BACKGROUND The combination of magnetic compression anastomosis(MCA)and endoscopy has been used to treat biliary stricture after liver transplantation.However,its use for the treatment of complex biliary obstruction af...BACKGROUND The combination of magnetic compression anastomosis(MCA)and endoscopy has been used to treat biliary stricture after liver transplantation.However,its use for the treatment of complex biliary obstruction after major abdominal trauma has not been reported.This case report describes the successful use of MCA for the treatment of biliary obstruction resulting from major abdominal trauma.A 23-year-old man underwent major abdominal surgery(repair of liver rupture,right half colon resection,and ileostomy)following a car accident one year ago.The abdominal drainage tube,positioned at the Winslow foramen,was draining approximately 600-800 mL of bile per day.During the two endoscopic retrograde cholangiopancreatography procedures,the guide wire was unable to enter the common bile duct,which prevented placement of a biliary stent.MCA combined with endoscopy was used to successfully achieve magnetic anastomosis of the peritoneal sinus tract and duodenum,and then a choledochoduodenal stent was placed.Finally,the external biliary drainage tube was removed.The patient achieved internal biliary drainage leading to the removal of the external biliary drainage tube,which improved the quality of life.CONCLUSION Magnetic compression technique can be used for the treatment of complex biliary obstruction with minimal operative trauma.展开更多
BACKGROUND Magnetic anchor technique(MAT)has been applied in laparoscopic cholecystectomy and laparoscopic appendectomy,but has not been reported in laparoscopic partial hepatectomy.AIM To evaluate the feasibility of ...BACKGROUND Magnetic anchor technique(MAT)has been applied in laparoscopic cholecystectomy and laparoscopic appendectomy,but has not been reported in laparoscopic partial hepatectomy.AIM To evaluate the feasibility of the MAT in laparoscopic left lateral segment liver resection.METHODS Retrospective analysis was conducted on the clinical data of eight patients who underwent laparoscopic left lateral segment liver resection assisted by MAT in our department from July 2020 to November 2021.The Y-Z magnetic anchor devices(Y-Z MADs)was independently designed and developed by the author of this paper,which consists of the anchor magnet and magnetic grasping apparatus.Surgical time,intraoperative blood loss,intraoperative accidents,operator experience,postoperative incision pain score,postoperative complications,and other indicators were evaluated and analyzed.RESULTS All eight patients underwent a MAT-assisted laparoscopic left lateral segment liver resection,including three patients undertaking conventional 5-port and five patients having a transumbilical single-port operation.The mean operation time was 138±34.32 min(range 95-185 min)and the mean intraoperative blood loss was 123±88.60 mL(range 20-300 mL).No adverse events occurred during the operation.The Y-Z MADs showed good workability and maneuverability in both tissue and organ exposure.In particular,the operators did not experience either a“chopstick”or“sword-fight”effect in the single-port laparoscopic operation.CONCLUSION The results show that the MAT is safe and feasible for laparoscopic left lateral segment liver resection,especially,exhibits its unique abettance for transumbilical single-port laparoscopic left lateral segment liver resection.展开更多

METHODS: Paraffin embedded materials from 23 cases of benign biliary stricture were analyzed for members of the TGF-β/Smad/CTGF signaling pathway. TGF-β_1, TβRⅠ, TβRⅡ, Smad4, Smad7 and CTGF protein were detected by immunohistochemical strepto-advidinbiotin complex method, and CTGF mRNA was evaluated by hybridization in situ, while 6 cases of normal bile duct served as controls. The percentages of positive cells were counted. The correlation between TGF-β_1, Smad4 and CTGF was analyzed. RESULTS: The positive expression ratios of TGF-β_1, TβRⅠ , TβRⅡ , Smad4, CTGF and CTGF mRNA in 23 cases with benign biliary stricture were 91.3%, 82.6%, 87.0%, 78.3%, 82.6% and 65.2%, respectively, signifi cantly higher than that in 6 cases of normal bile duct respectively (vs 33.3%, 16.7%, 50.0%, 33.3%, 50.0%, 16.7%, respectively, P < 0.05). The positiveexpression ratio of Smad7 in cases with benign biliary stricture was 70.0%, higher than that in normal bile duct, but this difference is not statistically signifi cant 70.0% vs 50%, P > 0.05). There was a positive correlation between positive expression of TGF-β_1, Smad4 and CTGF in cases with benign biliary stricture. CONCLUSION: The high expression of TGF-β/Smad/ CTGF signaling pathway plays an important role in the pathogenesis of benign biliary stricture.展开更多

