Objective:To investigate the effect of out-of-hospital extended nursing on the compliance behaviors and therapeutic effect of brace treatment of patients with idiopathic scoliosis.Methods:54 patients with idiopathic scoliosis between February 2015 and December 2017 were randomly divided into control group and observation group.Patients in the control group received pelvic suspension traction,gymnastic exercises,and brace wear at discharge,on the basis of which patients in the observation group were added with extended care outside the hospital.The compliance behaviors and the changes of scoliosis angle(Cobb angle)of patients in the 2 groups were evaluated.Results:Compared with the control group,patients in the observation group had better compliance behaviors in completion status of functional exercise(χ2=5.594,P=0.018),brace wear(χ2=6.171,P=0.013),subsequent visit on time(χ2=9.247,P=0.002).Cobb angle was improved significantly in both groups at the last follow-up compared with that on admission,and the improvement was more significantly in the observation group(P<0.001).Conclusion:Through the implementation of out-of-hospital extended nursing,the compliance behaviors and clinical effect of brace treatment for idiopathic scoliosis patients are obviously improved,and this active nursing model is worth popularizing in clinic. 展开更多

Correlation analysis of real-time three-dimensional ultrasound in diagnosis of left ventricular function, portal hemodynamics and severity of liver function in patients with liver cirrhosis

Objective:To analyze the correlation between real-time three-dimensional ultrasound in diagnosis of left ventricular function, portal hemodynamics and severity of liver function in patients with liver cirrhosis.Methods: 90 patients with cirrhosis admitted to our hospital from January 2017 to December 2018 were enrolled in the cirrhosis group, and the cirrhosis components were group A, B and C according to the Child-Pauh classification criteria. During the same period, 90 healthy subjects who underwent physical examination were selected as the control group;real-time three-dimensional ultrasonography was performed to examine left ventricular function and portal hemodynamic parameters;Pearson correlation was used to analyze left ventricular function, portal hemodynamics and liver. Functional severity relationship.Results: The left heart E and E/A levels in the cirrhosis group were significantly lower than those in the control group (P<0.05), and the A level was significantly higher than the control group (P<0.05). The Dpv and Q levels in the cirrhosis group were significantly higher than those in the healthy control group. The difference was statistically significant (P<0.05), and the level of Vpv was significantly lower than that of healthy controls (P<0.05). There were significant differences in E, A and E/A levels between different Child-Paugh patients (P<0.05). There were significant differences in portal hemodynamics Dpv, Vpv and Q between the different Child-Paugh grades, and the difference was statistically significant (P<0.05);left heart function E and E/A and liver function severity There was a significant negative correlation (P<0.05). There was a significant positive correlation between left cardiac function A and liver function severity (P<0.05). Portal vein hemodynamics Dpv, Vpv and QE were significantly associated with liver function severity. Positive correlation (P<0.05). Conclusion: Real-time three-dimensional ultrasound can effectively detect left ventricular function and portal hemodynamics changes in patients with cirrhosis, and left heart function, portal hemodynamics and liver function severity are significantly correlated. 展开更多

Clinical significance of combined detection of EASI lead and serum H-FABP, IMA in the diagnosis of coronary heart disease patients with myocardial ischemia

Objective:To explore the significance of combined detection of EASI lead and serum H-FABP, IMA in the diagnosis of coronary heart disease patients with myocardial ischemia.Methods:The 106 patients with myocardial ischemia of coronary heart disease treated in our hospital from March 2016 to June 2018 were taken as observation group and 106 healthy adults who were examined in our hospital at the same time were defined as control group. The abnormal rate of electrocardiogram (ECG), serum heart-type fatty acid binding protein (H-FABP) and ischemic modified albumin (IMA) in them, the diagnostic efficacy of single and combined detection in diagnosing myocardial ischemia of coronary heart disease and the area under receiver operating characteristic (ROC) curve of different joint detection were compared. Results: The abnormal rate of ECG, serum H-FABP and IMA levels in the observation group were higher than the control group. The specificity of EASI lead, serum H-FABP and IMA in the diagnosis of coronary heart disease patients with myocardial ischemia was different, the diagnostic specificity of H-FABP was higher than that of EASI lead and serum IMA. TThe sensitivity and negative predictive value of different parallel combined detection in diagnosing coronary heart disease were different. The specificity of different series of combined detection in the diagnosis of coronary heart disease was different. The areas under ROC curve of EASI lead+ H-FABP, H-FABP +IMA and EASI lead+ H-FABP +IMA were 0.713, 0.782 and 0.914, respectively, and the areas under ROC curve of series of combined detection were 0.709, 0.724 and 0.883, respectively. The area under the ROC curve of EASI lead+H-FABP+IMA was higher than that of EASI lead+H-FABP and H-FABP+IMA.Conclusions: The combination of EASI lead and serum H-FABP and IMA is of great significance in the diagnosis of coronary heart patients with myocardial ischemia.
