Adrenocorticotropic hormone is recommended worldwide as an initial therapy for infantile spasms. However, infantile spasms in about 50% of children cannot be fully controlled by adrenocorticotropic hormone monotherapy...Adrenocorticotropic hormone is recommended worldwide as an initial therapy for infantile spasms. However, infantile spasms in about 50% of children cannot be fully controlled by adrenocorticotropic hormone monotherapy, seizures recur in 33% of patients who initially respond to adrenocorticotropic hormone monotherapy, and side effects are relatively common during adrenocorticotropic hormone treatment. Topiramate, vitamin B6, and immunoglobulin are effective in some children with infantile spasms. In the present study, we hypothesized that combined therapy with adrenocorticotropic hormone, topiramate, vitamin B6, and immunoglobulin would effectively treat infantile spasms and have mild adverse effects. Thus, 51 children newly diagnosed with West syndrome including infantile spasms were enrolled and underwent polytherapy with the four drugs. Electroencephalographic hypsarrhythmia was significantly improved in a majority of patients, and these patients were seizure-free, had mild side effects, and low recurrence rates. The overall rates of effective treatment and loss of seizures were significantly higher in cryptogenic children compared with symptomatic children. The mean time to loss of seizures in cryptogenic children was significantly shorter than in symptomatic patients. These findings indicate that initial polytherapy with adrenocorticotropic hormone, topiramate, vitamin Be, and immunoglobulin effectively improves the prognosis of infantile spasms, and its effects were superior in cryptogenic children to symptomatic children.展开更多

BACKGROUND A prostatic stromal tumor is deemed to be a rare oncology condition.Based on the retrospective analysis of clinical data and scientific literature review,a case of prostatic stromal tumor was reported in th...BACKGROUND A prostatic stromal tumor is deemed to be a rare oncology condition.Based on the retrospective analysis of clinical data and scientific literature review,a case of prostatic stromal tumor was reported in this article to explore the diagnosis,treatment and prognosis of this rare disease.CASE SUMMARY The present case involved an older male patient who was admitted to our department for a medical consultation of dysuria.Serum prostate-specific antigen was 8.30 ng/mL,Ultrasound and magnetic resonance imaging suggested evident enlargement of the prostate and multiple cystic developments internally.Considering that the patient was an elderly male with a poor health status,transurethral resection of the prostate was performed to improve the symptoms of urinary tract obstruction.Furthermore,based on histopathologic examination and immunohistochemical staining,the patient was pathologically diagnosed with prostatic stromal tumor.The patient did not receive any further adjuvant therapy following surgery leading to a clinical recommendation that the patient should be followed up on a long-term basis.However,during the recent follow-up assessment,the patient demonstrated recurrence of lower urinary tract symptoms and gross hematuria.CONCLUSION Referring to scientific literature review,we believe that the management of these lesions requires a thorough assessment of the patient.Furthermore,the treatment of prostate stromal tumors should be based on the imaging examination andpathological classification. Active surgical treatment is of great significance to theprognosis of patients, and subsequent surveillance after the treatment iswarranted.展开更多
BACKGROUND Hem-o-Lok clip(HOLC)has been widely used in laparoscopic surgery due to its ease of application and secure clamping,though the rare complications associated with this technique should not be ignored.The rar...BACKGROUND Hem-o-Lok clip(HOLC)has been widely used in laparoscopic surgery due to its ease of application and secure clamping,though the rare complications associated with this technique should not be ignored.The rare complications of laparoscopic partial nephrectomy consist of the clip migrating into the renal pelvis and acting as a nidus for stone formation.CASE SUMMARY The case described here involved a 63-year-old woman who was found with stones in the right kidney and upper ureter during a recent reexamination following laparoscopic partial nephrectomy.We performed percutaneous nephrolithotomy for her,but during the operation,it was found that the center of the stone within the kidney was a HOLC,which was removed with forceps.For this reason,we speculate that the HOLC,which was employed to halt tumor wound bleeding,spontaneously drifted into the renal pelvis and formed kidney stones,with the clip being initially misdiagnosed as a kidney stone.CONCLUSION By reviewing related case reports,we conclude that in order to prevent complications related to HOLC,loose clips should be actively searched for and retrieved from the wound during urinary tract surgery,while the deployment of clips in close proximity of anastomotic stoma of collecting systems should be avoided.展开更多
Electrochemical water splitting is quite seductive for eco-friendly hydrogen fuel energy production,however,the attainment of highly efficient,durable,and cheap catalysts for the hydrogen evolution reaction(HER)remain...Electrochemical water splitting is quite seductive for eco-friendly hydrogen fuel energy production,however,the attainment of highly efficient,durable,and cheap catalysts for the hydrogen evolution reaction(HER)remains challenging.In this study,molybdenum oxides stabilized palladium nanoparticle catalysts(MoO_(x)-Pd)are in situ prepared on commercial carbon cloth(CC)by the facile two-step method of dip-coating and electrochemical reduction.As a self-supported Pd-based catalyst electrode,the MoO_(x)-Pd/CC presents a competitive Tafel slope of 45.75 mV·dec^(-1),an ultralow overpotential of 25 mV,and extremely long cycling durability(one week)in 0.5M H_(2)S0_(4)electrolyte,superior to unmodified Pd catalysts and comparable to commercial Pt mesh electrode.On the one hand,the introduction of MoO_(x)can inhibit the growth of Pd particles to obtain ultrafine Pd nanoparticles,thus exposing more available active sites.On the other hand,density functional theory(DFT)calculation revealed that MoO_(x)on the surface of Pd metal can regulate the electronic structure of Pd metal and enhance its intrinsic catalytic activity of HER.This work suggests that transitional metal nanoparticles stabilized by molybdenum oxides are hopeful approaches for obtaining fruitful hydrogen-producing electrocatalysts.展开更多

To assign the possible configurations of these two bands, experimental B(M1)/B(E2) ratios and signature splittings have been evaluated for the △I = 1 band, and calculations based on the geometrical model, cranked shell model and total Routhian surfaces model have been performed. They are suggested to be a near-oblate band built on the (πg7/2/d5/2) (vh11/2)2 configuration and a prolate band built on the πg7/2 (πh11/2)2 configuration, respectively.展开更多
On September 20 th, 2015, twenty satellites were successfully deployed into a near-polar circular orbit at 520 km altitude by the Chinese CZ-6 test rocket, which was launched from the Tai Yuan Satellite Launch Center....On September 20 th, 2015, twenty satellites were successfully deployed into a near-polar circular orbit at 520 km altitude by the Chinese CZ-6 test rocket, which was launched from the Tai Yuan Satellite Launch Center. Among these satellites, a set of 4 Cube Sats conform the atmospheric density detection and precise orbit determination(APOD) mission, which is projected for atmospheric density estimation from in-situ detection and precise orbit products. The APOD satellites are manufactured by China Spacesat Co. Ltd. and the payload instruments include an atmospheric density detector(ADD), a dual-frequency dualmode global navigation satellite system(GNSS) receiver(GPS and Beidou), a satellite laser ranging(SLR) reflector, and an S/Xband very long baseline interferometry(VLBI) beacon. In this paper, we compare the GNSS precise orbit products with colocated SLR observations, and the 3 D orbit accuracy shows better than 10 cm RMS. These results reveal the great potential of the onboard micro-electro-mechanical system(MEMS) GNSS receiver. After calibrating ADD density estimates with precise orbit products, the accuracy of our density products can reach about 10% with respect to the background density. Density estimates from APOD are of a great importance for scientific studies on upper atmosphere variations and useful for model data assimilation.展开更多