Comparing and evaluating the Nationally Determined Contribution(NDC) is an important element in global stocktake in the post-Paris climate negotiations, aimed at closing the emissions gap with the Paris Agreement goal...Comparing and evaluating the Nationally Determined Contribution(NDC) is an important element in global stocktake in the post-Paris climate negotiations, aimed at closing the emissions gap with the Paris Agreement goals. To date, however, there has still been no explicit guideline or method. By applying emissions allowance allocated by 16 schemes as benchmarks, this paper tries to compare and evaluate the NDCs of the top six emitters, which jointly account for about 70% of the world's CO_2 emissions. Results show that the four developed countries' NDCs lack ambition with respect to most allocations under 2℃ and all under 1.5℃, indicating they need to substantially ratchet up their NDCs and lead elevating mitigation. Evaluating cumulative emissions is more likely to clarify the ambition and fairness of China's NDC. If considering cumulative emissions, China's NDC is aligned with the median of cumulative allowances under 2℃ and within the 1.5℃ range. The Paris Agreement invited the Parties to communicate the mid-century low emissions strategies. This paper also tries to explore the mid-century mitigation in the perspective of allocations, which might provide decision-makers with some useful information when envisaging the post-NDC mitigation.展开更多

In this investigation,a new silicone rubber-based MRE material was prepared to be used as a forming medium in manufacturing thin-walled complexshaped Ni-based tubes through the bulging process.Thus,it is significant ...In this investigation,a new silicone rubber-based MRE material was prepared to be used as a forming medium in manufacturing thin-walled complexshaped Ni-based tubes through the bulging process.Thus,it is significant to investigate the effect of magnetic field intensity,magnetic field loading time,and angle on the mechanical properties of the prepared MRE material during the curing process.The obtained results showed that increasing the magnetic field intensity during the curing process can improve the orientation of the chain structure in the elastomer matrix effectively.However,its mechanical properties are the best under the corresponding magnetic field intensity of 321 mT.Besides,by extending the magnetic field loading time in the curing process,the orientation of the chain structure was optimized,at the same time,the mechanical properties were also improved,and the best loading time is about 20–25 min.By changing the loading angle of the magnetic field during the curing process,the mechanical properties of the MRE were improved.When the loading angle of the magnetic field is 90°,the elastomer showed the best compression mechanical properties and excellent compression reversibility.Besides,for the anisotropic MRE material,the performance with magnetic compression is always better than that without magnetic compression.展开更多
A new type of microstructure inspired by the cross section of barchan dunes was proposed to reduce windage,which was considered as a passive drag reduction technology in aerospace manufacturing field.Computational flu...A new type of microstructure inspired by the cross section of barchan dunes was proposed to reduce windage,which was considered as a passive drag reduction technology in aerospace manufacturing field.Computational fluid dynamics method was carried out to discuss the effect of the microstructure on the skin friction reduction under high velocity flow condition.Different microstructure heights were employed to survey the reduction of drag.The results illustrated that the appearance of microstructure led to a generation of pressure drag in non-smooth model(with microstructures inspired by cross section of barchan dune)in contrast to smooth model.However,the microstructure significantly increased the thickness of the low-speed fluid by 11.4%in the near-wall flow field,causing the low-speed fluid to rise and decreasing the velocity gradient near the wall,thereby reducing viscous resistance.In addition,high-speed fluid flowed above the microstructure units instead of along the inner side of the units due to the influence of micro-vortex,resulting in a reduction of friction near the surface.Further-more,micro-vortex was considered to be the significant internal factor to achieve turbulent drag reduction since it could not only reduce the viscous resistance by promoting the fluid flow above the microstructure but also pro-vide a reverse thrust force.The understanding of the mechanism of drag reduction provides theoretical guidance for further fabrication of drag reduction coatings using renewable materials.展开更多
BACKGROUND Biliary adenomas that occur in the extrahepatic biliary tree are rare.It is difficult to distinguish it from cholangiocarcinoma or cholangiolithiasis by various imaging examinations,and it is very easy to b...BACKGROUND Biliary adenomas that occur in the extrahepatic biliary tree are rare.It is difficult to distinguish it from cholangiocarcinoma or cholangiolithiasis by various imaging examinations,and it is very easy to be misdiagnosed.AIM To evaluate the cumulative experiences including clinical characteristics and treatments of nine patients diagnosed with extrahepatic biliary adenoma admitted to the First Affiliated Hospital of Xi’an Jiaotong University from 2016 to 2022.METHODS A total of nine patients were included in our study.The laboratory examinations,disease diagnosis,therapy and pathological characteristics,and follow-up of every patient were evaluated.RESULTS Our cohort consisted of six females and three males with an average diagnosis age of 65.1 years(range 46-87).Six extrahepatic biliary adenomas were located in the common bile ducts and three in the hepatic duct.On initial presentation,all of the patients have symptom of biliary origin,including obstructive jaundice(4/9,44.4%),abdominal pain(6/9,66.7%),and fever(3/9,33.3%).Preoperative imaging examination considered bile duct carcinoma in 6 cases and bile duct calculi in 3 cases.All the patients received surgical treatment and were confirmed by pathology as biliary adenoma.The symptoms improved significantly in all 9 patients after surgery.Seven of nine patients recovered well at follow-up without tumor recurrence.One patient died 2 mo after the surgery due to heart failure.One patient developed jaundice again 8 mo after surgery,underwent endoscopic retrograde cholangiopancreatography and biliary stent placement.CONCLUSION Benign extrahepatic biliary tumors are rare and difficult to diagnosis preoperatively.Intraoperative choledochoscopy and timely biopsy may offer great advantages.展开更多

