性肿瘤导致上消化道出血比率随年龄增长而增加。

上消化道出血患者院内病死率分析的单中心回顾性研究

目的分析上消化道出血患者院内病死率及死亡原因。方法回顾性分析2012年1月至2018年1月首都医科大学附属北京友谊医院收治的1 103例因上消化道出血住院的患者的临床资料,包括性别、年龄、住院天数、主要诊断、是否行内镜检查及治疗、是否入住监护室、是否输血及院内死亡等。结果 1 103例上消化道出血患者中,共74例患者死亡,院内病死率为6. 71%(74/1 103),死亡原因分别为胃及胆道系统恶性肿瘤、肝脏功能衰竭、心脏功能衰竭、呼吸衰竭及失血性休克等。按死亡原因分组比较,心、肺功能衰竭组患者年龄最大,肝脏功能衰竭组患者年龄最小。结论恶性肿瘤、脏器功能衰竭等非出血原因已成为上消化道出血的主要死因;因肝脏功能衰竭合并上消化道出血死亡的患者年龄最小,应成为临床关注的重点。

miR-26b调控Sox6和Bhlhe22基因表达影响胰岛素分泌的机制研究

目的阐明miR-26b通过调控Sox6和Bhlhe22基因介导胰岛素的分泌的机制。方法进行胰岛β细胞INS-1培养,miR-26b的过度表达通过慢病毒转染,miR-26的干扰通过miR-26bminics。INS-1细胞可分为3类:野生型INS-1细胞、miR-26b过表达INS-1细胞和miR-26b抑制INS-1细胞。构建miR-26b沉默细胞株和过表达细胞株与验证,采用慢病毒转染技术再次分别构建Sox6和Bhlhe22基因沉默细胞株和Sox6和Bhlhe22基因过表达细胞株,使用酶联免疫吸附(ELISA)试剂盒检测胰岛素含量;采用实时聚合酶链式反应(QT-PCR)和免疫印迹法(Western blotting)方法检测Sox6和Bhlhe22基因及蛋白的表达。结果与对照组相比,miR-26b过度表达组Bhihe22、Sox6基因及蛋白表达明显降低,差异具有统计学意义(P<0.05);miR-26b抑制组Bhihe22、Sox6基因及蛋白的表达明显增加,差异有统计学意义(P<0.05)。与对照组相比,miR-26b过度表达组胰岛素释放增加68.9%,而miR-26b抑制组胰岛素释放减少45.5%,差异有统计学意义(P<0.05)。结论miR-26b通过影响Sox6和Bhlhe22基因的表达,最终可能影响胰岛素的分泌,miR-26b可能参与调控胰岛素的分泌。

Syntheses, Crystal Structures and IR Spectra of Two Mixed Metal Complexes:〔Cr_nFe_(3-n)(μ_3-O)-(μ-O_2CC_2H_5)_6 Py_3〕Cl(n=1,2)

Two mixed metal complexes [Cr2Fe(μ3-O) (μ-O2CC2H5)6Py3]Cl (A)and [CrFe2 (μ3-O) (μ-O2CC2H5 )6Py3] Cl (B) have been prepared in non-aqueous sol-vents. Crystal (A) is monoclinic, space group C2/c, with a= 13. 782(3), b= 18. 381(4), c= 16. 643(2) A, β= 92. 84(5)°, V=4211. 1 (4) A3, Z=4, Mr= 887. 03, Dc= 1. 40 g/cm3, μ= 5. 0 cm-1, F(000) = 1836 and R(Rw) = 0. 057(0. 062) for 2134 unique reflections with I> 3δ(I). Crystal (B) is also monoclinic, space group C2/c ,with a= 13. 793(4), b= 18. 401 (6), c= 16. 657(5) A’, β= 93. 18(3)°, V=4221. 3(5) A3, Z= 4, Mr=890. 88, Dc= 1. 40 g/cm3, μ= 10. 5 cm-1, F(000) = 1844 and R(Rw) = 0. 067 (0. 069) for 2131 unique reflections with I>3δ(I). Both (A) and (B)have been characterized by IR spectra. Besides, the relations between the structuresand IR are discussed.

Unit Construction in the Formation of Metal Clusters──Investigation on the Metal Cluster CompoundsContaining Fe_2S_2(CO)_6-units

The syntheses and structures of trinuclear Mo (W)-Fe-S cluster[MFe2S2(CO)8 (S,CNSEt2)]- (M=Mo, W), hexanuclear Fe-S cluster [Fe6S6-(CO)12]2- and undecanuclear Cu-Fe-S cluster [Cu5Fe6S6(CO)18(PPh3)2]-, containingFe2S2(CO)6-units bave been summarized and the important vestiges left in their struc-tures reflecting the formation processes of the clusters have been found and discussed.Further inspecting some other typical clusters a regular unit construction in the forma-tion of the metal cluster compounds containing Fe2S2(CO)6-units has been figured outand applied to speculate and predict several new cluster compounds containing Fe2S2(CO)6-units.

Synthesis and Crystal Structure of[Fe_2V (μ_3-O) (μ-O_2CCH_3)_6 (THF)_3]Cl· 3H_2O

The compound [Fe2V(μ3-O) (μ-O2CCH3), (THF)3]Cl. 3H2O (THF=tetrahydrofuran) crystallizes in monoclinic system, space group C2, with cell di-mensions a= l2. 847(2), b= l5. 875 (6), c= lO. 211 (3) A, β= 109- 34 (3)', V=1965. 1(4) A3, Z= 2, D,c= 1. 73 g/cm'3 Mr= 838. 5 . F(000) = 1040, λ(MoKa) =14. 0 cm-1. The final R=0. 094, Rw= 0. O95 for 13O4 observed reflections with I≥3. 0σ (I). The result of structure analysis indicates that the framework of this com-pound is a typical trimeric oxo ccntered carboxylato compound.
