Objective: Accurate estimation of lymph node metastasis (LNM) in intramucosal gastric cancer is essential to select less invasive treatment options and even avoid surgery. The aim of this study was to evaluate combined clinicopathological features to predict the presence of LNM. Methods: A retrospective review of data from 386 intramucosaL gastric cancer patients who underwent gastrectomy with extended lymphadenectomy from 2003 to 2010 was conducted. The mutual relation between clinicopathological characteristics and LNM was analyzed. Results: LNM was detected in 40 (10.4%) of the 386 patients. Histological typeand vascular or lymphatic invasion presence showed a positive correlation with LNM occurrence by univariate analysis. Multivariate analysis revealed that histological type was the only factor associated with LNM. Combined clinicopathologic characteristics would be more predictable for LNM. We found no LNM when we used combined clinicopathological characteristics conforming to Japanese absolute indications for endoscopic therapy. The LNM rate was as high as 8.7% when Japanese expanded criteria were used. Univariate analysis in cancer conformity to expand endoscopic submucosal dissection (ESD) indication also revealed that the undifferential type was the only significant factor for LNM. Conclusions: It was possible to predict intramucosal gastric cancer cases without LNM using combined clinicopathological characteristic analysis. Extended indication for ESD should be cautiously used for intramucosal gastric cancer patients. 展开更多

Infrared(IR)birefringent materials with large optical anisotropy and wide transparency range are important for efficient light manipulation in various IR optical devices.Herein,two new IR birefringent materials AMgGeSe_(3)(A=Li,Na)with large optical anisotropy were rationally designed by a rigid octahedron and flexible dimer combined strategy and fabricated in experiment.The introduction of rigid[LiSe_(6)]/[NaSe_(6)]and[MgSe_(6)]octahedra effectively regulates the geometry and arrangement of the flexible[Ge2Se6]dimers,resulting in the birefringence as large as 0.334@1,064 nm in LiMgGeSe_(3) and 0.445@1,064 nm(the largest one in the reported[Ge_(2)Se_(6)]dimer-contained selenides)in NaMgGeSe_(3).Density functional theory(DFT)calculations and statistical analyses highlight the influence of polarizability anisotropy,density,arrangement of units,as well as layer distance on birefringence.The results indicate that AMgGeSe_(3)(A=Li,Na)crystals are the promising IR birefringent materials and it gives an insight into the exploration of new IR birefringent materials with large birefringence based on the clamping effect from rigid groups. 展开更多

目的探讨索拉非尼联合卡培他滨治疗肝细胞肝癌(HCC)的效果及对患者血清miR-212和miR-132水平的影响。方法抽取2018年2月至2020年6月郑州人民医院收治的HCC患者126例,采用随机数字表法分为单一组和联合组,每组63例。单一组给予卡培他滨治... 目的探讨索拉非尼联合卡培他滨治疗肝细胞肝癌(HCC)的效果及对患者血清miR-212和miR-132水平的影响。方法抽取2018年2月至2020年6月郑州人民医院收治的HCC患者126例,采用随机数字表法分为单一组和联合组,每组63例。单一组给予卡培他滨治疗,联合组给予索拉非尼联合卡培他滨治疗。比较两组的化疗效果、近期生存率、血清miR-212和miR-132水平及不良反应发生率。结果联合组总有效率(79.36%,50/63)高于单一组(61.90%,39/60),P<0.05。治疗后,两组血清miR-212和miR-132 mRNA、蛋白水平高于治疗前,且联合组高于单一组(P<0.05);治疗后1年,联合组无进展生存率为73.02%(46/63),高于单一组的52.38%(33/63),P<0.05。联合组不良反应总发生率(26.98%,17/63)与单一组(17.46%,11/63)比较差异未见统计学意义(P>0.05)。结论索拉非尼联合卡培他滨治疗HCC在调节患者血清miR-212和miR-132水平、提高疗效和近期生存率方面有积极作用,且联合用药未明显增加不良反应的发生。 展开更多

Although the genus Phoenicopsis is the most common member of Czekanowskiales and is considered an indicative representative of the Middle Jurassic Coniopteris–Phoenicopsis assemblage in northern China, little was known about its taxonomy and biodiversity based upon the leaf cuticular structure. In this paper, three subgenera of the genus Phoenicopsis from the Middle Jurassic, Yan'an Formation of the Ordos Basin, China, Phoenicopsis(subg. Phoenicopsis)(Heer) Samylina, P.(subg. Windwardia)(Florin) Samylina and P.(subg. Culgoweria)(Florin) Samylina are identified based upon the leaf epidermal features for the first time. This discovery demonstrates that Phoenicopsis was quite diverse during the Middle Jurassic in northern China. Its biodiversity is significant for further understanding Middle Jurassic paleophytogeography and paleoclimatology of northern China. The current findings show that the three Phoenicopsis subgenera coexisted and flourished in the mixed zone(ecotone) between the Indo-Sinian Floristic Region and the Siberian Floristic Region as seasonal deciduous plants(trees or shrubs) during the Middle Jurassic. They were important elements in the Middle Jurassic deciduous gymnospermous forest community in warm temperate regions with seasonal temperature and precipitation variations. They are interpreted as growing on the hillsides and sloping fields along meandering streams, lakes and swamps in a vast sedimentary basin in a wet and warm environment. 展开更多