Autistic spectrum disorder(ASD)is a male-biased,heterogeneous neurodevelopmental disorder that affects approximately 1%e2%of the population.Prenatal exposure to valproic acid(VPA)is a recognized risk factor for ASD,bu...Autistic spectrum disorder(ASD)is a male-biased,heterogeneous neurodevelopmental disorder that affects approximately 1%e2%of the population.Prenatal exposure to valproic acid(VPA)is a recognized risk factor for ASD,but the cellular and molecular basis of VPA-induced ASD at the single-cell resolution is unclear.Here,we aim to compare the cellular and molecular differences in the hippocampus between male and female prenatal mice with ASD at the single-cell transcriptomic level.The transcriptomes of more than 45,000 cells are assigned to 12 major cell types,including neurons,glial cells,vascular cells,and immune cells.Cell type-specific genes with altered expression after prenatal VPA exposure are analyzed,and the largest number of differentially expressed genes(DEGs)are found in neurons,choroid plexus epithelial cells,and microglia.In microglia,several pathways related to inflammation are found in both males and females,including the tumor necrosis factor(TNF),nuclear factor kappa B(NF-kB),toll-like receptor(TLR),and mitogen-activated protein kinase(MAPK)signaling pathways,which are important for the induction of autistic-like behavior.Additionally,we note that several X-linked genes,including Bex1,Bex3,and Gria3,were among the male-specific DEGs of neurons.This pioneering study describes the landscape of the transcriptome in the hippocampus of autistic mice.The elucidation of sexual differences could provide innovative strategies for the prevention and treatment of ASD.展开更多

文摘A new type of microstructure inspired by the cross section of barchan dunes was proposed to reduce windage,which was considered as a passive drag reduction technology in aerospace manufacturing field.Computational fluid dynamics method was carried out to discuss the effect of the microstructure on the skin friction reduction under high velocity flow condition.Different microstructure heights were employed to survey the reduction of drag.The results illustrated that the appearance of microstructure led to a generation of pressure drag in non-smooth model(with microstructures inspired by cross section of barchan dune)in contrast to smooth model.However,the microstructure significantly increased the thickness of the low-speed fluid by 11.4%in the near-wall flow field,causing the low-speed fluid to rise and decreasing the velocity gradient near the wall,thereby reducing viscous resistance.In addition,high-speed fluid flowed above the microstructure units instead of along the inner side of the units due to the influence of micro-vortex,resulting in a reduction of friction near the surface.Further-more,micro-vortex was considered to be the significant internal factor to achieve turbulent drag reduction since it could not only reduce the viscous resistance by promoting the fluid flow above the microstructure but also pro-vide a reverse thrust force.The understanding of the mechanism of drag reduction provides theoretical guidance for further fabrication of drag reduction coatings using renewable materials.

