Circulating micro RNA, mi R-122 and mi R-221 signature in Egyptian patients with chronic hepatitis C related hepatocellular carcinoma 被引量:18

AIM: To explore the potential usefulness of serum miR-122 and miR-221 as non-invasive diagnostic markers of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS: This prospective study was conducted on 90 adult patients of both sex with HCV-related chronic liver disease and chronic hepatitis C related HCC. In addition to the 10 healthy control individuals, patients were stratified into; interferon-na?ve chronic hepatitis C(CH)(n = 30), post-hepatitis C compensated cirrhosis(LC)(n = 30) and treatment-naive HCC(n = 30). All patients and controls underwent full clinical assessment and laboratory investigations in addition to the evaluation of the level of serum miR NA expression by RT-PCR.RESULTS: There was a significant fold change in serum mi RNA expression in the different patient groups when compared to normal controls; mi R-122 showed significant fold increasing in both CH and HCC and significant fold decrease in LC. On the other hand, mi R-221 showed significant fold elevation in both CH and LC groups and significant fold decrease in HCC group(P = 0.01). Comparing fold changes in miR NAs in HCC group vs non HCC group(CH and Cirrhosis), there was non-significant fold elevation in miR-122(P = 0.21) and significant fold decreasing in miR-221 in HCC vs non-HCC(P = 0.03). ROC curve analysis for miR-221 yielded 87% sensitivity and 40% specificity for the differentiation of HCC patients from non-HCC at a cutoff 1.82. CONCLUSION: Serum miR-221 has a strong potential to serve as one of the novel non-invasive biomarkers of HCC. 展开更多

Background: Pneumatic arterial tourniquet is a very commonly used technique in limb surgeries to provide bloodless field to facilitate dissection and decrease blood loss. However, arterial tourniquet has many deleterious effects including hemodynamic changes, serum lactate and potassium level changes and tourniquet-induced pain which sometimes can be severe and intolerable. Aim of the study: To evaluate the effect of different regional blocks: femoral-sciatic, spinal and epidural blocks on serum lactate and potassium levels and the degree of arterial tourniquet-induced pain in patients undergoing lower limb orthopedic surgeries. Methods: 60 patients underwent lower limb orthopedic surgery with application of tourniquet for duration not more than 90 minutes. Patients were assigned randomly to one of three groups (20 each) Group I had sciatic-femoral block, Group II: patients had spinal anesthesia and Group III: patients had epidural anesthesia. Intraoperative hemodynamics, changes in serum potassium and lactate levels and tourniquet pain after tourniquet inflation & deflation, were recorded. Results: There was no statistically significant difference among the three groups regarding tourniquet pain after tourniquet inflation (p = 0.872) and deflation (p = 0.902), and regarding serum levels changes of potassium (p = 0.067) and lactate (p = 0.051). However, each group showed statistically significant increase in post deflation tourniquet pain (p = 0.003, 0.002, 0.003, in groups F, S, E respectively) and serum potassium (p = 0.004, 0.006, 0.000, in groups F, S, E respectively) and lactate levels (p = 0.004, 0.000, 0.000, in groups F, S, E respectively) when compared to the pre-deflation values, and the increase was directly proportional to the duration of tourniquet. Conclusion: the three different types of anesthesia (femoral-sciatic, spinal and epidural block) have the same effect on serum lactate and potassium levels and the degree of tourniquet pain, which were related to the duration of tourniquet inflation. 展开更多

Background: Diabetes mellitus is an important risk factor for erectile dysfunction. Renin-angiotensin system with its branches Angiotensin II and Angiotensin 1-7 [Ang-(1-7)] are altered in diabetes and could affect erection. So, in this study we determine the level of Ang-(1-7), nitrite (the major nitric oxide metabolite) and histopathological changes in penile tissues of type I diabetic rats. A total of 60 male albino rats were divided into two groups: group I (control) and group II (diabetic) for either 4 weeks in group IIa, or 8 weeks in group IIb. Diabetes was induced by intraperitoneal injection of streptozotocin (60 mg/kg). Penile levels of Ang-(1-7), nitrite and histopathological examination were assessed at 4 and 8 weeks after diabetes induction. Results: Ang-(1-7) and nitrite were decreased in diabetic rats at 4 weeks and continued to be lower at 8 weeks for Ang-(1-7) only. Loss of corpus cavernosum smooth muscle was present in 25% and 85% of rats at 4 and 8 weeks of diabetes respectively (P Conclusion: Diabetes induced progressive decrease in the release of Ang-(1-7) and nitric oxide from the corpora cavernosa in a time-dependent manner with concomitant fibro-muscular changes that end by corporal fibrosis affecting subsequently erectile functions. 展开更多
