培菲康联合肠内营养支持对胃癌术后患者胃肠功能及营养评估指标的影响 被引量:13

背景胃癌(gastric cancer,GC)多见于中老年群体,近年来GC发生率呈逐渐升高趋势.GC根治术是治疗早期GC的首选方案,有报道指出部分患者术后易出现消化道异常,影响胃肠功能及营养状况同时.肠内营养支持为现代外科综合治疗的重要组成部分,... 背景胃癌(gastric cancer,GC)多见于中老年群体,近年来GC发生率呈逐渐升高趋势.GC根治术是治疗早期GC的首选方案,有报道指出部分患者术后易出现消化道异常,影响胃肠功能及营养状况同时.肠内营养支持为现代外科综合治疗的重要组成部分,培菲康为肠道益生菌,能够改善肠道屏障功能,维持人体健康.本研究首次将培菲康联合肠内营养应用于GC术后患者,探究其对患者胃肠功能及营养评估指标的影响.目的探究培菲康联合肠内营养支持对GC术后患者胃肠功能及营养评估指标的影响.方法选取2017-09/2019-09我院107例GC患者,随机数字表法分为对照组(n=53)和研究组(n=54).两组均行胃部分切除术,对照组给予肠内营养支持,研究组给予培菲康联合肠内营养支持.比较两组胃肠功能恢复情况,生活质量提高率,不良反应发生率,治疗前后营养评估指标[前白蛋白(pre-albumin,PAB),转铁蛋白(transferrin,TFN),白蛋白(albumin,ALB)],肠屏障功能[二胺氧化酶(diamine oxidase,DAO),内毒素脂多糖(endotoxin lipopolysaccharide,LPS),D-乳酸]及肠道菌群.结果研究组肠鸣音恢复,首次排便,首次排气,首次进食等时间均短于对照(P<0.05);研究组治疗后血清PAB,TFN,ALB水平高于对照组(P<0.05);研究组治疗后肠球菌,双歧杆菌,消化链球菌水平高于对照组(P<0.05);研究组治疗后血浆DAO,D-乳酸,LPS低于对照组(P<0.05);研究组生活质量提高率(70.37%)高于对照组(50.94%,P<0.05);研究组不良反应发生率(9.26%)与对照组(11.32%)无显著差异(P>0.05).结论采用培菲康联合肠内营养支持能显著改善GC术后患者肠道菌群水平及肠屏障功能,促进胃肠功能恢复,改善机体营养状况,提高患者生活质量,且具有一定安全性. 展开更多

目的研究胶质细胞源性神经营养因子(GDNF)对1-甲基-4-苯基-1,2,3,6-四氢吡啶(MPTP)诱导帕金森模型小鼠黑质酪氨酸羟化酶(TH)、Ephrin B2、磷酸化c-Jun(p-c-Jun)表达的影响,探索GDNF治疗帕金森病的价值。方法选取84只小鼠并采用MPTP复制... 目的研究胶质细胞源性神经营养因子(GDNF)对1-甲基-4-苯基-1,2,3,6-四氢吡啶(MPTP)诱导帕金森模型小鼠黑质酪氨酸羟化酶(TH)、Ephrin B2、磷酸化c-Jun(p-c-Jun)表达的影响,探索GDNF治疗帕金森病的价值。方法选取84只小鼠并采用MPTP复制帕金森病模型,选取72只模型复制成功的小鼠随机分为实验组和模型组,每组36只;另随机选取36只健康小鼠作为对照组。对实验组小鼠右侧侧脑室注射GDNF,模型组和对照组小鼠注射等量生理盐水。10 d后比较3组小鼠TH神经元变化、检测TH mRNA、Ephrin B2和p-c-Jun蛋白的表达。结果模型组和实验组毁损侧神经元数、神经纤维密度少于对照组(P<0.05),实验组神经元数高于模型组(P<0.05);模型组TH mRNA表达水平低于对照组和实验组(P<0.05)。模型组小鼠Ephrin B2、p-c-Jun蛋白表达水平高于对照组和实验组(P<0.05)。结论 GDNF能够提高MPTP诱导帕金森病模型小鼠黑质TH表达,降低Ephrin B2和p-c-Jun蛋白表达,进而对小鼠多巴胺能神经元产生保护作用,提示GNDF可能在治疗人帕金森病上具有一定价值。 展开更多