文摘BACKGROUND Biliary adenomas that occur in the extrahepatic biliary tree are rare.It is difficult to distinguish it from cholangiocarcinoma or cholangiolithiasis by various imaging examinations,and it is very easy to be misdiagnosed.AIM To evaluate the cumulative experiences including clinical characteristics and treatments of nine patients diagnosed with extrahepatic biliary adenoma admitted to the First Affiliated Hospital of Xi’an Jiaotong University from 2016 to 2022.METHODS A total of nine patients were included in our study.The laboratory examinations,disease diagnosis,therapy and pathological characteristics,and follow-up of every patient were evaluated.RESULTS Our cohort consisted of six females and three males with an average diagnosis age of 65.1 years(range 46-87).Six extrahepatic biliary adenomas were located in the common bile ducts and three in the hepatic duct.On initial presentation,all of the patients have symptom of biliary origin,including obstructive jaundice(4/9,44.4%),abdominal pain(6/9,66.7%),and fever(3/9,33.3%).Preoperative imaging examination considered bile duct carcinoma in 6 cases and bile duct calculi in 3 cases.All the patients received surgical treatment and were confirmed by pathology as biliary adenoma.The symptoms improved significantly in all 9 patients after surgery.Seven of nine patients recovered well at follow-up without tumor recurrence.One patient died 2 mo after the surgery due to heart failure.One patient developed jaundice again 8 mo after surgery,underwent endoscopic retrograde cholangiopancreatography and biliary stent placement.CONCLUSION Benign extrahepatic biliary tumors are rare and difficult to diagnosis preoperatively.Intraoperative choledochoscopy and timely biopsy may offer great advantages.

文摘The water infiltration problem of water conservancy projects is one of the more common problems in the construction of water conservancy projects,which seriously affects the overall construction quality and safety of water conservancy projects.This article studies the causes of water infiltration in water conservancy projects and introduces related antiseepage technologies,hoping to be a reference for the implementation of anti-seepage projects.Through research,it is found that the factors that cause water infiltration in water conservancy projects include water conservancy project structural changes,environmental factors and construction factors.Relevant personnel can use concrete anti-seepage wall construction technology and high-pressure jet grouting technology to do the relevant anti-seepage work of water conservancy projects.

文摘Autistic spectrum disorder(ASD)is a male-biased,heterogeneous neurodevelopmental disorder that affects approximately 1%e2%of the population.Prenatal exposure to valproic acid(VPA)is a recognized risk factor for ASD,but the cellular and molecular basis of VPA-induced ASD at the single-cell resolution is unclear.Here,we aim to compare the cellular and molecular differences in the hippocampus between male and female prenatal mice with ASD at the single-cell transcriptomic level.The transcriptomes of more than 45,000 cells are assigned to 12 major cell types,including neurons,glial cells,vascular cells,and immune cells.Cell type-specific genes with altered expression after prenatal VPA exposure are analyzed,and the largest number of differentially expressed genes(DEGs)are found in neurons,choroid plexus epithelial cells,and microglia.In microglia,several pathways related to inflammation are found in both males and females,including the tumor necrosis factor(TNF),nuclear factor kappa B(NF-kB),toll-like receptor(TLR),and mitogen-activated protein kinase(MAPK)signaling pathways,which are important for the induction of autistic-like behavior.Additionally,we note that several X-linked genes,including Bex1,Bex3,and Gria3,were among the male-specific DEGs of neurons.This pioneering study describes the landscape of the transcriptome in the hippocampus of autistic mice.The elucidation of sexual differences could provide innovative strategies for the prevention and treatment of ASD.

文摘3D free bending process,an advanced metal forming technology,has attracted much attention due to its unique geometrical flexibility and efficiency.Filling tubes with materials may effectively reduce the distortion in the cross-section area and restrain wrinkling and collapse of tube wall.In the present study,the impacts of filling different materials on the copper tubes during 3D free bending process were investigated.We have found that copper tubes filled with low melting point alloys could induce the most uniform stress distribution,which result in better formability and moderate thickness changing rates when compared to SS304 steel balls and PU rubbers.The lowest ellipticity reached to 1.467%.
